항암 화학요법을 받는 환자의 식욕부진 정도와 식이양상에 관한 연구 (A Study on the Anorexia and Diet Patterns in Cancer Chemotherapy Patients)

  • This study was designed to identify the severity of anorexia and the diet patterns in receiving chemotherapy. The identification of anorexia would provide useful and basic information to oncology nursing care. The subjects of this study were 102 hospitalized chemotherapy patients in a September 10, 1994. The subjects were 20 years old or above and who agreed to participate in this study and could understand the questionnaire. Three-days diet history were collected and analysed. The study subjects change, food aversions, the severity of anorexia. Data related to demographic and other mecdical characteristics such as age, diagnosis, and medication were collected by review of patient medical record. The results of this study were summarized as follows : 1) The score of anorexia was 73.7 in Anorexia Visual Analogue Scales and mean amount of fool intake per a day was 823cc. The larger the anorexia VAS score, the more severe of anorexia. 2) The food preferences of subjects were identified. Those were aversions, or dislikes the meat such as beef, pork, and chicken, and greesy or fried foods. The Caffeinated drinks such as coffee and tea were disgusted, also. The patients preferred vegetables and Korean traditional food especially kimchee and soy bean soap. 3) Those who were administered analgesics and cisplatin suffered more severe in anorexia than those who analgesics and cisplatin was not administarted. The patients with gastrointestinal cancer has more severe anorexia than those who have the other site cancer ; head and neck, genirourinary etc. The result of this study in turn provide valuable nursing practice guidelines for nutritional counseling in cancer chemotherapy patient. Nurses working with chemotherapy ward should identify the severity of anorexia and diet patterns. In conclusion, the severity of anorexia in cancer chemotherapy patients is very important problems. Health care personnels recognize the potential problems of anorexia and encourage the nutritional counseling in cancer chemotherapy patients.

Food-borne outbreaks, distributions, virulence, and antibiotic resistance profiles of Vibrio parahaemolyticus in Korea from 2003 to 2016: a review

  • Background: Vibrio parahaemolyticus is one of the most common causes of seafood-borne illnesses in Korea, either directly or indirectly, by consuming infected seafood. Many studies have demonstrated the antibiotic susceptibility profile of V. parahaemolyticus. This strain has developed multiple antibiotic resistance, which has raised serious public health and economic concerns. This article reviews the food-borne outbreaks, distributions, virulence, and antibiotic resistance profiles of V. parahaemolyticus in Korea during 2003-2016. Main body: V. parahaemolyticus infections appeared to be seasonally dependent, because 69.7% of patient infections occurred in both August and September during 2003-2016. In addition, the occurrence of V. parahaemolyticus in marine environments varies seasonally but is particularly high in July, August, and September. V. parahaemolyticus isolated from aquaculture sources on the Korean coast varied in association with virulence genes, some did not possess either the tdh (thermostable direct hemolysin) or trh (tdh-related hemolysin) genes, and a few were positive for only the trh gene or both genes. The high percentage of ampicillin resistance against V. parahaemolyticus in the aquatic environment suggests that ampicillin cannot be used to effectively treat infections caused by this organism. Short conclusion: This study shows that the observed high percentage of multiple antibiotic resistance to V. parahaemolyticus is due to conventionally used antibiotics. Therefore, monitoring the antimicrobial resistance patterns at a national level and other solutions are needed to control aquaculture infections, ensure seafood safety, and avoid threats to public health caused by massive misuse of antibiotics.

천식환자에 대한 표준화된 병원약사대상 교육과 반복적인 환자대상 복약지도를 통한 성과 분석 (The Analysis of the Outcome of Standardized Hospital Pharmacist Training and Repeated Asthma Patient Education)

  • Training for asthma patients by professionals has been highly emphasized for patients' self-management. In the present study, three standardized training sessions targeting hospital pharmacists were performed. On the basis of the number of training sessions taken, a non-educated group (CG), an once-educated group (NG), and a twice more-educated group (IG) were allocated. The most frequent errors of using an inhalant were shaking before the use in MDI while taking breathe out before inhaling in the case of DPI, and the total average number of errors were the biggest for CG and the smallest in IG. On comparison in the number of the four symptoms of asthma according to the level of seriousness, it was revealed that the total average number was the biggest in CG and the smallest in IG. In the level of awareness regarding the contents of training for patients, patients over 50 tended to answer that they were aware of education contents, and particularly in mouth rinsing and the use before/after a meal, significant similarity was examined. Regular asthma patient training performed by hospital pharmacists appeared to reduce errors in the inhaler usage, increase the quality of life of an patient, and help a patient remember the contents of the training. Especially among patients over 50, it was found to be more effective and retraining of more than two sessions was requisite.

Treatment and retention of relapsed anterior open-bite with low tongue posture and tongue-tie: A 10-year follow-up

  • The purpose of the current report is to present 6-year long-term stability and 10-year follow-up data for an adult patient who was treated with a tongue elevator for relapsed anterior open-bite. The 19-year-old male patient presented with the chief complaint of difficulty in chewing his food. Collectively, clinical and radiographic examinations revealed an anterior open-bite, low tongue posture, and tongue-tie. The patient opted for orthodontic treatment alone, without any surgical procedure. A lingual frenectomy was recommended to avoid the risk of relapse, but the patient declined because he was not experiencing tongue discomfort. Initial treatment of the anterior open-bite with molar intrusion and tongue exercises was successful, but relapse occurred during the retention period. A tongue elevator was used for retreatment, because the approach was minimally invasive and suited the patient's requirements regarding discomfort, cost, and time. The appliance changed the tongue posture and generated an altered tongue force, which ultimately resulted in intrusive dentoalveolar effects, and a subsequent counterclockwise rotation of the mandible. The results showed long-term stability and were maintained for six years through continual use of the tongue elevator. The results of this case indicated that a tongue elevator could be used not only as an alternative treatment for open-bite, but also as an active retainer.

  • 중증 질환 환자의 대부분은 치료 적기를 놓쳐 병을 키우게 된 경우가 많다. 높은 자각 증상이 나타나거나 강력한 통증이 없는 경우 병원 방문을 심각하게 고려하지 않는다. 치은염의 경우 잇몸에서 출혈을 일으키며 치아 붕괴를 일으키는 질환이지만 초기의 증상이 경미하다. 이 시점에서의 병원 치료는 매우 높은 효과를 가진다. 그러나 치료시기를 놓칠 경우 잇몸과 구강의 건강에 심각한 문제를 일으켜 음식을 섭취 할 수없는 상황까지 병증이 진행된다. 본 논문에서는 환자의 치주 이미지를 스마트 폰으로 촬영하여 실시간으로 치의료 기관에 구축된 환자 관리 시스템으로 정보를 전송한다. 전송된 정보를 담당 의사가 판단하며 이에 대한 치료 적기 정보를 환자에게 제공한다.

조기 덤핑증후군 환자의 복통에 감초분밀탕을 사용한 1례 (A Case Study of a Dumping Syndrome Patient Treated with Gamchobunmil-tang)

  • The purpose of this study is to explore the effectiveness of Gamchobunmil-tang as a treatment for early dumping syndrome. A patient reported abdominal pain after eating, was not able to eat more than two spoons of thin rice gruel, and had lost 8 kg of his weight. The patient was treated with Gamchobunmil-tang three times a day for 12 days. The effects of the treatment were evaluated using the Visual Analogue Scale and the amount and form of the patient's intake. After treatment, the pain due to the dumping syndrome decreased. The Visual Analogue Scale score dropped significantly from 9 to 2, and increased amounts and novel forms of the food the patient consumed were observed. Despite the lack of a case, the results showed that Gamchobunmil-tang was effective in reducing the pain caused by the early dumping syndrome. Further research might prove that Gamchobunmil-tang could be an alternative treatment method for early dumping syndrome.

성인 남성의 혈압과 식이섭취 실태 및 기타 환경 요인과의 관계에 대한 사례연구 (Correlations among Blood Pressures, Dietary Intakes and Other Environmental Factors in Korean Adult Males)

  • The present survey was designed to investigate correlations among blood pressure, nutritional intakes, and various environmental factors in Korean adult males, and to observe the difference in dietary intakes between hypertensive patients and normotensive subjects. Two hundred forty-three adult men from Seoul and rural areas were interviewed about their food intakes, dietary patterns, food habits, and general environmental factors from December 20, 1982 to February 15, 1983. Their dietary intakes were surveyed by means of 24-hour recall diet method. Dietary score card was used in order to evaluate the quality of routine diet of the subject. Variables of environmental factors and factors related to hypertension were expressed as percentage, and the differences in nutrient intake and food score of each sub-group were examined by means of t-test. Correlation of all the variables were analyzed by Pearsons Correlation Coefficient. The results obtained are summarized as follows: 1. 38% of the subjects were between the ages of 40~49, and about one-half of the subjects were middle or high school graduates. 28% of the subjects earned 210, 000~350, 000 Won per month, and income distribution was almost identical to that of the urban population of Korea. 2. There was more overweight or obese found in the hypertensive patients than in the normotensive subjects. About 40% of the hypertensive patients reported a family history of hypertension, and their knowledge about hypertension was rather poor, but slightly better than the normotensive subjects. The patient group had less adherence to smoking and alcohol compared with the normotensive subjects, but no difference was observed in coffee drinking. The mean value of the blood pressure for the normotensive subjects was 127/86mmHg and 175/109mmHg for the hypertensive group. 3. Nutrient intake for the hypertensive group was lower for most of nutrients than in the healthy group. Hypertensive patients on diet therapy consumed less calories and carbohydrates compared with those who do not follow diet therapy. 4. There was no difference intake of these food groups between patients and the healthy group. Food score was strongly related to consumption of animal protein, saturated fats, Calcium, Iron, Vitamin A, Riboflavin, and Vitamin C, which are prone to be deficient in the typical Korean diet. 5. Blood pressure had a strong positive correlation with age, body weight, and socioeconomic level. In addition, monthly income was positively related with age and education level. 6. Three was a positive relationship between blood pressure and consumption of vegetable protein, and a negative relationship with consumption of Calories, Carbohydrates, Calcium, Iron, Vitamin A, Vitamin B1, Niacin, and Vitamin C. Animal protein intake increased as income and education levels increased, but this tendency was opposite for carbohydrates. In summary, nutritional intakes are strongly correlated with various environmental factors and the level of blood pressure. There was no difference between normotensive and hypertensive subjects with respect to the quality of food consumed except restricting energy and carbohydrate in the patient group.

가압가열 처리에 의한 시판 돈육 햄과 베이컨의 알레르겐성 저감화 효과 (Reduction of Allergenicity of Domestic Pork Ham and Bacon by Autoclave Treatment)

  • 대표적인 돈육 가공품인 햄과 베이컨에 가압가열 처리를 한 후 알레르겐성 변화를 살펴봄으로써 가압가열 처리법을 응용한 hypoallergenic pork의 개발 가능성을 알아보았다. 구입한 돈육 햄과 베이컨 중 ci-ELISA를 실시하여 가장 결합력이 높은 2개의 제품에 대해 가압가열 처리를 하였다. 먼저, 돈육 햄의 알레르겐성에 대한 가압가열 처리 영향을 살펴본 결과, 돈육 햄은 30분 가압가열 처리구의 경우 p-IgG와의 결합력이 조금 감소하였으며 환자혈청을 사용하였을 때는 10분과 30분 처리구 모두 무처리구보다 결합력이 다소 낮게 나타났다. 베이컨은 무처리구에서 p-IgG와의 결합력이 각각 60% 및 91%로 높았지만, 가압가열처리에 의해 약 16% 및 11% 이하로 크게 감소하였다. 환자혈청을 사용한 경우에도 베이컨 sample b와 c의 무처리구는 95% 및 126%의 높은 결합력을 나타내었지만, 가압가열 처리에 의하여 각각 22% 및 34% 이하로 크게 감소하였다. Immunoblotting 결과에서도 베이컨 두 제품 모두가압가열 처리구의 PSA band가 p-IgG와 반응하지 않았다. 특히, sample c는 무처리구의 PSA band가 2명의 환자혈청과 약하게 반응하였지만, 가압가열 5분 처리구에서는 PSA band가 나타나지 않아 베이컨의 알레르겐성이 가압가열 처리에 의해 효과적으로 감소되었음을 확인하였다. 이상의 결과를 종합해 볼 때 돈육 가공품의 종류와 처리조건에 따라 알레르겐성 감소 정도는 다르게 나타났다. 가압가열처리에 의해 돈육 햄의 알레르겐성은 큰 변화를 나타내지 않았지만, 조금 감소하는 경향을 보였으며 베이컨은 알레르겐성이 크게 감소하였음을 알 수 있었다. 따라서 본 연구는 돼지고기 민감성 환자들을 위한 돈육 가공품 개발에 가압가열 처리법이 적용 가능하다는 것을 시사하였다.

인삼 복용 후 발생한 비출혈 및 심계항진을 주소로 한 소양인 환자 1례 (A Case Study of a Soyangin Patient with Palpitation and Epistaxis that Occurred after Taking Panax Ginseng)

  • Objectives The purpose of this case study was to report improvement of palpitation, epistaxis, nausea, and anorexia after treatment with Dojeokgangki-tang gamibang in a middle-aged woman who has been taking Panax Ginseng continually. Methods Upon beginning her Dojeokgangki-tang gamibang regimen the patient was asked to self-evaluate the degree of discomfort in her symptoms using a using a number scale from 0 (no symptoms) to 10 (most severe). Evaluation of palpitation and chest discomfort was made a high priority. Results The palpitation the patient experienced, and self-evaluated as an "8" on the number scale at the time of her admission to the hospital, had disappeared (become "0") by the fourth day of hospitalization. The numerous symptoms which accompanied her palpitations (chest discomfort, nausea, anorexia, general weakness, weight loss, and depression) also improved accordingly. Conclusions Health functional food can pose serious health risks such as sudden bleeding and palpitations in individuals with pre-existing health problems. One must exercise extreme caution in deciding to take such supplements. As seen in the case of the middle-aged woman, the quality of life of patients who had experienced adverse reactions to such supplements can be treated through the administration of Dojeokgangki-tang gamibang.

  • The purpose of this study is to identify the effect on a patient with bulimia nervosa-purging type and depression according to the oriental medical treatment and cognitive-behavioral therapy. In this case, a 28 year-old female patient had a binge eating and reward behaviors for 2 years. This patient was treated with acupunture therapy, Soyo-san for 5 months after cognitive-behavioral therapy for 12 sessions(2 times a week). The result of this research showed that bulimia nervosa with depression was improved by our treatment.
