  • Periodontal disease, trauma, deformity of tooth can jeopardize the esthetics of oral and maxillo-facial region. Moreover, increasing the demand and concern about the esthetics, clinicians should place high value on esthetics during periodontal treatment. Analysis of various anatomical considerations; lip line, shape and location of marginal gingiva, and biologic width; and diagnosis should be performed prior to periodontal plastic surgery.

  • Since they were introduced by Ward in 1923, periodontal dressing have been routinely used following the periodontal surgery to avoid pain, infection, desensitizing teeth, inhibiting food impaction of the surgical areas, and immobilizing injured areas. Recently, however, the value of periodontal dressings and their effects on periodontal wound healing have been questioned, several authors have been reported that the use of dressing has little influence on healing following periodontal surgical procedures. In addition, there is evidence that when good flap adaptation is achieved, the use of a periodontal dressing does not add to patient comfort nor promote healing. The purpose of this study was to evaluate patient postoperative pain experience and discomfort with and without the use of periodontal dressing following periodontal surgery. Twenty-eight patients, 11 male and 17 female. were selected for this study; The age range was 31 to 56, with an average of 40.2years. Patient selection was based on existence of two bilateral sites presenting similar periodontal involvement, as determined by clinical and radiographic assessment, and requiring comparable bilateral surgical procedures. Using a splitmouth dressing. one site received a periodontal dressing while the other site did not. Pain assessment was made according to a horizontal, rating scale(0-10). After at least a two-week period, the second surgical precedure was performed using the alternate postoperative treatment. At the conclusion of the trial, a self-administered questionnaire on postoperative experience was administered, and were asked of their preference of either, dressed or undressed. The results were as follows: 1. A similar trend for mean pain and discomfort scores as assessed by patients both dressed and salinetreated procedures was evident during 7-day postsurgical period. 2. Statistical analysis of differences between the dressed side and salinetreated side with respect to pain, discomfort and patient's experiences revealed that both treatment sides behaved similarly at any postoperative day(P>0.05). 3. Considering the patient's preference, on the basis of pain and discomfort experienced, 43% preferred the saline-mouthwash and 32% preferred the dressing, 25% showing no preference for either the dressing or the saline-mouthwash. There is evidence to support the use of a periodontal dressing in retention of an apically positioned flap by preventing coronal displacement, or its use to provide additional support to stabilize a free gingival graft. However, there will always be a use for periodontal dressing although routine use of dressings may decrease because of better surgical techniques and the use of antibacterial mouth rinses.

Relationship between Early Menopause and Periodontal Disease in Korean Postmenopausal Women

  • The purpose of this study was to evaluate the relationship between early menopause and periodontal disease in postmenopausal women using data from the 6th Korean National Health and Nutrition Examination Survey (2013~2015). A study was conducted with 2,048 postmenopausal women aged 45 to 74 years. Participants were divided into the early menopause group (menopause occurring at age 45 years or before) and normal menopause group (menopause occurring after age 45 years). A community periodontal index greater than or equal to code 3 was used to define periodontal treatment needs. A chi-square test was used to confirm the bivariate relationship between the variables. Multinomial logistic regression analysis was used to adjust for covariates (age, education, family income, body mass index, alcohol drinking, smoking, visiting dentist in the last year, use of oral care products, frequency of tooth brushing per day). The risk of periodontal disease was higher in the early menopause group after adjusting for potential confounders (odds ratio, 1.59). In particular, the relationship between early menopause and periodontal disease was more evident in women with low education and those who did not use oral care products. The findings of this study suggest that early menopause is a significant factor of periodontal disease in Korean women.

서울시 및 경기도 지역에 거주하는 청소년의 치주상태에 의한 치주처치수요도에 관한 연구 (A STUDY ON PERIODONTAL CONDITIONS AND TREATMENT NEEDS OF ADOLESCENTS IN SEOUL SPECIAL CITY AND KYOUNGKI-DO)

  • The author made a periodontal survey on 2,000 Korean adolescents in Seoul special city and Kyoungki-Do, to detect periodontal conditions and treatment needs. The obtained results were as follows: 1. 1.20% of adolescents had healthy periodontal tissues. 2. 11.95% showed only the gingival bleeding on probing, 66.55% had the calculus. 3. The proportions of adolescents with pocket depths about 4 to 5mm and over 6mm were 19.95% and 0.35% respectively. 4. Periodontal treatment need proportion showed 98.80% in oral hygiene instruction and 86.65% in oral prophylaxis; mean number of sextants affected was 2.96. 5. 0.35% was needed complex periodontal treatment. 6. In the sex and regional distribution, girls were slightly higher than boys in periodontal treatment needs and seoul than Kyoungki-Do; but the difference was not significant statistically.(p<0.05) 7. In the age distribution, periodontal treatment needs did not increase evidently with age; but comparing those for 15 years old with 19, the latter was slightly higher than the former.(p<0.05)
