Status of Herbal-drug-associated Adverse Drug Reactions Voluntarily Reported by EMR (1개 대학 한방병원에서 EMR을 통해 보고된 한약에 의한 약물유해반응의 현황)

  • Objectives : The aim of this study was to systematically investigate herbal-drug-associated adverse drug reactions (herbal ADRs) reports submitted by a single oriental hospital and to analyze the general characteristics, causative agents, clinical manifestations, severity and types of herbal medicines which caused herbal ADRs. Methods : This study proceeded with IRB approval. The data on herbal ADR were collected prospectively from January 2008 to February 2012 by EMR of Dongguk University Ilsan Oriental Hospital. The World Health Organization (WHO)-Uppsala Monitoring Center (UMC) criteria was used to determinate causality for each herbal ADR. WHO-Adverse Reaction Terminology (WHO-ART) System Organ Class (SOC) code and WHO severity category were also used in this study. Results : A total of twenty eight cases were reported. Twenty two cases were assessed to have over possible relations with herbal medication. The gender ratio of these cases were 64.6 percent female and 36.4 percent male, demonstrating no statistical significance. Patients aged over 60 were 59.1%. Gastro-intestinal system was reported to be the most frequently affected organ (38.8%), and followed by psychiatric system (22.4%), and integumentary system (22.4%). The most common clinical symptom was headache (12.2%), followed by diarrhea (10.2%), and pruritus (10.2%). The severity of most cases was assessed to be mild (89.8%). The percentage of moderate ones was 10.2%, and there were no severe cases. Conclusions : Progressive study and further analysis on herbal ADRs are warranted for safety in the clinical use of herbal medicines.

  • Dengue fever is an important health problem for international travelers to all endemic areas. The steadily increasing numbers of tourists visiting endemic areas raise the risk of exposure, and imported dengue cases are increasingly observed in nonendemic area. Dengue has a wide spectrum of clinical presentations, often with unpredictable clinical evolution and outcome. While most patients recover following a self-limiting, non-severe clinical course, a small proportion progress to severe disease such as dengue hemorrhagic fever or dengue shock syndrome. Therefore, it is important to suspect dengue fever in every febrile patient returning from the tropics. Whenever it is suspected, a quick diagnosis and adequate managements are essential to avoid complications. We report two cases of imported dengue fever in Korean children presenting with fever, headache, nausea, and rash.

  • The main symptoms of allergic rhinitis are sternulation, rhinorrhea and rhinanchon. It is also characterized by frontal headache, ohotophobia and epiphora. Alleric rhinitis occurs when inhaled alleren is in contact with nasal mucosa which causes immune response. Bojungiki-Tang is one of the most frequently used medical treatment for the allergic rhinitis. Two chronic perennial allergic rhinitis sutterers, whose alleraies are thought to be due to house dust and ticks, were investigated through the eosinoohil numerical index and the subject symptom numerical index for three months. After treatment, both patients showed a lower index for both tests. The symptom index scores diminished from 17 to 3, and from 16 to 5. The eosinophil numerical index taken through blood tests, diminised from 4 to 2, and from 4 to 3. The above results suggest that Bojungiki-Tang has the inhibitory effects on allergic rhinitis.

A STUDY ABOUT THE VARIATION OF STYLOID PROCESSES IN PANORAMIC RADIOGRAPHS (파노라마 방사선사진상에서 경상돌기의 변이에 관한 연구)

  • The purpose of this study was to investigate the variation in the length and shape of styloid processes and the relationships between the elongated styloid processes and the styloid process syndrome, using panoramic radiogrpahs and questionaires. The subjects were 342 patients consisted of 170 males and 172 females aged from 11 to 78 years, not showing facial asymmetry, who visited infirmary of dental college of Yonsei University. Also, the accuracy was determined for measuring the length of styloid processes from panoramic radiographs, using dried skulls. The results were as follows: 1. The length of styloid processes was magnified approximately 1.1 times, but there was no statistically significant differencies in the magnification rate of length between right and left side. 2. The mean radiographic length of styloid processes was 29.72±7.92㎜ in males, 27.93±6.69㎜ in females, and 28.82±7.37㎜ in total. And elongated styloid process (>30㎜) was seen in 31.3% of total subjects. 3. The growth in the length of styloid processes was completed in the third decade. 4. The most common shape of styloid processes was straight followed by segmented and bent form. The incidence of segmentation was reduced with increasing age. 5. The 74% of subjects with elongated styloid process (>30㎜) showed symptoms of the styloid process syndrome. The most frequent symptom was headache followed by discomfort in the neck when turning the head from left to right, tinnitus or earache, vague facial pain, discomfort or pain when swallowing, feeling that an object is caught in throat.

  • Post-herpetic neuralgia(PHN) is a chronic pain syndrome associated with the reactivation of a primary infection with varicella zoster virus(chinken pox), which leads to a chronic infection of dorsal root ganglia. The most common risk factor for shingles and its potential sequela, PHN, is advanced age. For a significant number of patients, the pain following healing of shingles can persist for months to years. If this pain, classified as PHN, persists longer than one month. PHN often leads to depression, disrupted sleep, decreased productivity, and utilization of health care. We treated a 60 year-old female patient who suffered pain and headache after Stellate Ganglion Blocks(SGB). In identifying points for differentiation of syndrom(辨證), this subject was diagnosed as Yangmyeong Merdian wind-heat syndrom(陽明經風熱證) and was administered Seungmagalgeuntanggamibang(revised Shengmagalgen-tang, 升麻葛根湯加味方). To ease pain, Western medication was administered as well. After fourteen days of treatment, pain and other symptoms improved.

The Effect of Ipidural Pain Block After Thoracotomy (개흉술후 지속적 경막외마취가 통증감소에 미치는 영향)

  • Continuous epidural pain block with a local anesthetic agents is a commonly employed technique for pain relief after thoracotomy. In this study, we evaluated the effectiveness of the continuous epidural pain block in 19 patients undergoing elective lateral or posterolatrral thoracotomy with control group(n=19) from November 1994 to July 1995, Epidural lidocaine and morphine mixtures were injected via an epidural catheter as a bolus after operation, and then bupivacaine and morphine mixtures were injected continuously following 5 or 6 days. The pain score, upper arm elevation(ROM score), and respiratory rate were significantly changed(P<0.05) from 30min after injection. The CO2 tension of arterial blood was decreased significantly(P<0.05) from 2hr after injection. The postoperative hospital days were decreased significantly(P<0.05). Side effects of the epidural pain block were urinary retention(n= 10), urticaria(n=2) and a case of headache. There was no postoperative lung atelectasis. We conclude that the continuous epidural pain block is good for prevention of the postoperative lung complication and early recovery after thoracotomy.

Effective Dosage of Intrathecal Morphine for Postoperative Pain Control of Perianal Surgery (항문 주위 수술후 통증관리를 위한 지주막하강내 Morphine의 유효량)

  • Background: Contraction of anal sphincter muscle produces severe pain in perianal surgery postoperatively. Recent reports have indicated that effective and prolonged pain relief can be obtained by the injection of small doses of morphine into the subarachnoid space. We attempted to use this technique for perianal surgery and investigated postoperative pain control and its side effects. Methods: Forty five patients scheduled for hemorrhoidectomy and anal fistulectomy were studied to determine the minimal effective dose of intrathecal morphine for postoperative analgesia. In order to control the pain, 7 mg of 0.5% hyperbaric bupivacaine with 0.05 mg (group I), 0.1 mg (group II) and 0.15 mg (group III) of morphine hydrochloride was injected with a 25 gauge spinal needle into the subarachnoid space. We estimated the duration of analgesia until the pain score attained to above 3 in 10 cm VAS (visual analogue scale) and incidence of itching, nausea and vomiting by percentage, headache, backpain and respiratory depression by positive and negative. We also checked the time of self-voiding. Results: The mean time of analgesia was $10.3{\pm}1.54$, $19.7{\pm}2.22$ and $20.3{\pm}2.29$ hours in group I, II and III respectively. Urinary retention of group I, II and III after block persisted for an average of $20.3{\pm}2.31$, $21.2{\pm}2.51$ and $23.3{\pm}3.74$ hours. Nausea and vomiting were observed 33%, 53%, 67% and itching was observed 53%, 67%, 80% in group I, II and III respectively and respiratory depression did not occur in all groups. Conclusions: It is not necessary to use more than 0.1mg of intrathecal morphine in perianal surgery because analgesia is not prolonged and side effects are increased.

  • Influenza is an infectious disease caused by an influenza virus with symptoms of high fever and headache. Since influenza especially mutates into multiple subtypes in the carrier's body, it is a serious threat for mankind such as Spanish influenza. The treatment of influenza infection mandates the use of antiviral drugs through rapid diagnostic test. Generally, immunochromatography-based rapid influenza diagnostic tests are used for rapid diagnosis in an emergency. In this paper, we propose an influenza analysis algorithm based on image processing to examine a large number of patients suspected of being infected with influenza. Also, we propose a robust influenza analysis algorithm based on the joint cumulative mass function under varying radiometric conditions such as illuminant and exposure differences. Simulation results show that the proposed algorithm significantly reduces the error of influenza diagnosis under different radiometric conditions.

COVID-19 vaccine-induced immune thrombotic thrombocytopenia: a review

  • Rare but serious thrombotic incidents in relation to thrombocytopenia, termed vaccine-induced immune thrombotic thrombocytopenia (VITT), have been observed since the vaccine rollout, particularly among replication-defective adenoviral vector-based severe acute respiratory syndrome coronavirus 2 vaccine recipients. Herein, we comprehensively reviewed and summarized reported studies of VITT following the coronavirus disease 2019 (COVID-19) vaccination to determine its prevalence, clinical characteristics, as well as its management. A literature search up to October 1, 2021 using PubMed and SCOPUS identified a combined total of 720 articles. Following the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guideline, after screening the titles and abstracts based on the eligibility criteria, the remaining 47 full-text articles were assessed for eligibility and 29 studies were included. Findings revealed that VITT cases are strongly related to viral vector-based vaccines, which are the AstraZeneca COVID-19 vaccine (95%) and the Janssen COVID-19 vaccine (4%), with much rarer reports involving messenger RNA-based vaccines such as the Moderna COVID-19 vaccine (0.2%) and the Pfizer COVID-19 vaccine (0.2%). The most severe manifestation of VITT is cerebral venous sinus thrombosis with 317 cases (70.4%) and the earliest primary symptom in the majority of cases is headache. Intravenous immunoglobulin and non-heparin anticoagulant are the main therapeutic options for managing immune responses and thrombosis, respectively. As there is emerging knowledge on and refinement of the published guidelines regarding VITT, this review may assist the medical communities in early VITT recognition, understanding the clinical presentations, diagnostic criteria as well as its management, offering a window of opportunity to VITT patients. Further larger sample size trials could further elucidate the link and safety profile.

  • In accordance with the new healthcare policy of government (Moon Jae-In Care) to strengthen health insurance coverage, the National Health Insurance (NHI) coverage of brain magnetic resonance imaging (MRI), brain/neck MR angiography (MRA), and head and neck MRI have been expanded since 2018 in Korea. This article has been reviewed focusing on the "Detailed matter concerning criteria and method for providing reimbursed services in the NHI. Some revisions" regarding reimbursement for MRI, which was revised from October 2018 to April 2020 and is currently in effect. It included the MRI reimbursement system in Korea, recent adjustment of the reimbursement coverage for patients with headache or dizziness, and reimbursement coverage, standard imaging, and radiologic report of brain MRI, brain/neck MRA and head and neck MRI. This article could help radiologists gain knowledge on health insurance to protect the expertise of the radiologist and to play a leading role in the hospital. As the policy changes, detailed matter concerning criteria and method for providing reimbursed services in the NHI may be revised. Therefore, radiologists should update issues related to insurance reimbursement for MRI continuously.
