Research Trends of Pattern Identification of Korean Medicine Using the Network Analysis (네트워크 분석을 통한 국내 변증 연구의 동향)

  • This study aims to analyze Korea's trend of 'pattern identification(PI)' research using network analysis. We selected 368 papers from OASIS (Oriental Medicine Advanced Searching Integrated System) by review of two Korean medicine experts. Network analysis between keyword of PI studies were performed. As a result, number of PI studies has increased over the last 10 years. 'Korean journal of oriental physiology and pathology' and 'Journal of Korean oriental internal medicine' have published most papers related to PI. As research category, there are 144 papers in the cross-sectional study, and 71 papers in case report study. In the case of randomized clinical trials, there are only 3 papers. In studies for PI, stroke, pattern analysis, and blood stasis were the keywords make up the main network.

Analysis of Tinnitus Pattern by Visceral Pattern Identification and Treatment Efficiency by Pattern Identification Type (장부변증에 따른 이명 양상과 변증유형별 치료효율 분석)

  • Objectives : By analyzing symptoms of tinnitus, we tried to find out the relationship between deficiency and excess(虛實) and visceral pattern identification(臟腑辨證). By testing Tinnitus Handicap Inventory(THI), we evaluated the quality of life of people suffering from various aspects of tinnitus, as well as comparing treatment efficiency. Methods : 52 patients were recruited in this study who was ill with tinnitus. They wrote out the questionnaire about tinnitus and the THI. Results : The vast majority of high-grade tiny noise patients are deficiency type(虛症). The vast majority of low-grade roar patients are excess type(實症). Tinnitus persistence and feeling of ear occlusion were prominent in low-grade roar patients. low-grade roar patients suggest that overall quality of life is lower than high-grade tiny noise patients. In terms of treatment efficiency, spleen-stomach weakness(脾胃虛弱)-type, stomach heat(胃熱)-type and phlegm-fire(痰火)-type was higher than kidney essence depletion(腎精虧損)-type and liver qi depression(肝氣鬱結)-type regardless of the aspect of tinnitus. Conclusions : The present study suggests that tinnitus pattern may be helpful in differentiating patients with tinnitus, and the effectiveness of treatment can be predicted through differentiation.

  • State Administration of Traditional Chinese Medicine of the People's Republic of China lead Research on Chao medicine's Constitutional Treatment project by which could involve the following principles. First, constitution can be identified, second, constitution correlates to ones susceptibility to diseases, third, constitution can be harmonized, fourth, prescription made according to ones constitution and patterns. These ideas is originated by Lee Je-ma's theory of Four Constitution Type in "DongEuiSooSeBoWon"(Longetivity and Life Preservation in Eastern Medicine). This is much similar to currents trends of personalized medicine in western medical sciences. Therefore further developing Lee's theory would have significant value on various fields of medicine.

Preliminary Research for Development of Instrument for Cold-Heat & Deficiency-Excess Pattern Identification of Dementia (치매(痴呆)의 한열허실(寒熱虛實) 변증(辨證)을 위한 지표 문항 개발에 관한 기초 연구)

  • This study was performed to develop cold-heat and deficiency-excess pattern identification for dementia, as well as for standard Korean medicine diagnosis and treatment. Five experts comprised of 4 neuropsychiatrists of Korean medicine and 1 statistician to develop cold-heat and deficiency-excess pattern identification for dementia. We searched studies about pattern identification and selected 507 articles using Oasis search terms provided by the KIOM. As a result, 10 pattern identification research study were recruited. Moreover, we analyzed neuropsychological assessments for dementia that evaluate Behavioral and Psychological Symptoms of Dementia (BPSD) and cognitive function using experts conferences and we selected neuropsychological instruments using pattern identification. Six cold patterns, six heat patterns, ten deficiency patterns, and four excess patterns were identified according to the cold-heat and deficiency-excess pattern identification of dementia. We selected the Caregiver-Administered Neuropsychiatric Inventory and the Korean Mini-Mental State Examination as neuropsychological assessments of dementia, which examine behavioral symptoms and cognitive function, suspectively. We formed positive and negative correlation between Korean medicine pattern identification and neuropsychological assessments for dementia. We developed and suggested a forecast module of pattern identification for dementia. But, it is necessary to perform additional clinical trials to verify its validity and accuracy.

The correlation between average temperature & precipitation and Pattern Identification of the patients with stroke (중풍환자의 변증(辨證)과 기온 및 강수여부와의 상관성에 관한 연구)

  • Objectives : This study was to investigated the correlation between average temperature & precipitation and pattern identification of the patients with stroke. Methods : 115 stroke patients within 30days of onset were registered during the study period. Pattern identification of stroke was classified into Fire and heat, Turbid phlegm and others. We took the mean of average temperature according to pattern identification and analyzed pattern identification into two groups according to climatological normal of average temperature in Seoul. We also analyzed pattern identification into two groups according to existence of precipitation. Results : Heat and fire, and other type of pattern identification had high frequency when average temperature was higher than 12.2℃. And there was no correlation between precipitation and pattern identification of stroke.

  • There are many reports about correlations between meteorological elements and stroke. In Oriental medicine, it is recognized that the weather affects the human body and diseases, but there are few studies about the correlation between meteorological elements and pattern identification of stroke. 105 stroke patients classified into fire-heat pattern or dampress-phlegm pattern were registered during the study period. We took the measurement of each meteorological element (atmospheric pressure, temperature, humidity, wind speed) according to pattern identification and analyzed pattern identification into two groups according to mean of each meteorological element during the study period. Mean temperature was higher with the heat-fire pattern than with the dampness-phlegm pattern. Heat-fire pattern also had higher frequency when temperature was higher than mean temperature. There was no correlation between atmospheric pressure, relative humidity, or wind speed and pattern identification.

  • The study was to investigate the distribution for the diagnosis of pattern identification questionnaire and agreement rate between diagnosis of pattern identification based on obesity pattern identification questionnaire and the clinical diagnosis of pattern' identification by medical specialist. The distribution for the diagnosis of pattern identification based on obesity pattern identification questionnaire was shown in order of stagnation of liver Gi, retention of undigested food, deficiency of Yang at scale of 5, 3, 2 score and the diagnosis rate of single pattern identification at scale of 5, 3, 2 score was 89.96%, 79.33%, 54.64%, respectively the agreement rate between the diagnosis of pattern identification based on obesity pattern identification questionnaire and the clinical diagnosis of pattern identification by medical specialist was 0.1013. Therefore, the complementary management in CRF questionnaires with consultation from experts and the study for score difference of pattern identification will improve the accuracy and agreement rate, which will will be helpful for pattern identification of obesity by clinical experts.

Analysis of Pattern Identification and Related Symptoms on Idiopathic Short Stature -Focusing on Traditional Chinese Medicine Literature- (특발성 저신장의 변증 유형 및 변증별 증상 분석 -중의학 논문을 중심으로-)

  • Objectives We aimed to analyze traditional Chinese medicine (TCM) literatures in regards to the pattern identification and related symptoms of idiopathic short stature (ISS). Methods We searched relevant literatures published up to September 29, 2020 through three Chinese electronic databases. We performed frequency analysis of the selected studies by extracting information on pattern identification, clinical symptoms, and TCM treatments presenting pattern identification of ISS. Results Sixteen studies were included. Spleen deficiency, kidney deficiency, dual deficiency of spleen-kidney, and liver-kidney yin deficiency were frequently reported. Clinical symptoms of the spleen deficiency include sallow complexion, body constituent weakness, anorexia, lack of qi and no desire to speak, and loose stools. Herbal medicines (HMs) such as Sijunzi-tang were frequently reported. Clinical symptoms of the kidney deficiency include cold limb and fear of cold, soreness and weakness of waist and knees, and clear and long urine. HMs such as Bishendihuang-wan were frequently reported. Clinical symptoms of the dual deficiency of spleen-kidney include body constituent weakness, spirit lassitude and lack of strength, anorexia, soreness and weakness of waist and knees, and cold limb and fear of cold. HMs such as Sijunzi-tang plus Bishendihuang-wan were frequently reported. Clinical symptoms of the liver-kidney yin deficiency include tidal fever and night sweating, heat in the palms and soles, dizziness, and dry throat. HMs such as Liuweidihuang-wan were frequently reported. Conclusions This was the first study to analyze the frequency of pattern identification and related symptoms on ISS. In the future, a standardized Korean medicine pattern identification system should be established.

Study of the Herbal Medicine for Stroke by Korean Standization of Pattern Identification for Stroke (한국형 중풍변증표준안-Ⅱ의 변증별 사용 처방에 대한 연구)

  • This study was performed to investigate using of herbal medicine based on pattern identification of symptoms, such as Qi deficiency pattern, Yin deficiency pattern, Phlegm -dampness pattern, Static blood pattern, Fire-heat pattern. These patterns settled by Korean Standard Differentiation of the Symptoms and Signs for Stroke in 2005. This study was done with 177 patients in Daejeon University Oriental Medical Hospital in the period of November 2006 to July 2007. Among the five types of pattern identification, Phlegm -dampness pattern showed significantly high frequency especially in 3 weeks after stroke. The sort of herbal medicine was 43 and Dodamhwalhyoel-tang(26.61%), Sunhwanki 1 hobang(14.52%), Banhabaekchulcheonma-tang(4.84%), Ansinchongnoi-tang(4.84%), Chongryuldodam-tang(4.03%) were most frequently used. Especially Dodamhwalhyoel-tang showed significantly high frequency in 2 weeks after stroke. Based on these results, it is suggested that more practical Korean Standard Differentiation of the Symptoms and Signs for Stroke would be established through continuous clinical studies by giving weight on relationship between herbal medicine and pattern identification.

Proposal of Form-Color-Pulse-Symptom Diagnostic System for Enhancement of Diagnostic Rate of 8 Principle Pattern Identification - Focusing on Cold Heat Pattern Identification - (팔강변증의 진단율 향상을 위한 형색맥증진단(形色脈證診斷)시스템 설계 - 한열변증을 중심으로 -)

  • In order to enhance the 8 principle pattern diagnosis rate comparing with diagnostic method by self-report questionnaire on cold/heat pattern in the clinical practice, a new diagnostic method using form-color-pulse-symptom (FCPS) system is proposed. FCPS system is composed of outputs of cold/heat pattern through the calculation process of contribution degree to the cold, heat pattern and qi, blood, yin, yang deficiency patterns, based on analysis of 16 mechanisms of disease calculated by diagnostic system of oriental medicine (DSOM) first. And second component is an output of differentiated 8 principle patterns in detail through binding and calculating process with digital informations of pulse, color, form, constitution obtained by computerized measurement system. Putting together above two processes consecutively, cold-heat complex or true/false cold/heat patterns and personalized characters of cold/heat patterns of each patient can be subdivided through a computation method of determining each pattern. In conclusion, 8 principle pattern identification can be performed more accurately using FCPS system than existent self report questionnaire method. These hypothetic proposal is needed to be proven by clinical trial for the future and then the accurate numbers used in each calculational function should be revised properly.
