뇌졸중환자에서 재원기간과 퇴원장소 예측을 위한 K-MBI의 유용성 (Utility of Korean Modified Barthel Index (K-MBI) to Predict the Length of Hospital Stay and the Discharge Destinations in People With Stroke)

  • The purpose of this study was to utilize the K-MBI (Korean Modified Barthel Index) and subscales of K-MBI in predicting the length of hospital stay (LOS) and the discharge destinations for stroke patients. The study population consisted of 97 stroke patients (57 men and 40 women) admitted to the Seoul National University at the Bundang Hospital. All participants were assessed by K-MBI at admission and discharge after rehabilitation therapy and the information available was investigated at admission. The data were analyzed by using the Mann-Whitney U test, the stepwise multiple regression and the logistic regression. The median LOS was 30 days (mean, 32.8 days; range, 22 to 43 days). The K-MBI score at initiation of rehabilitation therapy (p<.001), the type of stroke and living habits before a stroke were the main explanatory indicators for LOS (p<.05). Within the parameters of K-MBI measured at initiation for rehabilitation, feeding and chair/bed transfer were the explanatory factors for LOS prediction (p<.01). Confidence in the prediction of LOS was 20%. Significant predictors of discharge destination in a logistic regression model were the discharge K-MBI score, sex and hemiplegic side. Dressing in items of discharge K-MBI was the significant predictor of discharge destination. The K-MBI score was the most important factor to predict LOS and discharge destination. Knowledge of these predictors can contribute to more appropriate treatment and discharge planning.

GPU를 이용한 기타의 음 합성을 위한 효과적인 병렬 구현 (An Effective Parallel Implementation of Sound Synthesis of Guitar using GPU)

  • 본 논문에서는GPU 환경에서 기타의 음합성을 위한 물리적 모델링의 효율적인 병렬구현 방법을 제안한다. 물리적 모델링을 이용하여 기타의 개방현(E2, A2, D3, G4, B3, E4)들의 기본음을 합성하기 위해 각 개방현 음 합성을 위한 적절한 필터 계수를 사용하였고, 지연 라인의 길이를 조절하였다. 또한 물리적 모델링 알고리즘을 분석한 결과 지연 라인의 길이만큼 병렬성을 갖는 것을 확인하였다. 따라서 각 개방현의 기타 음을 합성하기 위해 지연 라인의 길이만큼CUDA 코어를 할당한 후 최적의 성능을 보이도록 알고리즘을 병렬 구현하였다. 모의실험결과, GPU를 이용하여 합성한 기타 음과 원음과의 스펙트럼이 매우 유사하였고, GPU는 기존 고성능 TI DSP보다 68배, CPU보다 3배의 성능 향상을 보였다. 또한, 본 논문에서는 물리적 모델링 알고리즘을 멀티 GPU시스템에서도 구현하고 성능을 분석하였다.

발목관절 근력훈련과 균형조절 훈련이 발목전략을 이용한 균형반응에 미치는 영향 (The Effect of Adjusted Balance Training and Muscle Training on Balance Using Ankle Strategy)

  • Purpose: The purpose of this study was to investigate the effect of balance using ankle strategy during adjusted balance training and muscle training in normal young female adults. Methods: There were eighteen participants. All subjects were randomly assigned to three groups, the control group (n=6), the adjusted balance training group (n=6) and the muscle training group (n=6). Subjects were assessed for each subject by a pretest and a posttest at 1 week, 2 weeks, and 3 weeks, measuring the anterior-posterior limit of stability, trace length and EMG data for the tibialis anterior and lateral gastrocnemius. Results: The anterior-posterior limit of stability significantly increased in the training group (p<0.05). The trace length significantly decreased in the training group (p<0.05). The muscle activity of the tibialis anterior and lateral gastrocnemius also significantly decreased in the training group (p<0.05). Conclusion: We confirmed that both the balance training group and the muscle training group significantly increased the anterior-posterior limit of stability and decreased trace length and muscle activity. Further studies are needed to analyze the long-term effects on subjects resulting from these changes.

아기마사지가 미숙아의 신체성장과 스트레스 반응에 미치는 영향 (Effects of Infant Massage on Physical Growth and Stress Response in Preterm Babies)

  • Purpose: This study was done to investigate the effects of an infant massage on physical growth and stress response in preterm babies. Method: A nonequivalent control group with pre-posttest design was used with 56 preterm babies in the NICU of E medical center at Daejeon. Data were collected from July 2004 to May 2005. The intervention was given for 15 minutes, once a day for 7 days. Physical growth was measured by weight, length, head circumference, and stress response was measured by serum cortisol level. Mean, %, paired-test, t-test with the SPSS/W in 16.0 program were used to analyze the data. Results: After the intervention, physical growth variables (weight, length, head circumference) in experimental group were higher than that of the control group. Also, the serum cortisol level in the experimental group was lower than that of the control group. But none of these results were not statistically significant. Conclusion: The results of this study suggest that a 7-day intervention period may not be long enough to confirm the effects of infant massage on physical growth and stress reaction. Therefore it is suggested that a longer period of infant massage should be tested to determine if it is effective in improving the physical growth and stress reduction in preterm babies.

Immediate Effect of Eye Movement on Static Balance according to Age in Elderly Individuals

  • Purpose: This study investigated changes in static balance during eye movement while in an upright standing position and compared static balance in elderly individuals according to age. Methods: A total of 154 elderly persons (male 23, female 136) were enrolled in the study. Participants followed an eye movement program that consisted of randomly occurring pursuit eye movement and saccadic eye movement. Participants were asked to remain in the double-leg standing position for 2 minutes 30 second while fixating their eyes on a specific target, after which they were instructed to perform eye movements for 2 minutes and 30 seconds. Static balance was measured in terms of the area, length, and average speed (cm/s) of the center of pressure (COP) displacement before and after eye movement intervention with the eyes open. Results: The area, length, and velocity of COP displacement improved significantly (p< 0.01) after intervention. No significant differences among age groups were observed. Conclusion: The results indicated that eye movement was an effective intervention for improving static balance in elderly persons.

뇌졸중 환자의 체중지지 비대칭과 보행 대칭성의 관련성 (The Relation between asymmetric weight-supporting and gait symmetry in patients with stroke)

  • Purpose : The aim of this study was to investigate the relationship between weight-supporting asymmetry and gait symmetry in patients with stroke. Methods : Sixty two stroke patients with hemiplegia stood quietly with eye opens on a force platform to calculate weight-supporting asymmetry from vertical reaction force. The GAITRite was used to evaluate their gait parameters. The data were analyzed using Pearson correlation. Results : The results of this study was showed that the medio-lateral index (ML) was correlated with symmetry rate (SR), symmetry index (SI), and Gait asymmetry (GA) of step time and length but stronger correlation with spatial gait symmetry than temporal symmetry. In gait symmetry, step length has stronger correlation with weight-supporting asymmetry than step time. Conclusions : The results of this study shows weight-supporting asymmetry was correlated with more spatial gait symmetry than temporal symmetry.

Effects of Motor Imagery Training and Balance Training on Static Balance: A Quasi-Experimental Study

  • Background: Although studies on physical motor learning through motor imagery training have been conducted in various fields, studies on its effectiveness are still considered insufficient. Objective: To investigate the effect of motor imagery training and balance training on static balance of asymptomatic adults in their 20s. Design: A quasi-experimental study. Methods: Thirty-six adults in their 20s who passed the tandem stance test were randomized to the motor imagery training group (MIG, n=12), motor imagery with balance training group (MIBG, n=12), and balance training group (BG, n=12). Each group underwent their respective interventions three times a week for four weeks, and changes in static balance were analyzed using multivariate analysis of variance. Results: Trace length was significantly lower in the MIBG than in the MIG and BG (P<.05), and a significant reduction in trace length in the MIBG was observed after the intervention as compared to the baseline (P<.05). Furthermore, a significantly lower velocity was observed in the MIBG than in the MIG and BG (P<.05), and a significant reduction of velocity in the MIBG was more observed after the intervention compared to the baseline (P<.05). Conclusion: These results suggest that motor imagery training enhance static balance in healthy college students.

하지근력강화운동이 뇌성마비 아동의 균형과 보행에 미치는 영향 (The Effects of The Lower Extremity Muscle Strengthening Exercise on Walking and Balance of Children with Cerebral Palsy)

  • Purpose: The aim of this study was to ascertain the effects of the lower extremity muscle strengthening exercise on balance and ambulation of children with cerebral palsy. Methods: 10 subjects who participated in this research undertook the 12-week the lower extremity muscle strengthening exercise program, which consisted of a series of mat exercises and sling exercises. The statistical significances were examined by using Wilcoxon signed-rank test, a non-parametric test, for evaluating the improvement of balance and ambulation of the subjects. In order to evaluate the correlation among the variables, Pearson's correlation coefficients were also calculated. In all statistical analyses the significance level was selected as ${\alpha}$=0.05. Results: Berg balance scale(BBS) was significantly increased after the intervention(p<.05). Percentage weight bearing(PWB) was decreased after the intervention, but there was no significant difference. Time up and go test(TUG) value was significantly decreased(p<.05). Gait velocities was increased after the intervention, but there was no significance. Stride length, step lengths of the affected side and the sound side were significantly increased after the intervention(p<.05). In the correlation analyses of the measures before the intervention, TUG had significant negative correlation to BBS and gait velocity(p<.05). Stride length, step lengths of the sound side and the affected side had significant positive correlation between themselves(p<.01). In the correlation analyses of the measures after the intervention, TUG had significant negative correlation to BBS and gait velocity(p<.05). BBS revealed significant positive correlations to stride length, step lengths of the sound side and the affected side(p<.05). Stride length, step lengths of the sound side and the affected side had significant positive correlation between themselves(p<.01). Conclusion: Based upon the outcomes as above, it is likely that the muscle strength exercises have substantial effects on balance and ambulation of children with cerebral palsy. Thus various lower extremity muscle strengthening exercise programs are required to be studied and developed in order to contribute to functional improvements of children with cerebral palsy.

The effect of visual information on gait parameters with induced ankle muscle fatigue

  • Objective: The purpose of this study was to investigate the effect of visual block with ankle joint fatigue on gait and dynamic balance ability. Design: Cross-sectional study. Methods: Thirty healthy young adults (men=15, women=15) between 22 to 25 years of age voluntarily participated in this study. All subjects performed the gait and dynamic balance test successively in two conditions: the visual block and the open eyes condition. Before the gait and dynamic balance test, muscle fatigue on the ankle joint was induced to all subjects by doing ankle dorsiflexion and plantarflexion alternately, and then gait parameters (step length, stride length, cadence, velocity, single limb support, and double limb support) were assessed by walking on the GAITRite system (CIR Systems Inc., USA). Subjects also performed the functional reach test (FRT) for assessment of dynamic balance. This study examined gait parameters and FRT scores in each visual block and open eyes condition. Results: The results showed that FRT scores with the visual block condition significantly decreased compared to without visual blocking (p<0.01). Step length, stride length, cadence, and velocity of gait parameters decreased significantly in the visual block condition (p<0.01) while there was no significant difference for single limb support. However, double limb support increased significantly in the visual block system (p<0.01). Conclusions: Therefore, blocking of visual information with muscle fatigue of the ankle joint can affect gait and balance ability of young adults and increase the risk of falls.

The Effect of Hinged Ankle-Foot Orthosis on Walking Function in Children With Spastic Diplegic Cerebral Palsy: A Cross-Sectional Pilot Study

  • Purpose: The aim of the current study was to examine the effects of hinged ankle-foot orthosis (HAFO) on walking function in children with spastic diplegic cerebral palsy (CP). Methods: Thirty-two children (mean age: $6.79{\pm}0.35years$, age range: 5-7 years) who were diagnosed with spastic diplegic cerebral palsy participated in the study. Each subject typically walked through 10 meters of a gait platform with markers on the subject's proper body segments and underwent 3-D motion analysis system with and without hinged ankle-foot orthosis. The HAFOs were all custom-made for individual CP children and had plantarflexion stop at $0^{\circ}C$ with no dorsiflexion stop. The interventions were conducted over three trials in each group, and measurements were performed on each subject by one examiner in three trials. 3-D motion analysis system was used to measure gait parameters such as walking velocity, cadence, step-length, step-width, stride-length, and double support period in two conditions. Results: The walking velocity, cadence, step-length, and stride-length were significantly greater for the HAFO condition as compared to the no HAFO condition (p<0.05). However, no significant difference in step-width and double support period was observed between two conditions. Conclusion: These findings suggest that using the HAFO during walking would suggest positive evidence for improving the spatiotemporal parameters of gait in children with spastic diplegic cerebral palsy.
