A New Framework of 6lowpan node for Neighboring Communication with Healthcare Monitoring Applications

  • The proposed technique uses cyclic frame structure, where three periods such as beacon period (BP), mesh contention access period (MCAP) and slotted period (SP) are in a data frame. This paper studies on a mechanism to allow communication nodes (6lowpan) in a PAN with different logical channel for global healthcare applications monitoring technology. The proposed super framework structure system has installed 6lowpan sensor nodes to communicate with each other. The basic idea is to time share logical channels to perform 6lowpan sensor node. The concept of 6lowpan sensor node and various biomedical sensors fixed on the patient BAN (Body Area Network) for monitoring health condition. In PAN (hospital area), has fixed gateways that received biomedical data from 6lowpan (patient). Each 6lowpan sensor node (patient) has IP-addresses that would be directly connected to the internet. With the help of IP-address service provider can recognize or analyze patient data from all over the globe by the internet service provider, with specific equipments i.e. cell phone, PDA, note book. The NS-2.33 result shows the performance of data transmission delay and data delivery ratio in the case of hop count in a PAN (Personal Area Networks).

Comparison of Intensive Care Unit Nurses' and Family Members' Priorities of Patient and Family-centered Care in Ghana

  • Purpose : Life-threatening illnesses represent a crisis for individual patients and their families. Little has been made to understand the priorities or perspectives in developing a care plan. This results in poor outcomes, and patients and families return home without being satisfied with the care provided. This study aimed to address nurses' and families' care priorities on patient and family-centered care principles and compare those priorities. Methods : A quantitative comparative descriptive research was conducted. The data were part of a study that was carried out to elicit and compare nurses' and families' perceptions of complying with patient and family-centered care (PFCC) principles in intensive care units (ICU) in Ghana. The respondents were ICU nurses (n=123) and family members of hospitalized patients in the ICU (n=111). The tool for the study was a "modernized version of a hospital self-assessment inventory on PFCC," and data analyses were performed using SPSS version 20.0. Results : Nurses and families differed significantly in their priorities of care based on the principles of PFCC. The means and p-values were significantly different for the definition, pattern of care and access to information/education, and the overall total scores of the patient and family-centered care principles (PFCCP) Conclusion : To render care that aligns with the care priority of families and patients in the ICU, nurses must plan care in consultation with their families.

일개 공공병원 응급실 방문건수 관련 요인 (Factors affecting the number of emergency room visits in a public hospital in Korea)

  • Purpose: Korean medical services are not balanced across regions and social classes. To prevent mortality gaps, Korea must distribute its medical resources more efficiently. Patient factors affecting emergency room visits serve as basic data for determining best practices for public healthcare distribution. Methods: The data included 18 473 visits by 14 949 de-identified patients who visited a public emergency room over one year. The dependent variable was the number of emergency room visits. A Poisson regression was conducted with the independent variables, comprising sociodemographic, socioeconomic, and spatial accessibility factors and patient characteristics. Results: Older men with higher Korean Triage and Acuity Scale scores visited more frequently. Greater patient-hospital distance decreased visits; however, the presence of a hospital within 1 km of a patient's residence did not affect the number of visits. The use of 119 services was negatively correlated with the number of visits. Visits increased with more medical benefits. Conclusions: Patient age, distance to hospital, use of 119 services, and medical benefits should be considered when planning or managing public hospitals in Korea.

Database for Patient Information Management in Radiation Oncology Department

  • The purpose of this study is to build a database of patient information for efficient radiotherapy management. Microsoft Office Access was used to build the database owing to its convenience and compatibility. The most important aspect when building the patient database is to make the input and management of patient information efficient at every step of radiotherapy process. The information input starts from the patient's first visit to the radiation therapy department and ends upon completion of the radiotherapy. The forms for each step of radiotherapy process include the patient information form, the radiotherapy schedule form, the radiotherapy information form, the simulation order form, and the patient history form. Every form is centrically connected to the radiation oncology department's patient information form. A test revealed that the database was found to be efficient in managing patient information at each step. An important benefit of this database is improved efficiency in radiotherapy management. Information on patients who received radiotherapy is stored in a database. This means that this clinical data can be found easily and used in future, which will be helpful in research studies on the radiation oncology department. Benefits such as these will potentially contribute to improved radiotherapy quality.

Clinical Analysis of Hemodialysis Vascular Access: Comparision of Autogenous Arterioveonus Fistula & Arteriovenous Prosthetic Graft

  • Background: Mature autogenous arteriovenous fistulas have better long term patency and require fewer secondary interventions compared to arteriovenous prosthetic graft. Our Study evaluated vascular patency rates and incidence of interventions in autogenous arteriovenous fistulas and grafts. Material and Methods: A total of 166 vascular access operations were performed in 153 patients between December 2002 and November 2009. Thirty seven caeses were excluded due to primary access failure and loss of follow-up. One group of 92 autogenous arterioveous fistulas and the other group of 37 arteriovenous prosthetic grafts were evaluated retrospectively. Primary and secondary patency rates were estimated using the Kaplan-Meier method. Results: The primary patency rate (84%, 67%, 51% vs. 51%, 22%, 9% at 1, 3, 5 year; p=0.0000) and secondary patency rate (96%, 88%, 68% vs. 88%, 65%, 16% at 1, 3, 5 year; p=0.0009) were better in autogenous fistula group than prosthetic graft group. Interventions to maintain secondary patency were required in 23% of the autogenous fistula group (average 0.06 procedures/patient/year) and 65% of prosthetic graft group (average 0.21 procedures/patient/year). So the autogenous fistula group had fewer intervention rate than prosthetic graft group (p=0.01) The risk factor of primary patency was diabetus combined with ischemic heart disease and the secondary patency's risk factor was age. Conclusion: Autogenous arteriovenous fistulas showed better performance compared to prosthetic grafts in terms of primary & secondary patency and incidence of interventions.

Effect of pre-operative medication with paracetamol and ketorolac on the success of inferior alveolar nerve block in patients with symptomatic irreversible pulpitis: a double-blind randomized clinical trial

  • Background: The efficacy of local anesthesia decreases in patients with symptomatic irreversible pulpitis. Therefore, it was proposed that the use of premedication with an anti-inflammatory drug might increase the success rate of pulpal anesthesia in mandibular posterior teeth with vital inflamed pulp. Methods: One hundred thirty-four patients who were actively experiencing pain willingly participated in this study. The Heft Parker (HP) visual analog scale (VAS) was used to record the initial pain intensity. Patients were randomly allocated to receive a placebo, 10 mg of ketorolac, and 650 mg of paracetamol. The standard inferior alveolar nerve block (IANB) was administered to all patients using 2% lidocaine with 1:200,000 adrenaline after one hour of medication. After 15 min, the patient was instructed to rate the discomfort during each step of the treatment procedure, such as access to remaining dentin, access to the pulp chamber, and during canal instrumentation on the HP VAS. IANB was considered successful if the patient reported no or mild pain during access preparation and instrumentation. Moderate or severe pain was classified as a failure of IANB and another method of anesthesia was used before continuing the treatment. Results: The rate of successful anesthesia in the placebo, paracetamol, and ketorolac groups was 29%, 33%, and 43%, respectively, and no statistically significant difference was found between the groups. Conclusion: Preoperative administration of paracetamol or ketorolac did not significantly affect the success rate of IANB in patients with irreversible pulpitis. No significant difference was observed between the paracetamol and ketorolac groups.

웹 기반 의료정보시스템 다중 접근제어를 위한 소프트웨어아키텍쳐 설계방법 (A Study of Software Architecture Design Methods for Multiple Access Con trol under Web-based Medical Information System Environment)

  • 웹 기반 의료정보는 많은 편리성을 제공하지만 공개된 네트워크 환경에서 나타나는 보안 취약성을 해결하지 않은 채 정보노출의 위험속에 사용이 확대되고 있다. 웹 기반 의료정보 접근시 보안문제에 대한 안전한 방법론 강구없이 기술만 발전시키려한다면 또 다른 위협의 요소를 증가시키는 것이다. 따라서 웹에 기반한 정보활용 보안대책 으로서 웹 기반 의료정보 접근제어 보안 메커니즘 기반 설계가 필요하다. 본 논문은 소프트웨어 아키텍쳐 설계사상을 기반으로 하여 의료정보시스템 접근제어 보안 메커니즘 기반을 설계 하였다. 그 방법론은 새로운 설계절차를 도출하고 아키텍쳐를 설계하며 기능 메커니즘 알고리즘을 구성하는 것 이다. 이를위해서는 웹 기반 다중 환자 정보 접근제어를 위한 보안 아케텍쳐 인프라스트럭가 필요하다. 제안하는 소프트웨어아케텍쳐는 소프트웨어 프레임워크를 도출하고 기능 메커니즘을 구성하는 기반에 관한 구조도를 도출했다. 제안된 시스템를 활용하여 의료정보 어플리케이션을 설계할때 의료정보 사용자는 실시간으로 데이터를 검색하면서도 통합화된 접근제어알고리즘의 보장하에서 정보관리 안전성을 확보하는 시스템 설계가 가능하다.

  • 의료기관에서 환자의 개인건강정보는 진료를 위해 의료진의 정보열람이 반드시 필요한 사항이다. 그러나 이러한 정보의 불필요한 노출은 개인정보보호와 관련이 있어 민감하게 취급되어야 하며, 의료기관에 종사하는 사용자들이라 할지라도 접근에 있어 역할에 따른 제한이 필요하다. 따라서 본 논문에서는 의료진과 그 이외의 직원들 간의 사용자 식별을 통한 개인건강정보의 접근 통제뿐만 아니라 업무에 따른 조건을 추가하여 사용자 직종 내에서도 상황에 따른 접근 통제에 대해 연구하였다. 응급상황, 담당과 여부에 따른 접근 통제, 그리고 환자가 정하는 본인의 개인정보에 대한 접근통제를 포함하여 확장된 개념의 역할기반 접근제어를 합으로써 의료기관내에서 환자의 개인건강정보의 불필요한 접근이나 유출을 최소화 할 수 있다.

Efficacy of Transradial Cerebral Angiography in the Elderly

  • Objective : Transradial angiography has become popular among many cardiologists as a diagnostic and therapeutic tool. However, transradial cerebral angiography is not utilized to the same extent. The purpose of this study is to present our experience regarding the usefulness of transradial cerebral angiography, especially in elderly patients. Methods : Between May 2011 and February 2012, a total of 126 cerebral angiographies were performed via a transradial approach in a single center. Of them, only 47 patients were over 60 years old. In our institution, we shifted the initial access from the right femoral artery to the right radial artery in all patients requiring cerebral angiography in 2011. We did not attempt radial access in 40 cases for variable reasons. Results : The procedural success rate was 92.2%. We have four failures of transradial angiography; two because of loop formations of the radial and brachial artery and two due to multiple puncture failures. All supra-aortic vessels were successfully catheterized. However, the selective catheterization rates of the left side distal vessels were lower, as success rates were 89.7% for the right internal carotid artery and 75% for the left internal carotid artery. Procedure-related vascular complications, such as puncture site hematoma, hand ischemia, pseudoaneurysm, arteriovenous fistula and arterial dissection were not observed in our series. However, intraprocedural thrombosis developed in one patient, which was resolved completely by intraarterial thrombolytic agents. Conclusion : With advancing patient's age, we believe that transradial cerebral angiography is a useful tool to decrease patient's discomfort and more effectively manage the vessel tortuosity.

장애인 구강진료 접근성 제약 - Part 2. 장애인 환자 보호자 측면에서의 고찰 (TREATMENT BARRIERS OF SPECIAL NEEDS PATIENTS - PART 2. CONSIDERATION FROM THE PERSPECTIVES OF CAREGIVERS)

  • The aim of this study was to determine the factors contributing to the barrier of dental treatment for special needs patients perceived by the main caregivers of the patients. A questionnaire was developed with three parts: patient-related factors, caregiver-related factors, and factors for treatment barriers. Patient- and caregiver-related factors were analyzed for correlations with the barrier factors. Fisher's exact test and Chi-square test were used at the level of 0.05. A total of 52 caregivers (mean age [SD]=50.2 [11.5] years) for the patients with intellectual and cognitive disabilities (mean age [SD]=38.9 [21.3]) responded to the questionnaires. Oral hygiene status, need for assistance in tooth brushing, and cooperation for tooth brushing were correlated with the level of treatment cooperation of the patients (p<0.05). The above three factors were also correlated with the treatment barrier despite patients presenting oral symptoms (p<0.05). Two thirds of the caregivers felt the treatment cost as severe or a considerable financial burden. For patients, insurance types and need for assistance in tooth brushing, and for caregivers, education levels and financial status were significant factors contributing to financial burden (p<0.05). There were background factors of the patients and their caregivers associated with access to dental treatment for special needs patients. Deficient cooperative skills and financial burdens were the main obstacles to treatment access. Tooth brushing skills and oral hygiene status can be proxy measures to cooperative ability for dental treatment.
