Management of a Severe Cervical Degloving Injury in a Bean Goose (Anser fabalis serrirostris)

  • A wild juvenile bean goose (Anser fabalis serrirostris) was rescued after sustaining an extensive degloving injury around the neck and chin region. Except for the degloving injury, physical and radiographic examination detected no other trauma-induced abnormality. On the day of presentation to the hospital, the patient underwent extensive debridement and suturing. A twice-daily force feeding with diluted parrot weaning food was required during the hospitalization due to severe anorexia. On day 18, the bean goose was moved to the aviary to undergo rehabilitation and feeding by itself. Management of the severe degloving injury of the cervical region with medical intervention and nutritional assistance was successful in this case, and the results of this case shows the proper management can be implemented to produce satisfactory outcomes.

  • The foreign bodies in the food passage and airway are very important in the Otolaryngological field, and practically there are many reports of them. Also the kinds of these foreign bodies are variable and almost all things around us are likely to be foreign bodies by momentary mistake. According to kinds and locations, clinicians often make mistakes in diagnosis with other physical examination except endoscopy, so foreign body was impacted for a long time and sometimes it may bring a grave outcome to the patient. The authors have recently experienced, in a 13-year-old boy, a pink-colored plastic pencil cap in the right main bronchus which was very similar to the color and shape of the bronchial lumen, and so we had some difficulty in removal of it. This is the report of clinical findings on this case along with literature review.

Oral and Human Microbiome Research

  • In the past gut microbiome has been the main focus of microbiome research. Studies about the microbiome inside oral cavities and other organs are underway. Studies about the relationship between noninfectious diseases and periodontal diseases, and the negative effects of harmful oral microbes on systemic health have been published in the recent past. A lot of attention is being paid towards fostering a healthy oral microbial ecosystem. This study aimed to understand the roles and effects of the microbiome inside the human body can potentially help cure various diseases including inflammatory bowel diseases with no known cure such as Crohn's disease, atopic dermatitis, obesity, cancer, diabetes, brain diseases and oral diseases. The present study examined technological trends in the correlation between the human microbiome and diseases in the human body, interactions between the human body's immunity, the metabolic system, and the microbiome, and research trends in other countries. While it has been proven that human microbiome is closely correlated with human diseases, most studies are still in the early stage of trying to compare the composition of microbiomes between health and patient groups. Since the oral environment is a dynamic environment that changes due to not only food intake but also other external factors such as lifestyle, hygiene, and drug intake, it is necessary to continue in-depth research on the microbiome composition characteristics to understand the complex functions of oral microorganisms. Analyzing the oral microbiome using computational technology may aid in disease diagnosis and prevention.

Analysis of the Prescription Patterns of Medications that List Suicide in Use Cautions using the HIRA Claims Data (건강보험청구데이터를 이용하여 사용상의 주의사항에 자살이 기재된 약물의 처방 양상 분석)

  • Objective: Suicide has recently become an important social problem. Thus, we analyzed prescription drugs that cause suicidal ideation. Methods: Of 156 drugs on the the Minister of Food and Drug Safty (MFDS) EZ-Drug site that had "suicide" listed as a side effect, 78 had "suicide" listed as a warning or contraindication; those 78 drugs were analyzed using data from the 2016 Health Insurance and Review and Assessment Services National Patient Sample (HIRA-NPS). Results: 51 "suicide risk" drugs was identified. Of all patients, 5.2% had received such drugs. The prescription rate was 0.8% of all prescriptions, accounting for 1.6% of all prescription days. From logistic regression analysis, the prescription rate for the drugs was approximately 1.1 times higher for women than for men. With regard to age, the prescription rate for patients 66 years and older was 15.5 times higher than those for patients 25-years and lower. With regard to medical departments, the prescription rates in psychiatry and dermatology departments were 8.1 times higher and 0.6 times lower than those in internal medicine departments, respectively. With regard to region, the prescription rates in Daegu and Jeju were 1.3 times higher and 0.79 times lower than those in Seoul, respectively. Conclusion: Drug-induced suicidal behavior is possible, and therefore efforts are needed to prevent it.

Breast implant-associated anaplastic large-cell lymphoma (BIA-ALCL)

  • Breast implant-associated anaplastic large-cell lymphoma (BIA-ALCL) is a rare T-cell non-Hodgkin lymphoma characterized as CD30 positive and anaplastic lymphoma kinase (ALK) negative. In 2016, the World Health Organization declared BIA-ALCL as a new disease entity. The first case of BIA-ALCL was reported in 1997, and as of July 2019, the United States Food and Drug Administration had cited a total of 573 United States and global medical device reports of BIA-ALCL, including 33 deaths. In all clinical case reports, except for those with unknown clinical history, the patient had received at least one textured surface breast implant. Although the etiology is not yet clear, chronic inflammation has been proposed as a potential precursor to tumorigenesis. The most common presentation of BIA-ALCL is peri-implant fluid collection following aesthetic or reconstructive implantation with textured surface breast implants. It can be accompanied by breast swelling, asymmetry, pain, skin lesions, lymphadenopathy, and B-type symptoms. Most cases are detected on average 7 to 10 years after implantation. Diagnostic specimens can be obtained with fine-needle aspiration or biopsy. BIA-ALCL is CD30 positive, epithelial membrane antigen positive, and ALK negative. It can be cured with complete surgical excision at the T1-T3 stage.

  • According to the COVID-19, development of various medical software based on IoT(Internet of Things) was accelerated. Especially, interest in a central software system that can remotely monitor and control ventilators is increasing to solve problems related to the continuous increase in severe COVID-19 patients. Since medical device software is closely related to human life, this study aims to develop central monitoring system that can remotely monitor and control multiple ventilators in compliance with medical device software development standards and to verify performance of system. In addition, to ensure the safety and reliability of this central monitoring system, this study also specifies risk management requirements that can identify hazardous situations and evaluate potential hazards and confirms the implementation of cybersecurity to protect against potential cyber threats, which can have serious consequences for patient safety. As a result, we obtained medical device software manufacturing certificates from MFDS(Ministry of Food and Drug Safety) through technical documents about performance verification, risk management and cybersecurity application.

  • Technological advancement in human genome analysis and ICT (information & communication technologies) brought 'precision medicine' into our clinical practice. Precision medicine is a novel medical approach that provides personalized treatments tailored to each individual by precisely segmenting patient populations, based on robust data including a person's genetic information, disease information, lifestyle information, etc. Precision medicine has a potential to be applied to treating a range of tumors, in addition to non-small cell lung cancer, in which precision oncology has been actively practiced. In this article, we are reviewing precision medicine in head and neck cancer (HNC) with focus on tumor agnostic biomarkers and treatments such as NTRK, MSI-H/dMMR, TMB-H and BRAF V600E, all of which were recently approved by U.S. Food and Drug Administration (FDA).

  • In case with severely atrophied mandibular ridges, it may be difficult to achieve adequate retention and stability of dentures through the normal functional impression process of complete dentures. In these patients, an approach using the neutral zone raising technique may be an effective alternative. The neutral zone is defined as the area where the pressure of the tongue towards the outside of the mouth and the pressure of the cheek and lips towards the inside are balanced during the normal oral function of the muscular-nervous system. Complete dentures made through neutral zone impression not only improve retention and stability, but also provide adequate tongue space in the posterior teeth area. Additionally, food residues are reduced in the area around artificial teeth, and aesthetics are improved through appropriate facial support.

A Survey on Radiation Exposure of Patient in Mammography (유방 X선촬영 시 피폭선량에 대한 조사 연구)

  • The purpose of this study was to examine both patient exposure dose during mammography and the utility status of mammograpy equipments. The data of this study were collected through questionnaire survey for 278 medical facilities registered at Korean Hospital Association and finally 161 medical facilities's data were analyzed. According to data analysis, medical facilities of 14.9% used the average glandular dose of less than 0.5 mGy, $0.51{\sim}1.0\;mGy$ 8.6%, $1.01{\sim}1.5\;mGy$ 14.9%, $1.51{\sim}2.0\;mGy$ 11.1%, $2.01{\sim}2.5\;mGy$ 9.8%, $2.51{\sim}3.0\;mGy$ 33.3%, and 7.4% more than 3.01 mGy. It was found that medical facilities of 92.6% used less than 3 mGy, showing that this figure is similar to the limit value of 3 mGy recommended by Korea Food & Drug Administration(KFDA). Recently, international organizations such as ICRP associated with radiation protection suggests that less than 3 mGy of average mammary gland dose be used during mammography in case of using Mo target+Mo filter, film/screen system and craniocaudal projection with the breast pressed to 4.2 cm. The standard dose is being strictly observed and that of the limits is going down to 2 mGy or 1.5 mGy. The major results of this study indicate that interests and a counterplan to reduce patient dose during mammography should be considered. Based on this study, the authors of this study will continue to measure exposure dose to set a new standard for patient exposure dose during mammography.

Study on the Effect of Chicken Egg Containing IgY against Helicobacter pylori (항헬리코박터 IgY 항체의 위염(위궤양) 치료효과에 관한 연구)

  • This study was conducted to investigate the effect of chicken egg containing IgY against H. pylori in patients with gastritis. Sixty three H. pylori-infected volunteers (20∼43 year, Male Female=49 : 14) were randomized into four groups which were treated with one chicken egg containing IgY b.i.d. (IgY group; n=17) or omeprazole 20 mg b.i.d., amoxicillin 1.0 g b.i.d. and clarithromycin 500 mg b.i.d. (OAC group; n=17) or omeprazole 20 mg b.i.d., amoxicillin 1.0 g b.i.d., clarithromycin 500 mg b.i.d. and one chicken egg containing IgY b.i.d. (OAC with IgY group; n=16) for 2 weeks or lyophilized IgY 1 g b.i.d (lyophilized IgY group) for 1 month. $\Delta$$^{13}$ $CO_2$ before and after treatment, the eradication rate of H. pylori and histologic change including H. pylori density, acute and chronic inflammation activity, intestinal metaplasia and glandular atrophy by updated sydney system were evaluated. Eradication rate of OAC with IgY group (94%) was higher than IgY group (0%), lyophilized IgY group (0%) and OAC group (88%). $\Delta$$^{13}$ $CO_2$at 2 weeks after treatment in one patient of IgY group was decreased. But that was not changed in the other patients. $\Delta$$^{13}$ $CO_2$ at 1 week after treatment in 15 patients of OAC with IgY group was significantly lower than pretreatment level (p<0.05), and $\Delta$$^{13}$ $CO_2$ at 1 week and 2 week after treatment was decreased in the other patient. Acute inflammation activity at antrum was significantly decreased after treatment in IgY and lyophilized IgY group (p<0.01), H. pylori density at antrum was significantly decreased after treatment in IgY and lyophilized IgY group (p<0.05). Chronic inflammation activity at body was decreased after treatment in lyophilized IgY group. Intestinal metaplasia and glandular atrophy at antrum and body were not changed after treatment in IgY group. Mild intestinal metaplasia in one patient of lyophilized IgY group changed to normal after 1 month treatment. Gandular atrophy at antrum and body were not changed after treatment in lyophilized IgY group.
