  • PURPOSE: While underwater, patients with hemiplegia experience unwanted limb flotation on their paretic side due to low muscle mass and high body fat. However, only a limited number of studies support the effectiveness of this practice. Therefore, the purpose of this study was to determine how the balance and walking abilities of patients with hemiplegia due to stroke were affected by wearing an aquatic cuff on their ankles during underwater treadmill walking. METHODS: Twenty stroke patients were divided into an experimental group comprised of 20 patients who would wear an aquatic cuff and a control group comprised of 10 patients without an aquatic cuff. Both groups underwent a six-week intervention for 30 minutes a day three times a week. To evaluate the groups' balance and walking abilities before and after the intervention, the 10 m walking test, timed up go test, Berg Balance Scale, functional reaching test, and the GAITRite system were used. RESULTS: The results of the 10 m walking test, timed up go test, differences between the left and right gait cycles, and functional reaching test showed statistically significant differences in the rates of change between the two groups (p<.05). CONCLUSION: The study results suggest that underwater treadmill training in stroke patients can be more effective when they wear an aquatic cuff on their ankles compared to wearing no aquatic cuff.

  • PURPOSE: This study measured the impact of a 12-week fall-prevention exercise program on balance, ambulatory ability, lower limb strength, and psychosocial characteristics in older adults diagnosed with dementia. METHODS: The participants comprised 31 older adults (9 men, 22 women) diagnosed with Alzheimer's or vascular dementia at a long-term care hospital located in Gunsan City. A fall-prevention exercise program was provided to the experimental group, while the control group was only provided with instruction and materials related to the fall-prevention exercise program. The participants were evaluated before the intervention, 6 weeks after the intervention, and 12 weeks after the intervention on static and dynamic balance abilities (using Timed Up and Go test: TUG, Tinetti-balance scale, one-leg standing test: OLS), gait (Tinetti-balance scale, 6-minute walk test: 6MWT), lower limb strength (sit to stand test; STS), and psychosocial characteristics (Short Form 36 Health Survey-Korean version, Korean Mini-Mental State Exam). RESULTS: An independent samples t-test and repeated measures analysis of variance were used for the statistical analysis. There were statistically significant improvements after the intervention (p<.05) in dynamic balance abilities (TUG and OLS using the left foot), gait (6MWT), and lower limb strength (STS) for the experimental group, but not for the control group. No difference was seen in psychosocial characteristics. CONCLUSION: Older adults with dementia who participated in the fall-prevention exercise program showed significant improvements in their static and dynamic balance abilities, lower limb strength, and ambulatory ability after the intervention.

  • The Goal of this study was to assess the effect of aerobic exercise on cognitive function of elderly people. The participants' cognitive functions were measured before exercise. Exercise was practiced three times a week for nine weeks. The aerobic exercises consisted of warm-up exercises for five minutes followed by cycling for 30 minutes. The exercise intensity was set to 65%~75% of the intensity for the maximum heart rate(220-age). The control group did not perform any exercises. The subjects' cognitive functions were measured nine weeks later. In the aerobic exercise group, between before and after the exercises, significant increases appeared in total K-MMSE scores and scores of some sub items comprising memory registration, concentration and calculation abilities but not in other items. In the control group, no significant differences appeared in any items between before and after the experiment. In a comparison between the aerobic exercise group and the control group, significant differences appeared in total K-MMSE scores and scores of two sub items comprising concentration and calculation abilities but not in other items(p<.05). Aerobic exercises were effective in the improvement of cognitive functions and among sub items of cognitive functions, concentration and calculating abilities were improved. Aerobic exercises performed by elderly persons are considered to be effective in improving cognitive functions.

  • Purpose: The purpose of this study was (1) to examine the effects of a Health Promotion Exercise Program (HPEP) on functional improvement, physical strength (PS) and muscle activation (MA), and the interactive influence with activities of daily living (ADL) and instrumental activities of daily living (IDAL) in the elderly, and (2) to develop and provide a HPEP for the elderly in order to prevent declines and impairments in integrated nerve function and physical capacity. Methods: Our study included relatively healthy elderly people aged 65 years or older. The experimental group, which was composed of 30 people, participated in a HPEP 50~60 minutes a day, 3 times a week, for a total of 12 weeks. Results: Confirmatory factor analysis (CFA) validated the measurement models for MA, physical strength, ADL and IADL with a p<0.01. This confirms (i) the beneficial effects of the ADFP on their MA and PS, and (ii) that their improvement in PS, in turn, can improve their IADL. The results of this study indicates that ADFP can help Korean seniors 65 years or older improve their MA, PS, ADL, and IADL, and do so synergistically. Conclusion: An HPEP, when organized in such a way that the elderly can easily do it, and when exercise items and intensity can be programmed and reorganized accordingly to individual physical and physiological characteristics by presenting 5 categorized health domains of physical strength, may be useful, especially because it can be practiced irrespective of time and place.

  • Elderly care services are health, mental health, social health, and residential services provided to temporarily or chronically disabled older persons over an extended period of time with a goal of enabling them to function as independently as possible. And elderly care facilities such as nursing homes are places in which elderly care services have to be delivered effectively. It is, therefore, desirable to compose the space of elderly care facilities to meet the mental and physical abilities of frail elder people. This study has proposed the hierarchical space organization of elderly care facilities with an aim to fulfill the goals of them. Frail older people in hierarchically designed homes are anticipated to use the inner and outer spaces of care facilities as independently as possible in accordance with their physical and mental abilities.

  • Purpose: The purpose of this study was both to examine the effects of proprioceptive neuromuscular facilitation (PNF) lower leg taping and treadmill training on the gait and balance abilities of patients with hemiplegia resulting from a stroke and to provide a taping method based on the PNF concept. Methods: Twenty patients with hemiplegia resulting from a stroke were randomly and equally assigned to a control group (n=10), which received treadmill training, and to an experimental group (n=10), which received PNF lower leg taping and treadmill training. The intervention was conducted five times per week for six weeks. In order to measure changes in the gait ability of the subjects, a 10-meter walking test (10MWT) and a 6-minute walking test (6MWT) were conducted, and in order to measure changes in the subjects' balance ability, a timed up and go test (TUG) was performed. In order to compare differences within each group before and after the intervention, a paired-t test was carried out, and in order to compare differences between the two groups, the analysis of covariance was utilized. All statistical significance levels were set at ${\alpha}=0.05$. Results: There were significant differences before and after the intervention within both groups in changes of 10MWT, 6MWT, and TUG (p<0.01). Regarding differences between the two groups, the experimental group underwent more effective changes than the control group in 6MWT and TUG (p<0.05). Conclusion: This study applied PNF lower leg taping and treadmill training to patients with hemiplegia resulting from a stroke, and this resulted in improvement in the subjects' gait and balance abilities. Taping and treadmill training based on the PNF concept is considered to be usefully applied as one of the programs to improve hemiplegic patients' gait and balance abilities.
