후기 급성호흡곤란증후군환자에서 스테로이드의 사용 효과 (The Effect of Steroid Therapy in Patients with Late ARDS)

  • 연구배경: 급성호흡곤란증후군(ARDS)환자에서의 고용량 및 단기간이 스테로이드의 사용은 사망률은 감소시키지 못하나 최근 섬유증식기로 진행한 후기 급성호흡곤란증후군 환자의 일부에서 스테로이드의 투여가 환자의 예후를 개선시킨다는 몇몇 연구 보고가 있었다. 이에 후기 급성호흡곤란증후군 환자에게 투여한 스테로이드가 사망률 및 호흡 역학에 미치는 영향을 후향적으로 분석하였다. 방 법: 1998년 1월부터 2001년 7월까지 서울중앙병원 내과계 중환자실에 입원한 급성호흡곤란증후군 환자들 중 일주일 이상 기계호흡을 했고 이전에 스테로이드를 쓴 적이 없으면서 기계환기, 복와위, 폐포모집술(recruitment maneuver), 산화질소 흡입 요법 등의 치료에도 급성호흡곤란증후군 진단 8일째 PF ratio가 200이하인 48명 중 스테로이드 치료군 14명과 비치료군 34명의 임상적, 생리적 지표들을 비교 분석했다. 평균${\pm}$표준편차로 표기했다. 결 과: 스테로이드 치료군과 비치료군 사이의 급성호흡곤란증후군의 원인, 연령, 급성호흡곤란증후군 진단 당시와 8일째 PF ratio, LIS, 폐 정적 탄성(static compliance), SAPS II score들의 통계학적 차이가 없었다. 스테로이드 치료군과 비치료군의 생존군만 비교시 치료 후 7일째 PF ratio, GOCA, APACHE III score가 스테로이드 치료군에서 의미 있게 호전되었고 사망률 역시 치료군에서 유의하게 낮았다(42.4% vs 73.5% p=0.02). 결 론: 후기 급성호흡곤란증후군 환자에서 스테로이드의 사용은 호흡역학 및 예후를 개선시킬 것으로 사료된다.

자궁경부암의 방사선치료 후 급성 장관 합병증 (Early Bowel Complications Following Radiotherapy of Uterine Cervi)( Carcinoma)

  • 목적 : 저자는 자궁경부암으로 외부 방사선치료와 고선량 강내치료를 받은 신선 증례에서 급성 장관 방사선 합병증에 대하여 관찰하고 이와 연관된 여러 인자들을 밝히고자 한다. 대상 및 방법 : 1994년 4월부터 1998년 12월까지 충북대학교병원 치료방사선과에서 근치적 방사선치료를 시행한 56례를 대상으로 급성 장관 합병증에 대하여 후향적 분석을 실시하였다. 모든 환자는 본과의 치료원칙에 따라 4cm 미만의, 병기 IB1 와 IIA 경우는 41.4Gy의 체외조사후 30Gy의 강내조사를 A점 기준으로 실시하였으며 그 이외에는 50.4Gy의 체외조사후 24Gy의 강내조사를 실시하였다. 급성 장관 합병증의 기준으로는 EORTC/RTOG morbidity criteria를 이용하였으며 이 기준에 따라 치료 첫날부터 90일까지 발생한 합병증을 급성으로 간주하였다. 급성 합병증의 빈도 및 경중을 파악하였으며 급성 장관 합병증과 연관된 여러 치료전 임상 요인을 밝히기 위하여 단변수 분석과 다변수 분석을 실시하였다. 결과 총 66 명의 환자 중 30명(46$\%$)에서 급성 장관 합병증이 발생하였고 그 중 25명(38$\%$)은 grade 1, 2의 경미한 합병증이었으며 4명(6$\%$)은 grade 3, 1명(2$\%$)은 grade 4의 중증 합병증이었다. 거의 모든 합병증은 방사선 치료 시작 3주 후에 발생하였으며 누적 발생빈도는 10주에 41$\%$였다. 고연령과 복부 및 골반부 수술 과거력이 급성 장관 합병증과 유관한 인자였으며 3명의 환자에서는 급성 장관 합병증으로 전체 치료기간이 약 2주 증가하였으며 결국 3명 모두 골반 내 재발을 경험하였다. 결론 : 46$\%$의 환자가 급성 장관 합병증을 경험하였으며 대부분은 경미하여 임상적인 문제를 야기하지 않았다. 다만 고연령 및 수술병력이 있는 환자에서는 급성 장관 합병증으로 인하여 방사선치료에 대한 환자의 순응도(compliance)가 낮아질 수 있으므로 보다 더 적극적인 주의가 필요하다고 하겠다.

급성호흡곤란증후군 환자에서 복와위(prone position)와 산화질소흡입(nitric oxide inhalation) 병용 치료의 효과 (The Combined Therapy of Inhaled Nitric Oxide and Prone Positioning Has an Additive Effect on Gas Exchange and Oxygen Transport in Patients with Acute Respiratory Distress Syndrome)

  • 연구배경 : 급성호흡곤란증후군(ARDS) 환자의 폐 산소화를 개선시키기 위한 보조적 치료법인 복와위 자세에서의 인공환기는 대상 환자의 약 61%에서, 흡입 산화질소 투여는 60-80%에서 효과가 있는 것으로 보고되어 있다. 산소화 호전의 주된 기전은 복와위시는 이환이 심한 등쪽 폐의 환기 호전에 의한 단락 감소이며, 산화질소 투여 시는 이환부위로부터 정상 폐포로의 폐 혈류의 재분포에 의한 단락 감소안 것으로 알려져 있다. 그러므로 복와위와 산화질소의 병용 치료 시 산소화 개선에 상승 효과를 기대할 수 있으나 이에 관한 임상 연구는 없었다. 이에 저자들은 ARDS환자에서 두 치료의 병합이 가스 교환 및 혈류 역학에 미치는 영향을 관찰하였다. 방 법 : ARDS 환자 12명(연령 $56{\pm}12$ 세, 남 : 여=9 : 3)을 대상으로 앙와위에서 호흡 및 혈류역학적 지표를 측정한 후 복와위로 전환하였다. 복와위 30분과 2시간에 동일 지표들을 측정한 뒤 산화질소를 투여하고 (5-10 ppm), 이후 30분, 2 시간 및 산화질소 투여 중단 후 10분에 각각 동일 지표들을 재 측정하였다. 결 과 : 가스 교환 지표 : 복와위에서 산화질소 병용 치료시, 앙와위 및 복와위에서보다 $PaO_2/FiO_2$가 증가되었고(각각 p< 0.01) 폐동맥혈 산소분압차($AaDO_2$)는 감소하였다(각각 p<0.005). 호흡 역학적 지표 : 폐 탄성, 호흡기계 탄성, 기도 저항 및 흡기말 기도압은 치료 방법에 따른 차이가 없었다. 혈류 역학적 지표 : 복와위에서 산화질소 병용 치료시, 앙와위 및 복와위에서보다 심 박출량 및 조직산소전달량($DO_2$)이 증가되었으며(각 P< 0.05), 폐혈관 저항, 평균 폐동맥압 및 폐동맥쐐기압은 감소되었다(각 P< 0.05). 결 론 : 급성호흡곤란증후군 환자에서 복와위 및 산화질소 홉업의 병용 치료는 복와위 단독 치료에 비해 폐산소화 호전에 상승 작용이 있으며, 조직으로의 산소 전달량도 증가시키므로 급성호흡곤란증후군 환자의 보조적인 치료법으로써 유용할 것으로 사료되었다.

폐암의 치료에 관한 일반 의사들의 견해 (Doctors' Opinions on Lung Cancer Treatment)

  • 연구배경 : 폐암으로 진단 받은 환자 혹은 보호자들은 친지 또는 친구 의사들에게 치료방법이나 예후에 관해서 상의하는 경우가 많으며 환자들은 이들의 권고에 따르는 경우가 많다. 따라서 의사들의 폐암의 치료 및 예후에 관한 견해는 환자들의 치료순응도에 많은 영향을 미칠것으로 생각된다. 저자들은 일반의사들의 폐암의 치료와 예후에 관한 견해를 조사함으로서 폐암환자들의 치료순응도를 향상시키기 위한 대책올 수립하는데 필요한 지침을 제공하고자 본 연구를 시행하였다. 방 법 : 대구 및 경상북도의 개원의와 소종합병원에 근무하는 의사들을 대상으로 우편을 통한 설문조사를 동하여 폐암의 예후와 치료에 관한 견해를 조사하였다. 통계분석은 SPSS/PC+ 통계 프로그램을 이용하여 분석하였다. 결 과 : 설문지에 충실히 답한 응답자는 228명이었으며 연령별 분포는 40세 이하가 26명, 41-50세가 113명, 51-60세가 57명, 61세 이상이 32명이었고, 성별분포는 남자가 213명, 여자가 15명이었다. 응답자의 68.3%가 폐암의 치료에 관한 문의를 받은 경험이 있었으며 응답자의 연령이 증가할수록 문의를 받았다는 응답자가 많았다. 진단명을 환자에게 통고해 주는 것이 좋다는 응답자가 51.5% 였으며 보호자의 뜻에 따라 결정하는 것이 좋다는 의견이 21.6% 였다. 치료선택에 있어서는 주치의가 가장 적합하다고 생각되는 치료를 적극적으로 원하는 것이 바람직하다는 응답자가 86.9%로 대부분 주치의가 주도적으로 결정하는 것을 선호하였다. 수술이 가능한 고령(80세) 환자에서는 응답자의 71.3%가 비수술적 치료를 선호하였으며 응답자의 연령이 증가할수록 비수술적 치료를 권하는 응답자가 유의하게 많았다. 방사선요법 혹은 항암화학요법에 관해서는 적극적으로 권하거나 주치의의 지시에 따르라고 권고하겠다는 응답자가 대부분이었으나 방사선치료와 항암화학요법의 효과에 대해서는 부정적인 생각을 갖고 있는 응답자가 비교적 많았다. 결 론 : 폐암의 치료와 예후에 관한 의사들의 견해는 매우 다양하였으며 고령의 환자 혹은 진행된 병기의 환지들에서는 적극적인 치료를 선호하지 않는 경향이 있었다.

폐결핵환자에서 초치료실패에 대한 요인 분석 (Analysis of Causes for Primary Treatment Failure of Pulmonary Tuberculosis)

  • 배 경 : 최근 약제내성결핵이 결핵의 치료에 문제가 되고 있으며 그 수도 증가하고 있다. 불규칙 복약, 잘못된 처방 그리고 불량한 치료순응도가 주된 원인으로 생각되고 있다. 그러나 표준단기초치료 처방으로 규칙적인 복약을 했음에도 치료에 실패하는 경우도 적지 않으며 이에 대한 임상적 자료를 살펴보고 가능한 요인을 알아보고자 하였다. 방 법 : 1996년 10월 현재 국립마산결핵병원에서 치료중인 환자중 표준단기초치료 처방으로 규칙적인 복약을 하였음에도 불구하고 초치료에 실패한 환자 50명을 대상으로 나이 및 성별, 항결핵제 치료중 음주 및 흡연력, 결핵의 가족력, 처음 결핵을 진단받은 계기 및 의료기관, 의료기관으로부터 결핵의 치료과정에 대한 충분한 설명을 들었는지의 여부, 초치료가 실패했다고 판단되는 시점에서의 흉부 X-선 사진상 병소의 특징 및 부위, 과거치료력 그리고 약제감수성검사의 결과등에 대해 descriptive cross-sectional study를 시행하였다. 결 과 : 1) 연령별로는 19세에서 67세까지 비교적 고른 분포를 보였으나 20대가 16명으로 34%를 차지하였으며 성별로는 남자가 44예 그리고 여자는 6예로 남자가 압도적으로 많았다. 2) 초치료기간중 흡연과 음주여부에 대해서는 22예에서 흡연을, 24예에서 음주를 하였다. 3) 17예에서 가족력이 있었다. 4) 최초진단을 받은 의료기관은 보건소가 27예로 가장 많았다. 5) 결핵을 진단받게된 증상이 있었던 경우가 42예로 가장 많았으며 자각증상이 없이 우연히 발견하게된 경우는 8예였다. 6) 결핵 및 치료과정에 대한 경우는 38예였다. 7) 흉부 X-선 사진상 병소의 부위 및 특정은 양측성이 24예(48%)로 가장 많았고 43예(86%)에서 공동이 있었다. 8) 과거치료력은 29예에서 있었으며 모두 규칙적으로 복약하였다. 9) 약제감수성검사는 알 수 있었던 41명의 환자 전 예에서 약제내성이 있었으며 이 중 초회내성이 21예, 획득내성이 20예였으며 INH와 RFP을 포함하는 2가지 이상의 약제에 내성을 보인 경우는 36예였다. 결 론 : 20대의 남자환자, 결핵의 가족력, 흉부 X-선 사진상 양측성 병소 혹은 잔존하는 공동성 병변 그리고 약제 감수성검사에서 INH와 RFP에 내성을 보이는 경우는 초치료실패의 요인이 될 가능성이 높을 것으로 생각된다.

인후두 역류질환에서 N-Acetyl Cysteine의 증상 개선 효과에 대한 다기관 평가 (Multicenter Evaluation on the Efficacy of N-Acetyl Cystine in Relieving the Symptoms of Laryngopharyngeal Reflux Disease)

  • Background and Objectives : Laryngopharyngeal reflux disease (LPRD) is relatively common disease. N-acetyl cysteine (NAC) has both mucolytic and antioxidant effect, also may be beneficial in inflammatory airway diseases. The purpose of this study was to evaluate the efficacy and safety of inhaled NAC therapy in LPRD. Materials and Method : We retrospectively reviewed the medical records of 525 LPRD patients at 12 medical centers. Finally 401 patients subjected to inhaled NAC therapy for 2 months were enrolled in the study. We analyzed the change of Reflux Symptom Index (RSI) and Reflux Finding Score (RFS) after use of NAC for 4 weeks and 8 weeks in addition to the patient's compliance of the treatment. Results : The RSI score significantly decreased from $19.87{\pm}6.34$ to $12.78{\pm}6.93$ after 4 weeks and to $10.65{\pm}7.47$ after 8 weeks. The RFS score also significantly decreased from $9.29{\pm}3.4$ to $7.17{\pm}3.41$ after 4 weeks and to $6.1{\pm}3.73$ after 8 weeks (p<0.05). During the treatment periods, 42 patients (10.4%) reported to have 80 episodes of discomfort. Throat discomfort (33%) and nausea (28%) were most common complaints, but the duration of discomfort was usually less than 4 weeks. Conclusion : Inhaled NAC treatment is highly effective for the reduction of both subjective and objective findings in LPRD patients. This study will provide the evidence of new treatment option for patients with LPRD. However, further studies will be needs to assess the real effect of inhaled NAC therapy as a standard treatment regimen of LPRD.

직장암에 대한 수술 전 동시병용 방사선-항암 화학요법: 초기 치료결과 보고 (Preoperative Concurrent Radio-chemotherapy for Rectal Cancer: Report of Early Results)

  • 목적: 수술 전 동시병용 방사선-항암 화학 요법의 초기 치료성적과 급성 부작용에 대해 알아보고자 하였다. 대상 및 방법: 1999년 6월부터 2002년 4월까지 T3또는 T4 병기의 직장암으로 진단 받고 완전절제가 어렵거나, 종양이 하부직장에 위치하여 수술 시 항문 괄약근의 보존이 불가능하리라고 예견된 40명의 환자에게 수술 전 동시병용 방사선-항암 화학요법을 적용하여 37명의 환자가 수술 전 치료방침을 완료하였다. 방사선치료는 전 골반부에 일일 1.8 Gy씩 5주간에 45 Gy를 엎드린 자세로 조사하였다. 항암 화학요법은 경구 UFT와 Leucovorin (LV)(12명), 정주 5-fluorouracil (FU)와 LV (10명), 정주 5-FU 단독요법(일시 정주 10명, 지속 정주 5명)을 각각 시행하였다. 수술은 수술 전 치료 종료 후 4$\~$6주경에 예정하였으며 35명에 대해서 수술을 시도하였다. 결과: 본 연구의 수술 전 방사선-항암 화학요법에 대한 순응도는 매우 높았. (92.5$\%$, 37/40). 수술 전에 간과 폐에 새로운 원격전이가 확인된 2명을 제외한 35명의 환자에서 절제수술을 시도하였고, 22명(62.9$\%$)에서 항문 괄약근의 보존이 가능하였으나, 2명(5.7$\%$)은 개복 후 절제수술을 포기하였다. 절제수술을 시행한 33명 중 30명은 육안적 완전절제를, 1명은 육안적 불완전절제를 시행하였으며 2명은 병리소견에 관한 정보를 얻을 수 없었다. 수술 및 병리소견에 근거한 병기하강률은 45.5$\%$ (15/33), 절제연 음성 완전절제율은 78.8$\%$ (26/33)였다. 수술 전 방사선치료 도중 급성 부작용으로 Grade 3$\~$4 호중구 감소가 4명(10.8$\%$)에서 발생하였다. 절제수술 후 국소 재발이 12.1$\%$ (4/33), 수술 전 치료방침 완료 후 원격 전이는 21.6$\%$ (8/37)에서 각각 나타났으며, 3년 생존율은 87$\%$였다. 결론: 직장암에 대한 수술 전 동시병용 방사선-항암 화학요법은 낮은 부작용으로 높은 수술절제율, 병기하강률, 완전절제율, 항문 괄약근 보존율 등을 얻을 수 있는 효과적인 방법으로 판단되며 앞으로 장기간에 걸친 추적관찰을 요한다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm.
