티벳의학에 대한 연구 - "사부의전(四部醫典).근본의전(根本醫典)"을 중심으로 (Study on "Four Tantras", the Prime Textbook of Tibetan Medicine)

  • The following conclusions are obtained from the studies on the chapters concerning phisiology, pathology, daily conduct, materia medica, pulse, and urinalysis from , of the "Four Tantras". 1. The theoretical basis forming the Tibetan Medicine is the substance of phlegm, bile, and wind each of which is divided into five kinds. These phisiological substances can be transformed into pathological factors when certain environment is formed. 2. In embryology, the semen of the father and menstral blood of mother is considered the most important condition in conception, and the Five factors are regarded as important. There is a detailed explanation of development of the fetus while it stays in the womb of mother during 38weeks, such as the formation of the viscera, channels, sense organs, etc. 3. There is metaphor which compares the human body with the king's palace. With the development of human anatomy, there is the detailed anatomical picture of anterior and posterior aspects of human body. And also there is the measurement of physiological constitution and the three fluids. 4. In division of the channels, they concerned the now of the blood and distribution of the nerve fibers, and each channel is connected with one another. The division of the cannel is namely embryonic channel, channel of existence, channel of connection, and the course of life principle. 5. The seven bodily constituents and three factors of phlegm, bile, and wind are important in sustaining the life of human body as well as growth and maturization, and when their equilibrium is broken, the human body is degnerated, and finally death comes. 6. The signs of death is divided into distant sign, remote sign, certain sign. and uncertain sign, and is used as a clue in diagnosis of the disease. Especially there is a mention about the mechanism of the dreams, and different dreams according to the condition of the patient. 7. In pathology, there is the cause of the disease, the environmental factors which can induce disease, the path by which disease come into the human body, the characteristics of the disease, and the kinds of disease. 8. There is a mention about the conduct, and it is divided into the daily conduct, the mental attitude, and the side-effects which can occur when one puts up with the physiological actions of the body. 9. The daily diet is divided into food and beverage. The food is divided again into grain, meat, fat, boiled food, and spiced food, and the beverage into milk, water, and wine. 10. The pulsation should be taken in lift hand to diagnose heart, small intestine, stomach, spleen, kidney, reproductive organ, and in right hand jungs, large intestine, liver, gall bladder, kidney, and bladder. In the healthy person, the pulse moves 5times in one perspiration, and the type of pulse is constant while the pulse moves 100times. But unhealthy person's pulse is different from this. The urinalysis is the unique part of Tibetan Medicine, and is important in examination of the disease. One should decide which of the three factors are dominant by the obervation of the color, amount of the steam and how long it lasts, odour, foam, and the shape of the swirl in three different states when the urine is hot, warm, and cold. One can determine the life of the patient and which viscera is the cause of the disease by the pulse, and whether the nature of the disease is hot or cold by the urinalysis. 11. The materia medica contains gemstones, minerals, plateau medication, and meat products.

입원환자의 초기영양평가를 위한 단순영양검색도구 개발 및 도구 간 신뢰도 검증 (Development of a simplified malnutrition screening tool for hospitalized patients and evaluation of its inter-methods reliability)

  • 본 연구는 입원환자의 전자의무기록에서 용이하게 얻을 수 있는 지표들을 중심으로 초기영양평가를 신속하게 진행하기 위한 영양검색도구를 개발하고, 새로 개발한 영양검색도구에 대한 신뢰도을 검증하기 위한 목적으로 수행되었다. 이를 위하여 입원환자 903명을 대상으로, 의무기록 및 개별면담을 통해 자료를 수집하여 PGSGA를 기준으로 양호군과 영양불량군을 나누었다. 1) 로지스틱 회귀분석을 통해 양호군과 영양불량군으로 범주화하기 위한 최종 영양평가지표로 연령, BMI, 혈중 알부민, 혈중 콜레스테롤, 총단백질, 헤마토크리트, 체중변화, 섭취변화여부가 선정하였다. 특히 체중변화, 섭취변화여부의 경우는 예/아니오의 주관적 단답식 조사로 간단히 이루어졌다. 2) SMST의 영양평가결과 총 903명 중에서 양호군은 620명 (68.7%), 영양불량군은 283명 (31.3%)으로 나타났으며, 양호군과 영양불량군사이에 연령, 혈중 알부민, 총임파구수, BMI, 헤모글로빈, 헤마토크리트, 총단백질, 혈중 콜레스테롤, 재원기간 모두 유의적인 차이를 보였다. 3) SMST의 신뢰도를 검증하기 위해서 PGSGA와 NRS2002와의 민감도, 특이도, 카파비를 통한 일치도를 분석하였다. SM-ST는 PGSGA와 민감도 94.4%, 특이도 88.4%, 카파비 0.747로 'Substantial agreement'하며 (p < 0.001), NRS2002와 민감도 96.1%, 특이도 79.5%, 카파비 0.505로 'Moderate agreement' (p < 0.001)로 나타났다. 본 연구에서 개발한 새로운 초기영양검색 도구인 SMST의 영양평가지표는 의무기록 및 단답형의 질문으로 이루어져, 신속하고 간단한 초기영양평가가 가능하다. 특히 기존에 개인면담을 통해 시간적, 인력적 요구가 높은 영양도구들에 비해, 입원당시 기본적 신체계측과 함께 단답형으로 이뤄진 체중변화 및 식이섭취변화 여부가 기록될 경우 추가될 경우 추가조사 없는 의무기록으로 평가가 가능하다. SMST는 PGSGA, NRS2002와 높은 일치도를 보여 기존에 타당도가 높다고 알려진 두 영양검색 도구와 도구간 신뢰도가 검증되었다. 하지만 SMST는 합병증의 유무, 사망률, 재입원률, 병원비용 등 환자의 예후와 관련한 연관성 및 보편적 대상자에서의 유용성은 검증되지 않았으므로, 이에 관한 추가적인 연구가 필요하다고 사료된다.

제2형 당뇨병 환자의 평소 식사 중 당질섭취량과 glycemic index 관련 연구 (Carbohydrate Consumption and Glycemic Index of the Usual Diet in Type 2 Diabetes Mellitus Patients)

  • The possibility that high, long-term intake of carbohydrates that are rapidly absorbed as glucose may increase the risk of type 2 diabetes has been long-standing controversy. A high consumption of carbohydrates with a high glycemic index produces greater insulin resistance than did the intake of low glycemic index carbohydrates. This study was designed to evaluate the cabohydrate intake status include glycemic index and correlation carbohydrtae intake status with anthropometry factors & other nutrients in usual diet of the Korean type 2 diabetes mellitus. In 104 tpye 2 diabetes mellitus patients(mean age : 51.8yr, male=44.femal=60), we determined carbohydrte intake status include glycemic index with 24hr recall method and measured anthropometry. Mean daily carbohydrtae intakes and glycemic index were 307.3g(male 323.1g, female 295.5g) and 90.7(male 93.4, female 88.8), respectively. We found a strong and statistically significant association between carbohydrate ratio and glycemic index in obese factors, other nutrient. But carbohydrate intake/kg of body weight was low a significant differences in obese factors, other nutrient. Also glycemic index was effected by total energy intake and carbohydrate ratio than carbohydrate intake/kg of body weight. In conclusion, emphasis for dietary modification should be total energy intake and carbohydrate ratio in diabetes mellitus patient.

소아 환자의 위 주름술에 있어서 복강경과 개복술의 비교 (Comparison between Laparoscopic and Open Nissen Fundoplication in Pediatric Patients)

  • Fundoplication is a common surgical procedure for gastroesophageal reflux Disease (GERD). Recently the procedure has been performed with increased frequency laparoscopically. The aim of this study is to compare laparoscopic Nissen fundoplication (LNF) and open Nissen fundoplication (ONF) for GERD in children. We studied retrospectively the 88 pediatric patients who underwent the Nissen fundoplication for GERD as primary antireflux surgery from 1994 and 2009. ONF was performed in 34 cases and LNF was in 54 cases. 58 patients have neurologic impairment. Time to initial food intake after the surgery were reduced in the LNF group (p= 0.032). Recurrent GERD symptom occurred in one patient in LNF group and four patients in ONF group within 1 year after the surgery (p= 0.012). There were no statistically significant differences in post operative morbidity and mortality between both groups. In conclusion, our practice of Nissen fundoplication indicates that LNF takes priority in most pediatric patients.

한국 성인당뇨병 환자의 식생활에 관한 문화기술적 연구 (An Ethnographic Study on Eating Styles of Adult Diabetics in Korea)

  • Purpose: The purpose of this study was to explore adult diabetics' eating styles and factors which influence them. Method: The study adopted an ethnographic method based on a perspective which views the eating style as a cultural phenomenon. Data was collected through a personal interview, participant observation, and documented materials from Oct.2001 to Sept. 2002. In this study, fifteen adult diabetics, with an average age of 57, participated. Data analysis was done by the Spradley's taxonomic analysis technique. Result: The patients' eating styles were rooted in their viewpoint on illness as well as the meaning of food. Eating styles were classified into 4 types: Pathology-centered, symptom-centered, need-centered, and role-centered. Conclusion: A conventional approach to the treatment and management of diabetes did not consider the patient's inner world which may play an important role in the successful management of the disease. We found that it was critical for health care personnel to understand patients' values, beliefs and their way of life in order to facilitate the most successful self-care diet.

심혈관질환자의 영양교육이 자기효능감, 식행동양상 및 심혈관 위험요인에 미치는 효과 (Effects of the Nutrition Education Program on Self-efficacy, Diet Behavior Pattern and Cardiovascular Risk Factors for the Patients with Cardiovascular Disease)

  • Purpose: This study examined the effects of a nutrition education program on self-efficacy, diet behavior pattern and cardiovascular risk factors for patients with cardiovascular disease (CVD). Method: Sixty-four CVD subjects (37 experimental, 27 control) were recruited from a cardiac center, at a university hospital located in D city, Korea. All subjects attended a first heart camp where pretest measures were performed, and a second heart camp at 6 months for the posttest measures. During the 6 month study period, the experimental group was required to attend five monthly nutrition education sessions, while the control group received only routine outpatient follow-ups. Data were analyzed by $x^2$-test and independent t-test using the SPSSWIN 11.5 program. Result: Group comparisons revealed that the experimental group had significantly more improved self-efficacy, frequency of food selection, gustation of salt, systolic blood pressure, and serum total-cholesterol compared to the control group. Conclusion: A nutrition education program may be effective in improving self-efficacy, diet behavior pattern and cardiovascular risk factors for patients with cardiovascular disease.

양성형 골화석증 (Benign Osteopetrosis : Report of a Case)

  • A 37-year-old male with the complaint of intermittent gingival swelling and dull pain associated with the food impaction of the mandibular third molar area was referred to our department of Chonbuk National University Hospital. The dental history of the patient showed that he had extracted the maxillary left third molar without complications two years ago. Intraoral and panoramic radiographs showed diffuse increased radiopacity of the maxilla and mandible. The trabecular pattern was obliterated. Postero-anterior skull and lateral skull radiographs showed thickening of cortical bone and diffuse increased radiopacity of the skull. Additional radiographs showed similar changes in the lumbar spine, clavicles, iliac bone and femur. However, no evidence of osteomyelitis was observed clinically and radiographically. Laboratory findings showed normal values of serum calcium, phosphorus, and alkaline phosphatase. Based on the radiographic examinations and the laboratory findings, final diagnosis was made as a benign osteopetrosis.

임플란트 선택동기 및 인지도 조사연구 -울산지역 성인 남, 여를 대상으로- (An Investigation of a Motive for Dental Implant Treatment in Clinical Cases)

  • This study was to find a patient's motive for receiving implant treatment by age and gender using a self-reporting questionnaire survey with adults(men & women) in Woolsan. The result was as follows: 1. As a result of questionnaire survey with 155 patients in total(86 men, 69 women), 24 of 86 men(27.9%) valued the natural mastication feature of dental implant most, that is as similar as natural teeth, while 18 of 69 women(26.1%) had the most priority over the longer life of implant than common dental prostheses. By age, those in their 20s and 30s had a preference to implant treatment because it does not need to pulling out teeth for prostheses, while those in their 40s and over had a priority to the natural mastication feature of implant that is similar as much as natural teeth. By dental treatment, all of respondents said that the natural mastication feature is the most important in getting dental implant. It was founded that whether one can taste food as it is or not is less important, regardless of age or gender. 2. The greatest obstacle to implant surgery was expensive medical fees, 88 of 155 respondents(56.8%), with a fear of surgery itself being the least obstacle. 3. 90.2% of the total respondents said that they will want to receive dental implant treatment if their economy allow.

부식성 식도협착부위에 발생한 식도암 - 14례 보고 - (Esophageal Corrosion Carcinoma at the Site of Caustic Stricture - A Report of 14 Cases -)

  • Between Dec. 1958 and Oct. 1989, we had been experienced 14 cases of the esophageal corrosion carcinoma among 562 cases of corrosive esophageal stricture, which incidence was revealed 5.6 % of total esophageal carcinoma cases and 2.5 9o of total benign esophageal stricture cases. The age distribution was from 15 to 64 years, and the mean age was 45.6 years. Ten of the patients were women. The caustic agents were lye in 12 patients and hydrochloric acid in 2 patients. The latent time from injury to the development of carcinoma was from 12 years to 45 years, and the mean latent time was 29 years. In majority of cases, the developing site of carcinoma was just or just above the benign stricture portion of the esophagus. Corrective surgery were made in seven cases; three cases were made esophagectomy and esophagogastrostomy, and the other four cases were made esophagectomy and colon interposition. Palliative surgery including feeding gastrostomy were made in seven cases. All cases had a longstanding history of difficult oral swallowing especially at the site of stricture. We think chronic food irritations of stricture portion of the esophagus induced the development of corrosion carcinoma. In conclusion, we had to suspect possibility of carcinoma in patient with long standing history of benign esophageal stricture. So we recommended preoperative esophagoscopic biopsy in such cases.

영양상담이 유방암 수술환자의 영양상태, 식습관 및 영양지식에 미치는 영향 (The Effect of Individual Nutrition Counseling on Diet and Nutrition Status of Postoperative Breast Cancer Patients)

  • This study was conducted to assess the effect of nutrition counseling for postoperative female breast cancer patients (N = 38). In baseline survey, we investigated the nutrition parameters by measuring anthropometric and biochemical blood index and assessing nutrient intake using recall method. Individual nutrition counseling was performed 3 times with 2 - 3 weeks intervals. Patients were offered nutrition information related to breast cancer and desirable life style after cancer operation. The effects of counseling program were assessed 2 month later. Of biochemical parameters, serum total cholesterol level was significantly reduced after nutrition counseling program. Nutrient intakes of postoperative breast cancer patients were generally below the RDA level at the baseline survey. After nutrition counseling, mean daily intake of total energy, protein, calcium were significantly elevated. Of dietary habit, meal regularity and control of portion size were improved and fruits and vegetables intake were significantly increased after nutrition counseling. Control of fatty food intake and alcohol drinking were significantly improved after nutrition counseling program. Score of general nutrition knowledge and knowledge about breast cancer, were significantly increased after nutrition counseling from 2.1 to 3.2 and from 2.3 to 3.8, respectively. It can be postulated that the dietary habit and nutrients intake of postoperative breast cancer patients can be improved by individual nutrition counseling program. In further study, the systematic group nutrition counseling program is needed.
