Baicalin suppresses lipopolysaccharide-induced matrix metalloproteinase expression: action via the mitogen-activated protein kinase and nuclear factor κB-related protein signaling pathway

  • Periodontal disease is an inflammatory disease that affects the destruction of the bone supporting the tooth and connective tissues surrounding it. Periodontal ligament fibroblasts (PDLFs) induce overexpression of matrix metalloproteinase (MMP) involved in periodontal disease's inflammatory destruction. Osteoclasts take part in physiological bone remodeling, but they are also involved in bone destruction in many kinds of bone diseases, including osteoporosis and periodontal disease. This study examined the effect of baicalin on proteolytic enzymes' production and secretion of inflammatory cytokines in PDLFs and RAW 264.7 cells under the lipopolysaccharide (LPS)-induced inflammatory conditions. Baicalin inhibited the expression of the protein, MMP-1 and MMP-2, without affecting PDLFs' cell viability, suggesting its possibility because of the inhibition of phosphorylation activation of mitogen-activated protein kinase's p38, and the signal transduction process of nuclear factor κB (NFκB)-related protein. Also, baicalin reduced the expression of MMP-8 and MMP-9 in RAW 264.7 cells. This reduction is thought to be due to the inhibition of the signal transduction process of NFκB-related proteins affected by inhibiting p65RelA phosphorylation. Also, baicalin inhibited the secretion of nitric oxide and interleukin-6 induced by LPS in RAW 264.7 cells. These results suggest that baicalin inhibits connective tissue destruction in periodontal disease. The inhibition of periodontal tissue destruction may be a therapeutic strategy for treating inflammatory periodontal-diseased patients.

Periodontal Wound Healing of the Experimental Subepithelial Connective Tissue Graft in Dogs (성견의 실험적 상피하 결합조직 이식시의 치주조직의 치유)

  • Several methods have been used for regeneration of tissue lost by periodontal disease. Subepithelial connective tissue graft technique, one of the technniques of mucogingival surgery, is used for the regeneration in esthetic problems such as recession, and denuded root coverage. This study is performed to evaluate the healing process and the regeneration and reattachment of periodontal tissue, including the reconstruction of junctional epithelium, and connective tissue. Alveolar defects in five adult dogs were treated with periodontal surgery and were attained by removing the marginal alveolar bone by $4{\time}3mm$ from CEJ in the labial side of incisors, and root surfaces were planed. The experimental sites were divided into two groups as follows. 1. root planing alone(control group) 2. with connective tissue graft (Experimental Group) In the two groups flaps were positioned and sutured tightly, the healing processes were observed and were histologically compared with each other after 2days, 4days, 1week, 2weeks, 4weeks. The results were obtained as follows : 1. In the two groups blood clots were observed as early as 2 and 4 days, and were resorbed at 1 week. 2. In the two groups moderate inflammation was observed as early as 2 and 4 days, decreased at 1 and 2 weeks, and disappeared at 4 weeks. 3. Junctional Epithelium migration was more significant in the control group, and was restrained by graft materials in the experimental group. 4. Features of connective tissue fiber attachment partially showed the parallel pattern in the two groups from 2 weeks, and entirely from 4weeks. 5. Anastomosis, between graft and connective tissue, appeared from 4 days in the experimental group and the border between them was not discriminated at 4weeks.

THE EFFECTS OF HORIZONTAL ORTHODONTIC TOOTH MOVEMENT ON THE PERIODONTALLY DISEASED TISSUE IN DOGS (성견 치주질환 이환치아의 수평이동이 치주조직에 미치는 영향)

  • Most adults, unlike growing children, have some periodontal problems which can influence the outcome of the orthodontic treatment. In cases where periodontal disease progression resulted in marked reduction of periodontium, orthodontic treatment could result in the worsening of the periodontal conditions, and therefore orthodontic treatment planning in such adult patients requires special considerations for the periodontal problems. This study investigates the effects of horizontal orthodontic tooth movement on the changes in the mesial, distal and furcation areas of the disease affected periodontium of adult dogs with advanced bone loss. Six adult dogs with healthy periodontium were selected, and mandibular 2nd premolars were extracted. In the mandibular 3rd premolars, angular bony defects in the mesial and distal sides, and horizontal bony defects in the furcation areas were created. Those that received the flap operation and plaque control were designated as the control, those that had horizontal tooth movement without plaque control after the flap operation as Experimental group I, those that had horizontal tooth movement under plaque control without the flap operation as Experimental group II, and those that had horizontal tooth movement under plaque control after the flap operation as Experimental group III. The control group was sacrificed 2 months postoperatively, and the experimental groups were sacrificed 5 months after the initiation of tooth movement. Specimens were histologically analyzed under light microscope. The results were as follows; 1. After the horizontal tooth movements, Experimental group I and II showed angular bony defects in the mesial sides of the roots and the distal side of the furcation areas, which correspond to the pressure sides. 2. After the horizontal tooth movements, Experimental group I and II showed decreased level of alveolar bone crest in the distal sides of the roots, which correspond to the tension sides. 3. Long junctional epithelium in the control group has not been replaced by periodontal connective tissue after the horizontal tooth movements. 4. Limited formation of new bone was observed in the angular bony defects in the mesial and distal aspects of the roots in the control group. 5. Inflammatory cell infiltration in the connective tissue was most severe in the Experimental group I, followed by Experimental group II, III, and the control group in that order. These results seem to indicate that plaque control was the most influencing factor in the alteration of the periodontal tissue after the horizontal tooth movements in the periodontal tissue with alveolar bone defects.

  • Purpose: In advanced case of periodontitis, surgical treatment without bone contouring may result in residual pockets inaccessible to proper cleaning during post-treatment maintenance. This problem can be avoided or reduced by applying guided tissue regeneration. Materials and Methods: All of 3 patients had deep periodontal pocket depth and bleeding on probing, and radiograph revealed osseous defect, so we planned guided tissue regeneration using resorbable membrane with or without xenograft. Result: 6 months later, periodontal pocket depth and bleeding on probing was improved and gingiva was stable. Conclusion: Guided tissue regeneration using resorbable membrane with or without xenograft in osseous defect is predictable.

  • Electrospinning was performed using an eco-friendly solvent composed of acetic acid, ethyl acetate and distilled water to investigate the effect of gelatin concentration on mechanical properties and cytotoxicity of absorbable poly(D,L-lactic acid) (PDLLA)/gelatin blend membrane. The tensile stress, strain at break, and WUC of the PDLLA/gelatin (97/3) scaffold at 26 wt% concentration were determined to be 3.9 ± 0.7 MPa, 37 ± 1.3 %, and 273 ± 33 %, respectively. FT-IR results revealed that PDLLA and gelatin were bound only by van der Waals interactions. The cell viability of PDLLA/gelatin membranes containing 0 %, 1 %, 2 %, 3 %, and 4 % gelatin were more than 100 %, which makes all membranes highly suitable as a barrier membrane for absorbable periodontal tissue regeneration due to their marketed physical properties and biocompatibility.

AN ELECTRON MICROSCOPIC STUDY ON THE TISSUE CHANGES IN THE PRESSURE ZONES OF RAT MOLAR PERIODONTIUM INCIDENT TO ORTHODONTIC TOOTH MOVEMENT (백서구치의 교정적 치아이동중 압박측 치주조직의 초기변화에 관한 연구)

  • Incipient changes of the periodontal tissue in the pressure zones of rat molar subjected to the experimental force were studied by the transmission electron microscope. Experimental animals were consisted in 3 control and 21 experimental rats, of which one maxillary first molar was moved buccally with a fixed appliance which were exerting the force of 15 gm. After experimental period of 1 hour, 3 hours, 6 hours, 24 hours, 2 days, 3 days and 7 days, the animal were sacrificed with cardiac perfusion of $2.5\%$ glutaraldehyde in the sodium cacodylate buffer and the experimental teeth with surrounding periodontal structures were processed for electron microscope. At the beginning of the tooth movement, periodontal ligaments of the pressure were compressed and collagenous fibers were arranged parallel to the root of the teeth and cell free zones in company with cell necrosis were followed. Cell free zones at the periodontal ligaments appeared in the 3 hour survival group, and getting severe with time lapse it became widespread in 2-3 day survival group and undermining bone resorption as a healing process was observed in 7 day survival group. Dilatation of mitochondria and swelling of the rER in the fibroblast and other connective tissue cells in the periodontal ligament were observed in the 3 hour survival group, which were characteristics of the incipient changes in the compressed periodontal ligament. Dilatation of nuclear membrane and pyknosis were followed by the destruction of the nucleus and cell membrane. There were no evidence in cell damage or necrosis of the alveolar bone adjacent to the hyalinized area of periodontal ligaments.

  • Objective : This study was performed to evaluate the effects of Palmijihwang-hwan (Baweidehuang-wan) and Obaeja (Galla Rhois) on the regeneration of periodontal tissue. Methods : In this study, we used MC3T3-El cells, such as osteoblast-like cell lines and human periodontal ligament fibroblasts, for experimental material. We separated each type of cells into a control group and an experimental group. In the control group, the cells were cultivated for 48 hours with distilled water and media which contained 10% fetal bovine serum (FBS) and penicillin (l00unit/ml)-streptomycin ($l00{\mu\textrm{g}}/ml$) at $37^{\circ}$ in 5% $CO_2$ gas. In the experimental group, the cells were cultivated for 48 hours with Palmijihwang-hwan extract and Obaeja extract (concentrations $1{\mu\textrm{g}}/ml,{\;}25{\mu\textrm{g}}/ml,{\;}50{\mu\textrm{g}}/ml$) under the same conditions as the control group. Investigating the regeneration of periodontal tissue was performed by evaluating proliferation, the activity of alkaline phosphatase and the synthetic ability of proteins using those cultivated cells by means of microculture tetrazolium (MTT) assay, alkaline phosphatase substrate kit and protein assay kit. Results : 1. In vitro, Palmijihwang-hwan extract increased the proliferation of MC3T3-El cells. 2. In vitro, Obaeja extract increased the activity of alkaline phosphatase and the synthetic ability of protein in MC3T3-El cells and human periodontal ligament fibroblasts depending on Obaeja extract's concentration. Conclusion : Obaeja extract can be developed as a subsidiary medicine for the regeneration of periodontal tissue. Further studies to evaluate the different concentrations the Obaeja extract and clinical trials in vivo are suggested.

  • The goal of periodontal therapy is the regeneration of the periodontium lost by periodontal disease. The purpose of this study was to evaluate the regenerative potential of the autogenous bone graft and guided tissue regeneration in the treatment of periodontal bony defect in dogs. Experimental periodontitis were induced in the mandibular left 3rd premolar and right 3rd and 4th premolars of 5 dogs using orthodontic ligature wire. After 6 weeks, the ligature wire removed, surgical procedure were performed as follows. 1) control group : Flap operation(Mn.Lt 3rd premolar) 2) experimental group I : Flap operation + autogenous bone graft (Mn.Rt. 3rd premolar) 3) experimental group II : Flap operation + Gore-Tex membrane (Mn.Rt. 4th premoalr) Thereafter, dogs were sacrificed on the 1,2,4,8,16th week and the specimens were prepared and stained with hematoxyline-eosin stain for the light microscopic examination. The results of this study were as follows. 1. The apical migration of junctional epithelium was most remarkable in the flap operation and the experimental group II was less than the experimental group I. 2. In the formation of new alveolar bone, it was found in experimental group I,II and experimental group I is more than II. In the control group, few bone formation was found. 3. In the formation of new cementum, it was found in experimental group I,II and experimental group II is more than I. So, the periodontal therapy combined with autogenous and guided tissue regeneration will be produce the periodontal regeneration.

  • The investigation was undertaken to determin the altered function and structure of granulation tissue fibroblasts. Human granulation-tissue fibroblasts were cultured from periodontal chronic inflammatory lesions (SBI index : above 3) and compared with healthy gingival connective tissues fibroblasts a control(SBI index : below 1). Granulation tissue fibroblasts proliferated with a slower growth rate and exhibited larger cell size than control cells. Total protein profile of granulation tissue fibroblasts was almost identical to that of control cells with some exception. These results support tha theory that granulation tissue fibroblasts represent a distinct phenotype of fibrotic cells.

A Retrospective Study of the Clinical Outcome of Guided Tissue Regeneration in infrabony defects (비흡수성 차폐막을 이용한 치조골재생술의 임상적 효과)

  • The purpose of this study was to evaluate the extent and predictability of periodontal regeneration with barrier membranes in deep infrabony defects. 25 patients(40% smokers) were included in this study. Fourty-one deep infrabony defects treated with membranes(PPD>6mm) were evaluated 1 year postoperatively following a plaque control regimen. Probing pocket depth(PPD), gingival recession(REC), and probing attachment level(PAL) were evaluated at baseline and postoperative 1 year. Plaque score at baseline was 16.2 and plaque score at 1 year was 9.9 A PAL gain of $4.1{\pm}2.5mm$ along with a PPD reduction of $5.0{\pm}2.3mm$ were observed. A PAL gain of $4.1{\pm}2.5mm$ was observed at the smoking group and a PAL gain of $4.0{\pm}2.5mm$ was observed at the non-smoking sroup. It was concluded that periodontal regeneration with membrane represented the predictable and effective treatment modality in the deep infrabony defects.
