A Case of Sliding Hiatal Hernia associated with Bochdalek Hernia Repair (선천성 횡격막 탈장증 교정 후 발견된 활주형 탈장 1예)

  • This is a case report of a sliding hiatal hernia with severe gastroesophageal reflux(GER) after repair of congenital diaphragmatic hernia(CDH). It was not possible to determine whether the hiatal hernia is a de novo lesion which was missed at the original operation or a consequence of overzealous repair of the Bochdalek defect at the expense of weakening of the diaphragmatic crura. This case demonstrates that a sliding hiatal hernia can be a cause of severe gastroesophageal reflux that should be managed surgically.

  • Pediatric carpal fractures are rare and often difficult to detect. This paper reviews the current literature on pediatric non-scaphoid carpal fractures, with a case report of a lunate fracture associated with a distal radius and ulnar styloid fracture, managed nonoperatively in a 12-year-old boy. There is lack of consensus regarding the management of these fractures due to the low number of reported cases. A frequent lack of long-term follow-up limits our understanding of the outcomes, but good outcomes have been reported for both nonoperative and operative management. This case report brings attention to the current time period for the definition of delayed union in pediatric carpal fractures, and emphasizes the need for prolonged follow-up for the detection of delayed complications leading to functional impairment.

  • It has been reported that the operative treatment for the preservation of tooth having periapical lesion has shown over ninety percent of success whereas in the case where calcified canal is severe, canal is perforated, ledge is formed, tooth restored by post and core and cases where symptoms still exist after treatment, endodontic treatment may not all be the solution in attaining a successful treatment. Thus Apicoectomy would generally be used to prevent the loss of tooth having periapical lesion where treatment prognosis is poorly evaluated by endodontic treatment. In case 1 and 2, endodontic treatment was applied due to unilocular radiolucent lesion in periapical area but the symptoms somehow did not show improvement and therefore operated apicoectomy, and as in case 1, favorable results were attained but as in case 2, yet more observation has to be done. The followings are the results we have concluded : 1. In the process of endodontic treatment, absolute canal obturation has great effect on attaining successful treatment. 2. In cases where endodontic treatment is not highly recommended, for example, tooth having dental pulp and periapical lesion, favorable results can be attained by applying apicoectomy.

  • Neuroblastoma arises from the embryonic tissue of the adrenergic rest. It is commonly found in children and mostly in nonrenal tissue. We present a case of intrarenal neuroblastoma which was initially thought to be a Wilms' tumor. The patient was a 18 months-old girl treated with radical nephrectomy and adjuvant chemotherapy after operation. The neoplasm within the kidney in children cannot always indicate Wilms' tumor. Neuroblastoma of the adrenal gland or retroperitoneal tissue may often compress or invade the kidney directly or arise from the kidney. Clinical aspects that differentiate between neuroblastoma and Wilms' tumor are discussed with a review of the literatures.

  • Foreign body ingestion is a frequent event in the pediatric age group, but the incidence of foreign bodies in the appendix is very low. The authors report a case of ingested foreign body trapped in the appendix in a 7-year-old girl. The foreign body was successfully treated by laparoscopic appendectomy under fluoroscopic guidance.

Development of a knowledge-based medical expert system to infer supportive treatment suggestions for pediatric patients

  • This paper discusses the design, implementation, and potential use of an ontology-based mobile pediatric consultation and monitoring system, which is a smart healthcare expert system for pediatric patients. The proposed system provides remote consultation and monitoring of pediatric patients during their illness at places distant from medical service areas. The system not only shares instant medical data with a pediatrician but also examines the data as a smart medical assistant to detect any emergency situation. In addition, it uses an inference engine to infer instant suggestions for performing certain initial medical treatment steps when necessary. The applied methodologies and main technical contributions have three aspects: (a) pediatric consultation and monitoring ontology, (b) semantic Web rule knowledge base, and (c) inference engine. Two case studies with real pediatric patients are provided and discussed. The reported results of the applied case studies are promising, and they demonstrate the applicability, effectiveness, and efficiency of the proposed approach.

An Analysis on Clinical Education of Pediatric Nursing (아동간호학 임상실습교육 현황)

  • This study was conducted to analyse the current clinical education of pediatric nursing in baccalaurate nursing program, then to give basic data for enhancing the quality of future clinical education of pediatric nursing. Data were collected through self-reported questionnaire by mail from December 2001 to February 2002. The subjects were 29 schools of 50 baccalaurate nursing education programs. The data were analysed by double raters, researcher and assistant researcher. The results were summarized as follows: 1. Twenty-eight schools had the objectives of the clinical education of pediatric nursing, and 28 schools in pediatric ward, 23 schools in nursery, 22 schools in neonatal intensive care unit(NICU), 15 schools in objectives related to profession by clinical site. 2. Credits on clinical education of pediatric nursing were most 15 schools of 3 credits. 3. The clinical sites were mainly the hospital that sick children were admitted in. 4. The clinical teacher were 9 types including pediatric professor and field nurse. 5. On teacher's role, the professor instructed the case study and conference, and field nurse instructed the patient assignment and nursing procedures. 6. All of schools used explanation and conference as a method of clinical education, 1 or 2 schools used PBL or role play or field study. 7. On clinical education content, most of school included Apgar scoring system, physical examination in newborn assessment, respira- tion maintenance, temperature maintenance, infection prevention, nutrition, and bath in newborn care. 8. On clinical education content, most of school included care of incubator, phototheraty, infusion, gavage feeding and how to use the instruments in NICU. Eighteen schools included attachment promotion, and 20 schools case study. 9. On clinical education content, most of school included a checklist of nursing procedures, case study, assessment of growth and development in pediatric ward and other sites. 10.There were various evaluation types in scores, measuring items. In conclusion, the results of this study revealed that there were some discrepancy in the objectives and contents, clinical sites on hospital focused, teacher's role, and diversity of measurement items and ratings in clinical education of pediatric nursing. There is a need for a standardization of content, clinical site, and evaluation tool to improve a quality of clinical education of pediatric nursing based on this study.

  • Haddad syndrome, the association of Congenital Central Hypoventilation Syndrome (CCHS) and Hirschsprung's disease (HD), is a disease caused by mutation of PHOX2B gene. This disease, which occurs very rarely worldwide, is characterized by adequate ventilation during wakefulness and hypoventilation during sleep due to decreased ventilatory sensitivity. In this case report, we report a case of dental treatment under general anesthesia in a child with Haddad syndrome. A 3-year-old child with Haddad syndrome visited Seoul National University Dental Hospital for dental treatment. Dental treatment was planned under general anesthesia because of his medical condition. Treatment was successfully done without any postoperative complications. This case suggests that general anesthesia may be advantageous for dental treatment due to the risk of respiratory depression in a child with Haddad syndrome.

A Case of Conjoined Twins - Omphalopagus (결합쌍생아 증례보고 - 제대결합쌍생아 1례 -)

  • Conjoined twins are one of the rarest and most challenging congenital anomalies in pediatric surgery. Successful surgical separation is difficult because it the majority of conjoined twins in is technically typical to separate shared vital organs successfully. The timing of separation is variable, but separation is usually delayed until such infants are relatively mature(i.e, 9-12 months of age). Operative survival was 50 % in the neonatal period, but 90 % in those over 4 months of age. The present case was successfully separated early beacase of cardiac problems in one of the twins. These twins were omphalopagus and only the liver was shared through a bridge. The vascular and biliary trees were independent from each other. Successful surgical separation was on 11th day of life, but one died of sepsis 18 days after operation.

REPLANTATION OF COMPLETE AVULSED TEETH DUE TO TRAUMATIC INJURIES. : A CASE REPORT (완전 탈구된 치아의 재식에 대한 증례보고)

  • The treatment of complete avulsed teeth due to traumatic injuries is replantation procedure. The major complication of replantation are pulp necrosis, ankylosis, and root resorption. It is important to maintain the vitality of periodontal ligament to minimize the complications. The purpose of this case report is to consider the facts that affect the success rate and the prognosis of complete avulsed teeth after replantation. All of these cases had different conditions and transport media. The following results were observed: 1. The successive treatment of complete ayulsed teeth requires the maintainance of vitality of periodontal ligament. 2. The complication of replantation are inflammatory root resorption and ankylosis and in such case, proper endodontic treatment using calcium hydroxide medications and periodic observations are needed. 3. In case of delayed replantation, the use of fluoride and root planning procedure can help to achieve better result. 4. It is more esthetic and functional to maintain the avulsed tooth in mouth.
