일 대학병원 유방암 환자의 우울증상과 삶의 질 (A Study of the Depressive Symptoms and the Quality of Life in Patients with Breast Cancer in a University Hospital)

  • 연구목적 유방암 환자에서 우울증상의 동반 빈도는 매우 높은 것으로 알려져 있으며 우울증상은 건강과 연관된 삶의 질에 영향을 미칠 수 있다. 본 연구는 일 대학병원 유방암 환자의 우울증상의 유병율을 알아보고 우울증상이 건강과 연관된 삶의 질과, 우울증상과 삶의 질에 미치는 인구 사회학적 및 임상적 변인을 알아보고자 하였다. 방 법 원광대학교병원 외과에서 2010년 11월 1일부터 2011년 5월 31일까지 유방암으로 진단 및 수술을 받고 통원치료 중인 19세 이상 75세 미만인 여성 환자 52명에서 이전에 기분장애로 진단을 받았거나 평가 설문지 및 의학적 정보가 부족한 11명을 제외한 41명을 연구대상으로 하였다. 연구 대상자의 인구통계학적 변인 및 유방암에 대한 임상적 특성은 의무기록지를 통하여 조사하였다. 우울증상은 Beck 우울척도, 불안은 성격평가질문지의 불안 하위 척도를 이용하였으며, 건강과 관련된 삶의 질은 한국판 단축형 36항목 건강 설문지(Short Form 36 health Survey-Korean Version)를 사용하였다. Beck 우울 척도에 따른 우울증상 군과 비우울증상 군의 건강과 관련된 삶의 질을 비교하였으며, 인구사회학적 변인 및 임상적 변인을 예언 변인으로 하고 우울증상과 삶의 질을 준거 변인으로 한 중다회귀분석을 시행하였다. 결 과 유방암 환자 중 우울증상의 유병율은 36.4%였다. 우울 증상을 나타내는 유방암 환자의 건강과 연관된 삶의 질에서 신체 기능(p<.01), 신체 역할(p<.001), 전반적 건강(p<.05), 사회 기능(p<.001), 정서 역할(p<.001), 정신 건강(p<.001)의 하위 척도가 우울증상을 동반하지 않은 환자군에 비하여 유의하게 낮았다. 그러나 신체 통증 및 활력의 하위척도에서는 유의한 차이가 없었다. 유방암 환자의 우울증상은 불안과 교육 정도, 항암치료 시행 여부와 유의한 것으로 나타나 전체 우울증상의 63.6%의 설명 변량을 나타냈다. 유방암 환자의 건강과 관련한 삶의 질은 우울증상, 수술형태, 연령이 유의하였으며 55.8%의 설명 변량을 나타냈다. 결 론 유방암 환자의 우울증상은 36%에서 나타났으며 우울증상은 건강과 연관된 삶의 질을 낮추었다. 우울증상은 삶의 질에 높은 설명 변량을 가지고 있으며, 불안은 우울증상을 동반한 유방암 환자에서 높은 설명 변량을 갖고 있었다. 따라서 유방암 환자의 삶의 질을 높이기 위하여 우울증상에 대한 평가 및 치료가 필요할 것으로 여겨진다.

요실금(尿失禁) 환자의 삶의 질에 대한 전침치료 효과 (Effect of Electroacupuncture on Quality of Life of Patients with Urinary Incontinence)

  • Objectives : This study was designed to evaluated the effect of electroacupuncture on Quality of life of patients with urinary incontinence Methods : Subjects were voluntarily recruited by newspapers and internet. Electroacupuncture was performed three times a week for 3 weeks. Acupuncture point for EA group was B32, Electrical stimulation frequency was 2Hz, duration 20 minutes, and intensity was up to pain threshold according to patients. The patients's symptoms were assessed before, after 3 weeks of treatment by QOL item of International Prostate Symptom Score(IPSS), Medical Outcomes Study(MOS) 36-Item Short-Form Health Survey(SF-36). Results : QOL score of IPSS were significantly improved after 3 weeks(p<0.05) compared to the pre-treatment. There were significant changes in Social functioning(SF), role-physical(RP), role emotional(RE), mental health(MH), bodily pain(BP) score of SF-36 after 3 weeks(p<0.05), but there were no significant changes in physical functioning(PF), vitality(VT), general health(GH) score of SF-36. Conclusion : This study suggests that electroacupuncture treatments can be applicable to improve symptoms in patients with urinary incontinence.

중년여성의 건강증진을 위한 단전호흡 운동프로그램의 적용 효과 (The Effect of the DanJeon Breathing Exercise Program Applied to Health Promotion in Women in Midlife)

  • The purpose of this study was to develop a DanJeon Breathing Exercise Program for health promotion and to examine the effects of a DanJeon Breathing Exercise Program on the physical and emotional health promotion of women in midlife. The design utilized for this study was quasi-experimental with a nonequivalent control group pretest-posttest design. This study was done between February 1 and July 4, 2000 and the subjects of the study were 40 women in midlife living in Seoul. There were 20 members in experimental group who participated in the DanJeon Breathing Exercise Program and 20 members in the control group who were chosen as matched to members of the experimental group according to age, education and religion. The DanJeon Breathing Exercise Program was carried out for 80 minutes a day, 3 times a week for 12 weeks. The percent of body fat, back strength, flexibility (trunk flexion), balance and vital capacity were measured using the Health Management System developed by the Korea Physical Science Institution. Blood tests were done at D infirmary for total cholesterol, and the atherogenic index was calculated using an established formula. The scores of anxiety and depression were measured by a questionnaire with 10 questions on anxiety and 13 questions on depression. It was developed from the Korean Manual of Symptoms-Checklist-90 revision. The collected data were processed using the SPSS PC program and analyzed using $\chi^2$-test and t-test. The result of this study are as follows : 1. The percent of body fat for the experimental group who participated in the DanJeon Breathing Exercise Program was lower than that of the control group. 2. The degree of back strength, flexibility (trunk flexion) and balance of the experimental group that participated in the DanJeon Breathing Exercise Program was higher than that of the control group. 3. The vital capacity of the experimental group that participated in the DanJeon Breathing Exercise Program was higher than that of the control group. 4. The total cholesterol level and atherogenic index of the experimental group that participated in the DanJeon Breathing Exercise Program were lower than those of the control group. 5. The scores for anxiety and depression in the experimental group that participated in the DanJeon Breathing Exercise Program were lower than those of the control group. In conclusion, DanJeon Breathing Exercise Program promotes the physical and emotional health of women in midlife. The DanJeon Breathing Exercise Program can be utilized as a nursing intervention for the promotion of health in women in midlife.

대학생의 주의력결핍과잉행동장애 증상 출현율과 우울과의 관계 -보건계열 학생을 중심으로- (Relationship between prevalenc of attention deficit hyperactivity disorder symptoms and depression of college students -Focused o health related majors-)

  • 본 논문은 대학생들의 과잉행동성충동장애 증상 출현율과 우울과의 관계를 알아보고자 하였다. 대학생 350명을 대상으로 Conners 과잉행동성충동장애 평정척도와 Beck 우울척도를 이용한 조사를 실시하였다. 대학생들의 과잉행동성충동장애 증상 출현율은 5.1%이였고 그 중 부주의형이 가장 높았다. 우울 정도는 $8.38{\pm}6.86$점이었고 여학생이 남학생보다 우울정도가 높았으며, 보통 우울증상과 심한 우울증상자는 2.1%이었다. 과잉행동성충동장애가 우울에 영향을 주는 모형 설명력은 33.5%로 다소 높았으며, 과잉행동성충동장애 증상과 우울 증상의 공존율은 전체 학생 중 3.1%로 나타나서, 대학생들의 성인 과잉행동성충동장애 증상 출현율과 우울은 관계가 있는 것으로 보인다. 성인 과잉행동성충동장애로 인한 다양한 문제를 고려하면 이에 대한 사회적 차원의 대책 수립이 필요한 것으로 보인다.

유방암 관련 인터넷 상담 질문에 대한 내용분석 (Content Analysis of Questions Related to Breast Cancer Raised through Internet Counseling in Korea)

  • Purpose: The purpose of this study was to analyze the data regarding questions raised by women with breast cancer through Internet counseling in Korea. Methods: The data were collected from one internet web-site, providing counseling by physicians. A total of 617 questions were analyzed by content analysis method. Results: About 90 percent of the counselees were patients themselves. But most of the general and health-related characteristics of them were not known from the data. As a result of content analysis, 617 questions were grouped into 9 major categories. The most common major category was identified as "life after treatment" (212 questions, 34.2%), followed by "chemotherapy" (139 questions, 22.3%) and "hormone therapy" (115 questions, 18.9%). Questions regarding "physical symptoms" were the most frequent one in the major categories of "life after treatment", "chemotherapy", and "radiotherapy", while questions regarding "psychological problems" were the least. Conclusion: The findings of this study indicate that it is important for health professionals to provide continuous on-line informational support to women with breast cancer, even after all the treatment is over, especially focusing on physical symptoms. In addition, off-line program needs to be reinforced to provide emotional support that is not well delivered by on-line program.

  • Premenstrual syndrome (PMS) is a common disorder affecting the emotional and physical health of women during certain periods of the menstrual cycle. Many researchers who have previously studied PMS have believed that PMS is associated with changes in sex hormones and serotonin levels at the beginning of the menstrual cycle. However, recent studies suggest that progesterone/estrogen imbalance and elevation of prolactin-induced by dopamine low-secretion play a crucial role in increasing PMS symptoms. Because of this, we have focused on mitigating PMS symptoms through the mechanism of prolactin secretion inhibition by dopamine receptor activation. The inhibition of prolactin secretion by 61-kinds of medicinal herb extracts was investigated in GH3 pituitary cells. Among them, Inula heleniun L. root extract (IHE) showed excellent prolactin secretion inhibitory effect. IHEs were prepared using 30, 50, and 70% ethanol. And the yield, cytotoxicity, dopamine receptor activity and inhibition of prolactin secretion of each extract were measured. Through a series of experiments, we found that prolactin secretion was significantly reduced (P<0.01) by the components present in IHE and that dopamine receptor regulation was possible (P<0.05). Considering yield and safety, we suggest the use of 30% ethanol IHE in the development of PMS symptom relief products.

부인암 여성의 증상 클러스터(Symptom Cluster) (Symptom Clusters in Women with Gynecologic Cancer)

  • Purpose: Women with gynecologic cancer often experience various physical and psychological symptoms relating to the cancer and its treatment. The purpose of this study was to identify symptom clusters. Method: A survey was conducted on 184 women with diagnoses of cervical, ovarian or endometrial cancer. Fifty symptoms were assessed for prevalence, severity and interference, and symptom clusters were identified. Cluster analysis was done using SPSS version 12.0. Results: Fatigue was identified as the most prevalent symptom (81.52%), lack of vaginal lubrication (2.26) as the most severe symptom, and lack of vaginal lubrication as the most interfering one (2.15). Identified six clusters were: Anorexia-pain cluster (loss of appetite, taste change, weight loss, appearance change, alopecia, weakness, pain), Fatigue cluster (lack of concentration, lack of memory, fatigue, dry mouth), Urinary-bowel distress cluster (urinary difficulty, constipation), Abdominal discomfort cluster (lower abdominal pain, abdominal pain, bloating), Emotional distress (sadness, anxiety-worry, nervousness, restlessness), and Menopausal cluster (sweating, hot flush, fever). Conclusion: The result of this study provides fundamental data to health care professionals in developing interventions for effective symptom management for women with gynecologic cancer by understanding identified 6 symptom clusters.

Quality of Life in Children and Adolescents with Inflammatory Bowel Disease: Impact and Predictive Factors

  • Purpose: Inflammatory bowel disease (IBD) in children and adolescents is associated with high morbidity and possibly has a significant negative impact on their quality of life. This study aimed to evaluate the quality of life of children and adolescents with IBD and define the variables that impact these individuals. Methods: We administered the Pediatric Quality of Life Questionnaire (PedsQL) to 35 children and adolescents diagnosed with IBD and with available quantitative data from clinical records on epidemiology, clinical evolution, complementary tests, medical interventions, and disease activity. Data were evaluated according to the IBD type and compared with a control group of healthy children. Results: The study group showed a significantly lower PedsQL score than the control group (p<0.01). Significant factors contributing to poor overall quality of life included female sex, Crohn's disease, surgery, and food restrictions. Symptoms such as diarrhea and the fear of using public toilets were associated with low physical scores. Feeling sick had a negative impact on the emotional PedsQL scores. Patients with a fear of using public toilets, anthropometric scores below the 3rd percentile, and greater disease activity scored lower in the social domain. Regarding school and psychosocial evaluations, younger children with symptom onset after the age of 2 years had lower scores than younger children with symptom onset before the age of 2 years. Conclusion: IBD negatively affects the quality of life of children and adolescents based on its impact on the physical, emotional, social, and psychosocial statuses of these patients.

Preoperative Quality of Life in Patients with Gastric Cancer

  • Purpose: We evaluated the socio-personal and clinical factors that can affect preoperative quality of life to determine how to improve preoperative quality of life in patients with gastric cancer. Materials and Methods: The preoperative quality of life data of 200 patients (68 females and 132 males; mean age $58.9{\pm}12.6years$) with gastric cancer were analyzed according to socio-personal and clinical factors. The Korean versions of the European Organization for Research and Treatment of Cancer (EORTC) Quality of Life Questionnaire Core (QLQ) 30 and the EORTC QLQ-STO22, a gastric cancer-specific module, were used to assess quality of life. Patients were asked to complete the questionnaire preoperatively by themselves. Results: Patients with a higher academic background and stage I disease tended to have higher global health status scores. Highly educated younger men had better physical functioning scores. Highly educated and well-nourished patients with stage I cancer had higher role functioning scores. Married patients had better emotional scores. The symptom scales were affected by sex, age, education level, nutrition, and cancer stage. Conclusions: Preoperative quality of life in patients with gastric cancer can be improved by nutritional support and treatment of symptoms caused by disease progression. Psychological support may be helpful for patients with a poor quality of life.

Does the Impairment of Functional Life Increase the Probability of Suicide in Cancer Patients?

  • Background: Cancer affects patients in many ways including physical, social, emotional, psychological and economic and restricts the functional lives. Psychiatric problems seen among cancer patients may increase the suicide probability and patients perceive suicide as a peaceful death type. The aim of this study was to examine the correlation between functional life and suicide probability among cancer patients. Materials and Methods: This study was conducted with 105 cancer patients as descriptive. The Functional Living Index_Cancer (FLIC)," suicide probability scale" (SPS) and personal information form were used as data collecting tools. Data were evaluated by descriptive analysis, and Pearson's correlation. Results: It was determined that 34.3% of patients thought of suicide. Significant negative correlation was found between functional life and suicide probability (r=-.641, p=0.000), increase being evident in those with poor functional life. Conclusions: It is recommended that cancer patients should be supported for improving their functional lives with help in coping processes for illness and treatment symptoms. Evaluation of the patient mental status to prevent the suicide among this group is an important role for nurses.
