This study was done for understanding the desirable direction of eastern and western nursing by comparing the nursing practice for the Pyrexia patient occuring most commonly. Body temperature usually maintains around $37^{\circ}C$ owing to the thermoregulatory center but pyrexia is caused by exogenous pyrogen like infection, cancer or disturbances in body's homeostatic heat balance. Pyrexia is defined that body temperature rise above $37.2^{\circ}C$. It has chill phase, course of the fever, termination accompanied various symptoms. Oriental medicine explains that pyrexia comes from Yang's(陽) abundance and Yin's(陰) lack. Pyrexia mainly happens when body constitution is in bad condition by Six Dirty's(六陰) affection to Wi Area (偉分). It also occurs because of unbalance between Qi (氣) and Yin(陰) caused by the lack in Seven Emotions, labor, food. The Sanghanron(傷寒論), explains that pyrexia is categorized exogenous fever like Poong Han(風寒), Poong Yul(風熱), Sup Yul(濕熱) and endogenous fever due to the Qi and blood deficiency. Explained above, even though pyrexia has different meaning in oriental medicine and western medicine, but this study have compared the oriental and western medicine assuming that pyrexia is rise of body temperature. From this point of view, oriental and western nursing shows the similarity in the field of 1) use of antipyretic to control the body temperature 2) rest and comfort 3) watering 4) nutrition and case study was executed in nursing practice. From the above study, western nursing has superiority in decreasing the patient's risen body temperature using antipyretic and ice bag. But in case of empty heat, oriental nursing which recommends the patient's body keep warm and prevent the use of ice bag as the first step and helps patient's sweating by drinking of hot water was comparatively effective. In conclusion, it is desirable that oriental nursing emphasizing the supportive nursing and western nursing should be harmonized according to the status of pyrexia patient and it is needed to study the nursing method appropriate in our culture.

Food bolus impaction is the most common cause of esophageal foreign body obstruction in adults. Other causes include intentional ingestion in psychiatric patients or prison inmates. We experienced successful treatment of a patient with intentional ingestion of multiple sharp foreign bodies(25 cutter and razor fragments). A 47-year-old male patient who was suffering from chronic alcoholism was admitted, via the emergency room, with dysphagia and neck pain. He was suffering from alcoholic liver cirrhosis and psychiatric problems, such as chronic alcoholism, anxiety disorder and insomnia. The patient had intended to leave the hospital after having swallowed the sharp objects. Plain radiographs and computed tomography (CT) scan showed multiple, scattered metal fragments in the esophagus, stomach, and small bowel. We performed emergent endoscopy and successfully removed one impacted blade in the upper esophagus using by a snare with an overtube. The rest of the fragments had already passed through the pylorus, so we could not find them with endoscopy. We checked the patient with simple abdominal radiographs and careful physical examinations every day. All remaining fragments were uneventfully excreted through stool during the patient's 6 day hospital stay. Finally, we were able to confirm the presence of the objects in the stool, and radiographs were negative. The patient was discharged without complications after 14 days hospital stay and then was followed by the Department of Psychiatry.
purpose : The purpose of this paper is to report that the fasting treatment was quick and effective against the menopausal depression. So that the further study on the fasting treatment for the menopausal depression is expected. methods : This fasting treatment included three periods. The first was to reduce the food intake and the sodium intake as well. The second was for fasting for two weeks. During this period, the patient drank 250cc of the fermented herbal juice and 2500cc of water a day and had the lactobacillus supplement twice a day. Then, the third was for rebuilding. During the rebuilding period, the patient followed the limited sodium diet for two weeks. results : After the fasting treatment done, the symptoms that patient complained before went away. The BDI score went down from 19 to 3. And, of course, the patient lost her body weight and the fat. conclusions : The 49-year-old female patient in the menopause period was suffering from the lack of energy, depression, sleeping disorder, constipation, and weight gain. She took the fasting treatment along with the acupuncture at the holistic medical clinic, 'The Seasons' from Jan 16th 2009 to Feb 27th(the main fasting period was for 14 days). The BDI score indicated that she had the depression as her score was 19 at the first examination. After the treatment was completed, the BDI score dropped to 3. Therefore, I report the immediate effect of the fasting treatment again the depression. Women are twice as likely to get the depression as men. Especially, the women in menopause period tend to get the depression as they are facing the physical, mental, and social changes which can act as the stressors. Besides, women tend to get depressed when they gain weights. Thus, I suggest the further study on the fasting treatment should be carried on so to apply it more positively to various problems in the field of the psychiatry.
The purpose of this study was to investigate dietary intake and to evaluate patient satisfaction toward the quality of hospital foodservice. Questionnaires were distributed to 203 hospitalized patients in 3 hospitals having 300 beds. The intake rates for served amounts of rice, side dishes, and soup were 72.5%, 68.2%, and 62.6%, respectively. The main reasons for left-overs were 'no appetite' (25.8%) and 'not salty enough' (19.9%). The rate of patients eating outside food was about 33.5%. The average score for quality satisfaction of meal characteristics was 3.34 ${\pm}$ 0.61, and the average score for quality satisfaction of sanitation and service characteristics was 3.58 ${\pm}$ 0.61. 'Seasoning' showed the lowest score and 'temperature' showed the highest score for quality satisfaction of meal characteristics. In the quality satisfaction of sanitation and services, 'explanation of meals' showed the lowest score and 'exactness of meal times' showed the highest score. The patients hospitalized for 10 days showed significantly lower average scores than those hospitalized over 60 days for quality satisfaction of meal characteristics. The patients with 'little appetite' and 'regular appetite' showed significantly lower average scores than those with 'much appetite' for the quality satisfaction of meal, sanitation, and service characteristics. The patients who 'rarely had leftovers' showed significantly higher average scores than those who 'always had leftovers' and 'often had leftovers' for quality satisfaction of meal characteristics. Meal characteristic scores were significantly correlated with age (r = 0.216), length of admission (r = 0.310), appetite (r = 0.251), leftovers (r = 0.233), and intake of soup (r = 0.205). Also, sanitation and service characteristics scores were significantly correlated with age (r = 0.327), education (r = -0.202), length of admission (r = 0.168), and appetite (r = 0.155). Thus, it would seem to be desirable that hospital foodservices improve the taste and seasoning of meals and provide appropriate nutrition education and counseling in mid-sized hospitals.
The purpose of this study was to analyze the importance-performance of clinical nutrition management in convalescent hospitals. The research was carried out based on questionnaires administered from March to April, 2015 to 73 dietitians at 40 convalescent hospitals in the Gyeongnam area. There was a statistically significant difference between the mean scores for importance (4.01/5.00) and performance (2.95/5.00) of clinical nutrition management. The importance and performance grid analysis showed that participation in a nutritional management committee, administration of patients using a cooperation program among hospital departments, cooperation with a medical team on patient's nutrition status, nutrition initial assessment, nutrition care process for patients showing malnutrition, nutrition care process for tube feeding patients, management of a therapeutic diet, meal management using dietary slip instructions including a therapeutic diet, and explication of a therapeutic diet for patients scored high regarding importance and performance (doing great area). Medical records on patient's nutrition management, and nutrition counseling for requested patient scored low regarding the importance and high regarding performance (overdone area). Participation in medical rounds, personal nutrition education for patients, group nutrition education for patients, nutrition education for medical teams, development of a menu for therapeutic diet and standardized recipes, and provision of information on diet therapy for patients after discharge scored low regarding importance and performance (low priority area). Accreditation of convalescent hospitals and interest of medical professionals in clinical nutrition management were effective variables for the importance-performance gap of clinical nutrition management. In conclusion, the accreditation process and positive awareness of medical professionals with regard to clinical nutrition management had positive effects on reduction of the importance-performance gap in clinical nutrition management at convalescent hospitals. The strength of clinical nutrition management in the accreditation and development of an education program for increasing medical team or administrator interest in clinical nutrition management could lead to improvement of clinical nutrition management for elderly patients in convalescent hospitals.
Objectives: The nutritional status of cancer patients undergoing chemotherapy is closely related to the compliance of nutrition education. However, as chemotherapy is conducted repeatedly, compliance with nutrition management is lowered, leading to malnutrition. Malnutrition is related directly to the quality of life after surgery in cancer patients. Therefore, this study examined the factors related to compliance with nutrition management during chemotherapy. Methods: In this study, five subjects with colorectal cancer undergoing adjuvant chemotherapy were interviewed in-depth using the Giorgi study method. The contents of the nutrition education visits and in-depth interviews were transcribed in the language of the subject after recording, and the appropriateness of the data was improved by reflecting the subject's actions and facial expressions. Results: After conducting the in-depth interviews for each subject, the experience of the subject's diet and adjuvant chemotherapy was drawn into two domains, six elements, and 26 sub-elements. In the cognitive domain, the patients experienced physical and psychological changes, and the need for nutrition management was recognized by analyzing the dietary causes of the diseases. In the domain of practice, a knowing-doing gap was formed, unlike the patient's will. Factors that inhibited compliance with nutritional management included digestive problems, sensory changes, loss of appetite, and social interaction stress. Conclusions: Dietary management is very important for patients receiving periodic anticancer therapy, and step-by-step training and personal monitoring based on the chemotherapy order is necessary to maintain the patient's will and social and environmental support.

Purpose Bisphophonate-related osteonecrosis of the jaw (BRONJ) is an emerging problem. Extensive osteonecrosis of the jaw needs free flap reconstruction. Free fibular flap is the most useful flap for maxilla-mandibular hard and soft tissue reconstruction. The advantages of fibular free flap are simultaneous soft and hard tissue reconstruction and placing implant in reconstructed mandible and maxilla. In this study, four consecutive BRONJ patients who underwent fibula free flap reconstruction using simulation surgery were reviewed. Materials and Methods Four BRONJ patients who underwent free fibula reconstruction between May 2006 and September 2014 were included in this study. Male to female ratio was 1:3 and average age was 67.3 years old (62-70). All patients need mandibular bone reconstruction. Three patients suffered from osteoporosis and one male patient had multiple myeloma. Postoperative flap survival, functional reconstruction, esthetic results, food taking were evaluated. Results Three osseous flaps and one osteocutaneous flap were used. All the fibular flaps were survived and patients were recovered without complications. Oro-cutaneous fistula was resolved after operation. All patients were satisfied with the esthetic results. Patients reported improved solid food intake after operation with partial denture. One fully edentulous patient had semi-fluid diet after operation. Conclusion Treatment of the BRONJ is difficult due to lack of standard protocol. Fibular free flap using simulation surgery is the workhorse flap for mandibular hard and soft tissue reconstruction, especially in stage III BRONJ patient. In this study, functional and esthetic results were successful in all patients. Normal diet was possible with partial dentures.
Objectives : Seborrheic dermatitis is common abnormal skin condition characterized by flaking and itch. In seborrheic dermatitis, the flakes are greasy and yellowish. Inflammation is also observed. Through this paper we can understand the process of seborrheic dermatitis of the scalp and confirm the scalp changes in the treatment process. The purpose of this study was to report the clinical effects of herbal medicine on Seborrheic dermatitis Methods : We employed oriental medical treatments; herbal-medication (Hyeongbangpaedoksangamibang), infant acupuncture, moxibustion and blood therapy. We treated her three times a week. She took medicine three times a day after a meal. During taking medicine, we let her avoid fatty food, flour based food. Her father took a picture the patient's head parts and we compared the symptom with previous symptom. With the picture, we evaluated the patient's scalp condition. Results : After taking treatment, the scalp condition of the patient was much improved. At first, yellowish to reddish scaly pimples appear along the hairline and itching was severe. There were thick and black crusts on the scalp, red, greasy skin covered with flaky white or yellow scales. After that the crusts were fell off and the itching was disappeared during the treatment period. And then the hair loss was occurred. After a year of treatment, scalp condition and hair were back to normal. Conclusion : Herbal medicine (Hyeongbangpaedoksangamibang) with oriental medical treatments was effective in the treatment of seborrheic dermatitis of the scalp and it helped to improve regenerating the scalp condition.
Phytochemicals have long been known to hold a number of physiological benefits, including antioxidant, anticardiovascular activities and anticancer. The profitable effects of phytochemicals from food sources such as vegetables and fruits, with respect to neurodegeneration, are only beginning to receive increased attention. Alzheimer's disease(AD) is one of the major neurodegenerative diseases for which no treatment is available, and characterized by loss of cognitiion and memory. Many recent studies show that the brain of AD patient is subjected to increased oxidative stress resulting from free radical damage, and the resulting cellular malfunctions are widely believed to be responsible for neuronal degeneration in AD. In this study, the relative relation between D and phytochemicals were surveyed.
Benign prostate hyperplasia (BPH) is one of the most common elderly disease, and because of prolonged incubation period and many side effects of medication or surgical interventions, the use of dietary phytochemicals is considered as an effective measure for prevention of BPH. The purpose of this study is to investigate the mechanism of inhibition effect for BPH by flavonoids such as baicalein and kaempferol. BPH cells were collected through biopsy from patients with PSA of 4 or higher, followed by primary culture. In vitro experiments were conducted to evaluate mitochondrial respiration, intracellular reactive oxygen species (ROS) level and expression of inflammatory markers, genes, and anti-oxidants. In conclusion, baicalein and kaempferol have been demonstrated to inhibit BPH through lowering ROS, thereby reducing inflammation triggers, and reduced inflammation. This study is expected to be helpful in the development of flavonoids that have a clinical effect on suppressing BPH.
