  • 비만 환자를 대상으로 절식요법을 시행한 결과 체성분의 변화에서 체지방량의 감소에 비해 근육량의 감소가 적었으며, 혈액학적인 변화에서도 호전을 나타내었기에 보고하는 바이다.

  • Food waste left by patients in hospitals is an important indicator of the nutritional adequacy of the patients' diet and of their satisfaction with food. Food wasted by patients or staff in hospitals is one of the most serious problem in hospital foodservice systems. The purpose of this study was to evaluate the disposal practices of food waste in hospital foodservices for providing basic information for an efficient solid waste management. This approach was achieved using a variety of qualitative and quantitative information including general food waste practices and measurement of food waste left by patients and staff in 20 hospital foodservices. The average food wasted by patients and staff per day was 402.20kg and 206.98kg respectively, being total food waste of 578.08kg per day. The mean plate waste of a staff was 115.95g, which was much lower than that of a patient (221.03g). As means of food waste treatment, most hospitals(60%) are using animal feed, followed by means of collection by contracters(15%) and disposal of waste collection after condensing. An average monthly cost for disposing food waste was 915,000 won and average 138.58 minutes were spent to dispose food waste in hospital foodservices.

한국의 건강기능식품 시장의 성장과 한의계의 대응전략 (Progress of Functional Food Market in Korea and Strategy of Korean Medicine)

  • Objectives: To analyze the growth pattern of the functional food market in Korea, and thus produce essential information in mapping out a strategy for Korean medicine. Methods: A survey of literature or analysis reports about market changes related to functional foods in Korea was undertaken. Results: The Korean functional food market has been growing rapidly, to 1.4 trillion won in 2012 compared to 250 billion won in 2004, which is an annual growth rate of about 14%. In particular, individually-approved functional food is worthy of notice regarding its growth rate and functional position, which is similar to herbal drugs. The expanding growth of functional food affects negatively the decrease of herbal drug industry in Korea to date. Conclusions: To compete with functional foods, the Korean medicine community needs to develop patient-friendly and cost-effective herbal drugs, and strategies to adopt them as therapeutic modalities.

  • 고기를 먹고 체한 이후 재발한 소양인 건선 환자가 하지의 건선만 호소하고, 야간 소양감,오후의 재발하는 편두통, 어두운 설질과 어반 등의 증상을 호소하여, 장누수증후군과 소양인 식체비만과의 연관성을 고려하여 독활지황탕을 투여하여 증상이 호전된 치험례로, 향후 건선환자의 사상의학적 치료에 보탬이 되고자 보고하는 바이다.

  • Anorexia is a common symptom in the elderly patients. Causes of anorexia in the elderly are very diverse and multifactorial. Causes include physiological changes associated with aging, mental disorders such as depression, anorexia tardive, dementia, medical diseases such as cancer(lung and gastrointestinal cancer), benign gastrointestinal disorders, cardiac disorders, pulmonary disease, thyroid disorders, infection. Medications such as digoxin, theophylline have also been implicated in the problem. No cause is found in about one quarter of patients. Management is directing at treating causes and providing nutritional support. In Oriental medicine, the appetite has close relation to Biwi. The main cause of anorexia is the insufficiency of Biwi. The physiology of Biwi is that Bi sends clarity(food essence) upward and Wi sends digested food downward. Specially if the physiologic function of Wi is disordered by various factors, Wi cannot send digested food downward. As a result, the anorexia can present by the disorder of Wi function. We experienced a case of an 74 years old female patient with ill-defined severe anorexia differentiated as Wijoongheohan. The patient was managed with fluid therapy and Jeonghyangsiche-tang. The anorexia and other symptoms improved continuously during hospitalization.

대전.금산 지역 당뇨.고혈압 질환자의 인삼에 대한 인식도 조사 (Recognition of Ginseng by Diebetic and Hypertensive Patients in Daejeon and Geumsan Areas)

  • Research on recognition, preference, and consumption of ginseng was conducted with the goal of enhancing the consumption of ginseng, and commercializing local ginseng dishes in Geumsan. With help from the Eulji University hospitals in Daejeon and Geumsan, a survey was completed by a group of 200 patients, including 50 diabetic and 50 hypertensive patients from each center. Results based on a patient's residence (p<0.001), and illness (p<0.001), showed that the percentage of outpatient treatments was very high at the center in Geumsan. Based on a patient's residence (p<0.001) and illness, a preference for ginseng was very high in Geumsan. Based on a patient's residence (p<0.01) and illness (p<0.05), the willingness to try new ginseng dishes was stronger at the center in Geumsan compared to the center at Daejeon. Based on residence and illness (p<0.001), hypertensive patients were more enthusiastic towards taking ginseng herbs. Based on residence (p<0.001) and illness (p<0.001), percipience concerning the effects of ginseng was more positive in Geumsan. Based on residence (p<0.001) and illness, the frequency of ginseng herb intake was higher in Geumsan. The research provides the basis for establishing changes in the perception of ginseng, providing accurate information, and promoting ginseng in local economies.

우리나라의 두부 엑스선검사에서의 환자선량 권고량 (A Study for Establishment of Diagnostic Reference Level of Patient Dose in Skull Radiography)

  • 진단을 위한 엑스선검사는 의료분야에서 전리방사선을 가장 많이 사용하고 있으며 인공 방사선피폭중 가장 높은 비율을 차지하고 있다. 우리나라에서도 진단 엑스선검사에 의한 의료피폭은 전체 방사선 피폭 중 17.4%를 차지하고 있으며 인공방사선피폭 중에서는 92%를 차지하고 있다. 두부진단을 위한 엑스선 촬영 횟수도 2007년의 경우에는 111,567건으로서 2004년 이후 매년 3% 정도의 증가 추세를 나타내고 있다. 따라서 환자선량 권고량을 국내의료기관 실정에 맞도록 설정하여 두부촬영시 환자가 받는 방사선량을 줄이고 환자의 방사선 방어를 최적화 하는 것이 필요하다. 본 논문에서는 전국 114개 의료기관에서의 두부 촬영시 피폭되는 방사선량을 인체팬텀과 유리선량계를 사용하여 측정하고 환자선량권고량(DRL, Diagnostic Reference Level)을 확립하였다. 이 결과에 따라 두부 후전면 촬영에서의 환자선량 권고량은 2.23 mGy이며, 이는 세계보건기구, 국제원자력기구 등 국제기구가 권고하는 선량 5 mGy 보다 낮았으며, 두부 측면찰영에서의 환자선량 권고량인 1.87 mGy는 국제기구가 권고하는 선량 3mG 보다 낮았다.

식편압입의 발현에 관한 임상적 연구 (A CLINICAL STUDY ON THE OCCURRENCE OF FOOD IMPACTION)

  • The purpose of this study was to investigate the causes of food impaction and to explore solutions as well. For this study, 39 patients with food impaction were selected. 77 contact areas in these patients were investigated mobility, tightness of contact area, gingival index, plaque index, attachment loss, alveolar bone loss, proximal caries, marginal ridge distance and occlusal relationships. The results were as follows ; 1. Teeth without distal support were found to be the most frequent site of food impaction (41.6%). Food impaction was found to be more frequent in the upper teeth (65.2%) than the lower teeth (33.8%). 2. Food impaction was found in tight contact cases (71.4%). Alveolar bone loss was not found in the early stage of food impaction (83.1%) 3. The distance between the marginal ridges of food impaction sites (mean=0.48mm) was short-er than that of the control group. (mean=0.77mm) (p<0.001) 4. In 18.2% of the cases, proximal carries were found at the food impaction site. 5. Food impaction affected patient's occlusion with the following frequencies, cusp to marginal ridge relationship (72.7%), cusp to fossa relationship (3.9%) and stepped relationship (23.4%).

영양 상담서비스 품질에 대한 환자의 만족도와 인식조사 (Patient Satisfaction and Perception on Nutritional Counseling Services Quality)

  • 전국의 43개 상급종합병원과 20개 상급병원의 환자 1,095명을 대상으로 환자의 영양 상담서비스에 대한 인식을 조사하고, 만족도는 서비스 품질 개념을 적용하여 분석하였다. 환자의 영양 상담서비스 인식 평균점수는 4.54/5.00점이었다. 환자의 영양 상담서비스 만족도를 요인 분석한 결과 서비스 품질 5개 영역으로 추출되었고, 각각 공감성, 반응성, 유형성, 신뢰성, 전문성으로 명명, 분류하였다. 5개 영역의 전체 평균 점수는 4.56점/5.00이었고 공감성 4.62점, 반응성 4.52점, 유형성 4.53점, 신뢰성 4.65점, 전문성 4.61점이었다. 학력에 따른 차이에서 영양 상담서비스 품질 만족도 총 평균은 대졸 이상이 중졸 이하보다 유의적(P<0.001)으로 높았다. 수도권 병원의 만족도 점수는 비수도권 병원보다 전체 평균 점수와 반응성, 공감성, 전문성, 신뢰성(P<0.001), 유형성(P<0.05)에서 유의적으로 높았다. 상담횟수에서는 반응성 영역에서 상담을 1회 받은 군이 2회보다 유의적(P<0.01)으로 높은 만족도를 보였다. 환자의 영양 상담서비스 인식과 만족도 간에 신뢰성(r=0.721), 반응성(r=0.697), 공감성(r=0.690), 전문성(r=0.678), 유형성(r=0.622)에서 양(+)의 상관관계를 보였다. 이상을 살펴볼 때 비수도권 병원에서는 임상영양사 수의 확보를 통해 환자의 영양 상담서비스 품질을 향상시키는 것이 필요하겠다. 또한, 영양 상담이 반복될수록 환자의 기대치가 높아질 수 있으므로 임상영양사는 영양 상담 내용 및 기법에 변화를 주고 다양한 자료를 활용하여 영양 상담서비스 품질을 향상시킬 수 있을 것이다. 영양 상담서비스 품질 만족도와 영양 상담 인식 간에는 높은 상관관계가 있는데 서비스 품질 만족도에서 유형성과 반응성은 다른 영역보다 상대적으로 낮은 점수를 보였으므로, 상담실 분위기를 쾌적하게 하고 다양한 교육매체를 활용하며 환자와의 여유 있는 상담시간 및 선택적 상담시간 확보 등 해당 영역에 대한 관리가 특히 필요할 것으로 여겨진다.
