의과대학 교육과정 개편에 대한 인식과 교육과정 평가 (Pereeption on curriculum reformation and evalution of medical school)

  • Purpose: The aim of this study is to identify the elements of success in curriculum reformation using focus group interview and faculty survey. Methods: We analyzed the 105 questionnaires that were collected horn 450 faculties in Yonsei University College of Medicine from September 1 to September 12, 2005. The interview of focus group was conducted to 105 faculty and students using 10 interview questions from July 25 to August 29, 2005. The data of focus group interview and faculty survey were analyzed by content analysis. frequency and percentile. Results : 66.7% of faculty have pointed out that it is right a direction of new curriculum but there are some problems in curriculum management. The most frequently addressed problem in the new curriculum is as follows : ability of self-directed learning(15.3%). deficiency of faculty understanding(10.5%). The faculty comments on the improvement areas of new curriculum such as curriculum revision (8.6%). efficiency of curriculum management (6.7%), reward and incentive system of education(6.7%). The focus groups pointed out the important factors of curriculum reformation such as horizontal and vertical integrity between subjects, improvement of student achievement test, enhancement of self-directed learning. responsibility and authorities of course director. Conclusions : It should be concluded. to succeed curriculum reformation, which the purpose of curriculum reformation announces obviously and management group should effort continuously so that professors may understand the new curriculum. The course directors have to responsibility and authorities operating the subjects and the university should develop the students' evaluation system and faculty performance appraisal system.

National Registry Data from Korean Neonatal Network: Two-Year Outcomes of Korean Very Low Birth Weight Infants Born in 2013-2014

  • Background: The aim of this study was to observe long-term outcomes of very low birth weight infants (VLBWIs) born between 2013 and 2014 in Korea, especially focusing on neurodevelopmental outcomes. Methods: The data were collected from Korean Neonatal Network (KNN) registry from 43 and 54 participating units in 2013 and 2014, respectively. A standardized electronic case report form containing 30 items related to long-term follow up was used after data validation. Results: Of 2,660 VLBWI, the mean gestational age and birth weight were $29^{1/7}{\pm}2^{6/7}$ weeks and $1,093{\pm}268g$ in 2013 and $29^{2/7}{\pm}2^{6/7}$ weeks and $1,125{\pm}261g$ in 2014, respectively. The post-discharge mortality rate was 1.2%-1.5%. Weight < 50th percentile was 46.5% in 2013 and 66.1% in 2014. The overall prevalence of cerebral palsy among the follow up infants was 6.2% in 2013 and 6.6% in 2014. The Bayley Scales of Infant Developmental Outcomes version II showed 14%-25% of infants had developmental delay and 3%-8% of infants in Bayley version III. For the Korean developmental screening test for infants and children, the area "Further evaluation needed" was 5%-12%. Blindness in both eyes was reported to be 0.2%-0.3%. For hearing impairment, 0.8%-1.9% showed bilateral hearing loss. Almost 50% were readmitted to hospital with respiratory illness as a leading cause. Conclusion: The overall prevalence of long-term outcomes was not largely different among the VLBWI born between 2013 and 2014. This study is the first large national data study of long-term outcomes.

Relative contributions of weather systems to the changes of annual and extreme precipitation with global warming

  • The global patterns of annual and extreme precipitation are projected to be altered by climate change. There are various weather systems which bring precipitation (e.g. tropical cyclone, extratropical cyclone, etc.). It is possible in some regions that multiple weather systems affect the changes of precipitation. However, previous studies have assessed only the changes of precipitation associated with individual weather systems. The relative contributions of the weather systems to the changes of precipitation have not been quantified yet. Also, the changes of the relative importance of weather systems have not been assessed. This study present the quantitative estimates of 1) the relative contributions of weather systems (tropical cyclone (TC), extratropical cyclone (ExC), and "others") to the future changes of annual and extreme precipitation and 2) the changes of the proportions of precipitation associated with each weather system in annual and extreme precipitation based on CMIP5 generation GCM outputs. Weather systems are objectively detected from twelve GCM outputs and six models are selected for further analysis considering the reproducibility of weather systems. In general, the weather system which is dominant in terms of producing precipitation in the present climate contributes the most to the changes of annual and extreme precipitation in each region. However, there are exceptions for the tendency. In East Asia, "others", which ranks the second in the proportion of annual precipitation in present climate, has the largest contribution to the increase of annual precipitation. It was found that the increase of the "others" annual precipitation in East Asia is mainly explained by the changes of that in summer season (JJA), most of which can be regarded as the summer monsoon precipitation. In Southeast Asia, "others" precipitation, the second dominant system in the present climate, has the largest contribution to the changes of very heavy precipitation (>99.9 percentile daily precipitation of historical period). Notable changes of the proportions of precipitation associated with each weather system are found mainly in subtropics, which can be regarded as the "hotspot" of the precipitation regime shift.

위성영상기반 증발스트레스지수를 활용한 필지단위 논 가뭄 모니터링 (Drought Monitoring for Paddy Fields Using Satellite-derived Evaporative Stress Index)

  • Drought monitoring over paddy field area is an important role as the frequency and intensity of drought due to climate change increases. This study analyzed the applicability of drought monitoring on paddy crops using MODIS-based field surveys. As a satellite-based drought index using evapotranspiration for quantitative drought determination, ESI (Evaporative Stress Index), was applied and calculated through the ratio of MODIS- based actual and potential evapotranspiration. For the irrigated areas of Idong, Gosam, Geumgwang, and Madun reservoirs the availability of irrigation water supply, ponding depth, precipitation, paddy growth were investigated for the paddy field within one grid of MODIS. In addition, the percentile-based ESI drought severity was calculated to compare the growth process of paddy and changes in the drought category of ESI. The Idong area was irrigated about a week later than other reservoirs for the period of water supply, transplanting, and water drainage and the ESI drought category tended to be different. The Gosam, Geumgwang, and Madun area expressed moderate drought prior to the farming season, and indicated normal as the water was supplied. During the water drainage, the drought category intensified, indicating that the water available on land was decreasing. These results demonstrated that the MODIS-based ESI could be an effective tool for agricultural drought monitoring over paddy field area.

초음파를 이용한 임신초기 태아의 두개내투명대 표준치 제시 (Design of Standard Value Intracranial Translucency of Fetus in Early Pregnancy Using Ultrasound)

  • 본 연구는 임신초기 초음파 선별검사에서 태아의 후뇌 정중면을 이용하여 태아의 두개내투명대 기준 값을 제시하고 임상적인 의의를 찾고자 하였다. 2018년 8월부터 2020년 2월까지 I 병원 정기검진을 받는 산모들 중 머리엉덩길이 40.6 mm에서 78.3 mm에 해당되는 단태아 1529명의 두개내투명대를 측정하여 후향적으로 분석하였다. 회귀분석에서 머리엉덩길이가 증가할수록 두개내투명대는 54.3%의 설명력을 보이며 증가하는 경향을 보였다(P<0.001). 또한 빈도분석을 통해 임신주수별 두개내투명대의 백분위수 기준치를 산출하였다. 결과적으로 태아 두개내투명대의 한국인 기준치를 마련할 수 있었으며 임신초기 태아 후뇌의 평가에 있어서 척추이분증의 보조적 선별검사로 유용하게 적용될 것이다.

성장장애 한의표준임상진료지침 개발을 위한 한의사 인식조사 (A Survey on Korean Medicine Doctors' Recognition for Developing Korean Medicine Clinical Practice Guideline for Growth Disorders)

  • Objectives The purpose of this study is to provide data for development of the korean medicine (KM) clinical practice guidelines (CPG) for growth disorder (GD) by identifying the awareness and knowledge needs of KM doctors (KMD) through online survey. Methods Survey questionnaire was produced by referring to the previous recognition survey studies for clinical KMDs. The survey questionnaire was composed 18 questions regarding the current status of clinical care for GD, clinician's knowledge level about GD, and other details that clinicians use during practice. The survey was conducted from January 2021 to March 2021. An online survey was conducted on 101 KMDs from the association of pediatrics of KM. Results According to the survey, 96 respondents (40.3%) said the causation of GD without growth hormone deficiency needs to be included in CPG. 96 (23.5%) of the respondents wishes to utilize percentile in diagnosis and evaluation of the growth assessment. 24.7% of the clinicians were using the herbal medicine treatment. Currently, when treating with KM for GD, herbal medicine (100%) is the most widely used, followed by acupuncture (77.6%), and moxibustion (36.7%). In terms of a complex treatment, growth therapy efficacy (26.8%) is shown to be the most important factor to consider and needed to be included in CPG for the complex treatment, and diet (22.3%) is also considered to be important in GD. Conclusions In this study, we were able to understand the clinical KMDs' perception of GD, knowledge level, and the requirements in the CPG. The results of this study will provide the basic data for development of CPG for GD.

A Retrospective Comparative Study of Serbian Underground Coalmining Injuries

  • Background: During 2011, a study was undertaken to assess safety conditions in Serbian underground coalmines by analysis of injury data. The study covered all Serbian coalmines, identified week spots from the aspect of safety, and recommended possible courses of action. Since then, Serbia has made changes to safety and health legislation; all coalmines introduced new preventive measures, adopted international standards, and made procedures for risk management. After 10 years a new study has been performed to analyze the impact of these changes. Materials and methods: In this study, the injuries that have occurred in the Serbian underground coal mines over the last 20 years were analyzed. Statistical data analysis was performed by IBM SPSS Statistics v23. The injuries that occurred in the last ten years were compared with the results of the previous study (2000-2009). The average values of injury rates for both periods were compared for each of the categories (severity, age, body part, qualification), and the results were presented as absolute difference or percentile difference. Results: The results showed reduction in the number of injuries in the category of 20-30 years old workers, where the new training procedures for workers, which were set by mandatory legal regulations, certainly contributed. They also showed an increase in the number of injuries in the category of old workers, which indicates that the law did not have a positive effect on this category. Conclusion: The total number of injuries is still high; therefore, it is necessary to introduce mechanization and automation in mines and have a better policy for older workers who retire later nowadays.

Estimation of Lead Exposure Intensity by Industry Using Nationwide Exposure Databases in Korea

  • Background: In a previous study, we estimated exposure prevalence and the number of workers exposed to carcinogens by industry in Korea. The present study aimed to evaluate the optimal exposure intensity indicators of airborne lead exposure by comparing to blood lead measurements for the future development of the carcinogen exposure intensity database. Methods: Data concerning airborne lead measurements and blood lead levels were collected from nationwide occupational exposure databases, compiled between 2015 and 2016. Summary statistics, including the arithmetic mean (AM), geometric mean (GM), and 95th percentile level (X95) were calculated by industry both for airborne lead and blood lead measurements. Since many measurements were below the limits of detection (LODs), the simple replacement with half of the LOD and maximum likelihood estimation (MLE) methods were used for statistical analysis. For examining the optimal exposure indicator of airborne lead exposure, blood lead levels were used as reference data for subsequent rank correlation analyses. Results: A total of 19,637 airborne lead measurements and 32,848 blood lead measurements were used. In general, simple replacement showed a higher correlation than MLE. The results showed that AM and X95 using simple replacement could be used as optimal exposure intensity indicators, while X95 showed better correlations than AM in industries with 20 or more measurements. Conclusion: Our results showed that AM or X95 could be potential candidates for exposure intensity indicators in the Korean carcinogen exposure database. Especially, X95 is an optimal indicator where there are enough measurements to compute X95 values.

Long-term Outcomes of Augmentation Cystoplasty in a Pediatric Population With Refractory Bladder Dysfunction: A 12-Year Follow-up Experience at Single Center

  • Purpose: Augmentation cystoplasty (AC) is a surgical procedure used in adults and children with refractory bladder dysfunction, including a small bladder capacity and inadequate bladder compliance, and in whom conservative and medical treatment has failed. This study was aimed to determine the long-term outcomes of AC in children. Methods: A retrospective analysis was conducted of 42 patients (31 males; mean age, $14.2{\pm}6.2years$) who underwent AC for neurogenic and nonneurogenic bladder dysfunction, with a median $12.0{\pm}1.5years$ of follow-up. All patients underwent AC using the ileum with or without continent reconstruction. Pre-AC, concurrent, and post-AC procedures and complications were analyzed. Patients who underwent ureterocystoplasty, were lost to follow-up, or had less than 10 years of follow-up were excluded. The primary outcomes were the complication and continence rates, the post-AC linear rate of height and weight gain, and renal function. The Student t-test was used to evaluate between-group differences and the paired t-test was used to evaluate longitudinal changes in measured variables. Results: Renal function was stable or improved in 32 of 42 patients (76.2%), with a post-AC continence rate of 88.1%. Thirty patients (71.4%) required 72 procedures post-AC. There was no statistically significant difference in the mean percentile of height (P=0. 212) or weight (P=0.142) of patients in the pre- and post-AC periods. No cases of bladder perforation or malignancy were detected. Conclusions: We consider AC to be a safe and effective procedure that does not negatively affect future physical growth, while achieving a good rate of stable renal function. Patients need long-term follow-up to address long-term complications.

성장 치료를 목적으로 한방병원에 내원한 환아에 대한 임상적 고찰 (A Clinical Study of the Pediatric Patients Who Visited Oriental Medical Hospital for Growth Treatment)

  • Objectives The purpose of this study is to analyze the characteristic of pediatric patients who visited oriental medical hospital for growth treatment. Methods The study was conducted with 130 pediatric patients who visited ${\bigcirc}{\bigcirc}$ Korean medicine hospital from January 2016 to August 2017. The patient's data was classified by sex, age, pattern identification and diseases. The classified data was analyzed to find the correlations and characteristics among variables. Results There were 50.8% of boys and 49.2% of girls. 6.2% of the total patients were in 'early childhood', 50.8% were in 'late childhood' and 43.1% were in 'puberty' stage. In terms of pattern identification, 44.6% of the total patients were 'Spleen-lung Qi Deficiency', 53.8% were 'Lung-kidney Yin Deficiency' and 1.5% were 'Weakness of Heart and gallbladder'. 44.3% of the patients had various diseases including rhinitis (77.6% of 120.7%). Height percentile of the children with using the Growth chart of 2017 was lower than using the Growth chart of 2007. Predicted height based on the child's bone-age were higher than the predicted height based on the parental height, although both of the precited factors were correlated to the current child's height. Conclusions This study showed the characteristic of the pediatric patients who visited oriental medical hospital for growth treatment. It helps to determine prognosis and treatment, and to explain treatment measures.
