The Incidence of Hepatitis B in Military Service and the Effect of Asymptomatic HBsAg Carriers on the Incidence (군 입영자에서의 B형 간염 발생 규모와 B형 간염 바이러스 보유자가 이에 미치는 영향)

  • The purpose of this study was to investigate the incidence rate of hepatitis B in the military service and to examine the effect of the asymptomatic hepatitis B surface antigen(HBsAG) carries on the incidence of hepatitis B. The subject were 223,270 men who were conscripted to the Korean Army from 1991 to 1994. According to the conscripted year, four conscription cohort were constructed. At the screening examination for military service no test for hepatitis B were performed in 1991 and 1992. In 1993, a screening test for hepatitis B were performed and those who were confirmed as HBsAg positive o. showed high titers $(\geqq100IU)$ of nm glutamic-pyruvic transaminase(SGPT) were excluded from conscription. In 1994, the criteria for conscription was changed and those who were HBsAg positive were not excluded from conscription. Only those who showed $\geq$ SGPT 100IU were excluded. The main results were as follows ; 1. The positive rate of HBsAg is 5.5% in the conscripted men. 2. The incidence rates of the hepatitis B in 1991 and 1992 conscription cohort were 9.96 and 8.10 per ten thousand person-year, respectively. The incidence rate of the hepatitis B was 1.34 per ten thousand person-year in 1993 conscription cohort which was confirmed as HBsAg negative at the screning test, and 7.41 per ten thousand person-year in 1994 conscription cohort which included the HBsAg positive. 3. The incidence rate of hepatitis B was 99.98 per ten thousand person-year in HBsAg positive group and 2.25 per ten thousand person-year in HBsAg negative group. The incidence rate of the group with high SGPT and HBsAg positive was 255 times higher than that of normal population. 4. The incidence of hepatitis B in HBsAg negative group did not increase even though the probability of personal contact with HBsAg positive had been increased. from the above results, the men who have high SGPT with HBsAg positive should be excluded from military service, and it can not be said that asymptomatic HBsAg carriers influence on the hepatitis B incidence among the HBsAg negative through personal contact.

Elemental Analysis of herbal medicine, Foundry Air and Hair for the Study of Human Surroundings (인체 환경 연구를 위한 한약재, 작업장 공기 및 모발의 원소분석)

  • Hair provides important information about the body's mineral status. Therefore, we have studied the trace elemental distribution in human hair under different inhalation and ingestion environment using neutron activation analysis (NAA). NAA is a powerful analytical method which can be used successfully to determine trace elements in environmental and biological materials. Total diet and six different herbal medicine were analysed to study ingestion environment. Airborne dust in foundry was analysed and compared with outdoor dust to study inhalation environment. Human hairs of common person, herbal medicine taker and foundry worker were analysed to estimate the trace elemental distribution of people under different inhalation and ingestion environment. Analytical results show that herbal medicines contain higher micronutrients such as Ca, Mg, Mn and Cu that total diet and airborne dust in foundry has high Cr. The concentration of Ca, Mg and Mn in hair of herbal medicine taker is two times higher than in hair of common person and Cr con-centration in hair of foundry worker is about three times higher than in hair of common person. These results show that NAA can be used importantly to monitor human health through biological and environmental samples.

Application of NIOSH Lifting Equation to Analysis of Workload for Patients Transferring by Physical Therapist: a Case Study (물리치료사의 환자 운반 시 작업 부하 분석에 NIOSH 들기 공식 적용: 사례연구)

  • Purpose: The National Institute for Occupational Safety and Health (NIOSH) lifting equation (NLE) is a useful tool to ergonomically analyze a workload. The NLE has high reliability and it can assess tasks by analyzing the work process. The purpose of this case study was to try using the NLE to analyze the workload of transferring patients by physical therapists in the hospital setting. Methods: We observed a physical therapist (PT) transferring patients from a wheelchair to a tilt table and a therapeutic table in one day. Two types of patient transferring methods were evaluated; (1) the manual single person method of stand, pivot and transfer, and (2) manual two person lifting under the thigh and grasping the waist for totally dependent patients. Results: The NIOSH lifting indexes of a person grasping the waist during the manual two person lifting were 5.52~4.48 according to the patient's weight. The NIOSH lifting indexes were 3.34 and 4.48 for the tasks performed by the manual single person method. Conclusion: Because transferring patients is not done very frequently, patients transferring tasks by a PT are not included as one of the musculoskeletal disorder related risky work criteria of the Korea Ministry of Labor. But the NIOSH lifting indexes of a person grasping the waist during the manual two person lifting and the manual single person method were over the NIOSH recommended weight limit threshold.

Patient satisfaction in shoulder arthroscopy: telemedicine versus clinic follow-up visits

  • Background: The use of telemedicine for postoperative visits is increasing, especially in rural areas. Few studies have investigated its use for arthroscopic shoulder patients. This study aims to evaluate patient satisfaction with telemedicine for postoperative clinic visits following arthroscopic shoulder procedures in a rural setting. Methods: Patients were prospectively enrolled using the following exclusion criteria: <18 years, open procedures, and non-compliance follow-up at 6 weeks postoperatively. All patients completed a 13-question satisfaction survey, while telemedicine patients completed an additional, separate seven-question survey. Patients who switched groups completed a four-question prompt to determine the reasons for switching. Differences between groups were evaluated by either Student t-test or Mann-Whitney U-test. Results: The study enrolled 32 patients, with five patients following up by telemedicine and 27 in person. Age and distance from clinic were similar between patients who were assigned to the telemedicine group, completed the telemedicine visit, and opted for in-person visits (all p>0.05). Patient satisfaction did not vary significantly based on care by the surgeon, concerns being addressed, thoroughness of visit, overall clinical assessment at a prior visit, and improvements in pain and physical function (all p>0.05). Among patients who opted out of telemedicine visits, the most common reason was a preference to meet in-person but these patients agreed that telemedicine visits are a good idea. Conclusions: Regardless of type of follow-up, individuals reported similar levels of satisfaction with treatment during the visit and improvements in pain and physical function.

  • The purpose of this study was supply basic information and positive attitude needed to understand the disabled person through the analysing EMT Dept. student reaction on the disabled person. The subjects for this study were 251 students from G-college in In-Cheon City. The subjects were separated as the EMT dept. students and general dept student. The instrument used for this study were Lazer. Bonett and Granaff(1973). The scale of attitude on the disabled person. Data were analyzed by use of two-sample t-test and correlation coefficient with SAS program. Results are as follows ; 1. Frist, the EMT Dept. students showed more positive and optimistic attitude on the disabled person as safety prevention with their family life and safety prevention with first impression, but the general dept. student showed more positive and optimistic attitude on the disabled person as safety prevention with their self-consciousness, safety prevention with operative capacity, safety prevention with social life and safety prevention with school life. 2. Second, on attitude of 1 or 2 grade, first grade showed more positive and optimistic attitude on the disabled person as safety prevention with self-consciousness and safety prevention with operative capacity. Second grade showed more positive and optimistic attitude on the disabled person as safety prevention with social life, safety prevention with family life and safety prevention with school life. and first impression. But disabled person as safety prevention with self-consciousness, safety prevention with operative capacity, safety prevention with social life, safety prevention with school life and first impression. 3. Third, in interrelation effect analysing each section department and grade. They showed and interrelated more positive and optimistic attitude on the disabled person as safety prevention with their social life and first impression. As a results, it is important to understand disabled person whose physical condition. Especially, this study suggests to need to endeavor safety prevention for disabled person.

Analysis of the prescription for persons of Taiyin constitution (太陰人 Tae-eum) in the herbal formulas and cases found in classic texts (태음인 처방의 본초 조합 용례 분석 - 중요 고문헌을 중심으로 -)

  • Purpose : This study compares prescriptions in the 1901 edition of the Donguisusebowon (東醫壽世保元) and the 1901 edition of 24 Revised Tae-eum constitution prescriptions (新定太陰人病應用要藥二十四方) (1901PCDT) with medical herbs found in classical texts. This study also examines whether to include patterns which compose medical herbs for the person of Tae-eum constitution (太陰人). Method : The prescriptions for Tae-eum-in in the Shinchukbon and the ingredients of medical herbs of the prescriptions of the Complete Works of Zhang Jingyue (景岳全書), Secret Works of Universal Benefit (廣濟秘笈), Treasured Mirror of Eastern Medicine (東醫寶鑑), Comprised Edition for Formulas (方藥合編), Protecting Essence for Longevity (壽世保元), Introduction to Medicine (醫學入門), New Book for Saving People (濟衆新編 ENGLISH), and Compendium of Local Medicinals and Formulas (鄕藥集成方) were input into a database. The prescriptions were examined and combinations of medical herbs used to treat the person of Tae-eum constitution were noted. Result : Among the prescriptions for the person of Tae-eum constitution, similar examples of the 17 prescriptions of Kudzu Decoction to Relieve the Muscles (葛根解肌湯), Regulate the Stomach and Coordinate the Purity Decoction (調胃升淸湯), Clear the Heart and Lotus Pip Decoction (淸心蓮子湯), Decoction for Stabilizing Asthma with Ephedra (麻黃定喘湯), Profuse Heat and Sparse Cold Decoction (熱多寒少湯), Decoction for Coordinating Qi with Kudzu (葛根承氣湯), Major Decoction for Coordinating Qi with Kudzu (葛根大承氣湯), Minor Decoction for Coordinating Qi with Kudzu (葛根小承氣湯), Decoction for Dispersing the Exterior with Ephedra (麻黃發表湯), Boost the Lung Essence Decoction (補肺元湯), Major Supplementing Decoction with Deer Antler Velvet (鹿茸大補湯), Boosting Black Essence Pill (拱辰黑元丹), Honeylocust Fruit and Rhubarb Decoction (皂角大黃湯), Kudza and Duckweed Decoction (葛根浮萍湯), Sweet Flag and Polygala Powder (石菖蒲遠志散), Liriopis and Polygala Decoction Powder (麥門冬遠志散), and Cattle Gallstone Formula to Clear the Heart (牛黃淸心元) were found in important ancient literature and Dried Chestnut and Holotrichia Decoction (乾栗蠐螬湯) and Dried Chestnut and Tree of Heaven Root Bark Decoction (乾栗樗根皮湯) are the combination originally set by Lee Jema. Conclusion : Because 70.8% of the prescriptions in 1901PCDT could be found in literature which were pervasive in the period of the Joseon Dynasty (the period of Ming and Qing), it corresponds with Lee Jema's view that medical men in the Song, Yuan, and Ming Dynasties disclosed half of the prescriptions for Tae-eum person by studying these texts more.

A Study of Surface Electromyography Measurement of Facial Muscles in Normal Person (정상인의 안면부 운동 시 표면근전도 측정 연구)

  • Background or Objectives : The purpose of this study is to measure surface Electromyography(sEMG) of facial muscles in normal person and to find method for standardizing of sEMG's value. Methods : We measured 3points on face, frontalis muscle($GB_{14}$), zygomaticus muscle($SI_{18}$), orbicularis oris muscle($LI_{19}$) of 40 normal person by sEMG. 40 normal person consist with two groups, each 20 male, 20 female. Average age of subject was $26.50{\pm}4.79$. SEMG instrument QEMG-4 XL was used. After training exercise of facial muscles, sEMG's root mean square value was measured once. Results : 1. In whole experimental group, frontalis muscle's both side average was $78.36{\pm}40.87$, zygomaticus muscle's both side average was $84.70{\pm}49.81$, orbicularis oris's both side average was $104.83{\pm}38.81$. 2. Left side of Frontalis muscle, both side of zygomaticus muscle are high marked in male than female in statistically. 3. In whole experimental group, average of ratio comparing smaller value with bigger value in difference between left side and right side was $19.60{\pm}12.88$ %. 4. Average of asymmetry index(AI) was $11.46{\pm}8.36$ %. orbicularis oris muscle's average of AI had least difference was $8.95{\pm}7.50$ %. zygomaticus muscle's average of AI had most difference was $13.95{\pm}8.90$ %. Conclusions : The result of this study could provide useful information of field of sEMG is used in oriental medicine treatment of facial muscles. To assess efficacy of treatment in facial muscles, we need to standardize facial muscle's sEMG values by using AI, ratio comparing values and etc.

  • This study was carried out to investigate the present conditions and discuss the issues of the Subrogation payment system in emergency medicine. Hitherto preceding study is focusing on controversial of management or efficient control of Subrogation payment system in emergency medicine. The object of this study is legalistic study of the Subrogation payment system in emergency medicine. The Current legalistic issues of subrogation payment system in emergency medicine are the following aspects; Firstly, there are a claimant conformity to the standard limit. Secondly, the review system is not propriety of the promptitude. Thirdly, there is a lack of propriety claim for compensation of a support responsible person. Fourthly, there are objectivity and fairness of administrative appeal system Fifthly, the point where one starts counting of extinctive prescription. Sixthly, the administrative punishment is an illogical system. Lastly, equity and fairness of the Review Agency, as an insurance company and an review Agency are sameness In conclusion, we ought to improvement an unnecessary obstructions of promptitude in the Subrogation payment system in emergency medicine, and ensure a right of emergency medicine without delay.

Brain Activation Related to Perspective-Taking in Adolescents : Differences from Adults (관점 획득과 관련된 청소년에서의 뇌 활성화 : 성인과의 차이)

  • Objectives The purpose of this study was to investigate the differences between adolescents and adults, in the perspective-taking ability, as well as the brain activation patterns during the perspective-taking situation. Methods We recruited healthy adolescents aged 13 years to 15 years (n = 20) and adults aged 19 years to 29 years (n = 20). All the subjects were scanned while performing the perspective-taking task, in which an emotional situation was presented in the form of statements comprising first person, as well as third person perspectives. Differences in brain activation between groups were assessed by contrasting neural activity during the tasks. Results In the between-group analysis, while performing the third-person perspective-taking task, the adolescent group showed greater neural activities in the middle frontal gyrus and precentral gyrus as compared to the adult group. Positive correlation was observed between the activity in the frontal areas (Brodmann area 6/9) and the score of scales related to perspective-taking and social cognition in the adolescent group. Conclusions This study suggests that several frontal brain areas of adolescents needs to be overactivated in order to compensate for low perspective-taking ability when they ought to take another person's point of view.

Disability Weights for Diseases in Korea (한국인 질병의 장애가중치 측정 및 신뢰도 평가)

  • Objectives : This study aimed to develop an evaluation protocol of disability weights using person trade-off, and to test the reliability of the developed protocol in a Korean context. Methods : To develop the valuation protocol, the Global Burden of Disease(GBD) and the Dutch studies were replicated and modified. Sixteen indicator conditions were selected from the Korean version of disease classification, which was based on that of the GBD Study, and the person trade-off method referred to the Dutch method. Results : The disability weights were valued in a two step panel sfudy. The first step was a carefully designed group process by three panels, using person trade-off to establish the disability weights for sixteen selected indicator conditions. The second step consisted of interpolation of the remaining diseases, on a disability scale, by the individual members of three panels. The members of three panels were all medical doctors, with sufficient knowledge of the consequences of a broad variety of diseases. The internal consistency of the Korean disability weights was satisfactory. Considerable agreement existed within each panel and among the panels. Conclusions : It was feasible to use a modified evaluation protocol from those used in GBD and Dutch studies, This would provide a rational basis for an international comparative study of disability weights.
