손발저림의 원인(原因)에 대(對)한 동서의학적(東西醫學的) 고찰(考察) (Consideration of the Son-Bal Jeorim in oriental and western medicine)

  • Objectives & Methods: We investigated 28 books to study etiology and pathology of Son-Bal Jeorim. Result and Conclusion 1. The eiology of Son-Bal Jeorim is same as it of Bee Jeung(痺症). 2. Generally speaking, the cause of Bee Jeung was distributed Wind(風), Coldness(寒), Wetness (濕) of meridian. Bee Jeung can be devided into SilBi(實痺) and HeoBi(虛痺). In SilBi(實痺) there are PungHanSeupBi(風寒濕痺) and YeolBi(熱痺). In HeoBi(虛痺), there are GiHyeolHeoBi(氣血虛痺), EumheoBi(陰虛痺) and YangHeoBi(陽虛痺). 3. Son-Bal Jeorim belong to peripheral neuropathy in western medicine. 4. Syndrome of acute motor paralysis with variable disturbance of sensory and autonomic function, subacute sensorymotor paralysis, syndrome of chronic sensorimotor polyneuropathy, neuropathy with mitochondrial disease, syndrome of mononeuropathy or nerve plexusopathy. 5. Peripheral neuropathy is caused by carpal tunnel syndrome, diabetic neuropathy, uremic neuropathy, hepatic neuropathy, hypothyroid neuropathy, hyperthyroid neuropathy, neuropathy due to malnutrition, neuropathy due to toxic material, neuropathy due to drug, paraneoplastic neuropathy, hereditary neuropathy, etc. 6. Cerebral apoplexy, myelopathy, peripheral circulatory disturbance, anxiety syndrome cause symptoms of peripheral neuropathy

말초신경질환과 하지불안증후군의 연관성에 관하여 (The Relation of Restless Legs Syndrome with Diseases of Peripheral Nerves)

  • Many neurologists, particularly whose subspecialty is in peripheral neurology, may agree that patients with peripheral neuropathy often complain of the "restless legs" symptoms. These symptoms seem to share the typical features of the so-called "restless legs syndrome (RLS)", i.e., unpleasant sensations in the leg/feet, worsening in the evening or at night, and the partial relief of the positive sensory symptoms by the movements such as walking, shaking or rubbing. In fact, a higher incidence of RLS was reported among the neuropathic patients, and peripheral neuropathy was found to be more prevalent in patients with RLS than in general population. Moreover, RLS share many risk factors with peripheral neuropathy such as diabetes, uremia, amyloidosis and cryoglobulinemia, which suggests that peripheral neuropathy may play a pathophysiologic role in the development of RLS.

말초신경병증으로 추정되는 비증(痺證) 치험 1례 (A Case of Treating the Paresthesia Suggested from Peripheral Neuropathy)

  • Peripheral neuropathy is a disease of multiple Peripheral nerves. Tingling, pins-and needles, numbness, burning and raw sensation are symptoms of peripheral neuropathy. These symptoms are frequently complained by patients who suffered from obstructive syndrome of Ki and blood (痺證). This case was reported to evaluate the effects of oriental medicine therapy on a patient with obstructive syndrome of Ki and blood (痺證). The subject was a female patient who had obstructive syndrome of Ki and blood (痺證). She complained of calf pain, cold sense, walking discomfort, insomnia, dyspepsia, anorexia etc. We administered the medicine with dispeling the cold pathogenic factor (寒邪), adjusting the constructive and defensive energy (調營衛), tonifying the Ki and blood (補氣血) by stage. After the treatment, the symptoms improved. This case suggests that oriental medicine therapy can be applicable to improve in symptoms with Peripheral neuropathy.

항암제 투여 후 발생한 수족비증(手足痺症) 환자 치험례 (Traditional Korean Medical Treatment for Chemotherapy-induced Peripheral Neuropathy - a Case Report)

  • Chemotherapy has many known side effects. Neuropathy is a common side effect when using cisplatin. The clinical course varies and depends on the agents used and their cumulative dose. Although symptoms can resolve completely, in most patients chemotherapy-induced peripheral neuropathy is either only partially reversible or completely irreversible. Current management for chemotherapy-induced peripheral neuropathy is symptomatic relief using membrane stabilizing medications and antidepressants. Dysaesthesia and pain involving the feet and hands are described in traditional korean medicine. In traditional korean medicine, the pathogenesis is related to the inability to direct Qi and Blood to the extremities, and is associated with Qi, Blood, Yang and Kidney deficiencies. We report a case of a 52 years old female patient treated with acupuncture and bleeding treatment for the goal of improving the symptoms of chemotherapy induced peripheral neuropathy. Further study and other medical method will be needed in order to improve the effect and determine the long-term effect of traditional korean medicine in treating Chemotherapy-induced Peripheral Neuropathy.

항암화학요법으로 인한 암환자의 말초신경병증 관련 특성과 삶의 질 (Characteristics and Quality of Life in Patients with Chemotherapy-Induced Peripheral Neuropathy)

  • Purpose: The purpose of study was to identify how patients experienced chemotherapy-induced peripheral neuropathy (CIPN) and quality of life related to CIPN. Methods: This was a descriptive research. We collected data from 105 patients with chemotherapy-induced peripheral neuropathy. They completed a self-reported questionnaire including Eastern Cooperative Oncology Group (EORTC) CIPN20 and items related to their disease and peripheral neuropathy. The investigators filled in part of items about disease and treatment. Results: In the study, duration of peripheral neuropathy was 9.4 month and 54.3% of patients used pharmacological or non-pharmacological interventions. Patients reported the highest score for sensory scale and it's score was $38.74{\pm}20.24$. The scores for motor scale and autonomic scale were $21.95{\pm}19.19$ and $26.61{\pm}21.0$ respectively. This showed that patients more suffered from sensory neuropathy than any other domain of neuropathy. The most frequently selected two items were 'did you have tingling fingers or hands?' and 'did you have tingling toes or feet?'. Conclusion: The results of this study will provide useful information for chemotherapy-induced peripheral neuropathy.

Paraproteinemic neuropathy

  • Paraproteinemia is caused by a proliferation of monoclonal plasma cells or B lymphocytes. Approximately 10% of idiopathic neuropathies are associated with paraproteinemia, where a certain paraprotein acts like an antibody targeted at constituents of myelin or axolemma in peripheral nerves. The relationship between paraproteinemia and peripheral neuropathy remains unclear despite this being of interest for a long time. Neurologists frequently find paraproteinemia during laboratory examinations of patients presenting with peripheral neuropathy, especially in the elderly. The possibility of a relationship with paraproteinemia should be considered in cases without an explainable cause. We review the causal association between paraproteinemia and neuropathy as well as clinical, laboratory, and electrophysiologic features, and the treatment options for paraproteinemic neuropathy.

  • The paraproteinemia is a disorder in which a single clone of plasma cells (monoclonal gammopathy) is responsible for the proliferation of monoclonal proteins (M-proteins). Approximately 10% of patients with idiopathic peripheral neuropathy have monoclonal gammopathy. Some M-proteins have the properties of an antibody to the components of peripheral nerve myelin, but the pathophysiological relationship between the neuropathy and the M-protein is often obscure. The relationship between peripheral neuropathy and monoclonal gammopathy requires the appropriate neurological and hematological investigations for precise diagnosis and treatment. In this review, we provide an update on the causal associations between peripheral neuropathy and monoclonal gammopathy as well as characteristics of clinical and electrophysiologic features.

  • Carbon monoxide (CO) intoxication is a leading cause of the variable neuropsychiatric impairment. Despite of widely known central nerve system complications after CO intoxication, peripheral neuropathy due to CO poisoning is rare and has been under-recognized. We report interesting case of a 29-year-old male who suffered from motor weakness and sensory abnormalities in his lower extremity following acute CO intoxication. The patient revealed direct and indirect signs of peripheral neuropathy of the left inferior gluteal and sciatic nerve on magnetic resonance imaging.

  • Ischemic monomelic neuropathy and myopathy are rare complications of peripheral arterial occlusive disease. We report a case of ischemic monomelic neuropathy of the right sural, common peroneal and posterior tibial nerves and ischemic myopathy of the right tibialis anterior resulting from the occlusion of the right common femoral arteries despite successful revascularization. Ischemic monomelic neuropathy and myopathy can occur as a result of occlusion of the specific peripheral artery.

암 환자의 말초신경병증 관련 일상활동장애와 삶의 질: 심리적 디스트레스의 매개효과 (Disturbance in ADL from Chemotherapy-induced Peripheral Neuropathy and Quality of Life in Cancer Patients: The Mediating Effect of Psychological Distress)

  • Purpose: The purpose of this study was to examine the mediation of psychological distress in the relationship between disturbance in ADL from chemotherapy induced peripheral neuropathy and quality of life in order to provide a basis for planning nursing interventions to improve the quality of life in cancer patients. Methods: A purposive sample of 130 patients treated with chemotherapy were recruited in the cross-sectional survey design. Data were collected using self-report questionnaires. The instruments were the Chemotherapy Induced Peripheral Neuropathy Assessment Tool (CIPNAT), Hospital Anxiety Depression Scale (HADS), and Functional Assessment of Cancer Therapy-General (FACT-G). Results: The mean score for disturbance in ADL from chemotherapy induced peripheral neuropathy was 3.30. Overall quality of life was 2.48. The mean score was 1.04 for psychological distress. The prevalence was 35.4% for anxiety and 47.7% for depression. There were significant correlations among the three variables, disturbance in ADL from chemotherapy induced peripheral neuropathy, psychosocial distress, and quality of life. Psychosocial distress had a complete mediating effect (${\beta}$= -.74, p <.001) in the relationship between disturbance in ADL from chemotherapy induced peripheral neuropathy and quality of life (Sobel test: Z= -6.11, p <.001). Conclusion: Based on the findings of this study, nursing intervention programs focusing on disturbance of ADL management, and decrease of psychological distress are highly recommended to improve quality of life in cancer patients.
