의식불명 환자에서 경과급식에 의한 혼합형 식사와 상업용 조합식이의 효과 비교 (Comparison of Blenderized Diets and Commercial Enteral Formulas in the Unconscious Tube-fed Patients)

  • Blenderized diets are not recommended because of difficulties in providing a constant content of nutrients, phoblems with viscosity and osmolarity, time needed for preparation, and increased risk of contamination. To determine the possibility of substituting commercial formulas for blenderized diets, 27 unconscious patients were randomized into two different groups : blenderized diet group(n=11) and commercial formula group(n=16). Blenderized diets were composed of food sources such as rice, milk, egg, juices and others, and its energy percentage of protein, fat, and carbohydrate was 15%, 30%, and 55%, respetively. Commercial enteral formulas provided 1 kcal/ml and the energy percentage of protein, fat and carbohydrate was 25%, 20% and 55% in greenbia(G) and 20%, 20%, and 60% in Greenbia-DM(GD), repectively. In commercial formula group, five patients with diarrhea, constipation, and high blood glucose level received GD, and the other eleven patients received G. All patients received their lipuid diet for 4 weeks through 16 French rubber nasogastric tube by bolus feeding 6 times daily. The patients in commercial formula group had a similar mean daily calorie intake to the patient in blenderized diet group, 32-34kcal/kg/d and 30-35kcal/kg/d, respectively. Patients given commercial formula gained an averge of 1.7kg in weight but those given blenderized diet lost an average of 2.6kg. There was a trend toward an increase in percent ideal body weight in commercial formula group(94$\pm$5 vs 99$\pm$6%). However, blenderized diet group showed a trend toward a decrease in percent ideal body weight(106$\pm$5 vs $101\pm$6%). The initial levels of total lymphocyte counts, serum albumin and tranferrin were below the normal rante, The mean daily protein intake was significanlty higher for commercial formula group(2.0-2.1g/kg/d) than for blenderized diet group(1.1-1.3g/kg/d). Patient fed cormmercial formula for 4 weeks showed a trend toward an increase in serum albumin(8.1%) and a significant increase in serum transferrin(32.1%) without increasing the levels of blood glucose. GOT, GPT, blood urea nitrogen and serum creatinine. In the blenderized diet group, however, no significant improvement were obseved in the concentration of serum albumin and transferin, compared to initial value. There was a trend toward a decrese in the level of hemoglobin and hematocrit in blenderized deit group but no significnat change in commercial formulas goups. All patients tolerated both diets well and no significant complications were encountered. The results indicate that the commercial enteral formulas tested in this study can be an effective substitution for blenderized diet in unconscious tube-fed patients.

유산소운동이 여자 아동의 뇌세포 생성 관련인자에 미치는 영향 (Effect of Aerobic Exercise on Factors Relative to the Brain Nerve Growth in Girls)

  • 운동은 신경의 생성, 적응, 보호 기능을 통해 뇌 기능 및 생성에 긍정적인 영향을 미치며, 유산소운동은 뇌신경 성장인자(GH, IGF-1, estrogen, serotonin)를 변화시키는 것으로 보고되었다. 본 연구는 여자 아동의 뇌기능 발달 및 뇌세포 생성 유도인자의 유산소운동 효과를 알아보고자 유산소운동 프로그램에 따른 뇌신경 성장인자(GH, IGF-1, estrogen, serotonin)의 변화를 분석하여 다음과 같은 결론을 얻었다. 반복측정 분산분석에 의해 운동군과 대조군의 평균을 비교한 결과 유산소운동은 집단간 유의한 차이를 보이며 체중, BMI, 체지방량, 체지방률을 감소 시켰으며, 제지방률을 증가시켰다. 또한 뇌신경 성장인자인 serotonin을 감소시켰으며, GH와 IGF-1을 증가시켰다. 본 연구결과 유산소운동 프로그램이 아동의 체중, BMI, 체지방, 체지방률 감소와 제지방률 증가에 효과적이다. 또한 serotonin이 운동 후 감소된 결과를 볼 때 유산소운동 프로그램이 아동의 중추피로 감소에 효과적이며, GH, IGF-1의 운동 후 증가된 결과를 볼 때 유산소운동이 아동의 뇌기능 발달 및 뇌세포 생성 유도에 긍정적인 프로그램임을 알 수 있다.

노인 건강증진용 신발의 운동과학적 효과분석 (The Scientific Analysis of Aged' Shoe for Health Promotion)

  • 본 연구에서는 국내외 실버화와 일반적인 운동화에 대한 1차 운동기능학적 분석을 통하여 노인에게 가장 적합한 신발을 정하고 2차 운동생리학적 실험을 통해 체지방률변화, 운동강도에 따른 대사량의 변화 그리고 에너지 소비량의 변화를 분석하였다. 본 연구의 대상으로는 특별한 질병을 가지고 있지 않은 남자 어른들 20명을 대상으로 하였으며, 평균나이는 $62.78{\pm}4.32$세, 신장은 $170.89{\pm}3.56cm$ 그리고 몸무게는 $75.12{\pm}8.76kg$이었다. 실험 전 실험에 대한 충분한 설명을 하고 동의서를 받은 후 데이터를 수집하였다. 본 연구를 위하여 운동 기능학적 실험과 운동 생리학적 실험을 수행하였으며, 본 연구결과 걷기나 달리기 시 최초 발 뒤꿈치 접촉 시 발이 회내(pronation)되는 정도와 최대 회내가 되는 정도를 알아 본 결과 걷기속도가 가장 빠르게 나온 B형의 신발이 회내값($-2.3{\pm}1.05^{\circ}$)이 가장 크게 나타났으며 일반걷기용 신발에서 가장 작은 값($-1.5{\pm}0.49^{\circ}$)을 보여주고 있다. 또한 노인들의 일상생활에서 착용하는 건강증진용 신발을 대상으로 운동 생리학적 부분을 살펴보면 체지방율의 변화는 모든 그룹에서 감소는 하였으나, 통계적으로 유의한 차이가 나타나지 않았는데, 이는 체지방율의 변화를 가져오기 위해서 일시적인 운동이 아닌 규칙적이고 장기적인 운동이 필요할 것으로 사료되어지며, 노인들이 건강증진용 기능화를 장기적으로 착용하여 운동할 경우 체 지방율에 긍정적인 영향을 미칠 것으로 사료되어진다.

비만 중학생의 비만도와 영양지식, 영양섭취상태 및 혈액성상과의 상관성 (Relation Between Obesity Indices and, Nutritional Knowledge Nutritional Status and Blood Parameters in Obese Middle-School Students)

  • 본 연구에서는 청소년의 효과적인 비만관리 프로그램을 실시하는데 필요한 기초자료를 제시하기 위하여 부천시 보건소에서 개최한 청소년 캠프에 참여한 남녀 비만 중학생 총 42명 (남 28명, 여 14명)을 대상으로 신체 계측, 24시간 식사섭취조사, 혈액성상 분석 그리고 영양지식, 영양태도, 자아만족도 등에 대한 설문조사를 실시하고 비만도와 영양지식, 영양섭취상태 및 혈액성상과의 상관성을 살펴보았다. 그 결과를 요약하면 다음과 같다. 연구대상자들의 평균 연령은 13.9세였으며, 체중, 체질량지수, 비만지수, 허리엉덩이둘레 비, 체지방률은 남학생이 각각 75.5 kg, 29.1 kg/$m^2$, 30.1, 0.89, 33.3%이었고, 여학생이 각각 67.8 kg, 27.5 kg/$m^2$, 25.3,0.81, 34.6%이었다. 영양태도 점수(40점 만점)는 남학생이 30.3점, 여학생이 30.1점으로 남녀 모두 비교적 긍정적인 영양태도를 보이고 있었으며, 자아만족도(50점 만점)는 남학생이 34.7점, 여학생이 33.8점으로 자신에 대하여 보통수준 이상의 만족도를 가지고 있었다. 영양지식 점수(16점 만점)는 남학생이 12.7점, 여학생이 12.4점으로 유사한 영양지식 수준을 보였다. 열량 섭취량은 남학생이 2137.7kcal,여학생이 2059.7kcal로 권장량의 각각 85.5%와 98.1% 섭취수준을 보였다. 단백질은 권장량 대비 남학생 110.3%, 여학생 107.3%섭취 수준이었으며, 총 열량에 대한 당질, 단백질, 지질의 섭취비율은 남학생 57:14:29, 여학생 60:13:26으로 남녀학생 모두 총 열량에 대한 지질의 섭취비율이 높았다. 인, 비타민 B$_1$, B$_{6}$, E를 제외한 칼슘, 철, 아연, 비타민 A, B$_2$, C, 나이아신, 엽산 등 대부분의 미량 영양소 섭취량은 권장량에 미치지 못하였다. 혈청 COT, GPT는 남학생이 여학생보다 각 항목에서 유의하게 높았고, 혈청 총 콜레스테롤, 중성지질, HDL-콜레스테롤, LDL-콜레스테롤은 남녀학생 간에 유의한 차이가 없었다. 적혈구지수의 경우 적혈구수, 헤모글로빈, 헤마토크릿, MCHC가 남학생이 여학생보다 유의하게 높았다. 체중, 체질량지수, 비만지수는 모두 수축기 혈압과 유의한 정의 상관관계를 보였고(p<0.01, p<0.05, p<0.05), 체지방률은 영양지식과 유의한 부의 상관관계를 보였다(p<0.05). 비만도와 영양소 섭취량과의 관계에서 체중, 체질량지수, 비만지수는 콜레스테롤 섭취량과(p<0.01, p<0.05, p<0.05) 각각 유의한 정의 상관관계를 보였다. 비만도와 혈액성상과의 관계에서 체중은HDL-콜레스테롤과 유의한 부의 상관관계를(p<0.05), 적혈구수, 헤모글로빈, 헤마토크릿과는 유의한 정의 상관관계를(각 p<0.05) 보였다. 체질량지수와 비만지수는 각각 HDL-콜레스테롤과는 부의 (각 P<0.05), 적혈구수와는 정의(각 p<0.05) 상관관계를 보였다. 허리엉덩이둘레비는 혈청 GPT, glucose, MCV와 각각 유의한 정의 상관관계를 보였다(각 p<0.05). 이상의 연구결과를 종합할 때 남녀 비만 중학생 모두 총 열량 섭취량 중 지질로 인한 열량 섭취비율이 높았고 비만도가 증가할수록 콜레스테롤의 섭취가 높게 나타났으며, 대상자의 체지방률이 증가할수록 영양지식 점수가 감소하는 결과를 나타내어 비만 청소년을 대상으로 한 영양교육의 중요성이 제시되었다. 또한 여러 비만도 평가지표들은 혈액성상의 변화와도 유의적인 관련성을 보였으나, 본 연구는 대상자의 수가 적고 비만 청소년만을 대상자로 하고 있는 제한점을 가지고 있으므로 향후 좀더 많은 대상자를 확보하고 정상 대조군과의 비교를 통하여 비 만에 있어 다양한 혈액성상의 변화 및 역할규명에 대한 연구가 이루어져야 할 것이다.

복합운동의 운동유형 순서변화가 비만여성에게 미치는 영향 (The Effect of Changing the Order of Exercise Types on Body Composition and Blood Lipid in Obese Women)

  • 본 연구는 복합운동 유형 순서 변화에 따른 생리적, 생화학적 변화를 비교 하고자 40대 비만여성(n=18)을 대상으로 12주간 복합운동을 실시하였다. 복합운동 유형은 유산소성 운동과 저항성 운동그룹 A(n=9) 그리고 저항성 운동과 유산소성 운동그룹 B(=9)로 구성하였다. 체지방량의 경우 A그룹의 평균변화량이 B그룹 평균변화량보다 더 많이 감소하였고 통계적으로 유의한 차이를 보였다(p<0.01). 혈당의 경우는 A그룹보다는 B그룹이 더 많이 감소하였으며 통계적으로 유의한 차이를 보였다(p<0.01). 따라서, 비만 중년 여성의 체중감량을 위한 신체구성 변화 유도를 위해서는 복합운동의 순서 변화에 따른 운동유형이 선택적으로 사용되어질 수 있다.

유유기백서서(乳幼期白鼠)의 단백질부족(蛋白質不足)에 관(關)한 영양학적(營養學的) 연구(硏究) (Studies on Early Protein Undernutrition of Rats)

  • These experiments were designed to study the influence of early protein undernutrition on growth, behaviors toward food, general attitude toward a new environment, brain size and body composition of the experimental rats. The following experimental groups were studied. Lactation period (3 weeks) (Diets of mother rats) 25% Casein diet 12% Casein diet 25% Casein diet 25% Casein diet 12% Casein diet 12% Casein diet After-weaning protein deprivation period None deprivation (25% Casein diet) None deprivation (25% Casein diet) 5% Casein diet (4 weeks) 5% Casein diet (8 weeks) 5% Casein diet (4 weeks) 5% Casein diet (8 weeks) After a long period of rehabilitation with 25% casein diet the following results were obtained. 1. Growth rate during lactation period is closely related with the protein levels of the diet for mother rats. The average body weight of offsprings of the mother rat fed 25% casein diet is 46.0 grams at 21 days old. However, that of the mother rat fed 12% casein diet is only 25.0 grams. 2. The group of protein undernutrition during lactation (S weeks) (offsprings of mother rat fed low protein diet, 12% casein diet) could never catch up with the normal group in its growth even after twenty-four (24) weeks of rehabilitation. 3. However, the groups of protein undernutrition during either four (4) or even eight (8) weeks after weaning could catch up with the normal group in their growth after long period of rehabilitation. 4. The absolute amounts of carcass protein and fat of the normal group are larger than those of the protein deficient groups. In terms of percent carcass, however, the normal group showed higher body fat and lower body protein than the early deficient groups. However, there is no difference between preweaning (3 weeks) and postweaning (8 weeks) deficient groups. It is assumed, from these differences in body composition, that there might be any differences in physiological and metabolic functions among these various groups, and also that the basic formation of various metabolic regulators (protein-nature) might be fixed mostly during lactation and postweaning period. 5. The groups of protein undernutrition during either three (3) weeks lactation or four (4) weeks after weaning are not so remarkably different from the normal group in their amounts of food intake and spillage. However, the groups of undernutrition during either eight (8) weeks postweaning or eleven (11) weeks (3 weeks lactation period plus 8 weeks postweaning period) showed higher amounts of food intake and spillage. In these respects, it seems that desire for food is closely related with the degree of early hunger in protein and also seems that the longer be deficient in early life the more food spillage is found. 6. Both preweaning and postweaning deficient groups showed generally nervous and restless. The normal group is staid and showed less mobilities. 7. The average size of the brains of the group subjected to protein deficiency during three (3) weeks lactation period is smaller than that of the group of the eight (8) weeks postweaning deficiency. This means that the development of the brain is made mostly during lactation period. The group of the eleven (11) weeks postnatal deficiency is significantly different from the normal group in its brain development. It is assumed, in connection with the results of various maze tests reported, that the brain size is closely related with the intellectual ability.

12주간 필라테스 매트 운동이 임산부의 신체구성, 요부근력, 요통 및 임신 스트레스에 미치는 영향 (The Effects of 12 Weeks Pilates Mat Exercise on Body Composition, Lumbar Muscle Strength, Low Back Pain and Pregnancy Stress in Pregnant Women)

  • 임신 기간 중 임산부는 체중 증가 및 근골격계 변화로 요통과 골반 통증을 유발한다. 필라테스는 유, 무산소성 복합운동으로 자세 교정 및 근 기능을 강화시키는 것으로 보고되고 있다. 따라서 이 연구는 필라테스 매트 운동이 임산부의 신체구성, 요부근력, 요통 및 임신 스트레스에 미치는 영향을 규명하는데 목적이 있다. 본 연구를 수행하기 위해 임신 중기(16~25주) 임산부 19명을 대상으로 필라테스 매트 운동 실험집단(n=11)과 비교집단(n=8)으로 구분하였으며, 실험집단은 하루 60분, 주 2회 12주간 필라테스 매트 운동을 실시하였다. 신체구성 중 체수분량, 체지방량, 체질량지수, 체지방률 및 기초대사량에는 집단 간 변화가 없는 것으로 나타난 반면 실험집단에서는 골격근량에서 유의한 증가가 나타났다. 또한 요부근력의 경우 엉덩이관절 신전근과 요추 신전 가동범위는 필라테스 매트 운동을 수행한 집단에서 유의하게 증가하였다. 요통 수준과 임신 스트레스 수준을 분석한 결과 집단 간 차이는 없는 것으로 나타났다. 따라서 필라테스 매트 운동은 임산부의 근육량을 증가시키고, 요부근력에는 긍정적인 영향을 나타내어 임산부에게 안전하게 적용할 수 있는 운동이라고 할 수 있다.

초등학생의 비만 이환율 및 비만도에 따른 지방 섭취량ㆍ혈청 총콜레스테롤/중성지질 수준 연구 (A Study on the Obesity Incidence Rate and Fat Intake, Serum Total Cholesterol/Triglyceride Concentrations by Obese Index in the Elementary School Students)

  • The obesity incidence rate of children in Korea has increased from 2 to 3% in 1970 to 15% in 2004. The purpose of this study was to evaluate the effect of diet on obesity, and serum cholesterol and triglyceride concentrations by obese index in Korean elementary school students. The subjects were three hundred and thirteen students(male 157, female 156). Height, weight and body fat were measured. Dietary records by the 24-hr recall method were taken for 3 days, and serum cholesterol and triglyceride concentrations were determined. The obesity index was calculated by the Korean of Pediatrics Association standards. Nutrient intakes were calculated by CAN PRO. Almost twenty percent(19.8%) of the subjects were underweight, 60.1 % were normal, 10.2% were overweight and 9.9% were obesity. The obesity incidence rate of the males was not different from the females. Energy and nutrients intakes containing fatty acids were not significantly different by the obesity index. All the subjects had 59.7%~61.1% of carbohydrate and 24.2%~25.6% of fat for energy. Serum cholesterol concentrations of the obesity group(177.3 mg/dl) was higher than those of the underweight group(161.8 mg/dl) and normal weight group(163.5 mg/dl) and overweight group(163.8 mg/dl). Twenty nine percent of the obesity group had serum cholesterol concentrations over 200mg/dl, as compared with 4.8% of the normal and underweight group. Serum triglyceride in the obesity group(103.0mg/dl) was higher than those of the underweight group(79.7 mg/dL), normal weight group(81.0 mg/dl) and overweight group(81.1 mg/dl). Serum total cholesterol concentrations were correlated with the obese index, PIBW, R hrer index at p<0.001, with BMI at p<0.01 and with waist/hip ratio at p<0.05. Serum triglyceride concentrations were correlated with the obese index, PIBW, R hrer index at p<0.01. It is concluded that obesity increases serum total cholesterol concentrations and triglyceride concentrations, and that the obesity preventive program for normal weight children, and weight reduction program for obese children are necessary for elementary school children's education.

Growth Performance and Carcass Evaluation of Jeju Native Cattle and Its Crossbreds Fed for Long Fattening Period

  • This study compared the growth performance and carcass evaluation of Jeju native cattle (JNC) and its crossbreds (CBK = 25 JNC:50 Charolais:25 Brahman and BCBK = 62.5 JNC:25 Charolais:12.5 Brahman). Eight male calves of each JNC, CBK and BCBK were weaned at 4 month of age and were fed for 24 months of age. All animals grazed a pasture between 5 to 10 months of age then they were fed growing ration at the rate of 1.5% of their BW along with ad libitum supply of Italian ryegrass hay between 11 to 16 months of age and thereafter switched to ad libitum feeding of finishing ration and hay between 17 to 24 months of age. Mean body weight (BW) and BW gain were higher in CBK compared with BCBK and JNC at 4, 10, 16 and 24 months of age. Average daily BW gain during 4 to 10 months of age was the highest for CBK followed by BCBK and JNC. However, daily BW gain was higher in BCBK than in CBK and JNC during 11 and 16 months of age. During fattening period (17 to 24 months) average daily BW gain was higher in JNC than in CBK and BCBK. Slaughter weight, hot and cold carcass weight were higher in CBK compared with JNC and BCBK. Weight of bones, boneless meat, ribs, excluded rib meat, retailed cut meat, tender loin, sir loin, strip loin, sticking, top round, bottom round, fore leg, shank, and thin-flank were higher in CBK than in BCBK and JNC. Fat weight in these carcass cuts and kidney fat was similar in JNC and its crossbreds. Logissimus dorsi and its ratio were higher in CBK compared with BCBK and JNC. Percent moisture, crude protein, and ash contents of beef were similar in JNC and its crossbreds. Percent beef fat was higher in JNC and BCBK than in CBK. Cooking loss and water holding capacity of beef was similar in JNC and its crossbreds. Sheer force was lower in BCBK compared with JNC and CBK. Juiciness, tenderness and flavor of beef were higher in BCBK compared with JNC and CBK. In conclusion, CBK has shown higher growth rate and produced heavier carcasses with good degree of fatness compared with JNC and BCBK. However, fattening for longer period could increase the maintenance cost in CBK and BCBK because of their higher BW which they attained during growing period.

비만 여성의 체중조절 실시를 통한 효과 측정 (The Effect of Weight Control on Obese Women)

  • This study was conducted to investigate the effect of weight control by use of commercial formula diet and nutrition education on 36 obese women over a period of 6 months. During the study it motivated ways for women to control their weight and improve their health, thus analyzing variables that influence weight control. To help weight control, for the first 3 weeks, along with 1 regular mea, 2-3 liquid formula diets of 135㎉ per pack a day were provided within the 1,200-800㎉ range. For 6 months since then, all 3 meals were taken regularly and it was recommended that energy which is 500㎉ less than energy requirement be taken. To evaluate the effect of weight change by the weight control program, anthropometric measurements, biochemicl parameter, and food behavior are measured. The mean body mass index(BMI) was 30.1$\pm$3.8(25.0-43.6) and it was in the upper 5 percentile of the mean BMI of Korean women. The percent of body fat was significantly reduced from 29.1$\pm$2.4 to 26.4$\pm$2.3, showing that the weight control program induced not only a change in weight but also a change in body composition. The body circumference such as waist and hip circumference and skinfold thickness measurements of biceps, triceps, subscapular and surprailiac were reduced significantly, but the mean waist-to-hip ratio didn't change. Singnificantly improved food behavior score measured by the questionnaire indicated that the weight-contorl program induced the change of the food behavior, as well. Weight loss also induced biochemical parameters of the subject, thus total cholesterol, LDL-Cholesterol, and triglycerol which were in the high range of the normal values were reduced to 20.2%(p<0.01), 22.2%(p<0.01), 25.8%(p<0.01), repectively. HDL-cholesterol didn't change significantly, but the ratio of HDL to total cholesterol increased(p<0.01).
