Prosthetic rehabilitation by double-processing technique for edentulous patient with soft palate defect after maxillectomy: A case report (연구개를 포함한 상악골 절제술을 받은 완전 무치악 환자에서 이중 온성법으로 제작한 구개 폐색장치를 통한 보철수복: 증례 보고)

  • A patient who went through maxillectomy can have soft palate defects including oronasal fistulas and suffer from dysphagia and dysarthria due to velopharyngeal insufficiency. This defect causes the food to enter nasal cavity and creates hypernasal sound which debilitates a quality of life. An obturator can rehabilitate the substantial oral tissue defects. The maxillary obturator separates the nasopharynx from the oropharynx during speech and deglutition by closing of the defect. For edentulous obturator patient, it is difficult to obtain proper retention due to reduced peripheral sealing. Therefore, the contours of the defects must be used to maximize the retention, stability, and support. Hollow type obturator can improve physiologic function by reducing weight than the traditional obturator. This case report describes a patient with hemi-maxillectomy who recovers mastication, speech, deglutition, and appearance with a maxillary obturator using physiological border molding of the velopharyngeal area and double-processing method.

  • Drug-induced liver injury (DILI) is a common cause of acute hepatitis. Regarding the risk of DILI from herbal preparations, there are controversial issues such as exaggerated reports straying far from the truth and lack of discrimination between herbal drugs and folk remedies or food supplements. This study reported one case of a patient with DILI caused by Smilacis Chinae Radix and cured in an Oriental hospital. Smilacis Chinae Radix has been used as an anti-inflammatory, anti-microbial, detoxification and anticancer compound, as a folk remedy. This report would provide helpful information for management of DILI by folk remedies or herbal drugs.

  • A 58-year old woman with no molar teeth on her upper left side wanted these missing teeth to be replaced with a fixed implant prosthesis. Incision, from distal to 2nd premolar to hamulus, was performed and impression of canine fossa anteriorly, zygoma buccally, pterygoid hamulus posteriorly and palatal groove lingually was taken by Optosil (Bayer Dent.). The implant frame was made of Vitallium, and was inserted the next day after the impression had been taken. 12-days later, the 4-units fixed prosthesis was constructed. The patient was extremely comfortable, had no pain, and was able to chew food more efficiently.

  • Hydatid disease is a zoonotic infection in humans. The disease is endemic in some parts of the world, including Africa, Australia, and Asia, where cattle grazing is common; the disease is spread by an enteric route following the consumption of food contaminated with the eggs of the parasite. Failure to identify this parasite results in delayed diagnosis and increased morbidity to the patient. Upon diagnosis, every possible step should be taken, both surgical and medical, to prevent anaphylactic reactions from the cystic fluid. Postsurgical long-term follow up along with periodical ultrasonography of the liver and computed tomography scan of the abdomen is essential to rule out possible recurrence.

  • Proper hygienic care of removable dentures is an important means of maintaining a healthy oral mucosa on denture wearers. Denture cleansing is often poor due to improper mechanical and the inefficient chemical cleansing of dentures. Dentists and patients should realize that microbial plaque on dentures may be harmful to both the oral mucosa and the patient's general health. This literature review was aimed to evaluate materials and methods for cleansing dentures and to discuss different means of keeping dentures plaque-free. A routine denture cleansing regimen should be designed to remove and prevent reaccumulation of microbial plaque and also to remove mucin, food debris, calculus, and exogenous discoloration. The combined use of chemical and mechanical cleansing is highly recommended for patients to clean their denture effciently.

Free jejunal graft for replacement of cervical esophagus (유리 장 이식편을 이용한 식도 재건)

  • Reconstruction of the pharynx and cervical esophagus is a difficult surgical problem. A successful case of reconstruction of cervical esophagus by free jejunal graft upon a 23-year-old female who had stricture in the pharynx and cervical esophagus after ingestion of hydrochloric acid is presented. This procedure was done after the primary traditional reconstruction with right sided colon had resulted in restenosis due to necrosis of the cervical portion of the graft. A proximal jejunal segment, about 12 cm in length was isolated for free graft preserving its vascular arcade. Both superior thyroid artery and vein were anastomosed to the graft vessels in end to end by continuous suture of 8-0 monophil. Nylon. The postoperative course was uneventful. The patient has been followed for 4 months after operation and she can eat every kind of food without dysphagia. We think free jejunal graft offers an excellent and safe method of reconstructing cervical esophagus and pharynx with definitive advantages over other traditional techniques.

Personal digital assistants: Essential tools for preparing dietetics professionals to use new-generation information technology

  • Rapid integration of information technology into health care systems has included the use of highly portable systems-in particular, personal digital assistants (PDAs). With their large built-in memories, fast processors, wireless connectivity, multimedia capacity, and large library of applications, PDAs have been widely adopted by physicians and nurses for patient tracking, disease management, medical references and drug information, enhancing quality of health care. Many health-related PDA applications are available to both dietetics professionals and clients. Dietetics professionals can effectively use PDAs for client tracking and support, accessing to hospital database or information, and providing better self-monitoring tools to clients. Internship programs for dietetics professionals should include training in the use of PDAs and their dietetics applications, so that new practitioners can stay abreast of this rapidly evolving technology. Several considerations to keep in mind in selecting a PDA and its applications are discussed.

  • The movements of oral surrounding muscles, lips, and tongue play a very important role while masticating, swallowing, and speaking. In case of the edentulous patient wearing dentures as well, every movement of the tongue can be seen almost the same as that of the dentulous jaw. The crushed food during mastication is swallowed and sent to pharynx. At this course, the contact of the posterior portion of tongue with dentures affects the stabilization of denture. The retention and stability of the complete dentures are related with buccinator muscles of buccal parts, lips in front, and tongue of the lingual sides. Strack(1946) insisted that the arrangement of artificial teeth in the same place as that of the natural dentition is the best way to maintain the stability of denture. In this report, I would like to introduce the several advantages of a new impression method taken simultaneously, and its procedures and clinical cases.

Hospital Food Service Systems in the United Kingdom With Special Reference to Patient Satisfaction

  • 본 연구는 영국의병원 급식체계를 분석하고 그에 대한 객관적, 주관적 평가를 시도하는데 목표를 두고 있다. 영국 보건성의 지원하에 12개 병원에 대한 급식체계를 비용편익 분석과 생산성 측정이라는 객관적 기법, 그리고 Indepth-interview 및 Self-Administered Qestionnaire 라는 주관적 방법을 통해 분석하였다. 이 중 본논문에서 다루는 것은 환자 만족도와 이에 영향을 미치는 병원 급식 체계의 결정요인 부분이다. 제1단계 연구에서는 급식대상 환자들에 대한 표본추출을 통해 설문조사를 실시하였다. 분석결과 환자의 인구통계학적, 정서적 변수가 환자의 만족도에 가장 큰 영향을 미치는 요소로 나타났다. 각병원에 대한 객관적 자료를 이용한 2차 희귀분석 결과 인적자원요소와 시스템요소로 대별된 독립변수 요인군종 어느 변수도 0.05(p) 수준에서 유의미하지 않은 것으로 밝혀졌다. 환자의 만족도는 병원 급식체계에 기준한 객관적인 결정요인에 의해 설명되어지지 않고 오히려 환자 자신의 변수에 의해 결정되어짐을 알수 있다. 본논문이 시사하는 바는 급식체계에 대한 평가에 있어 이론적 모형과 변수의 선정이 수정될 필요가 있다는 점과 방법론적 차원에서 평가대상 급식시스템의 유형상 특성이 고려되어야한다는 점이다.

  • In recent years, the pathology in the colon grows up annually since people of all ages prefer to have less fiber and more fat food. Therefore, the colonoscopy is generalized in developed countries. But it requires much time to acquire a dexterous skill to perform an operation and the procedure is painful to the patient. Therefore, some studies on the development of autonomous colonoscope are carried out. In this paper, we propose a new and simple locomotive mechanism that can be propelled by elliptic motion of a leg. It has several legs that have constant phase difference each other and those legs are disposed along the upper and lower of the body. In order to evaluate the performance of locomotive mechanism, we carried out the simulations of moving characteristics and the experiments in the colon of a dead pig.
