노인의 낙상과 연관된 위험요소와 균형 측정 방법 (Risk Factors and Methods in Balance Assessment Associated with Fall in Older Adults)

  • Purpose : The purpose of this study was to determine risk factors and methods in balance assessment associated with fall in older adults. Methods : This article describes many of the tools that can be used to evaluate the physical parameters associated with fall risk in older adults. Results : Composite ratings of performance(Tinetti balance assessment, Guralnik test battery, Berg balance scale, modified-physical performance test) measures the score compounding the balance measure to determine fall risk. Static balance instruments are composed of FICSIT-4 that measures the ability of maintaining foot positions and CTSIB that measures postural stability. Dynamic balance instrument is composed of functional reach test. To measure walking velocity and mobility, 8-foot up-and-go test and walking around two cones are used. We can use 1-RM and to measure muscular strength, isokinetic dynamometery, and 30-second chair stand to measure lower extremity muscle strength. Conclusion : The described instruments are easy to use and widespread. To select and use these tool kits carefully is considered to be helpful in identifying those who are most likely to fall. The final part of the article includes a brief discussion of the potential role of exercise training interventions to improve these physical parameters and prevent falls.

Multiobjective Optimization of Three-Stage Spur Gear Reduction Units Using Interactive Physical Programming

  • The preliminary design optimization of multi-stage spur gear reduction units has been a subject of considerable interest, since many high-performance power transmission applications (e.g., automotive and aerospace) require high-performance gear reduction units. There are multiple objectives in the optimal design of multi-stage spur gear reduction unit, such as minimizing the volume and maximizing the surface fatigue life. It is reasonable to formulate the design of spur gear reduction unit as a multi-objective optimization problem, and find an appropriate approach to solve it. In this paper an interactive physical programming approach is developed to place physical programming into an interactive framework in a natural way. Class functions, which are used to represent the designer's preferences on design objectives, are fixed during the interactive physical programming procedure. After a Pareto solution is generated, a preference offset is added into the class function of each objective based on whether the designer would like to improve this objective or sacrifice the objective so as to improve other objectives. The preference offsets are adjusted during the interactive physical programming procedure, and an optimal solution that satisfies the designer's preferences is supposed to be obtained by the end of the procedure. An optimization problem of three-stage spur gear reduction unit is given to illustrate the effectiveness of the proposed approach.

Research on safety assessment and application effect of nanomedical products in physical education

  • This study investigates the application of nano-composite materials in physical education, specifically focusing on improving the performance of sports hall flooring. The research centers on carbon nanotube reinforced polyvinyl chloride (PVC) composites, which offer enhanced mechanical properties and durability. The incorporation of carbon nanotubes as reinforcements in the PVC matrix provides notable benefits, including increased strength, improved thermal stability, electrical conductivity, and resistance to fatigue. The key parameters examined in this study are the weight percentage of carbon nanotubes and the temperature during the fabrication process. Through careful analysis, it is found that higher weight percentages of carbon nanotubes contribute to a more uniform dispersion within the PVC matrix, resulting in improved mechanical properties. Additionally, higher fabrication temperatures aid in repairing macroscopic defects, leading to enhanced overall performance. The findings of this study indicate that the utilization of carbon nanotube reinforced PVC composites can significantly enhance the strength and durability of sports hall flooring. By employing these advanced materials, the safety and suitability of physical education environments can be greatly improved. Furthermore, the insights gained from this research can contribute to the optimization of composite material design and fabrication techniques, not only in the field of physical education but also in various industries where composite materials find applications.

복합운동에 혈류제한 적용과 비적용이 여성노인들의 운동수행능력과 평형성에 미치는 영향 (The Influence of Combined Exercise Training with and without Blood Flow Restriction on Physical Performance and Balance in Elderly Females)

  • 본 연구는 12주간 복합운동 중 혈류제한을 적용한 것과 적용하지 않은 것이 65세 이상 여성노인들의 운동수행능력(심폐지구력, 하체근력)과 평형성(동적평형성, 낙상위험지수)에 미치는 영향을 알아보기 위해 실시하였다. 연구대상자는 혈류제한을 통한 복합운동(14명), 혈류제한을 하지 않은 복합운동(14명)과 통제집단(15명)으로 총 43명이 연구에 참여하였다. 혈류제한 복합운동 그룹과 복합운동 그룹 모두 주 3회 총 12주 동안 복합운동을 실시하였고, 통제집단은 훈련기간 동안 일상적인 생활을 하였다. 본 연구의 자료처리는 SPSS/ver22를 사용하였으며 사전값의 동질성 검사 후 이원반복측정분산분석(Two-way Repeated Measures ANOVA)과 대비검증(contrast)을 이용하여 분석하였고, 모든 자료의 유의수준은 α=.05로 설정하였다. 연구결과 혈류제한을 적용한 복합운동 집단과 적용하지 않은 복합운동 집단의 운동수행능력(심폐지구력, 하체근력)과 평형성(동적평형성)은 유의하게 증가하였고, 두 집단 모두 낙상위험지수는 유의하게 감소하였다. 하지만 통제집단에서는 유의한 변화가 없었다. 추가적으로 두 운동집단 중 혈류제한을 적용한 복합운동 집단이 사전과 사후 변화량(%△)과 효과크기(effect size)가 일반복합운동 집단 보다 높이 나왔다. 이는 혈류제한을 적용한 복합운동집단이 혈류제한을 적용하지 않은 복합운동보다 운동수행력과 평형성에 더욱 긍정적인 효과를 나타냈다.

노인요양병원 입원환자의 TUG, FRT와 SPPB와의 상관관계 연구 (Correlations between SPPB, FRT, and TUG in Hospitalized Frail People - The Timed Up and Go test, the Functional Reaching Test, and the Short physical performance battery -)

  • Purpose: The purpose of this study was to determine the correlations, if any, between SPPB (Short physical performance battery), FRT (Functional reach test), and TUG (Timed up and go test) in hospitalized frail people. Methods: Seventy frail elderly patients (male/female: 16/54) participated in this study. All subjects could walk with or without an assisting device and they had no mental problems. Subjects were scored on physical functioning using the SPPB, FRT, and TUG. The data were analyzed using Pearson Correlation Coefficients. Results: There was a statistically significant difference between (a) SPPB and (b) FRT (p<0.01) and TUG (p<0.01). SPPB and FRT scores showed a positive correlation (r = 0.38) and SPPB and TUG scores showed a negative correlation (r = -0.56). Conclusion: SPPB, FRT, TUG scores are correlated with each other. From these results, we conclude that the SPPB is a useful method for predicting walking, balance, and physical function in frail elderly people.

수행에 대한 지식의 상대적 빈도가 편마비 환자의 균형 재훈련에 미치는 영향 (Effects of Relative Frequency of Knowledge of Performance on Balance Retraining in Patients With Hemiplegia)

  • During therapy sessions, feedback is often provided concurrently by the physical therapist as the patient attempts to perform a movement and after the movement attempt. This feedback is provided to enhance the patient's balance abilities. However, recent studies in nondisabled populations have suggested that frequent feedback may be detrimental to retention or learning of motor skills. This study compared the effects of 100% relative frequency of knowledge of performance (KP) with 66% relative frequency of KP for motor learning on balance retraining in patients with hemiplegia. Twenty patients with hemiplegic were randomly assigned to one of two experimental groups. The acquisition phase consisted of 16 blocks of 5 trials for 2 days (80 total practice trials). The retention phase consisted of 2 blocks of a short-term retention test, one day after the end of the acquisition phase and a long-term retention test, one week after the end of the short-term retention test. In the 100% feedback condition, participants received feedback after every practice trial. A faded KP schedule was used in the 66% condition. No significant differences were found between the two groups during all experimental phases (acquisition and retention phases), (p>.05). However, there were significant decreases in balance index for both groups of acquisition phase (p<.05). These results suggest that 66% relative frequency of KP is not more effective than 100% relative frequency of KP with respect to retention over time when hemiparetic patients attempt to learn balance.

암 환자에 대한 한국어판 FACIT-호흡곤란 10개 항목 단축형 설문지의 신뢰도와 타당도 분석 (Reliability and Validity of Korean Version of FACIT-dyspnea 10 Item Short Form in Patients With Cancer)

  • Background: The Functional Assessment of Chronic Illness Therapy (FACIT) for Dyspnea was developed to assess multidimensional dyspnea using two subscales (experience of dyspnea and functional limitation) and a total score. Objects: This study aimed to assess the reliability and validity of the Korean version of the FACIT-dyspnea 10-item short form questionnaire (FACIT-dyspnea-K). Methods: Subjects were 163 patients with cancer. Dyspnea-related scales (modified Medical Research Council scale [mMRC], European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-Core 30 [EORTC QLQ-C30], Hospital Anxiety and Depression [HAD], and WHO Performance Scale) were used to validate the FACIT-dyspnea-K. Results: Internal consistency was confirmed by Cronbach's alpha values of 0.90 and 0.95 in factors 1 and 2, respectively. Convergence validity was determined by comparing the two factors and total score of the FACIT-dyspnea-K with conceptually related assessment tools measuring the physical and emotional effects of dyspnea, with which correlations ranged from 0.364 to 0.567. Criterion validity was established by significant differences in the FACIT-dyspnea-K score between groups when the patients were classified by performance status as assessed by the WHO performance scale. Furthermore, the FACIT-dyspnea-K showed notable correlations with other dyspnea scales (mMRC, EORTC QLQ-C30, and HAD) for cancer patients (r = 0.28 to 0.54). The test-retest reliability of the two factors and total score of the FACIT-dyspnea-K appeared to be excellent (Cronbach's alpha = 0.96 to 0.97). Conclusion: This study supports FACIT-dyspnea-K as a valid and reliable instrument to assess the dyspnea experience of cancer patients in clinical settings.

Predictive validity of the gait scale in the Performance Oriented Mobility Assessment for stroke survivors: a retrospective cohort study

  • Objective: The present study was to investigate the discrimination capacity of the Performance Oriented Mobility Assessment-Gait Scale (POMA-GS), for predicting falls in stroke survivors. Design: Retrospective cohort study. Methods: Data including the characteristics and number of falls of 52 chronic stroke patients from a rehabilitation center were collected. The number of falls each subject had experienced in the previous year were investigated through interviews. The subjects were divided into two groups depending on the number of falls: if falls occurred twice or more on the basis of the time of study after stroke, they were defined as the falls group and if there was no fall experience or one fall, they were defined as the non-falls group. The subjects were examined with the POMA-GS, and physical functions were examined using by the One Leg Stand Test (OLST), Sit to Stand Test (SST), 10-m Walk Test, Lower Extremity in Fugl-Meyer assessment (FM-LE), and Trunk Impairment Scale (TIS). The validity of POMA-GS for falls prediction was analyzed. Results: In the POMA-GS, which predicts falls in stroke survivors, the cut-off value was 8.5 (sensitivity 72%; specificity 65%) and the area under the curve was 0.75 (95% confidence interval: 0.60-0.90, p<0.007). There was a significant difference in the OLST, SST, FM-LE, and TIS between the subjects with POMA-GS >8.5 and the subjects with POMA-GS ${\leq}8.5$. Conclusions: The POMA-GS could be a useful tool in predicting falls in stroke survivors, as its discrimination capacity and predictive validity is proven satisfactory.

전통적 균형훈련이 노인의 균형능력에 미치는 영향 (Effect of Traditional Balance Training on Balance in Older Adults)

  • Purpose : This study was to evaluate the effect of traditional balance training on balance in older adults. Methods : The subjects of this study were thirty elderly over 65 years old. Thirty subjects ranging aged from 66 to 85($74.0{\pm}5.83$) completed the study and participated three times a week for 5weeks. Subjects were assessed by utilizing two different balance measure : Static standing balance was measured by balance performance monitor(BPM). Dynamic balance was measured by timed up and go test(TUG). The scale for static standing balance was measured by using frequence(Hz), sway area($mm^2$), sway path(mm), max sway velocity(mm/s), ant/post sway angle($^{\circ}$), left/right sway angle($^{\circ}$), and sway number. Results : The change in frequency were statistically significant on pre-test and post-test(P<.05), 2. The change in sway area were statistically significant on pre-test and post-test (P<.05), 3. The change in sway path were statistically significant on pre-test and post-test(P<.05), 4. The change in max sway velocity were statistically significant on pre-test and post-test (P<.05), 5. The change in ant/post sway angle were statistically significant on pre-test and post-test (P<.05), 6. The change in left/right sway angle were statistically significant on pre-test and post-test(P<.05), 7. The change in sway number were statistically significant on pre-test and post-test(P<.05), 8. The score on timed up and go test shows statistically significant increase on pre-test and post-test (P<.05). Conclusion : This study suggest that traditional balance training have an effect on balance performance ability for elderly people. Therefore, the traditional balance training is recommended for older adults to improve balance performance ability.

안구운동이 노인의 정적균형과 낙상효능감에 미치는 영향 (The Effects of Eye Movement Training on the Static Balance and Fall Efficacy in the Elderly)

  • Purpose: This study is to identify how eye movement influences the static balance and fall efficacy of the elderly who have experienced fall-related injuries. Methods: Thirty nine elderly who scored 24 points in the Korean mini mental state examination, were able to walk, and had no specific disease, were selected as the research subjects among elderly who have experienced fall-related injury more than once in the past year. Thus, 20 for the experimental group, and 19 for the control group were selected as subjects. The experiment was conducted for 6 weeks, including periods of evaluation before and after intervention. Results: The results with respect to the dependent variables are as follows: Substitute sample t-testing showed significant differences between each group in eye movement to check difference in balance performance and fall efficacy. When independent sample t-tests were conducted to compare static balancing performance and fall efficacy between the two groups after intervention, they showed significant differences in statistical terms (p<0.05). Conclusion: From the above results of the study, it was found that the application of eye movement combined with diverse fall prevention programs is effective, when enhancing static balance performance power and improving fall efficacy.
