Fatigue and Associated Factors among Airline Pilots

  • Purpose: This study aimed to identify the prevalence of fatigue and its associated factors leading to the fatigue among Korean commercial airline pilots. Methods: An anonymous, web-based questionnaire collecting data on sleep related characteristics was completed by airline pilots. The final 1,029 samples of completed questionnaires were analyzed. The association of the risk factors with fatigue was determined using logistic regression analysis. Results: The prevalence of fatigue was 60% (Fatigue Severity Scale index≥3.2), disturbed sleep 28% (Pittsburgh Sleep Quality Index≥9), daytime sleepiness 17% (Epworth Sleepiness Scale≥10), and sleep apnea (Berlin Questionnaire≥2) 11%. The fully adjusted logistic regression showed that quality of life (odds ratio [OR]=0.16, confidence interval [CI]=0.11-0.24), daytime sleepiness (OR=1.26, CI=1.18-1.34), sleep quality (OR=1.44, CI=1.29-1.61), mental workload (OR=1.10, CI=1.05-1.14), vigorous physical activity (OR=0.82, CI=0.72-0.92), late starts (OR=1.25, CI=1.06-1.46) and average weekly flying hours (OR=1.02, CI=1.00-1.03) were associated with higher levels of fatigue. Conclusion: Lower quality of life, disturbed sleep, more subjective sleepiness, shorter sleep duration, higher mental workload, less vigorous physical activity, frequent late starts, and longer flying hour were shown to be risk factors for fatigue in airline pilots. These findings should be taken into account in the development of sleep and fatigue countermeasures for airline pilots.

  • This paper examines the effects on mental fatigue of changes in the intensity of physical activity. A treadmill-equipped instrument and perception tester were used to attain several levels of physical activity In this paper, In order to determine the individual levels of physical activity of subjects, Borg-RPE scale, Heart Rate(HR) and Respiratory Quotient(RQ) were used. Also, an addition test in whole-body activity on treadmill-equipped instrument as an indicator of mental fatigue were performed. In the above experiments, the scores obtained in addition test administered before and after physical activity at each intensity level used. Restricted within the limits of this paper, the results of these tests showed that mental fatigue decreased after physical activity.

Effects of Induced Microcurrent Shoes on Fatigue and Pain in Painful Foot to Patients with Plantar Fascitis (족저근막염 환자의 미세전류발생 신발 착용이 족부 근피로 및 통증 완화에 미치는 영향)

  • Purpose: This study was to investigate how induced microcurrent shoes affect relief of feet pain, which often occurs to persons who are more than fifty years old and how they prevent fatigue of feet generated during everyday life. The subjects of this study were comprised af five males and five females, who agreed with this research and ate more than fifty years old. And they all have plantar faciitis and pain in their feet. Methods: Subjects ware the induced microcurrent shoes far more than 4 hours everyday during the test. When they wore those shoes, they also wore the specially produced shacks made of silver-mixed thread and they were asked to avoid serious exercise. Prior to starting test and after the test, assessments were carried out two times. In the examination before the test, general shoes were used and in examination after the test, induced microcurrent shoes used. Muscle fatigue was measured by electromyography(MP150, Biopac, USA). When the walking rate of subjects on a treadmill was increased from 2.Km/h to 3Km/h for 10 minutes, muscle activities of a lower leg were measured during the whale walking process by MP150 remote monitoring system(TEL 100C). And pain was measured by VAS. Results: The results are as follows. 1. Comparisons of VAS before and after experimentation showed that degree of pain was significantly decreased(p < .05). 2. Degree of fatigue of Tibialis anterior was significantly decreased after 6 weeks of treatment(p < .05). 3. Degree of fatigue of Soleus was slightly decreased after 6 weeks of treatment, but did not show significant difference (p > .05). Conclusion: The above results suggest that induced microcurrent shoes is helpful decreasing fatigue of feet and relief of feet pain.

The Effect of a 12-week Combined-Exercise Program on Physical Fitness and Fatigue for Cancer Survivors (12주간 복합운동프로그램이 암 생존자의 신체기능과 피로도에 미치는 영향)

  • Purpose: The purpose of this study was to analyze the effects of a combined exercise program on the level of physical fitness and fatigue for cancer survivors. Methods: The subjects were all of 61 persons consisted of prostate cancer(n=22), breast cancer (n=20), and stomach cancer(n=19) survivors. The exercise program was composed of a wall exercise, a chair exercise, a towel exercise and a bottle exercise which lasted 50-60 minutes each, two times a week for 12 weeks. The outcome measures were the fat mass, the muscle mass, the grip strength, the sit and reach, the 6 minutes walk, & fatigue. Results: At the completion of the 12-week combined exercise program, there were statistically significant differences in the grip strength, the sit & reach, the 6 minutes walk, & fatigue for the prostate, breast, and stomach cancer survivors. But no significant differences were found in the muscle mass between the pretest and posttest. Conclusion: As the 12-week combined exercise program was proved to be an effective intervention for improving muscle strength, flexibility, cardiopulmonary fitness & fatigue, it is thought that the program can be utilized as a rehabilitation program for cancer survivors.

Characteristics of Median Frequency According to the Load During Fatiguing Isometric Exercise (등척성 운동시 운동강도에 따른 중앙주파수의 특성)

  • Median frequency can be regarded as a valid indicator of local muscle fatigue. As local muscle fatigue develops, the muscle fiber conduction velocity decreases, the fast twitch fibers are recruited less, and consequently the median frequency shifts toward the lower frequency area. The aim of this study was to test the characteristics of the median frequency according to exercise load (30% and 60% of MVC on the biceps brachii, 40% and 80% of MVC on the vastus lateralis) during the fatiguing isometric exercise. Thirteen healthy male volunteer students of Yonsei University were recruited. After the testing maximal voluntary isometric contraction, three variables (initial median frequency, regression slope, fatigue index) from the regression line of MDF data were measured in each exercise load. The results showed that the regression slope and fatigue index were significantly different for the biceps brachii, but not for the vastus lateralis initial MDF was not significant difference according to the exercise load on both muscles. The regression slope and fatigue index could monitor physiologic muscle change during fatiguing isometric exercise. The results showed that two MDF variables reflect the local muscle fatigue according to the exercise load.

  • Background: Chronic neck pain (CNP) is associated with weakness in the deep neck flexor muscles, a shortening of the neck extensors, and a reduction in endurance. In addition, muscle imbalance can lead to neck pain and musculoskeletal dysfunction. This study compared neck extensor muscle fatigue, muscle strength, and muscle endurance time between patients with CNP and healthy adults during isometric neck extension. Methods: Thirty participants (15 patients with CNP and 15 healthy adults) were recruited in this research. The outcome measures included splenius capitis (SC) muscle fatigue, isometric neck extensor strength, and muscle endurance. The independent T-test was used to compare the continuous dependent variables between the CNP group and the healthy group. Results: The independent T-test indicated that muscle fatigue in the left SC differed significantly between the CNP group and the healthy group. A significant difference was also noted in the isometric neck extensor and neck extensor strength between the groups. Conclusion: Our results provided promising clinical evidence that patients with CLP have reduced neck extensor strength and endurance and increased SC muscle fatigue, which results in neck pain.

Clinical Investigation and Follow-up Monitoring of Fatigue Patients Visiting the Oriental Medical Hospital (한방병원 보양클리닉에 내원하는 피로환자의 임상적 분석 및 치료경과 관찰)

  • Objectives : Fatigue is a common symptom experienced by everyone. Nevertheless, clinicians have a tendency of ignoring it since fatigue itself is not considered a distinct disease. Actually, some limited research about chronic fatigue syndrome has been made within the country, but in reality, the probability of getting this syndrome is still considered very low due to the strict diagnosis standard. Therefore, there are tremendous numbers of patients who do not get enough attention from clinicians for their fatigue symptoms only because technically they do not belong to the syndrome. Therefore, a basic statistical database must be compiled and patient management programs must be developed. To accomplish this, we conducted this study by measuring degree of fatigue, clinical characteristics and processes of Oriental medical treatment of fatigue patients. Methods : The objects of this study were selected from the new patients who entered the tonification Clinic in Kynnghee Oriental Medical Center between August 11, 2000 and October 7, 2000. Their main complaint was fatigue and they did not suffer from any physical or mental problem either historically or at the time of the study. The objects were divided into two groups based on duration of fatigue; fatigue under 6 months is considered as acute fatigue and fatigue for longer than 6 months is chronic fatigue. The prepared survey sheet for measuring fatigue degree was distributed to the patients with their consent. The patients were divided again into three subgroups : the fIrst group went through 1st test and constitution test after tonification clinic; the second one went into constitution test skipping Ist measuring test; the third one went into only tonification clinic with neither 1st measuring test nor constitution test. Results : The total number of object patients was 47 and 80% of them were considered as 'fatigue patients' by the Chalder scale. Among all patients, 29.5% requested treatment for chronic fatigue, which is over 6 months. The average of scale II for all patients was 14.8, which indicates moderate fatigue. The averages of scale II-1, II-2, II-3 were respectively 7.5, 5.9, and 3.7 so the most common complaint was physical fatigue. When compared scale II based on occupations, student group scored 6.9 and office man group scored 8.5 in scale II-1, physical fatigue, but it was not significant. Conclusions : Numerous number of patients have come to Oriental medical centers or hospitals in Korea. Therefore, deeper statistical research and follow-up-monitoring are reqnired in the Oriental medical academic world. In this study, among all patients who entered the tonification Clinic in Kyunghee Oriental Medical Center, 29.5% requested treatment for chronic fatigue, which is over 6 months. This kind of statistical report is the first time trial in the Oriental medical academy world. Through these steps, more objective treatment can be made and standards of prognosis assessment can be established.tablished.

  • The purpose of this study is to find the relation between the detailed categories of the self-efficacy of workers in the field of railroad signaling and the detailed categories of their fatigue in the use of analysis of moment structure(AMOS) model. A self-administered questionnaire survey of workers in the field of railroad signaling had been conducted and a total of 341 copies were analyzed. This study had the assumption that their general self-efficacy and social self-efficacy would negatively influence their physical fatigue and mental fatigue. As a result, their general self-efficacy negatively influenced their physical fatigue and mental fatigue, whereas their social self-efficacy didn't influence their physical fatigue and mental fatigue. Therefore, it is possible to reduce the fatigue of the study subjects by enhancing corporate support to increase their general self-efficacy.

The Study on Respiratory Function, Spirometric Lung Pattern and Fatigue of Elderly in a Facility

  • The purpose of this study was to investigate spirometric lung pattern, respiratory function and degree of fatigue by lung function tests and fatigue tests of 39 elderly people in a care facility aged 65 and over. The respiratory function tests were used to the Spirovit SP-1 and fatigue tests were used modified Piper fatigue scale(mPFS). Regarding the respiratory function, the FVC was $1.41{\pm}0.36$l, the FVC % predicted was $69.10{\pm}14.98$%, the $FEV_1$ was $1.02{\pm}0.31$l, the $FEV_1$ % predicted was $63.27{\pm}16.05$%, the $FEV_1$/FVC was $72.77{\pm}13.40$%, and the fatigue score was $5.83{\pm}1.09$. As for the spirometric lung pattern, 19 patients had a restrictive pattern(48.7%), followed by 11 with a mixed pattern(28.2%), 5 with an obstructive pattern( 12.8%), and 4 with a normal pattern(10.3%). Regarding the respiratory function and fatigue by spirometric lung pattern, the FVC and the FVC % predicted of patients with a normal pattern or an obstructive pattern were greater than other groups at a statistically significant level. As for the $FEV_1$, that of patients with a normal pattern was significantly higher than others, and for the $FEV_1$ % predicted, that of patients with a normal pattern or a restrictive pattern was significantly higher(p<.001). Fatigue score by patients with a normal pattern was significantly less than patients of other patterns(p<.001). Therefore, pulmonary physical therapy is considered necessary to improve respiratory function and fatigue degradation of elderly in a facility.
