Neural Tract Injuries by Penetration of Foreign Body: a Diffusion Tensor Tractography Study

  • We presented with a patient who showed injury of the cingulum and fornix by penetration of a foreign body into the brain on diffusion tensor tractography (DTT). A 63-year-old man suffered a brain injury by a part of a power saw blade that was suddenly detached from a power saw during work. A part of the power saw blade penetrated his right frontal skull and advanced to the right posterior horn of the lateral ventricle. This penetration caused traumatic intracerebral hemorrhage in the right frontal lobe and intraventricular hemorrhage in the lateral ventricle. He underwent craniotomy and removal of intracranial foreign bodies (bony pieces and saw blade). The patient's Memory Assessment Scale scores were 74 (4%ile) for global memory, 78 (7%ile) for verbal memory, and 80 (9%ile) for visual memory. DTTs showed disruptions in the anterior portion of the fornical body, right fornical crus, the anterior portion of the right cingulum, and the middle portion of the left cingulum, compared to the control. It seems that the sustained memory impairment of this patient might be related to injury of the cingulum and fornix.

Nutritional Assessment of Cancer Patients in Tehran, Iran

  • Background and objectives: Weight loss and malnutrition are common among cancer patients, these two factors greatly affecting survival and quality of life during treatment. Since cancer is becoming increasingly common in the world and in order to provide better treatment measures, it is important to identify and prevent side effects. The present study has been conducted in 2010 on a sample of cancer patients in the oncology center of Shahid Beheshti University of Medical Sciences to determine the prevalence rates of malnutrition and the factors affecting it. Methods: The PG-SGA standard questionnaire was administered to 416 cancer patients to evaluate their nutrition status and determine the frequency of each malnutrition stage. Correlations and ANOVA tests were used to analyze the relationship between factors and weight loss and how they might affect the development of malnutrition. Results: The prevalence of malnutrition among the patients was 53.1% out of which 29.1% had moderate and 24% had severe malnutrition. The most common factors inducing nutritional symptoms were depression and anorexia. Some 35 % of the patients had over 5% weight loss in the last mouth. The average PG-SGA score was 10.1 with 49 being the highest. 46.1 percent of the patients scored over 9 (requiring critical nutrient intervention). Malnutrition has a high correlation with weight loss, activity limitations, nutritional symptoms, and cancer stage, but low correlation with treatment and pathologic type. Conclusion: Malnutrition has a high prevalence in Iranian cancer patients and has a close relationship with mortality, morbidity and treatment-related problems and also quality of life. Therefore, periodical assessment by PG-SGA to detect malnutrition in patients should be made so that appropriate nutritional interventions can be provided.

Quality of Life in Malaysian Colorectal Cancer Patients : A Preliminary Result

  • Objective: Rapidly increasing colorectal cancer (CRC) incidence in Malaysia and the introduction of cutting edge new treatments, which prolong survival, mean that treatment outcome measures meed to be evaluated, including consideration of patient's quality of life (QoL) assessment. There are limited data on QoL in CRC patients, especially in Malaysia. Therefore, this study was performed focusing on cancer stages and age groups. Methods: The cross sectional study was conducted from June to September 2011 at three public tertiary hospitals with the EORTC QLQ C-30 questionnaire in addition to face to face interview and review of medical records of 100 respondents. Results: The mean age was 57.3 (SD 11.9) years with 56.0% are males and 44.0% females, 62% of Malay ethnicity, 30% Chinese, 7% Indian and 1% Sikh. Majority were educated up to secondary level (42%) and 90% respondents had CRC stages III and IV. Mean global health status (GHS) score was 79.1 (SD 21.4). Mean scores for functional status (physical, emotional, role, cognitive, social) rangeds between 79.5 (SD 26.6) to 92.2 (SD 13.7). Mean symptom scores (fatigue, pain, nausea/vomiting, constipation, diarrhea, insomnia, dyspnoea, loss of appetite) ranged between 4.00 (SD 8.58) to 20.7 (SD 30.6). Respondents role function significantly deteriorates with increasing stage of the disease (p=0.044). Females had worse symptoms of pain (p=0.022), fatigue (p=0.031) and dyspnoea (p=0.031). Mean insomnia (p=0.006) and diarrhea (p=0.024) demonstrated significant differences between age groups. Conclusion: QOL in CRC patients in this study was comparable to that in other studies done in developed countries. Pain, fatigue and dyspnoea are worse among female CRC patients. Given that functions deteriorates with advanced stage of the disease at diagnosis, a systematic screening programme to detect cases as early as possible is essential nationwide.

사상의학적(四象醫學的)으로 접근한 노인성 입얼굴 이상운동증 환자 치험 2례 (2 Case Reports on Oro-facial Dyskinesia Patients Diagnosed and Treated in Sasang Constitutional Medicine)

  • Objectives This study was designed to report diagnosis and treatment by Sasang Constitutional Medicine to the patients with Oro-facial Dyskinesia. Methods These two patients were diagnosed as Soyangin and Taeeumin according to the result of Sasang constitutional diagnosis, and treated by Sasang constitutional medications and acupuncture. The progress was evaluated with the Global Assessment Scale (GAS). Result Oro-facial Dyskinesia was almost disappeared in each patients. the one patient was cured with Chongshimyeonja-tang and Yuldahanso-tang, and the other was cured with Hyeungbangjihwang-tang. Conclusion We can treat Oro-facial Dyskinesia whose cause is mostly idiopathic with the diagnosis and treatment of Sasang Constitutional Medicine.

Systematic Review of Recent Lipidomics Approaches Toward Inflammatory Bowel Disease

  • Researchers have endeavored to identify the etiology of inflammatory bowel diseases, including Crohn's disease and ulcerative colitis. Though the pathogenesis of inflammatory bowel diseases remains unknown, dysregulation of the immune system in the host gastrointestinal tract is believed to be the major causative factor. Omics is a powerful methodological tool that can reveal biochemical information stored in clinical samples. Lipidomics is a subset of omics that explores the lipid classes associated with inflammation. One objective of the present systematic review was to facilitate the identification of biochemical targets for use in future lipidomic studies on inflammatory bowel diseases. The use of high-resolution mass spectrometry to observe alterations in global lipidomics might help elucidate the immunoregulatory mechanisms involved in inflammatory bowel diseases and discover novel biomarkers for them. Assessment of the characteristics of previous clinical trials on inflammatory bowel diseases could help researchers design and establish patient selection and analytical method criteria for future studies on these conditions. In this study, we curated literature exclusively from four databases and extracted lipidomics-related data from literature, considering criteria. This paper suggests that the lipidomics approach toward research in inflammatory bowel diseases can clarify their pathogenesis and identify clinically valuable biomarkers to predict and monitor their progression.

소음인 태양병궐음증으로 진단한 갑상선 기능저하증으로 인한 수족냉증 치험 1례 (A Case of Cold Hypersensitivity from Hypothyroidism of Soeumin Diagnosed as Greater Yang Disease Reverting Yin Pattern)

  • Objectives The purpose of this case study was to report that cold hypersensitivity caused by hypothyroidism was alleviated by diagnosing and treating as Greater Yang Disease Reverting Yin Pattern. Methods The patient was administered Insamosuyu-tang and Sammisamyu-tang and treated with acupuncture. Global Assessment Scale (GAS) and Digital Infrared Thermal Imaging (DITI) was used to assess the improvements of symptoms. Results and Conclusions Cold hypersensitivity of hands and feet was improved to GAS 20, and hours of sleep were increased from one to six. On DITI, the increase of the temperature were 1-3 degree celsius on the chest and the abdomen, 0.2-1 degree on the forearms and the hands, 1-2 degree on the thighs, and 0.2-0.5 degree on the shins and the feet.

  • 입얼굴 이상운동증을 주증으로 입원한 환자에 대하여 소양인(少陽人) 흉격열증(胸膈熱證)으로 변증하고 한약, 침, 약침, 턱관절균형요법을 병행한 치료로 유의미한 호전 반응을 나타낸 1례를 보고하는 바이다.

  • Objectives : This case report aims to demonstrate the effectiveness of incisional needle therapy in treating facial scars resulting from autoimmune disorder. Methods : A single case of a 29-year-old female patient with facial scars due to systemic sclerosis was treated with incisional needle therapy. The treatment was applied over 14 sessions from February to September 2023. The efficacy of the treatment was evaluated using clinical photographs and the Physician's Global Assessment(PGA). Results : The treatment significantly improved scar appearance and reduced blood vessel visibility. The PGA scores, with an average of 2.66, indicating moderate improvement. Conclusions : Incisional needle therapy demonstrated potential as an effective treatment for scars caused by autoimmune disorders, due to minimal side effects and non-interference with ongoing treatments. It offers a promising alternative to conventional treatments, suggesting a need for further research for wider applications.

점액성 선암종으로 폐의 쐐기절제술을 시행한 환자에 대한 한방치료 1례 (A Case Report of a Mucinous Adenocarcinoma Patient Treated with Korean Medicine after Wedge Resection of the Lung)

  • The purpose of this study is to report and demonstrate the effect of Korean medical treatment on a wedge resection of a lung patient with mucinous adenocarcinoma. The patient was an 84-year-old female who was diagnosed with mucinous adenocarcinoma and underwent wedge resection. The patient was treated with acupuncture and herbal medicine (Banhahubak-tang and Gamiondam-tang). The EORTC Quality of Life Questionnaire, Core 30 (EORTC QLQ-C30), EORTC Quality of Life Questionnaire, Lung Cancer 13 (EORTC QLQ-LC13), numeric rating scale (NRS), and the Global Assessment (G/A) were used to assess the changes in symptoms, including operation site pain and dyspnea. After treatment, the patient showed improvement in operation site pain and dyspnea, based on the NRS and G/A. However, dyspnea and pain were aggravated based on the EORTC QLQ-C30 and EORTC QLQ-LC13. Therefore, Korean medicine may have a substantial benefit for patients with mucinous adenocarcinoma after wedge resection of the lung.

새싹테라피를 이용한 수두 흉터의 치료 16례 (A Clinical Report on Chicken Pox Scars Treatment with Sae-ssack Therapy)

  • 본 논문은 수두로 인한 위축성 흉터 환자 16명에 대한 새싹테라피(절개침) 시술 후 개선도를 평가하여 연구한 바를 기록하였다. 그 결과 새싹테라피는 수두 흉터에 대해 시술효과를 판단하는 척도인 PGA, SBSES에서 좋은 결과를 보였으며 환자 만족도도 3가지 항목으로 세분화하여 평가했을 때 높은 개선 점수를 보였다. 이런 임상 결과로 미루어 볼 때 새싹테라피는 수두로 인한 위축성 흉터에게 좋은 치료적 방법이 될 것으로 예상되며, 시술 중 통증 및 시술 후 붉은 기의 개선이 과제로 남기에 이에 대한 한의학적 치료법 연구가 추가로 요구되는 바이다.
