뇌졸중 환자의 배근력과 호흡기능의 상관관계 (The Correlation between Abdominal Muscle Strength and Respiratory Function in Stroke Patients)

  • Purpose: This study aimed to investigate the correlation between abdominal muscle strength and measures of respiratory function in stroke patients. Methods: The study participants comprised 17 (male: 12, female: 5) stroke patients hospitalized at W rehabilitation hospital in Busan, South Korea. Abdominal muscle strength was assessed using a digital manual dynamometer for 5 seconds contacting the sternal notch of the participants to bend the trunk. Respiratory function (forced vital capacity, forced expiratory volume in one second, forced expiratory volume in one second/forced vital capacity, and peak expiratory flow) was assessed using a spirometer. The collected data were analyzed using Pearson's correlation analysis, and the significance level was set 0.05. Results: A statistically significant correlation was found between abdominal muscle strength and forced vital capacity, forced expiratory volume in one second, and peak expiratory flow. However, abdominal muscle strength and forced expiratory volume in one second/forced vital capacity were not significantly correlated. Conclusion: This study demonstrated that there is a relationship between abdominal muscle strength and respiratory function. Exercise programs to strengthen the abdominal muscles are therefore necessary to improve respiratory function in stroke patients.

  • Peak expiratory flow rate(PEF) is one of the most important diagnostic parameters in spirometry. PEF occurs in a very short duration during the forced expiratory maneuver, which could lead to measurement error due to non-ideal dynamic characteristic of the transducer. In such case the initial slope of the flow rate signal determines the accuracy of the measured PEF. The present study considered this initial slope as a parameter to compensate PEF. The 26 standard flow rate signals recommended by the American Thoracic Society(ATS) were flown through the air flow transducer followed by simultaneous measurements of PEF and maximum transducer output$(N_{PEF})$. $N_{PEF}$-PEF satisfied a quadratic equation in general, however, two signals (ATS #2 and #26) having large initial slopes deviated from the fitting equation to a significant degree. The relative error was found to be in a linear relationship with the initial slope, thus, $N_{PEF}$ was appropriately compensated to provide accurate PEF with mean relative error less than only 1%. The 99% confidence interval of the mean relative error was less than a half of the error limit of 5% recommended by ATS. Therefore, PEF can be very accurately determined by compensating the transducer output based on the initial slope, which should be a useful technique for air flow transducer calibration.

Lung Function of Grain Millers Exposed to Grain Dust and Diesel Exhaust in Two Food Markets in Ibadan Metropolis, Nigeria

  • Background: Despite growing concern over occupational exposure to particulate matter (PM) such as grain dust and diesel exhaust, information about the exposure level and health implications among workers in small-scale milling enterprises in developing countries like Nigeria has not been adequately documented. The purpose of this study was to assess the level of exposure to grain dust and diesel exhaust and effect on lung function among grain millers in food markets in Ibadan metropolis, Nigeria. Methods: The study adopted descriptive cross-sectional design with a comparative approach. Sixteen grain milling shops each were randomly selected from two major food markets in Ibadan metropolis for indoor $PM_{10}$ and $PM_{2.5}$ monitoring. Seventy-two respondents each were proportionately selected from grain millers and shop owners for forced expiratory volume in one second and peak expiratory flow rate tests. Results: The $PM_{2.5}$ concentrations for both market locations ranged between 1,269.3 and $651.7{\mu}g/m^3$, while $PM_{10}$ concentrations were between 1,048.2 and $818.1{\mu}g/m^3$. The recorded concentrations exceeded the World Health Organization guideline limit of $50{\mu}g/m^3$ and $25{\mu}g/m^3$ for $PM_{2.5}$ and $PM_{10}$, respectively. As compared with control group (2.1 L), significantly lower forced expiratory volume in one second value (1.61 L) was observed among the exposed group (p < 0.05). Likewise, significantly lower peak expiratory flow rate value (186.7 L/min) was recorded among the exposed group than the control group (269.51 L/min) (p < 0.05). Conclusion: Exposure to grain dust and diesel exhaust accentuated respiratory disorders with declines in lung functions amongst grain millers. Improved milling practices and engaging cleaner milling facilities should be adopted to minimize exposure and related hazards.

  • Objectives : Nitrogen dioxide $(NO_2)$ has been inconsistently associated with gradual decreases in lung function. Here, we studied the effects of $NO_2$ exposure in asthmatics by examining the association between changes in lung function and concentrations of $NO_2$ which were personally measured. Methods : Peak expiratory flow (PEF) and daily personal exposures to $NO_2$ were recorded on 28 patients with asthma (confirmed by methacholine provocation test) over 4 weeks. We used generalized estimating equations to assess the relationship between personal $NO_2$ exposure and PEF, adjusting for potential confounders such as age, gender, outdoor particulate matter, temperature, humidity, and exposure to environmental tobacco smoke. Results : The personal $NO_2$ exposures were higher than the corresponding ambient levels. The mean personal: ambient ratio for $NO_2$ was 1.48. The personal $NO_2$ exposures were not associated with the morning PEF, evening PEF, or the diurnal PEF variability. However, environmental tobacco smoke was negatively associated with both the morning and evening PEF. Conclusions : Among the asthmatic adults who participated in this study, we found no apparent impact of personal $NO_2$ exposures on the peak expiratory flow.

수술후 폐기능상태 평가를 위한 시각적 상사척도(VAS)의 효용성에 관한 연구 (The Effectiveness of VAS for Evaluation of Pulmonary Condition in Postoperative Patients.)

  • This study was designed and undertaken to find out the effectiveness of VAS for evaluation of general anesthetic postoperative pulmonary-function. We compared the degree of perceived pulmonary function recovery with peak expiratory flow at postoperative 72 hours of subjects. The subjects of this study were collected 38 patients who had received upper abdominal operation in St. Paul Hospital, Catholic University Medical College, and Kangnam Scared Heart Hospital and Kangdong Scared Heart Hospital, Hallym University. Data collection period was from June 15th, to August 7th, 1992. The degree of pulmonary recovery function was measured with peak expiratory at 72 hours postoperatively. The degree of perceived pulmonary function of the patient was measured with ten points visual analog scale at 72 hours postoperatively. Peak expiratory flow and visual analog score was analyzed with Pearson correlation. Peak expiratory flow was expressed as a ratio of preoperative value. The result was as follows : The recovery of pulmonary function and the degree of perceived pulmonary function of the patient at 72hours postoperatively was revealed high correlation (r=.84). The above result suggested that patients with general anesthetic upper abdominal surgery should evaluate recovery of pulmonary function making use of VAS. We know that VAS is very useful in postoperative patients. We perceived that VAS is to take up a positive attitude of patients. Nurse should furnish the nursing care objectively and scientifically to patients. As VAS was economic and simple, VAS should be adviced for wider application.

  • PURPOSE: The purpose of this study was to examine the effects of respiratory muscle training on respiratory function, respiratory muscle strength, and cough capacity in stroke patients. METHODS: This study used a nonequivalent control group pre-post test design. We recruited thirty-four stroke patients(16male, 18female), who were assigned to intervention (n=17), or control (n=17) groups. Both groups participated in a conventional stroke rehabilitation program, with the intervention groups also receiving respiratory muscle training 20 minutes a day, three times a week, for 4 weeks. Respiratory function (forced vital capacity) and respiratory muscle strength (maximal inspiratory pressure, maximal expiratory pressure) were assessed by spirometry. Cough capacity (peak expiratory flow) was assessed using a peak flow meter. The collected data were analyzed by independent and paired t-tests. RESULTS: The intervention group showed a significant increase in the forced vital capacity (FVC), maximal inspiratory pressure (MIP), maximal expiratory pressure (MEP) and peak expiratory flow (PEF) at the end of the program, while the control group showed no significant changes. CONCLUSION: This study showed that respiratory muscle training increased respiratory function, respiratory muscle strength, and cough capacity in stroke patients and prevented a decrease in cough capacity. These findings suggest that respiratory muscle training effect on respiratory function, respiratory muscle strength and cough capacity for rehabilitation in patients with stroke.

  • The aim of this study was to investigate the effect of abdominal drawing-in maneuver (ADIM) on peak exploratory flow (PEF), forced exploratory volume in 1 second ($FEV_1$), and low back pain during forced expiration. Twenty-two subjects (14 subjects in experimental group, 8 subjects in control group) participated in this study. The stabilizer was used for ADIM training for five consecutive days. Vitalograph PEF/$FEV_1$ DIARY and visual analogue scale (VAS) were used to determine forced expiratory pulmonary function and low back pain, respectively. Independent t-test and analysis of covariance were used for statistical analysis with a significance level of .05. The findings of this study were as follows: 1) There were no significant differences of ADIM effect on PEF and $FEV_1$ between experimental group and control group. 2) There was a significant pain reduction in experimental group with ADIM. 3) PEF and $FEV_1$ increased significantly in the fifth day compared with the first day pre-exercise baseline. Therefore, it is concluded that ADIM was effective in improving PEF and $FEV_1$, and reducing VAS during forced expiration in patients with chronic low back pain.

The effect of Schroth's three-dimensional exercises in combination with respiratory muscle exercise on Cobb's angle and pulmonary function in patients with idiopathic scoliosis

  • Objective: The objective of this study was to investigate the effects of Schroth's three-dimensional exercises in combination with respiratory muscle exercise (SERME) on Cobb's angle and functional movement screen (FMS). Design: Randomized controlled trial. Methods: Fifteen subjects with scoliosis were randomly assigned to two groups. Eight subjects were assigned to the experimental group and seven subjects were assigned to the control group. The experimental group underwent SERME using SpiroTiger (Idiag, Switzerland), while the control group performed only the Schroth's three-dimensional exercises (SE). Both groups performed exercises for one hour per day, three times a week for eight weeks. Cobb's angle, pulmonary function (forced vital capacity, forced expiratory volume at one second, and peak expiratory flow) and FMS were measured before and after the experiment. Results: After intervention, the SERME group showed a significant difference in Cobb's angle, FMS scores, and pulmonary function as compared to before intervention (p<0.05). In the SE group, there was a statistically significant difference in Cobb's angle, pulmonary function, and FMS scores compared to before intervention (p<0.05). The SERME group showed a significant difference in Cobb's angle and peak expiratory flow in pulmonary function compared to the SE group (p<0.05). Conclusions: The results suggest that SERME could be a more effective intervention for improvement of the Cobb's angle and pulmonary function for scoliosis patients.

  • Background: Measurement of peak expiratory flow rate (PEFR) in a follow-up examination for a chronic airway disease is useful because it has the advantages of being a simple measurement and can be repeated during examination. The aim of this study was to examine the annual decrease of PEFR in asthma and chronic obstructive pulmonary disease (COPD) patients and to confirm the factors which influence this decrease. Methods: From May, 2003 to September, 2010, the annual decrease of PEFR was obtained from asthma and COPD patients attending an outpatient pulmonary clinic. PEFR was measured using a Mini-Wright peak flow meter (Clement Clarke International Ltd. UK), and we conducted an analysis of factors that influence the change of PEFR and its average values. Results: The results showed an annual decrease of $1.70{\pm}12.86$ L/min the asthmatic patients and an annual decrease of $10.3{\pm}7.32$ L/min in the COPD patients. Age and $FEV_1$ were the predictive factors influencing change in asthma, and $FEV_1$ and smoking were the predictive factors influencing change in COPD. Conclusion: We confirmed the annual decreasing PEFR in patients with chronic airway disease and identified factors that work in conjunction with $FEV_1$ to influence the change.

수동폐확장과 능동호흡강화 기법이 건강한 성인 폐기능에 미치는 영향 (The Effect of Passive Lung Expansion Technique and Active Respiration Enhancement Technique on Lung Function in Healthy Adults)

  • Purpose : This study was conducted to investigate the effect of positive active pressure technique and active breathing technique on lung function in healthy adults. Methods : In this study, the passive lung expansion technique and active respiration enhancement technique using an air mask bag unit were conducted in 30 normal adults to observe changes in pulmonary function with forced vital capacity (FVC), Forced expiratory volume at one second (FEV1). In order to observe the change in the level of respiratory function, we would like to investigate the peak expiratory flow (PEF) and the forced expiratory flow (FEF 25-75 %). Results : As a result of this study, there was no significant difference in comparison between the passive lung expansion technique and the active breathing enhancement technique (p>.05). The passive lung expansion technique effectively increased the effortful expiratory volume and the median expiratory flow rate of 1 second (p<.05). And the passive lung expansion technique effectively increased the effortless lung capacity and the maximum expiration flow rate (p<.05). Conclusion : The passive lung expansion technique effectively increases the range of motion of the lungs and chest cages, intrathoracic pressure, and elasticity of the lungs, and the active breathing technique increases the muscle functions such as the diaphragm and the biceps muscles. It is expected that it will be able to selectively improve the respiratory function of patients with respiratory diseases or functional limitations as it is found to be effective.
