Background: Promoting patients' safe return home at discharge and reducing length of stay in hospital is key for Restorative Rehabilitation Institution (RMI). Objects: This study was designed to identify the factors influencing the return to home and length of stay among various factors. Methods: A total of 120 stroke patients (76 males and 44 females) who were hospitalized in an adult inpatient unit of a RMI for more than 2 months were retrospectively analyzed for this study (multivariate logistic regression analyses, p < 0.001). As predictor variables for assessing the return to home and length of stay, demographic data (sex, age, duration between onset and admission, length of stay, caregiver after discharge, occupation after discharge, reason for discharge, and household type after discharge) were collected. Additionally, following measurements were selectively collected from patient's medical records: scores of Mini-Mental State Examination Korean version (K-MMSE), modified Barthel Index Korean version (K-MBI), Berg Balance Scale and Functional Ambulation Category were obtained at admission and discharge. Results: The K-MMSE at admission and K-MBI at discharge were found to be the predictors of return to home. Additionally, K-MBI at admission influenced the length of stay. Conclusion: This study suggests cognitive functioning at admission and the level of activities of daily living at discharge predicted the return to home and length of stay.
PURPOSE: The purpose of this study was to identify whether backward walking exercise was more effective than conventional physical therapy for balance and gait in hemiplegic stroke patients. METHODS: Eighteen patients with chronic stroke were randomly assigned to the experimental (n=9) or control (n=9) group. The experimental and control group performed backward walking exercise and conventional physical therapy, respectively, for 8 weeks. Stability Index (SI) and Weight Distribution Index (WDI) during standing were assessed using the Tetrax Balance System. The Timed Up and Go (TUG) test and Korean version of the Berg Balance Scale (K-BBS) were used to evaluate balance and fall risk. Walking speed, stride length, and step length on the affected side were measured using the 10-Meter Walk and ink foot printing tests. Wilcoxon signed-rank and Mann-Whitney U tests were used for within- and between- group comparisons, respectively. RESULTS: The experimental group showed significantly higher changes in SI (p<.01), WDI (p<.01), TUG (p<.001), and BBS score (p<.001) following intervention compared with the control group. The experimental group also showed significantly greater improvements in walking speed (p<.01), stride length (p<.001), and step length on the affected side (p<.001) after intervention compared with the control group. CONCLUSION: Backward walking exercise is an effective intervention to improve balance and gait in hemiplegic stroke patients.
Purpose: The purpose of this study was to investigate the effects of three-dimensional virtual reality horse riding simulator training using a head-mounted display on gait and balance in children with cerebral palsy. Methods: Ten children with cerebral palsy were randomly assigned to the horse riding simulator (HRS) group (n=5) or the horse riding simulator with virtual reality (HRSVR) group (n=5). To evaluate balance, center of gravity (COG) sway velocity and total sway distance of each group were assessed using the Wii balance board, and gait speed and stride length of each group were assessed using a gait analysis system. Results: Intra-group comparisons between pre- and post-intervention measures revealed that there were significant changes in all gait and balance variables such as stride length, gait velocity, COG sway velocity and COG sway distance in the HRSVR group (p<0.05). In the HRS group, there were significant changes in all variables except stride length (p<0.05). In addition, inter-group comparisons showed significant differences between the two groups in stride length, gait velocity and COG sway distance except COG sway velocity (p<0.05). Conclusion: The findings of this study suggest that horse riding simulator training combined with 3D virtual reality can be a new positive therapeutic approach for improving functional performance in children with cerebral palsy.
This study investigated the effects of axillary crutch length on trunk muscle activity and lumbo-pelvic-hip complex movements during crutch gait. Eleven healthy men participated in this study. The participants performed a three-point gait with optimal, shorter, and longer crutch lengths. Weight-bearing (WB) side was determined as the dominant leg side. The electromyography (EMG) activity of the bilateral rectus abdominis (RA) and erector spinae (ES) muscles and lumbo-pelvic-hip complex movements were monitored using a three-dimensional motion system with wireless surface EMG. Differences in the EMG activity of RA and ES muscles and range of motion (ROM) of lumbar spine, pelvis, and hip among conditions were analyzed using one-way repeated-measures analysis of variance, and a Bonferroni correction was conducted. There was less RA muscle activity on the WB side under the optimal crutch length condition compared with shorter and longer crutch length conditions (p<.05). The EMG activity of the RA muscle on the non-WB side and ES muscle on the WB side were significantly decreased under the optimal crutch length condition compared with shorter crutch length condition (p<.05). No significant differences in the EMG activity of the ES muscle on the non-WB side and ROM of lumbo-pelvic-hip complex were found among conditions (p>.05). These findings indicate that the optimal crutch length improves the trunk muscle efficiency during crutch gait.
Purpose: The purpose of this study was to investigate the correlations between functional movement screen (FMS) and ankle dysfunctions in subjects with chronic ankle instability (CAI). Methods: This study was a cross-sectional study of 20 participants with CAI. The ankle dorsiflexion range of motion (ROM), Foot and Ankle Disability Index (FADI), center of pressure (COP) path length, and COP velocity for ankle dysfunction were measured in all the subjects. All the subjects underwent the FMS concerned with ankle functions consisted of deep squats, hurdle steps and in-line lunges. The Spearman rank-order correlation coefficient was used to determine relationship between the ankle ROM, FADI, COP and FMS. Results: The results of the deep squat and in-line lunge exercises revealed a significant correlation with the ankle dorsiflexion ROM, FADI, COP path length, and COP velocity. The hurdle step showed no correlation with the ankle dorsiflexion ROM and FADI but a significant relationship with the COP path length and COP velocity. Conclusion: The results of this study showed that relationship deep squat and in-line lunge and it is suggested that an assessment tool using ankle dorsiflexion ROM and ankle instability would be clinically effective.
The purpose of this study was to examine the effects of the pelvic tilting exercise, pelvic tilting exercises with floor walking training, pelvic tilting exercises with treadmill walking training on quantitative gait function in patients with hemiplegia. Thirty patients with hemiplegia due to cerebrovascular disease participated in this study. Subjects were randomly assigned to one of pelvic tilting exercise group, pelvic tilting exercise with floor walking training group and pelvic tilting exercises with treadmill walking training group. The effects of each therapeutic method were evaluated by measurements of gait velocity, cadence, stride length, step length, base of support and foot angle using ink-foot prints. Data were analyzed statistically using paired t-test and one-way ANOVA. The results of this research are as followings : 1. After treatment, it turned out that pelvic tilting exercises with treadmill walking training has the most effect on gait velocity, cadence, stride length, step length and foot angle, which has significant difference in statistics (p<0.05). 2. Quantification of the gait velocity, cadence, both stride length and step length demonstrated a significant increase (p<0.05) after treatment in all groups when compared with values measured before treatment. The base of support and foot angle in affected side decreased significantly (p<0.05) after treatment in all groups when compared with values measured before treatment. 3. The lumbosacral angle noted a significant increase (p<0.05) after treatment in all groups, however, an ANOVA analysis did not reveal any differences between groups.
PURPOSE: The purpose of this study was to examine the effects of sciatic nerve mobilization techniques on straight leg raising (SLR) and walking ability in elderly women aged over 65. METHODS: Seventy women aged over 65 were examined using SLR test and forty women were selected as subjects. They were divided into Group I (under $70^{\circ}$ of SLR test of both legs, n=20) and Group II (over $70^{\circ}$ of SLR test of both legs, n=20). Nerve mobilization was applied to both groups (three times a week for 4 weeks). SLR angle was measured using digital goniometer and walking ability was measured by step length, stride length, velocity, double support, using GAITRite System. After 4 weeks, paired t-test was used to compare the changes of SLR test and walking ability within Group I and Group II. RESULTS: In Group I, SLR test, step length, stride length and gait velocity were significantly increased but double support was significantly difference. In Group II, SLR test, step length and stride length were significantly increased but gait velocity was not significantly increased and double support was significantly decreased. CONCLUSION: This study showed that sciatic nerve mobilization techniques had positive effects on the SLR angle and walking ability in elderly women.
Objective: Children with spastic diplegic cerebral palsy show weakness especially in the lower-extremity rather than upper-extremity muscles and display characteristics such as asymmetric alignment, deficits in postural control or balance ability, and slow walking speed. Various therapeutic interventions are applied to children with cerebral palsy, of which taping is widely used in the field of rehabilitation, however, there are few studies of the effects of kinesio taping on gait patterns of children with cerebral palsy. The present study investigated the effects of kinesio taping on gait parameters of children with cerebral palsy. Design: Cross-sectional study. Methods: Four children with spastic diplegia participated in this study. The participants' gait parameters while walking 10 m with and without kinesio taping (tibialis anterior, quadriceps femoris, and gluteus maximus) were recorded. Gait parameters including gait velocity, cadence, step length, stride length, single support time, and double support time were evaluated using the GAITRite. Mean values were obtained after having the subjects walk three times in each condition with a 5-minute rest period between each condition. The order of each condition was assigned randomly. Results: There were significant improvements in gait velocity, step length, stride length, and single support time of the right leg with kinesio taping condition compared to the without kinesio taping condition taping (p<0.05). However, there were no significant differences in cadence, single support time of the left leg, or double support time. Conclusions: The results show that kinesio taping may have a positive effect for improving gait parameters of children with spastic diplegia. However, its usefulness in the rehabilitation of children with cerebral palsy needs to be further investigated.

Objective: Hallux valgus (HV) is a common musculoskeletal deformity that is accompanied with pain and continues to decrease one's quality of life and ability to perform daily life activities by affecting gait and static stability. Therefore, this study aimed to investigate the effect of the angle of HV (HVA) and to compare the one-legged stance and gait parameters in young adults with less HV and severe HV. Design: Cross-sectional study. Methods: Forty young adults were divided into two groups, where HVA ≥15° (n=20) was defined as HV, and HVA <15° (n=20) was defined as normal. For balance ability, the center of pressure (COP) path, velocity, length of axis of the COP path, deviation of the x-axis and y-axis, and percentage of foot pressure were measured, and gait, the foot rotation angle, step length, percentage of each phase of the gait cycle, time change from the heel to forefoot, and maximum pressure of the forefoot and midfoot were measured. Results: Significant differences were found in sway length and time change from heel to forefoot during walking between the normal and HV groups (p<0.05). Most parameters were not associated with the HVA, but parameters such as length of axis and time to change from heel to forefoot were significantly associated with the HVA (p<0.05). Conclusions: These results suggest that most one-legged stance and gait parameters were not significantly affected by the HVA in young adults; therefore, future studies are needed in order to address other dynamic parameters and other methods of gait analysis for detecting clinically meaningful conditions.
