Kaltenborn의 관절가동 기법 (Kaltenborn's Joint Mobilization Techniques)

  • This study was introduce to Manual Mobilization of the Extremity Joints of Freddy M. Kaltenborn(1989). Much of Orthopedic Manipulative Therapy is devoted to the evaluation and treatment of joint and related soft tissue disorders and one of the primary treatment methods is mobilization. When examination reveals joint dysfunction, especially decreased range of motion, joint mobilization techniques are often utilized. Kaltenborn's joint mobilization techniques are performed as passive examination or treatment movements by the therapist. There are three basic joint play movements: (1) traction, (2) compression, and (3) translatoric gliding. The purpose of joint mobilization is to restore normal, painless joint function. Mechanically, the goal is to restore joint play and thus normalize roll-gliding which occurs during active movements.

  • This research recognized about general ancient temple of Alzheimer dementia. Research of cause of Alzheimer dementia is progressing up to now. Age, education, melancholia, estrogen, woman, smoking, thyroid gland disease, aluminum etc. are danger factor of an Alzheimer dementia. Familyish factor was proved in some degree by gene. Medicine in early patient's case imbecility some measure progress late in degree develop. However, ceilinged thing is true in treatment of Alzheimer dementia up to now, and must help so that their quality of life may can rise and laws of physical therapist must help to keep function in everyday life.

기능적 전기자극 치료의 최근 기법과 향후 발전 방향 (Current method of Functional Electric Stimulation and Direction of Henceforth Expansion)

  • Functional Electrical Stimulation is the clinical application of a small electric current to the intact nerves of the body, in order to trigger a muscle contraction. This contraction is then incorporated into a functional activity, for example walling. A clear distinction needs to be made between therapeutic stimulation and functional stimulation. The former being of an exercise orientation, where one would relax while the stimulation works on its own. Functional electrical stimulation on the other hand, incorporates this elicited muscle movement into an everyday activity, like standing, walking, reaching out etc. But recently, New method used to FES which updated electrode insult in muscle and small nerve branch. A lot of new research build up many countries such as England, USA, Japan, Spain, Canada. So I had been write this paper for introduce new FES method, and I hope to more enhanced motivation of therapist for the functional electric stimulation.

물리치료사들의 경추 가동범위에 관한 연구 (A Study for cervical range of motion in Physical Therapists)

  • The purpose of this study is to find out the influence of Physical Therapists' heavy work on their cervical spines. Subjects were 80 members of Physical Therapists, 40 males and 40 females who were between 3rd decades to 4th decades. The Cervcial Range of Motion instrument was used to measure six cervical range of motion such as flexion, extension, left and right lateral flexion, left and right rotation. The degree of rotation and both lateral flexion was appeared as decrement in 4th decades than 3rd decades(P<0.05). And the left lateral flexion in female Physical therapiest was increased than in male ones(P<0.05). By this results, it was proved that the older the PT's age, the less the cervical motion range and there were more decrement in male Physical Therapists than female Physical Therapists. And finally I think that the evaluating method used in this study will be clinically very useful and be a basic raw materials for evaluating the existence and the degree of functional disability as in the occupational hazards due to Physical Therapist's heavy work.

VDT 증후군 환자에 물리치료와 인간공학적 개입: 사례 연구 (Physical Therapy and Ergonomic Interventions in Patients with VDT Syndrome: 4 Cases Study)

  • Purpose: Among computer users, the awkward posture and workstation setups of workers contribute to work-related upper limb musculoskeletal disorders. The purpose of this study was to evaluate the efficacy of a traditional physical therapy and ergonomic intervention by physical therapists in 4 computer users. Methods: After checking Visual Analogue Scores (VAS), four subjects who were treated by physical therapy for neck and shoulder problems related to VDT syndrome were enrolled in the study. All subjects spent at least 40 hours per week at a computer workstation. All subjects had pain and ergonomic states evaluated using methods such as VAS, Neck Disability Index (NDI), Workstyle short form, Rapid Upper Limb Assessment (RULA), and OSHA VDT checklist (Occupational Safety and Health Administration video display terminal) before a physical therapy + ergonomic intervention. Participants were re-evaluated 1 month later. Results: Participants showed more improvement of their neck pain after being treated with a physical therapy plus ergonomic intervention than when treatment consisted only of physical therapy. Improvements in RULA, Workstyle short form, and OSHA VDT checklist also were achieved. Conclusion: This case study suggests the importance of examining the work habits and work-related postures of subjects who complain of neck and shoulder pain that is exacerbated by computer use. Personalized ergonomic interventions and physical therapy can lead to improvement of patients with VDT syndrome.

건강보험정책심의위원회의 이익집단 간 대립이 물리치료 수가에 미치는 영향 (Conflict of Interest Groups on the Health Insurance Policy Deliberation Committee Affect the Medical Insurance Cost of Physical Therapy)

  • Purpose: The aim of this study was to demonstrate that non-participation of physical therapists on the political decision-making committee results in invasion of their interests. Methods: To demonstrate the effects, we analyzed the change of medical insurance score decided by the Health Insurance Policy Deliberation Committee between 2001 and 2012 years, focusing on medical examination as the interest of the participation group and physical therapy cost as interest of the non-participation group. Results: Total medical insurance cost increased by 23.72%, on average. Medical examination cost increased by 23.90% and 37.66% in medical examination for new and established patients, respectively. However, physical therapy cost was reduced by 5.01%. The medical examination cost for physical therapy without medical checkup increased by 2.62%. Conclusion: This study shows that the physical therapy cost, related on the interest of the non-participative group in the Health Insurance Policy Deliberation Committee, rather decreased while the total medical insurance cost increased.These findings demonstrate the invasion of the non-participative group on the Health Insurance Policy Deliberation Committee. Thus, aggressive participation in political decision-making committee is necessary in order to protect and increase rights and interests of Korean physical therapists.

A Study on the Introduction of Home-Based Physical Therapy for Patients with Cerebrovascular Disease

  • Purpose: This study was conducted in order to present basic policy data for introduction of a home-based physical therapy (HBPT) policy. Methods: The study surveyed physical therapists and patients for six weeks, in order to identify satisfaction with physical therapy. The statistical significance of the management plan with regard to costs and cost estimation and payment methods was determined and managed according to the operating principals of HBPT, eligibility criteria, number of visits, treatment time, and team approach. Results: Through the survey all groups of patients and physical therapists recognized the need for HBPT. They felt that the most desirable methods for activation of physical therapy visits involve precise diagnosis and evaluation through a team approach. While making regular visits to the patient three times a week, an expert visiting physical therapist with 3 to 5 years of experience in the clinical field could provide central nerve developmental treatment in less than 60 minutes; this service could be provided at a reasonable cost, minimizing unnecessary hospital visits, reducing time, medical expenses and facilitating a quick recovery due to psychological satisfaction resulting from a decrease in the psychological anxiety patients often experience in medical institutions. Conclusion: Based on the study results provided above, if HBPT is to be introduced, appropriate pricing and programs should be based on the results of pilot projects.

후천성면역결핍증의 물리치료 (Physical Therapy for Acquired Immune Deficiency Syndrome)

  • The purpose of this paper is to review the literature of AIDS/HIV (acquired immune deficiency syndrome/human immunodeficiency virus) and to introduce physical therapy of the AIDS client. It was first reported that five young homosexual men, in 1981 in Los Angeles, died of pneumonia caused by pneumocystis carinii - called "pest of twentieth century". AIDS was implied a fatal breakdown of the functioning of the normal human immune system. The major routes of transmission has occurred via l)sexual contact between men, 2)heterosexual intercourse, 3)contaminated blood and blood products, and 4)intrauterine or pre-natal infection from mothers. AIDS clients who may require physical therapy for a wide range of problems due to respiratory, neurological, musculoskeletal and painful syndromes as well as general decline in fitness and function. AIDS is growing rapidly in our contury as well as all over the world. Therefore, physical therapist has to correct his understandings of AIDS in order to protect himself and to give proper physical therapy to the AIDS clients.

물리치료사의 직무만족에 영향을 미치는 요인 (Analysis of Factors that Influence the Job Satisfaction of Physical Therapists)

  • Purpose: This study aimed to analyze factors that influence the job satisfaction of physical therapists working in hospitals. Methods: The study was done using a self-administered questionnaire associated with job satisfaction, which was completed by 352 physical therapists working in hospitals. Results: The average job satisfaction score was 3.16. Sub-items scores for job satisfaction were higher for overall satisfaction (3.21) and lower for job satisfaction of achievement (3.12) than the average score. Job satisfaction of subjects showed statistically significant differences with regard to differences in sex, marriage, type of hospital, year of total career, type of patients and motivation level (p<0.05). The average job stress score was 2.38. The sub-items of job stress consisted of problems pertaining to physical environment (2.28), job demand (2.54), job autonomy (2.31), social support (2.14), job instability (2.36), organized management system (2.44), inner motive (2.24), and job culture (2.26). Multiple regression analysis revealed that job satisfaction became significantly lower with increasing levels of job stress. Conclusion: These results suggest that the job stress of physical therapists is closely related to their job satisfaction. An effort to reduce job stress may be helpful in improving the health and quality of life of physical therapists who work in hospitals.

A Survey on Sexual Harassment and Countermeasures of Physical Therapists in the Workplace

  • Purpose: This descriptive study aimed at physical therapists to determine the actual conditions of sexual harassment occurring in the workplace, their coping strategies, and effective coping methods. Methods: In total, 186 responses were collected through Google from June 28 to August 21, 2021. The questionnaire consisted of 102 questions about the subject's general characteristics, sexual harassment, psychological stress, physical stress, and sexual harassment prevention education. Statistical Package For The Social Sciences (SPSS) was used for analysis, frequency analysis, percentage, standard deviation, and corresponding sample t-test, and the significance level was set to 0.05. Results: The perception of sexual harassment was 7.1, which was lower than that of other occupations. The perpetrators of visual, verbal, and physical sexual harassment appeared in the order of patients, coworkers, and guardians. Psychological and physical stresses were higher than the average due to damage caused by sexual harassment, requiring some attention. Work stress showed an average level. With the higher perception, a negative correlation was observed in the face of mitigation (p<0.001). Conclusion: The number of victims of sexual harassment is increasing every year. To cope with sexual harassment, there should be a department capable of counseling and processing in the workplace, and what occurs should be analyzed. In addition, educational programs are needed to prevent sexual harassment in consideration of the characteristics of hospitals.
