The Effects of Changing Duty Cycle With Electrical Stimulation on Blood Lactate and Plasma Enzyme (전기자극 시 활동주기 형태의 변화가 혈중젖산과 혈장효소에 미치는 영향)

  • By measuring changes in blood lactate and plasma enzyme (CPK, GOT, GPT) with electrical stimulation applied at two duty cycles, this study is intended to look into which type of duty cycle may have more effects on blood lactate and plasma enzyme constituents through animal experiment so as to determine any duty cycle appropriate for electrical treatment. In this study, electrical stimulation was applied to total 20 Korean house rabbits (weight: 3~3.5 kg) by means of an electrical therapeutic apparatus called TS6000 (made in Netherlands) at duty cycle of 50% and 20% respectively for 30 minutes. Here, 5 cc of blood was collected from their carotid artery before stimulation and in 30 minutes after stimulation respectively to carry out biochemical experiment and analysis. As determined through the above experiment, blood lactate rate was increased to 333.07% at 50% duty cycle after experiment and 185.71% at 20% duty cycle after experiment respectively. In both cases, blood lactate rate was significantly increased to higher level after electrical stimulation than before. Moreover, the rate of change in the average of blood lactate rate at both duty cycles also showed significant differences. CPK rate was boosted to 301.82% at 50% duty cycle after experiment and 321.35% at 20% duty cycle after experiment respectively. In both cases, CPK rate was remarkably boosted to higher level after stimulation than before (p<.05). However, there was not any significant difference in the rate of change in average CPK at both duty cycles (p<.05). GOT rate was significantly boosted up to 38.97% at 50% duty cycle after experiment (p<.05), while it was slightly increased to 1.68% at 20% duty cycle after experiment without any significant difference. Rather, GPT rate dropped slightly at both duty cycles after experiment, but there was not any significant difference. Although blood lactate and GOT were relatively less generated at 20% duty cycle after electrical stimulation than at 50% duty cycle, the change of duty cycle didn't have any significant influence on CPK rate. In this regard, this study failed to come any consistent conclusion about the association between change of duty cycle and muscle fatigue. Therefore, it is advisable that follow-up studies seek various ways to a little more effectively apply electrical stimulation to laboratory animals by avoiding their muscle fatigue. GOT rate was significantly boosted up to 38.97% at 50% duty cycle after experiment (p<.05), while it was slightly increased to 1.68% at 20% duty cycle after experiment without any significant difference. Rather, GPT rate dropped slightly at both duty cycles after experiment, but there was not any significant difference. Although blood lactate and GOT were relatively less generated at 20% duty cycle after electrical stimulation than at 50% duty cycle, the change of duty cycle didn't have any significant influence on CPK rate. Comparison the Initial Effects of Nerve Mobilization Techniques, Static Stretching and Contract-Relax on Hamstring Flexibility and Walking Ability in Post-Stroke Hemiplegia Patients (뇌졸중으로 인한 편마비 환자의 슬괵근 유연성과 보행능력에 대한 신경가동기법, 정적 신장기법 및 수축-이완기법의 즉각적 효과 비교)

  • Purpose : The purpose of this study is to compare the initial effect of nerve mobilization (NM), static stretching (SS), and contract-relax (CR) techniques to find the best method in improving hamstring flexibility and gait function in patients with hemiplegia. Methods : Eleven patients with hemiplegia were included in this study. Passive knee extension (PKE) range of motion and the sit and reach (SR) test were used to measure hamstring flexibility, while timed up and go (TUG) and the 10m walking (10MW) test were used to measure the subject's gait. Measurements on each test were assessed prior to the experiment, immediately following the experiment, and 30 minutes after the experiment. Analysis of the results utilized a repeated measures analysis of variance to examine hamstring flexibility and the difference in walking ability. Results : The results suggest significant increases in NM, SS, and CR techniques as they relate to hamstring flexibility (p<.05) following (both immediate and 30 minutes post experiment) PKE range of motion and the SR test, but post-hoc showed no significant difference between the three techniques (p>.05). Additionally, the results suggest significant increases in NM, SS, and CR techniques as they relate to gait function (p<.05) following the TUG test, but found no significant difference in the 10MW test (p>.05). Post-hoc analysis between the three techniques suggests that only the NM technique significantly changed gait function. The time of TUG and 10MW test showed no significant difference between the three techniques before an experiment, just after an experiment, and 30 minutes following the experiment according to the measurement points in time (p>.05). Conclusion : This study suggests NM, SS, and CR techniques immediately improve hamstring length and flexibility while improving gait function in patients with hemiplegia.

The Comparison of Effective of Stretching Exercise and Stabilizing Exercise for The Patients with Myofascial Pain Syndrome at The Shoulder Girdle (견관절부 근막동통증후군의 근육내 전기자극치료시 신장운동과 안정화운동의 효과 비교)

  • Summary of Background Data: Myofascial pain syndrome (MPS) is a common painful muscle disorder caused by trigger points occurring in myofascial. MPS is a major cause of chronical pain and is the subject of further clinical examination. Purpose: To uncover effective intramuscular stimulation therapy (IMS), the patients' actively participation stretching exercise and stabilizing around shoulder girdle where trigger point provoking myofascial pain syndrome in usually occurred. Methods: 45 myofascial pain syndrome patients were randomly chosen and divided into 3 groups. The first group (G1) received only IMS therapy, The second group (G2) had both IMS and active stretching exercises administered and the final group (G3) was given IMS therapy and stabilizing exercises. Therapy intervention was given for 3 weeks, 3 times a week and then only stretching exercise for the second group and stabilizing exercise for the third group was given for another 3 weeks. The visual analogue scale was dine before the experiment, 3 weeks after the experiment and 6weeks after the experiment to measure subjective degrees of pain and pressure pain threshold to measure sensitivity improvement of trigger point and functional ability questionnaire to measure daily life performance. Results: There were no significant changes after 3 weeks but after 6 weeks, between GI and G2 and between G1 and G3 showed significant change of pain, pressure pain threshold and daily life performance. There were significant improvement of the measurement of degrees of pain, pressure pain threshold and improvement of daily life performance at different times for G1 showed change 3 weeks after the experiment, but there were no changes 6 weeks after the experiment. There were significant improvement of the measurement at different times for G2 and G3 showed change 3 weeks after the experiment and 6 weeks after the experiment. Conclusions: IMS therapy proved to be effective in inactivation of trigger points of the myofascial pain syndrome patients and stretching and stabilizing exercises beside it keep remedial value longer by improvement of pain and dysfunction that occurred by the trigger point.

Study on the Estimation of Discharge Coefficient of Sluice for Tidal Power Generation by Performing Physical Experiment (수리실험에 의한 조력발전용 수문의 유량계수 산정에 관한 고찰)

  • In this study, the influence of methodology of assessing water levels on the discharge coefficient of sluice for tidal power generation was investigated. A physical experiment was performed in a planar open channel by installing 1/70 scale model of the sluice caisson in the planar open channel. In front of and behind the sluice model, sloping bathymetry was made to reproduce corresponding field condition. By analyzing the experimental results, it was found that the location of measuring water levels significantly affects the estimates of the discharge coefficient, due to the variability of the parameter according to the head difference between the measuring locations. Therefore, it is necessary to be careful in estimating and utilizing the discharge coefficient in the relevant study of a tidal power generation.

Comparison of Effects of Non-surgical Continuous and Intermittent Traction on Pain, Balance and Physical Function in the Treatment of Knee Osteoarthritis

  • Purpose: This study examined the comparison of effects of non-surgical continuous and intermittent traction on pain, balance and physical function in the treatment of knee osteoarthritis. Methods: A total of 30 knee osteoarthritis patients were recruited and randomized to a continuous traction group (n=10), an intermittent traction group (n=10), and a control group (n=10). The continuous traction group and intermittent traction group received a non-surgical continuous and intermittent knee joint traction workout five times a week, for 4 weeks. All subjects were assessed with the numeric rating scale (NRS), timed up and go test (TUGT), and Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) before and after the 4-week treatment. Results: As a result of comparison within groups, the continuous traction group, intermittent traction group, and control group showed a significant difference for NRS, TUGT, and WOMAC after the experiment (p<0.05). According to the comparison of the three groups, the continuous traction group showed a more effectively significant difference than the intermittent traction group and the control group in the balance and physical functions before and after the experiment (p<0.05). Conclusion: This study showed that non-surgical continuous traction treatment was effective in improving pain, balance ability, and physical function in knee osteoarthritis patients.

The Effects of the Physical Activity Program on Body Composition, Depression and Risk Factors of Dementia in the Elderly Women (고령 여성들의 신체활동 프로그램 참여가 신체조성, 우울증 및 치매 위험 인자에 미치는 영향)

  • The purpose of this study was to examine the effects of a physical activity program on body composition, depression and risk factors of dementia in elderly women. Subjects were 40 women aged 65~75 years, classified into two groups (experiment group 20, control group 20). The factors measured in this study were body composition (weight, percent body fat, body mass index, body fluid), depression (BDI score) and risk factors of dementia (K-MMSE, LDL-C, HDL-C, insulin, ${\beta}$-amyloid, DHEAs) at 12 and 24 weeks before and after the program. Subjects of experiment group participated in a 24 week physical activity program (45~60 min, 3~4 day per weeks). Data were analyzed by repeated ANOVA method of SPSS, with accepting levels for all significances above ${\alpha}$=0.05. The results of this study were as follows. In body composition, the experiment group showed significant differences in the weight, percent body fat and body mass index compared to the control group. In depression score, the experiment group showed no significant differences regarding BDI score level compared to the control group. In risk factors for dementia, the experiment group showed significant differences in levels of K-MMSE, LDL-C, HDL-C, insulin, ${\beta}$-amyloid, and DHEAs compared to the control group. This study showed that a 24 week physical activity program had positive effects on almost all of the factors evaluated in elderly women, and that elderly women who did not participate in the program experiences no such positive effects. Therefore, regular participation in a physical activity program could be advocated as one strategy to enhance body fat, depression, and risk factors of dementia and may result in improving risk for diseases of age in elderly women.

The Effects of Sitting Posture on Cervical Flexion Angle and Pain during Smart Phone Use in Young Adults

  • Purpose: This study aimed to investigate the effects of sitting posture on cervical flexion angle and pain during smart phone use. Method: This research was performed on 10 adult female students who use smart phones regularly, Each of the two groups of participants used the provided smart phone for 20 minutes while maintaining the given default posture. The evaluation order was randomly selected and the two groups were photographed twice before and after the experiment and were asked to answer if they felt pain during or after the experiment. Result: First, both upright sitting position and vertebra bent position pre and post experiment readings showed significant statistical difference (p<.05). And it is shown to be a factor that induces pain around the neck via increase in neck flexion angle. Second, although there were no significant statistical difference between the pre and post experiment readings of the upright sitting position results and the vertebra bent position results, the vertebra bent position readings showed bigger changes to the neck flexion angle then the upright sitting position readings. Third, all participants from both groups claimed pain in all the tested postures of smart phone usage. Conclusion: Smart phone usage for an extended amount of time in all body postures may prove to have a negative effect making the "optimal" smart phone usage position as a controlled time with neck stretches included in between short sessions.

Skin Permeation Effects of Meloxicam Gel on Ultrasound Parameters by Phonophoresis (초음파의 매개변수에 따른 Meloxicam Gel의 경피투과 촉진효과)

  • This study conducted the following experiment to examine and compare transdermal permeation effects according to parameters of ultrasound and physiochemical characteristics of meloxicam. Permeation by ultrasound among these experimental drugs was relatively higher and it was involved in COX-2 inhibition unlike other drugs. Recently use of oral agents has been rapidly increased, but it was not generalized to transdermal agent and this study selected meloxicam that transdermal permeation research using ultrasound was not performed and conducted transdermal permeation experiment with skin of hairless mouse and analyzed permeation with HPLC. It made gel first and analyzed permeation depending on frequency and intensity of ultrasound of meloxicam with the same experimental procedures as the above experiment. The results of this study can be summarized as follows. Transdermal permeation by ultrasound frequency was higher in 1.0 MHz and it was higher as intensity increased. In comparison by parameters of ultrasound, there was similar permeation in $1.0\;W/cm^2$ of continuous mode and $3.0\;W/cm^2$ of pulsed mode and it was effective to high intensity for using pulsed mode. It was found that duty cycle of ultrasound affected transdermal permeation in meloxicam gel used in this experiment and transdermal permeation was higher in used ultrasound as phonophoresis than non-ultrasound for anti-inflammatory effects.

Effects of Multipath Electrical Stimulation on the Functional Recovery of Early Stage Patients of Total Knee Arthroplasty

  • PURPOSE: This research was intended to investigate the influence to function recovery at the early stage after surgery, by conducting Multipath Electrical Simulation and isometric exercise treatment as early stage medical treatment method for Total knee arthroplasty patients. METHODS: The subject of 30 patients having Unilateral Total knee arthroplasty over age 65, Multipath Electrical Simulation and isometric exercise (experiment group I), Conventional Electrical Simulation and isometric exercise (experiment group II) and isometric exercise (control group). The intervention was performed in 5 times per a week and 60 minutes per a day during 4 weeks. We performed research by conducting Neuromuscular Electrical Stimulation and isometric exercise together and measured pain, range of motion, muscle strength and gait ability before and after intervention. RESULTS: The result showed therapeutic improvement in experiment group I, experiment group II and control group, but Multipath Electrical Simulation and isometric exercise showed significant improvement in function recovery of early stage compared to Conventional Electrical Simulation and isometric exercise, only isometric exercise. CONCLUSION: Based on research result, in order for early state function recovery of Total knee arthroplasty patients, when conducting neuromuscular electrical stimulation and isometric exercise together, especially when applying Multipath Electrical Stimulation, we could know that it showed more significant improvement to function recovery after surgery. Also, we suggest that Multipath Electrical Simulation may become a useful tool as a method for intervention and performing in various diseases for weakening of Quadriceps muscle.

Gait Type Change for Decrease of Leg's Fatness (다리의 지방감소를 위한 보행형태 변화에 관한 연구)

  • We developed the shoes which is specially designed to decrease the leg's fatness during walking and to improve the body figure. The analysis of the gait cycle and the shoes gave changes at the swing phase. We examined the results to 30 girl students of Yeong Dong Junior College for 5 weeks from 27, October to 29, Nobember in 1996, and the results were also obtained in decrease of their thigh's size in circumference. The results of the analysis are as followings; 1) After experiment circumference of the right thigh was decreased 0.04cm in noncontrol group and significantly decreased 1.04 cm in control group(p<0.01). 2) After experiment circumference of the left thigh was decreased 0.27 cm in noncontrol group and significantly decreased 1.17cm in control group(p<0.01). 3) After experiment circumference of the right calf of the leg decreased 0.09 cm in noncontrol group and significantly decreased 0.54 cm in control group(p<0.01). 4) After experiment circumference of the left calf of the leg decreased 0.21 cm in noncontrol group and significantly decreased 0.47 cm in control group. 5) After experiment body weight were not changed significantly in both group. In these results, if the people want to exercise the analysis of fat in the thigh, put on the shoes which improves the body figure and do daily work, and it will be naturally decreased the fat in thigh and they will maintain the beauty lines of the legs. There weren't any changes in the body weight while the fat of thigh in circumsference was analysed and decreased. It means that because the weight of the fat is very light, there weren't any changes in body weight.
