Current Status of Children on Peritoneal Dialysis in Korea : A Cross-Sectional Multicenter Study (소아복막투석의 현황: 다기관 공동연구 결과보고)

  • Purpose : Peritoneal dialysis (PD) is the major form of dialysis in use for infants and children with end-stage renal disease (ESRD). The aim of this study was to gain insight into the current status of children on PD in Korea. Methods : In May 2008, questionnaires were sent to the pediatric nephrologists via e-mail. Four centers replied and those data were reviewed. Results : A total of 103 patients were included in this study. Male to female ratio was 1.6:1. Mean age was $11.5{\pm}4.9$ years (0-19 years). Primary renal diseases diagnosed were as follows: primary glomerular disease (34%), chronic pyelonephritis-reflux nephropathy (14.6%), systemic disease (9.7%), renal hypoplasia/dysplasia (8.7%), heredofamilial disease (6.8%), vascular disease (3.9%), drug-induced nephropathy (1.0%), and unknown (12.6%). PD modalities were as follows: CAPD (42.7%), CCPD (27.2%), NIPD (11.7%), and Hybrid (18.4%). Weekly total Kt/V was $2.1{\pm}0.7$ (0.3-4.1). Results of peritoneal equilibrium test were as follows: low 36.8%, low average 31.6%, high average 19.7%, and high 11.8%. Z-score for weight was $-1.00{\pm}1.20$ (-4.54~+2.50). Z-score for height was $-1.55{\pm}1.65$ (-9.42~+1.87). Growth hormone was administered in 24.3% of patients. Anti-hypertensive drugs were administered in 64.0% of patients. Laboratory findings were as follows: hemoglobin $10.5{\pm}1.4$ g/dL, calcium $9.7{\pm}0.7$ mg/dL, phosphorus $5.4{\pm}1.4$ mg/dL, and parathyroid hormone $324.2{\pm}342.8$ pg/mL. Conclusion : Primary glomerular disease was the most common cause of ESRD. CAPD was the most prevalent PD modality. Low and low average peritoneal transport type were common. Growth disturbance were noted in many patients. Some patients had hypertension even with anti- hypertensive drugs. Calcium-phosphorus levels were maintained adequately, but many patients had secondary hyperparathyroidism.

Effects of Cheonggisan and Gagamcheonggisan on the anti-allegic and immune responses in mice (청기산(淸肌散)과 가미청기산(加味淸肌散)이 마우스의 항(抗)알레르기 및 면역반응(免疫反應)에 미치는 영향(影響))

  • Cheonggisan(CGS) is well known for its effect on such allergic disease as urticaria and atopic dermatitis. Gagamcheonggisan(GCGS) was formulated by subtracting several herbs from CGS and adding several herbs to CGS. Even though it is being used frequently in the clinicai medicine for the treatment of above hypersensitivity diseases, basic study to make sure the mechanism of its action is rare. In this study the author tried to know the effect of CGS and GCGS on the vascular permeability, contact dermatitis, granular secretion from mast cells and function of macrophages. The results obtained in this study are as follows : 1. Administration of CGS and GCGS decreased the vascular permeability induced by serotonin and histamine. The decrease by serotonin is more typical and dose-dependent. 2. Administration of CGS and GCGS inhibited foot-pad and ear swelling responses induced by sheep red blood cells and picryl chloride respectively, the inhibition of foot-pad swelling responses is bigger than that of ear swelling responses and both of them are not dependent on the dose3. Treatment of peritoneal mast cells with CGS and GCGS water extract decreased the histamine release triggered by compound 48/80 in a dose dependent fashion 4. Administration of CGS and GCGS increased the phagocvtic activity of peritoneal macrophages and treatment of peritoneal macrophages with CGS activated phagocytic function in a dose dependent fashion. 5. Administration of CGS and GCGS enhanced such reactive oxygen intermediates(ROIs) as superoxide and hydrogen peroxide production from peritoneal macrophages. 6. Treatment of CGS and GCGS activated peritoneal macrophages for the production of ROIs. The above results show that CGS and GCGS decreased the hypersensitivity reactions by inhibiting non-specific inflammatory mediator release and vascular permeability without affecting general immune responsiveness.

  • It is now generalized to perform cardiac surgery in the patients with end-stage renal disease. The growing number of patients with chronic renal failure wake us up to the need to prepare for proper management. There are not only the prevalence of coronary artery disease, but also a great amount of valve dysfunction exist in this group. Peritoneal dialysis may be one of the obstacles for cardiopulmonary bypass but it is not a great hindrance in cardiac surgery with careful preparation and well organized perioperative care. The author has performed mitral valve replacement in a 33-year-old anuric female patient with chronic renal failure and severe mitral insufficiency. Preoperatively, the patient was kept in adequate fluid and electrolyte balance using peritoneal dialysis. Peritoneal dialysis continued and regulated according to the laboratory data in this patient during and after the surgery. She recovered well showing an uneventful course and was discharged on postoperative 1 th day.

Radionuclide Peritoneal Scintigraphy in Patients with Ascites and Pleural Effusion (방사성핵종 복막촬영술을 이용한 복수에 동반된 수흉의 감별 진단)

  • Simultaneous presence of ascites and pleural effusion has been documented in patients with cirrhosis of the liver, renal disease, Meigs' syndrome and in patients undergoing peritoneal dialysis. Mechanisms proposed in the formation of pleural effusion in most of the above diseases are lymphatic drainage and diaphragmatic defect. But sometimes, hepatic hydrothoraxes in the absence of clinical ascites and pleural effusion secondary to pulmonary or cardiac disease are noted. It is not always possible to differentiate between pleural effusion caused by transdiaphragmatic migration of ascites and by other causes based soly on biochemical analysis. Authors performed radionuclide scintigraphy after intraperitoneal administration of $^{99m}Tc-labeled$ colloid in 23 patients with both ascites and pleural effusion in order to discriminate causative mechanisms responsible for pleural effusion. Scintigraphy demonstrated the transdiaphragmatic flow of fluid from the peritoneum to pleural cavities in 13 patients correctly. In contrast, in 5 patients with pleural effusion secondary to pulmonary, pleural and cardiac diseases, radiotracers fail to traverse the diaphragm and localize in the pleural space. Ascites draining to mediastinal lymph nodes and blocked passage of lymphatic drainage were also clarified, additionaly. Conclusively, radionuclide peritoneal scintigraphy is an accurate, rapid and easy diagnostic tool in patients with both ascites and pleural effusion. It enables the causes of pleural effusion to be elucidated, as well as providing valuable information required when determining the appropriate therapy.

Frequently Consumed Dishes and Development of Recipes to Improve Bone Mineral densities in Continuous Ambulatory Peritoneal Dialysis Patients with Osteopenia (골감소증을 동반한 지속성 복막투석환자의 다빈도섭취 음식조사 및 골밀도 개선을 위한 레시피 개발)

  • The purpose of this study was to assess the frequently consumed dish consumption frequencies of continuous ambulatory peritoneal dialysis(CAPD) patients with osteopenia and develop recipes to improve bone mineral density of CAPD. The subjects were 96 CAPD patients with osteopenia(male 39, female 57) (osteopenia group) and 45 CAPD patients with normal BMD(male 24, female 21), matched with key variables(normal group). Fifty dishes(foods) that most frequently consumed were determined and food consumption frequency for each dish(food) for two groups were compared. Osteopenia group showed lower consumption frequency for ice-cream but higher frequency in apple. Of the 50 most frequently consumed dishes(foods), 20 dishes assessed as safe and recommendable for CAPD patients with osteopenia based on the contents of protein and mineral were selected : white boiled rice, white gruel, beef soup, steamed cabbage, roasted dried laver, fried egg, roasted bean-curd, cooked and seasoned bean sprouts, corn-starch jelly, cheese, ice-cream, orange juice, apple, grape, peach, peanut, raw lettuce, raw cucumber, and injulmi rice cake. wenty eight new dishes with modified recipes were developed for CAPD patients. Protein and mineral contents were analyzed for frequently consumed 17 dishes, assessed as modification of recipes are needed. The recipes were modified to decrease P, Na and K contents and to increase protein and Ca contents. Twenty dishes(foods) selected as having reasonable protein and mineral contents ratio or 28 newly developed dishes modified with protein and mineral contents or ratio would be helpful for nutrition education or counseling for CAPD patients with osteopenia. Dishes(foods) suggested in this study would also be useful for all CAPD patients for preventing osteoporosis.

Clinical Characteristics of Inguinal Hernia in Children on Peritoneal Dialysis (복막투석 중인 소아에서 발생한 서혜부 탈장의 임상상)

  • Peritoneal dialysis (PD) has been utilized for the children with end stage renal disease. Nevertheless, it is thought to promote inguinal hernia by increasing intraabdominal pressure. To investigate the clinical characteristics of inguinal hernia in children on PD, 155 cases of PD in children between January 1996 and June 2007 at Seoul National University Children's Hospital were reviewed retrospectively. Inguinal hernia developed in 16 cases (10.3 %, M:F=8:8). Hernia occurrence was not correlated to age. Eleven cases (69 %) of inguinal hernia developed in first 6 months after initiation of PD. All inguinal hernias were surgically repaired. No complications occurred related to inguinal hernia or surgery. Recurrent hernia developed in 1 patient (6.3 %) of 2 cases who had PD postoperatively on the day of surgery. In conclusion, inguinal hernia developed more frequently with children on PD than general population (3.5~5 %). The rate of hernia development was highest within the first 6 months following initiation of PD. After repair of hernia, we recommend to discontinue PD immediate postoperatively to prevent recurrence.

Severe hypermagnesemia presenting with abnormal electrocardiographic findings similar to those of hyperkalemia in a child undergoing peritoneal dialysis

  • In this report, we present a pediatric case of severe symptomatic hypermagnesemia resulting from the use of magnesium oxide as a laxative in a child undergoing continuous cyclic peritoneal dialysis for end-stage renal disease. The patient showed abnormal electrocardiography (ECG) findings, such as tall T waves, a widened QRS complex, and irregular conduction, which were initially misdiagnosed as hyperkalemia; later, the correct diagnosis of hypermagnesemia was obtained. Emergent hemodialysis successfully returned the serum magnesium concentration to normal without complications. When abnormal ECG changes are detected in patients with renal failure, hypermagnesemia should be considered.

  • Disseminated peritoneal leiomyomatosis (DPL) is a very rare benign disease, characterized by multiple solid subperitoneal or peritoneal smooth muscle nodules in abdominopelvic cavity and malignant transformation is extremely rare. Also, uterine smooth muscle tumors of unknown malignant potential (STUMP) is a rare tumor, which is regarded as subclassification in uterine smooth muscle tumors between benign and malignant criteria. Pathogenesis of DPL is uncertain, but increasing evidence of iatrogenic cause including laparoscopic myomectomy has been reported. We report a case of a 28-year-old female with previous history of laparoscopic myomectomy diagnosed with DPL with atypical feature and concurrent uterine STUMP using computed tomography (CT) and magnetic resonance imaging (MRI), as well as present a review of the literature.

Inhibition of Immediate Allergic Reaction by Cryptotympana atrata (선퇴에 의한 즉시형 알레르기 반응의 억제)

  • Effects of the aqueous extract of Cryptotympana atrata Fabricius (CAF) on the allergic reactions were investigated. In the present study, we examined the effect of CAF on compound 48/80-induced anaphylaxis in vivo and histamine release from rat peritoneal mast cells in vitro. CAF dose-dependently inhibited systemic anaphylaxis induced by compound 48/80 in mice. CAF significantly inhibited serum histamine levels induced by compound 48/80. CAF also inhibited histamine release from the rat peritoneal mast cells activated by compound 48/80. These results suggest that CAF may be useful for the prevention and treatment of allergy related disease.

Chrysanthemum morifolium inhibits inflammatory responses in IFN-γ and LPS-induced mouse peritoneal macrophages

  • Chrysanthemum morifolium (CM) is a herb widely used in medicine for the treatment of a variety of diseases. In this study, using mouse peritoneal macrophages, we have examined whether CM affects nitric oxide (NO), tumor necrosis factor $(TNF)-\alpha$ and interleukin (IL)-6 induced interferon $(IFN)-\gamma$ and lipopolysaccharide (LPS). CM inhibits $IFN-\gamma$ and LPS-induced NO in dose dependent manner. We also found that CM inhibits pro-inflammatory cytokine, $TNF-\alpha$ and IL-6. The expression of cyclooxygenase-2 was reduced by CM. These finding means that CM can be used in controlling macrophages-mediated inflammatory disease.
