전기자극 시 활동주기 형태의 변화가 혈중젖산과 혈장효소에 미치는 영향 (The Effects of Changing Duty Cycle With Electrical Stimulation on Blood Lactate and Plasma Enzyme)

  • By measuring changes in blood lactate and plasma enzyme (CPK, GOT, GPT) with electrical stimulation applied at two duty cycles, this study is intended to look into which type of duty cycle may have more effects on blood lactate and plasma enzyme constituents through animal experiment so as to determine any duty cycle appropriate for electrical treatment. In this study, electrical stimulation was applied to total 20 Korean house rabbits (weight: 3~3.5 kg) by means of an electrical therapeutic apparatus called TS6000 (made in Netherlands) at duty cycle of 50% and 20% respectively for 30 minutes. Here, 5 cc of blood was collected from their carotid artery before stimulation and in 30 minutes after stimulation respectively to carry out biochemical experiment and analysis. As determined through the above experiment, blood lactate rate was increased to 333.07% at 50% duty cycle after experiment and 185.71% at 20% duty cycle after experiment respectively. In both cases, blood lactate rate was significantly increased to higher level after electrical stimulation than before. Moreover, the rate of change in the average of blood lactate rate at both duty cycles also showed significant differences. CPK rate was boosted to 301.82% at 50% duty cycle after experiment and 321.35% at 20% duty cycle after experiment respectively. In both cases, CPK rate was remarkably boosted to higher level after stimulation than before (p<.05). However, there was not any significant difference in the rate of change in average CPK at both duty cycles (p<.05). GOT rate was significantly boosted up to 38.97% at 50% duty cycle after experiment (p<.05), while it was slightly increased to 1.68% at 20% duty cycle after experiment without any significant difference. Rather, GPT rate dropped slightly at both duty cycles after experiment, but there was not any significant difference. Although blood lactate and GOT were relatively less generated at 20% duty cycle after electrical stimulation than at 50% duty cycle, the change of duty cycle didn't have any significant influence on CPK rate. In this regard, this study failed to come any consistent conclusion about the association between change of duty cycle and muscle fatigue. Therefore, it is advisable that follow-up studies seek various ways to a little more effectively apply electrical stimulation to laboratory animals by avoiding their muscle fatigue. GOT rate was significantly boosted up to 38.97% at 50% duty cycle after experiment (p<.05), while it was slightly increased to 1.68% at 20% duty cycle after experiment without any significant difference. Electrical Stimulation Induces the Collagen Deposition and TGF-${\beta}$1 mRNA Expression in Skin Wound of Rat

  • Purpose: The purpose of this study was to investigate the effect of electrical stimulation (ES) on the wound closure rate, collagen deposition, and TGF-${\beta}$1 mRNA expression in skin wound of rat. Methods: Twenty male Sprague-Dawley rats (222~271 g) were randomly divided into ES (n=10) and control group (n=10). The ES group received a cathodal stimulation with 50 V at 100 pps for 30 minutes for 7 days, while the control group was not given electrical stimulation. The wound closure rate, collagen density and TGF-${\beta}$1 mRNA ratio were measured. Results: The mean wound closure rates in the ES and control groups were $83.79{\pm}16.35$% and $51.57{\pm}17.76$%, respectively (p<0.001). The collagen density in the ES and control groups were $46.67{\pm}10.68$% and $25.03{\pm}13.09$%, respectively (p<0.001). The TGF-${\beta}$1 mRNA ratio in the ES and control groups were $1.35{\pm}0.60$ and $0.63{\pm}0.30$, respectively at 6 hours post-wound (p<0.01) and $1.69{\pm}0.47$ and $1.32{\pm}0.28$, respectively, at 7 days post-wound (p<0.05). Conclusions: ES accelerated the wound closure rate of skin incision wounds and was accompanied by an increase in collagen deposition in the regenerating dermis. In addition, ES increased TGF-${\beta}$1 mRNA expression during wound healing process. These findings suggest that ES may activate TGF-${\beta}$1 expression, and may increase synthesis activities of fibroblasts in regenerating skin wounds in rats.

The effects of providing visual feedback and auditory stimulation using a robotic device on balance and gait abilities in persons with stroke: a pilot study

  • Objective: The purpose of this study was to investigate the effects of providing visual feedback and auditory stimulation using a robotic device on balance and gait abilities in stroke patients. Design: Randomized controlled pilot trial. Methods: Fifteen subjects were randomly divided into three groups where five subjects were in the visual feedback robotic device assist gait training (VRGT) group, five subjects in the auditory stimulation robotic device assist gait training (ARGT) group, and five subjects in the control group. Subjects received visual feedback and auditory stimulation while undergoing robotic gait training for 45 minutes, three times a week for 2 weeks, and all subjects had undergone general physical therapy for 30 minutes, five times a week for 2 weeks. All subjects were assessed with the Berg balance scale (BBS), timed up and go (TUG) test, and 10-meter walking test (10MWT) pre- and post-intervention. Results: All subjects showed that BBS, TUG test, and 10MWT scores significantly improved post-intervention (p<0.05), and the control group also had significantly improved post-treatment (p<0.05). The VRGT and the ARGT showed significant improvements in BBS, TUG, and 10MWT scores compared with the control group (p<0.05). The VRGT group showed a significant improvement in BBS, TUG, and 10MWT scores compared with the control group (p<0.05). In addition, it has been confirmed that VRGT had significantly improved in BBS, TUG test, and 10MWT scores compared with the auditory stimulation and control group (p<0.05). Conclusions: The results of this study showed improved balance and gait abilities after VRGT and ARGT groups compared with general physical therapy and was found to be effective in enhancing the functional activity of persons affected with stroke.

  • In most tissues, apoptosis plays a pivotal role in normal development and in regulation of cell number. Therefore inappropriate apoptosis is revealed in a variety of diseases. This study was carried out to investigate the effects of acupuncture and needle electrode electrical stimulation on the change of caspase-3, 9 and neuronal nitric oxide synthase (nNOS) immunoreactive cells in the sprague dawley rats (SD rat). In immobilized SD rats (n=5), enhanced caspase-3 and caspase-9 expression were detected in the reticular part of substantia nigra, and enhanced nNOS was detected in the dorsolateral periaqueductal gray (DL-PAG) of midbrain and the paraventricular nucleus (PVN) of the hypothalamus using immunohistochemistry. Following the immobilization, acupuncture (n=5) and needle electrode electrical stimulation (n=5, 2 Hz) was applied at H$\acute{e}$g$\breve{u}$ (LI4) acupoint of SD rats, respectively. The stress-induced enhancement in the expression of caspase-3, 9 and nNOS were The present results demonstrate that and needle electrode electrical stimulation are effective in the modulation of expression of caspase-3, 9 and nNOS induced by immobilization.

The Effects of Stair Climbing Training with Functional Electrical Stimulation on Muscle Strength, Balance, and Gait in Patients with Chronic Stroke

  • Objective: The weakness of muscle strength due to stroke affects the posture control and gait in the patients with stroke. Stroke This study examined the effects of the stair climbing training with functional electrical stimulation on muscle strength, dynamic balance, and gait in individuals with chronic stroke. Design: Randomized controlled trial. Methods: Total forty-eight patients were randomly assigned to the 3 groups. Participants randomly divided to stair climbing training with functional electrical stimulation group (SCT+FES group, n=16), stair climbing training group (SCT group, n=16) and control group (n=16). Subjects in the SCT+FES group and SCT group performed stair walking training with and without functional electrical stimulation for 30 minutes, 3 sessions per week for 4 weeks and all subjects received conventional physical therapy for 30 minutes with 5 sessions per week for 4 weeks. Outcome measurements were assessed using the sit-to stand Test for strength, timed up and go test and modified-timed up and go test for dynamic balance, and 10m walk test and GaitRite system for gait. Results: In the SCT+FES group, subjects have been shown the significant increase in lower extremity strength (p<0.05), significantly improve in dynamic balance (p<0.05), and significantly improve in their temporal gait parameter (p<0.05). The SCT+FES group was significantly better than other groups in all parameters (p<0.05). Conclusions: This result suggested that the SCT+FES may be effective strategy to improve muscle strength, dynamic balance, and gait for individuals with chronic stroke.

  • Evoked potentials(EP) are defined as electric responses of the nerves system to sensory stimulation. EPs are used mainly to test conduction in the visual, auditory, and somatosensory systems, especially in the central parts of these systems. Somatosensory evoked potentials (SEP) are the potentials elicited by stimulation of peripheral nerves and recorded at various sites along the sensory pathway. SEPs types consist mainly of SEPs to electric stimulation of arm or leg nerves. SEPs to arm stimulation are usually recorded simultaneously from clavicular, cervical, and scalp electrodes; SEPs to leg stimulation are recorded from lumbar, low thoracic, and scalp electrodes. Subject variables that have practical impotance are age, limb length, body height, and temperature. General clinical interpretation of abnormal SEPs wave decreases of peripheral conduction time, and abolition of SEPs recorded from different levels to identify lesions of peripheral nerves, plexus, nerve root, spinal cord, cauda equina, hemispheric brainstem, and cerebral parts of the somatosensory pathway.

간섭파가 노인의 교감신경계에 미치는 영향 (The Effects of Interferential Current Therapy on Sympathetic Nerve System in Senile patients)

  • The purpose of this study was to investigate the influence on sympathetic nerve system of interferential current therapy(ICT). The subjects were consisted of 20 senile patients, 10 males and 10 females with an average age of 71 years old. And systolic and diastolic blood pressure, temperature, heart rate, and respiratory were tested. The results were as follows: 1) Systolic and diastolic blood pressure were observed a statistical significance between before and during stimulation(p < .05). 2) Temperature was observed a statistical significance between before and during, and before and after 10 minutes stimulation(p < .05). 3) Heart rate and respiratory were not statistical significance(p > .05). These results are imply that electrical stimulation is directly or indirectly influence on sympathetic nerve system.

  • The purpose of this study is to investigate the relationship between effects of TENS (Transcutaneous Electrical Nerve Stimulation) and IFC(Interferrential Current Therapy) to the change of body surface temperature. Cases are 22 normal persons and 22 patients with low back pain. Digital Infrared Thermal Imaging system was used for the detection of body surface temperature. 50Hz in frequency and 25-35mA in intensity were applied to TENS and IFC, 15 and 10 minutes on each. The results were follows ; 1. TENS and IFC has on effect of decreasing surface temperature, which would be from cardiovascular factors. (P<0.001) 2. The influence of IFC to the body surface temperature is greater than TENS, and it seems to be vasoconstriction of sympathetic activity. 3. There were no significant differences of body surface temperature between the two groups before and after electrical stimulation.

  • The purpose of this study was to investigate the effect of functional electrical stimulation(FES) on sitting balance in child with cerebral palsy. Four cerebral palsy children were selected for this study. Functional electrical stimulation(FES) was applied to subject's abdominal muscle and electrospine muscle. Assessment was carried out before treatment for obtain baseline measurement of sitting balance and reassessment were carried out after treatment. The obtain results are as follows. 1. The result of this study were following that maximum perturbation area was significantly reduced after treatment compared with pre-treatment. 2.The result of this study were following that maximum perturbation velocity was significantly reduced after treatment compared with pre-treatment.

경피신경전기자극이 자율신경계에 미치는 영향 (The Influence of Transcutaneous Electrical Stimulation on Autonomic Function)

  • Purpose:The purpose of this study is to investigate the effect of transcutaneous electrical stimulation on autonomic nervous system using heart rate variability analysis. Methods:31 subjects were evaluated with HRV before and after a single high-frequency TENS for 15minutes treatment. The standard deviation of all the normal RR-interval(SDNN) and lower frequency/high frequency ratio(LF/HF ratio) were recorded with TAS-9. Results:After single TENS treatment, autonomic adaptation as analyzed by SDNN was increased from $42.47\pm12.96$ to $4.43\pm16.76$(p>0.05) and autonomic balance as analyzed by LF/HF ratio was increased from $1.4\pm0.45$ to $1.45\pm0.41$(p>0.05). Conclusion:In this study, SDNN and LF/HF ratio changed in health young students as a result of single TENS treatment. But there is no significant difference between pre-treatment and post-treatment of SDNN and LF/HF ratio.
