Regulatory System of Quasi-drugs in Korea (우리나라 의약외품 허가심사제도 고찰 및 개선 방안)

  • The study was aimed to recommend the ways for improving regulatory system of quasi-drugs in governmental authority by comparing with other countries. According to the regulations, the scope of quasi-drugs includes 3 categories of 1) the health aids made of textile, rubber and paper, 2) the health aids which have very minimal effects or no any effects on humans, and 3) disinfectants and pesticides. In US, these quasi-drugs in Korea are classified into 5 categories of medical device, cosmetics, OTC drugs, dietary supplements and pesticides. To improve quasi-drugs administration in Korea, it is concluded that several measures should be implemented : 1) establish clear criteria for classifying into quasi-drugs and more detailed guidelines on designation of quasi-drugs, 2) reform current regulations to meet 3-categories characteristics, supplement detailed guidelines on quasi-drugs administration for effective application process, and update relevant regulations for efficacy, safety and quality, 3) update quasi-drug monographs, 4) re-evaluate current classification of individual quasi-drugs, 5) develop comprehensive list by ingredients, 6) reform post-marketing management system for safety and quality, 7) strengthen the review agency function by increasing the number of experts, 8) develop the database for quasi-drugs for effective information management.

Success Factors for the Smoking Cessation Service of the Safe Pharmacy (세이프약국 금연서비스의 성공요인)

  • Objective: Seoul City has implemented the Safe Pharmacy in 2013. This study aimed to ascertain the success factors of the smoking cessation service of the Safe Pharmacy. Methods: The data for the smoking cessation registration cards were obtained from the pharmacies which participated in 2014 Safe Pharmacy. The sample included 289 smokers in 6 districts who participated at least one sessions of smoking cessation service. the service included both behavioral intervention and nicotine replacement therapy. In order to identify the effectiveness of the smoking cessation service, logistic regression analysis was used. The dependent variable was the success of quitting smoking at the 4th week. The independent variables included age, sex, employment, chronic disease, district, smoking amount, registration path, supporters for quitting smoking and number of service sessions. Results: Fifty eight point eight percent of the sample successfully quit smoking at the 4th week. Unemployment, higher smoking amount, and less service sessions were negatively related to the success of quitting smoking. There were differences in the success rate across districts. Age, sex, chronic disease, registration path, and anti-smoking supporters were not related to the success rate. Conclusion: Pharmacy can be an effective community resource for smoking cessation. Factors that could increase the effectiveness of smoking cessation service of the Safe Pharmacy and possible ways to enhance the participation of pharmacies in smoking cessation services were also discussed.

A Simple Method for Determining Residual p-arsanilic Acid in Aquatic Products using EDTA-Assisted Solvent Extraction and LC-MRM

  • A simple method was developed to determine residual p-arsanilic acid (ASA), an organo-arsenic compound used as a feed additive, in aquatic products (eel, halibut, and shrimp) using EDTA-assisted solvent extraction and LC-MRM. The method was successfully validated in terms of specificity, linearity (coefficient of determination ≥ 0.995), accuracy (recovery or R, 72.72-78.73%), precision (the relative standard deviation of R, 2.08-6.98%), and sensitivity (the lower limit of quantitation, 5 ppb) according to CODEX guidelines (CAC-GL 71-2009). The use of EDTA in the extraction solvent and water with a suitable pH modifier as the reconstitution solvent may be the key factors for successful results. This is the first method that can be used for monitoring residual ASA in aquatic products using LC-MRM and could contribute to establishing a better aquatic product safety management system.

  • Sepsis remains common surgical problems with high morbidity and mortality despite improvement in the management for septic patient. Although hepatocellular dysfunction occurs during sepsis, the mechanism responsible for this remains unclear. In sepsis, a state of severe oxidative stress is encountered, with host endogenous antioxidant defenses overcome. Therefore, the aim of this study was to determine whether specific abnormality exists in cytochrome P450 (CYP)-mediated metabolizing function associated with polymicrobial sepsis and whether role of ascorbic acid (AA) in the alterations during sepsis. Educational Goals Extracted from Homepages of Pharmacy Schools in Korea (우리나라 약학대학의 홈페이지를 통해 고찰한 교육이념)

  • Background: The current educational goals and missions of pharmacy schools in Korea were analyzed to examine the current orientation and future direction of pharmaceutical education. Methods: Educational mission statements were obtained from the homepages of 35 pharmacy schools and subjected to convert into codes. Themes and categories were induced using qualitative content-analysis from the codes and compared according to location of school (capital area versus province), public versus private, and date of initial enrollment (before versus in 2011). The themes and categories were compared with "the eight-star pharmacist" suggested by World Health Organization (WHO) and International Pharmaceutical Federation (FIP). Results: Twelve themes, 44 categories, and 496 codes were identified. Themes included pharmaceutical expertise, professionalism, contribution to society, basic educational ideology, sphere of activity, leadership, research, dealing with future change, problem-solving ability, self-management and development, cooperation, and respect for life. Mission statements of schools that initially enrolled in 2011 cited humankind level contribution (p=0.011), patient-centered care (p=0.026), and globalization (p=0.018) more frequently than those enrolled before 2011. Most schools mentioned about care-giver, researcher, and decision-maker which were stated in "the eight-star pharmacist". Conclusion: To meet the growing social requirements of a pharmacist's roles, wide-ranging active discussion on establishing educational goals should be made.

Phellinus baumii Inhibits Immediate-type Allergic Reactions

  • In this study, we investigated the effect of Phellinus baumii (PB) on immediate-type allergic reaction and inflammatory cytokine secretion. PB inhibited compound 48/80-induced systemic reactions in mice. PB inhibited compound 48/80-induced plasma histamine release. In addition, PB also inhibited the immunoglobulin (Ig) E-mediated local allergic reaction. Furthermore, PB decreased the phorbol 12-myristate 13-acetate plus calcium ionophore A23187-stimulated tumor necrosis $factor-\alpha$ and interleukin-6 secretion in human mast cells. These results indicate that PB may be beneficial in the treatment of immediate-type allergic reactions.

  • The elderly are more likely to have chronic medical conditions that require multiple drug therapies. Purposes of this study are to reveal necessity of elderly patient education by pharmacists, and to induce appropriate policy. We carried out literature research. Taking several drugs together increases risk of drug interactions and adverse reactions. We suggest that pharmacists have the legal authority to monitor prescription for efficient drug management, pharmacovigilance system be efficiently operated, and medication education fee be provided to allow pharmacists give more time to the elderly.

A Descriptive Research on Drug Use Pattern of Narcotic Analgesics: a Case of University Hospital in Korea (마약성 진통제의 사용양상에 관한 서술적 조사연구: 한 대학병원의 경우)

  • The objective of this study was to find out drug use pattern of narcotic analgesics in university hospitals in Korea. A university hospital located in Kyungbuk province was chosen for this study. The drug use pattern was analyzed in terms of ingredient, administration route, patient type, and attending department. Amount of drug usage was counted by unit dose defined by the number of ampule or vial for injectable, tablet or capsule for oral, and each for patch preparations. Result showed that 11 narcotic analgesic ingredients were used during 2007-2009, and the drug usage was increased by about 20% annually during the period. Proportion of oral preparations used for pain management was about two third of all narcotic analgesics usage and kept increasing during the period. Proportion of the drug usage for outpatients was also steadily increased. Notably, the usage of oral preparations of oxycodone, morphine, and hydromorphone was rapidly increased for the management of cancer pain while the usage of codeine and codeine-containing composite preparations for cancer pain were minimal (<10%). About 90% of all narcotic analgesics were used by physicians in Internal Department, especially in Oncology Division of the Department. These findings suggest that pain management is becoming more aggressive and in agreement with WHO's guidelines regarding selection of administration route. However, in terms of 3-step ladder for cancer pain management, the drug use pattern was not congruent to WHO's guidelines. Therefore, in conclusion, it appears that physicians need to try to be congruent to the guidelines when using narcotic analgesics for cancer pain.

A Survey of Pharmacy Students' Perceptions of National Drug Supply Strategy through Public Pharmaceutical Companies (약학대학 학생들의 공공제약사를 통한 국가의 의약품공급 전략에 대한 인식 조사)

  • Background: Currently establishment of public pharmaceutical companies became an issue for securing stable supply of national essential drugs in order to respond appropriately to national public health crisis. Pharmacy students as future pharmacists need to be interested in drug related issues under the discussion in our society. This study aimed to investigate perceptions of pharmacy students on the national drug supply strategy through public pharmaceutical companies. Methods: A 20-items questionnaire designed for this study was used. Pharmacy students nationwide were enrolled in July 2017. They responded to questions through on-line survey program. Data were analyzed descriptively. Results: 160 students from 26 pharmacy schools participated. 5th and 6th grade students accounted for two-thirds, and one third aspired future jobs in pharmaceutical industry. Among responders, 71.3% was unaware of current national essential drugs, 52.5% did not know the Rare and Essential Drug Center, and 91.9% was unaware of the legislation on the supply and management of essential drugs. 82.5% favored the establishment of public pharmaceutical companies. 80.6% agreed that government should intervene in pharmaceutical market for ensuring drug accessibilities, 73.8% agreed that public pharmaceutical companies make possible to reach a goal of stable supply of pharmaceuticals, and 85.1% agreed that active compulsory license by public pharmaceutical companies is necessary. Conclusion: We found that pharmacy students recognized the need for governmental control to achieve social values of pharmaceuticals. More attention and participation of the pharmacy students as well as pharmacists in the process of national debate on public pharmaceutical companies are expected.

Comparative Review of Pharmacological Treatment Guidelines for Bipolar Disorder (양극성 장애의 약물치료 가이드라인 비교)

  • Objective: Bipolar disorder displays a spectrum of manifestations, including manic, hypomanic, depressive, mixed, psychotic, and atypical episodes, contributing to its chronic nature and association with heightened suicide risk. Creating effective pharmacotherapy guidelines is crucial for managing bipolar disorder and reducing its prevalence. Treatment algorithms grounded in science have improved symptom management, but variations in recommended medications arise from research differences, healthcare policies, and cultural nuances globally. Methods: This study compares Korea's bipolar disorder treatment algorithm with guidelines from the UK, Australia, and an international association. The aim is to uncover disparities in key recommended medications and their underlying factors. Differences in CYP450 genotypes affecting drug metabolism contribute to distinct recommended medications. Variances also stem from diverse guideline development approaches-expert consensus versus metaanalysis results-forming the primary differences between Korea and other countries. Results: Discrepancies remain in international guidelines relying on meta-analyses due to timing and utilized studies. Drug approval speeds further impact medication selection. However, limited high-quality research results are the main cause of guideline variations, hampering consistent treatment conclusions. Conclusion: Korea's unique Delphi-based treatment algorithm stands out. To improve evidence-based recommendations, large-scale studies assessing bipolar disorder treatments for the Korean population are necessary. This foundation will ensure future recommendations are rooted in scientific evidence.
