Background: Testicular germ cell tumors (TGCTs) are a relatively common malignancy in young men. The aim of this study was to investigate the clinicopathological features and survival of young Turkish patients with TGCT. Materials and Methods: In this retrospective study, the clinical and pathological characteristics of young Turkish patients with TGCT who were monitored by the Department of Medical Oncology of a military hospital between 2008 and 2013 were investigated. Overall survival data were analyzed. Results: Ninety-six patients were included in the study. The mean age was 26.4 years. Among the patients, 17.7% had seminoma and 43.8% had mixed non-seminomatous germ cell tumors. Some 46.9% were Stage I, 30.2% were Stage II, and 22.9 were Stage III. Of the patients, 83.3% received chemotherapy, 25% underwent retroperitoneal lymph node dissection (RPLND), 3.1% received radiotherapy, and 12.5% were followed-up without treatment. In addition, 18.8% of the patients were administered salvage chemotherapy due to relapse or progression. The 5-year overall survival rate was 90.2% for all patients. The 2-year overall survival rate was 100% for Stage I patients, 94% for Stage II patients, and 70.2% for Stage III patients. The difference between the survival curves of stages was statistically significant (p=0.029). Conclusions: In young Turkish patients with TGCT, good results were obtained with appropriate treatment, most receiving chemotherapy. The prognosis of the disease was good even in the advanced stage.

Background Cryptotia is a congenital ear deformity in which the upper pole appears buried beneath the mastoid skin. Cryptotia is a common auricular malformation among Asians. The aim of this paper is to examine the surgical techniques for and complications of 34 cryptotic patients. Methods Surgery was performed for 34 cryptotic deformities (January 2005 to January 2012). Twenty-two patients (64.7%) were classified as having type I cryptotia, and 12 patients (37.5%) type II cryptotia. Among the type I cryptotia patients, 8 patients had mild deformity and 14 severe deformity. Among the type II cryptotia patients, 10 patients had mild deformity and 2 severe deformity. Results The mild deformities were corrected via Z-plasty, V-Y plasty, full-thickness skin graft, and transposition flap, while the severe deformities were corrected via cartilage graft or Medporfor the spread of cartilage adhesion of antihelix. There were two cases of reinvagination in the autologous cartilage graft group. Implant exposure occurred with Medpor (two cases). There were two cases of hypertrophic scar on the previous surgical wound with Medpor. There were no complications in the 18 patients who had mild deformities. Conclusions The type I cryptotia patients had more severe deformities than the type II cryptotia patients. As most of the type II cryptotia patients had only mild deformities, their deformities were corrected without using autologous conchal cartilage graft or Medpor, except for two patients. Through more case analyses, researchers should make an effort to identify methods for recurrence and prevention of complication.
The paper is based on the record of researching the patients with cancer in the Chun-nam National University Hospital from September 1985 to December 1988. The results are the as follows ; 1. Among the total O.P.D. 921, 028, the patients of Therapeutic Radiology (Opening the Therapeutic Radiology in September) are classified into 27,159(2.95%), (186 in 1985, 2,388 in 1986, 10,511 in 1987, and 14,074 in 1988) 2. Among the 4,925 cancer patients, cervix and uterus cancer patients are 1,138(23.10%), stomach cancer patients are 592(12.02%), brain and thyroid cancer patients are 565(11.47%), liver cancer patients are 400 (8.12%), lung cancer patients are 355 (7.20%) and sexual ratio appeared 1 : 1.13. Therefore, female patients are a slightly more than the male patients. 3. The age distribution of cancer was that of $45{\sim}54$ ages are 1,244(25.26%), $55{\sim}64$ ages are 1,119(22.72%) and $35{\sim}44$ ages are 773(15.70%) and the half of all the cancer patients are $45{\sim}64$ ages. 4. Among the 2,519 cancer patients, 742(29.46%) are in the uterus system, 620(24.62%) are in the brain and thyroid part, 402 (15.96%) are in the lungs. Therefore, these three kinds of cancer consist of 70%. 5. The occupational distribution of 3,067 cancer patients ($87{\sim}88\;year$) house wives are 636 (20.73%), orderly farmers are 622 (20.28%) public service personnells are 193 (6.29%), salarymen are 162(5.28%) and businessmen are 159(5.18%).
Purpose: The aim of this study was to describe our treatment experiences with patients who had acute abdomen (AA) with common bile duct (CBD) dilatation. Methods: The treatment outcomes in children with AA and CBD dilatation were retrospectively reviewed. According to the shape of the intrahepatic bile ducts on ultrasonography (US), the origin of the pain was estimated as choledochal cyst (CC) complication or choledocholithiasis in normal CBD. Patients with complicated CC underwent surgery, and patients with choledocholithiasis in a normal appearing CBD underwent symptomatic treatment initially. Results: Of the 34 patients, 30 (88.2%) were female. The mean age of the patients was $6.4{\pm}4.9$ (range, 0.8-17) years. Seventeen (50.0%) patients had CBD stones and 17 (50.0%) did not. Surgical treatment was performed in 20 (58.8%) patients, 2 of whom underwent preoperative stone removal with endoscopic retrograde cholangiopancreatography and an operation. Conservative treatment was applied in 12 (35.3%) patients (8 with and 4 without stones), 1 of whom developed symptom relapse and underwent an operation. Among the 8 patients with CBD stones, 4 (4/17, 23.5%) had complete resolution of the stones and recovery of the CBD diameter after conservative treatment. US findings of patients with stone showed a fusiform or cylindrical shape of the CBD in 14 (82.4%) patients. Conclusion: The presence of stones in the distal CBD and the US features of CBD dilatation may be helpful to diagnose and treat the causes of biliary dilatation. Conservative treatment can be considered as initial therapy in patients with uncomplicated CBD dilatation with stone.

Purpose: Recently, as the number of people who enjoy water sports life in river or lake easy to accessible are increasing, that of the patients who are injured in water sports also does gradually. We intend to investigate the type of the injured patients of water sports and the riskiness of the sports in this study. Methods: We retrospectively looked into the medical records of the patients who were injured in water sports and visited a general hospital in Gangwondo-province from 2010 to 2015. Results: Total 146 patients came to the hospital during six years. Patients mostly occurred at younger ages, in summer, and on holidays, rather than weekdays. The most common lesions of injuries were faces (53 patients). The most common types of injuries were contusions (62 patients), followed by fractures (32 patients) and lacerations (26 patients). The most frequent fracture sites were the upper extremities (11 patients). Most of the trauma patients were mild, but a small number of patients with aspiration pneumonia occurred and their severity was higher than trauma patients. Conclusions: In this study, facial injuries were most frequent in water sports injuries. In the fractures, upper extremities were the most common fractured lesions. In addition, even if there is no direct trauma, aspiration pneumonia is serious, so caution should be taken with protective equipment suitable for water sports.
Background: The prognosis for young patients generally considered to be poor. The purpose of this study was to determined whether the clinical characteristics and the survival rate in young lung cancer patients after surgical treatment differs from that in older patients. Material and Method: Of 526 patients, 28(5.3%) were 30 to 40 years old. We studied the clinical characteristics and prognosis of 28 patients aged 40 years or less(Group 1), in whom primary lung cancer was diagnosed and operated between 1990 and 1997, and compared them with those 498 patients aged more than 40 years(Group 2). Result: The differences in sex ratio that were higher for women in Group 1, but there was no significant difference (p=0.297). The percentages for smokers and symptoms in Group 1 were significantly less than in Group 2.(p=0.049, p=0.008). Adenocarcinoma was significantly more common (p=0.018) and squamous cell diagnosed was diagnosed as stage IIIb or stage IV in 9 patients(32.1%), while 12.0% of the patients older than 40 years of age had either stage IIIb or stage IV(p=0.002). The 5-year survival rate was 41.3% in Group 1 ; 37.7% in Group 2, and the median survival time was 24.3 months in Group 1 ; 27.0 months is Group 2. There were no significant difference in survival between two age groups(p=0.808). Conclusion: Younger patients have more adenocarcinoma, however have less squamous cell carcinoma, less symptoms and less smoking history. Although younger patients tended to have more advanced disease and less complete resection rate, the recurrence and the long term survival in these patients did not differ that of older patients.

Objective: Clostridium difficile Infection (CDI) is one of the common nosocomial infections. As elderly population increases, the proper treatment has been emphasized. We investigated the risk factors associated with CDI unimprovement in elderly patients. Furthermore, we performed drug use evaluation of old CDI patients and oldest-old CDI patients. Methods: It was a retrospective study using electronic medical record at Kangbuk Samsung Medical Center (KBSMC) from January 2016 to December 2017. Seventy three patients aged 65 years or older, diagnosed with CDI by Clostridium difficile Toxin B Gene [Xpert] were screened and they were assessed for risk factors regarding unimprovement status. We also evaluated drug use evaluation in old patients ($65{\leq}age$<80) and oldest-old patients ($80{\leq}age$) by assessing the use of initial therapy, severity, dose, route, treatment course, days of use, total days of use and treatment outcome of initial therapy. Results: Out of 73 patients aged over 65 years, four patients were excluded because they did not receive any treatment. There were 31 improved patients and 38 unimproved patients after initial therapy. We were able to find out patients with surgical comorbidity or endocrine comorbidity (especially, diabetes mellitus) had 2.885 more risk of becoming unimproved than those patients without surgical comorbidity or endocrine comorbidity. Drug use evaluation for CDI was generally fair, but vancomycin as initial therapy is more recommended than metronidazole. Conclusion: Although age, antibiotics exposure, use of antacids are all important risk factors for CDI, our result did not show statistical significance for these risk factors. However, the study is meaningful because the number of elderly population keeps increasing and recently updated guideline suggests the use of vancomycin as drug of choice for CDI.
There are few reports about the beliefs of psoriasis patients related to their disorder and therapy modalities except for the medical ones. We aimed to investigate the beliefs of psoriasis patients about their disorders, where they had attended therapies unrelated to dermatologists and what modality of unconventional therapies they used for psoriasis in Turkey. The study was performed on 141 psoriasis patients. The age, sex, education status and the number of visits to the dermatology clinics were recorded. How the patients decided to visit the dermatology clinic (themselves, directed by others), where they attended for treatment of psoriasis outside of the dermatologist, what they think about the causes of their disease and what they used for treatment of psoriasis were asked. The mean age of the patients was $33.44{\pm}15.13$ years. The patients whose ages were over 30 years significantly more went to dermatology clinics. 68.1% of the patients attended the dermatology clinic based on the individual decision, 18.4% were directed by family members and 5.7% by friends. 45.4% of the patients believed that stress was the cause of their disease. 54.6% of the patients had used at least one type of unconventional therapy for psoriasis. The most frequently used unconventional therapy forms were herbal (24.1%) and spiritual therapies (22%). One conclusion is psoriasis patients need education about their diseases. Dermatologists should have information about the tendencies of the patients and approach the patients with sensitivity and empathy and give objective knowledge about psoriasis.
Purpose: The objectives of this study were to examine the difference between scores assigned by dementia patients and their caregivers to the patients' anxiety, depression, and quality of life. Methods: After obtaining Institutional Review Board(IRB) approval, face-to-face interview with the patients and their caregivers respectively was conducted by trained graduate-level nursing students from December 2007 to February 2008. Patients' anxiety, depression, and quality of life were measured by patients and their caregivers. The data obtained were analyzed using the SPSS/WIN 14.0 program, which was used for frequency, percentage, mean, standard deviation, t-test, and Pearson's correlation. Results: Significant relationships were reported between the depression rated by patients and that rated by their caregivers(r = .37, p = .019). In addition, there was no difference between the quality of life rated by patients and that rated by their caregivers(t = -7.11, p = .479). However, there was no significant relationship between the anxiety rated by patients and that rated by their caregivers(r = .21, p = .195). Conclusion: There were no differences on level of depression and quality of life of dementia patients measured by dementia patients and caregivers, However, dementia patients' anxiety level has discrepancy between them.
