Influencing Factors and Consequences of Near Miss Experience in Nurses' Medication Error (간호사의 투약 근접오류경험의 영향 요인과 결과)

  • Purpose: This study aimed to predict the influencing factors and the consequences of near miss in nurses' medication error based upon Salazar & Primomo's ecological system theory. Methods: A convenience sample of 198 nurses was recruited for the cross-sectional survey design. Data were collected from July to September 2016. Using the collected data, the developed model was verified by structural equation modeling analysis using SPSS and AMOS program. Results: For the fitness of the hypothetical model, the results showed that $x^2$ ($x^2=258.50$, p<.001) was not fit, but standardized $x^2$ ($x^2/df=2.35$) was a good fit for this model. Additionally, absolute fit index RMR=.06, RMSEA=.08, GFI=.86, AGFI=.81 reached the recommended level, but the Incremental fit index TLI=.82, CFI=.85 was not enough to reach to the recommended level. With the path diagram of the hypothetical model, caution (${\beta}=-.29$ p<.001), patient safety culture (${\beta}=-.20$, p=.041), and work load (${\beta}=.18$, p=.037) had a significant effect on the near miss experiences in nurses' medication error, while fatigue (${\beta}=-.06$, p=.575) did not affect it. Moreover, the near miss experience had a significant effect on work productivity (${\beta}=-.25$, p=.001). Conclusion: These results have shown that to decrease the near miss experience by nurses and increase their work productivity in hospital environments would require both personal and organizational effort.

  • Objectives: The Journal of the Korean Academy of Child and Adolescent Psychiatry (JKACAP) has had a 31 year-long history, sharing research and reviews of children and adolescents' mental health to promote voluntary participation and communication of the members of this society. Here, we have reviewed the detailed history of the journal from the perspective of developmental progression of JKACAP and discussed the direction of further development. Methods & Results: We reviewed the journey of the journal by focusing on the effort it took to take the journal to a global standard, and discussed the future direction of progress of JKACAP, based on the opinions raised at the Editor-in-Chiefs' reunion. Conclusion: JKACAP has just stepped on the path to globalization by being indexed in Emerging Sources Citation Index, PubMed Central, and Scopus. It is time to progress to another dimension, by acknowledging and overcoming more complicated issues, such as augmenting impact of the journal, expanding domains of interdisciplinary collaboration, and more global cooperation.

Association Between Unplanned and Planned Readmissions in an University Hospital (비예정과 예정된 재입원 환자들간의 관련 요인 분석)

  • This study describes associated factors of readmission of 213 inpatients from an university hospital in Seoul. This retrospective study reviewed medical records of patients who discharged from a hospital stay for general diseases between 1 August 1995 and 31 October 1995, Cases were 68 discharge patients with an unplanned readmission within 30 days of discharge from an index stay. And the other cases are 145 patients who had more than two discharges and didn't have an unplanned readmission within 30 days. Logistic regression model was analyzed and the results were as follows; 1. duration of readmission, rate of unpayed, room, path, and risk of disease were more likely to be readmitted unexpectedly than the expected readmission patients. 2. early readmission, low risk condition group, and inadquateness of discharge plann for patients had unplanned radmissions rather than planned readmissions. Therefore, discharge planning education to health care provider is required and assessement of discharge planning should be evaluated. Readmissions are usually for related problems that arose during the original hopitaliztion and caused cost problems. Especially the unplanned readmissions are frequently preventable. Ultimately, models for readmissions can serve as a valuable clinical tool for target high-risk patients and older patients and with this kind of tools we can reduce hospital readmissions and maintain high-quality of inpatient care.

Study of stability and evolution indexes of gobs under unloading effect in the deep mines

  • The stress path characteristics of surrounding rock in the formation of gob were analysed and the unloading was solved. Taking Chengchao Iron Mine as the engineering background, the model for analysing the instability of deep gob was established based on the mechanism of stress relief in deep mining. The energy evolution law was investigated by introducing the local energy release rate index (LERR), and the energy criterion of instability of surrounding rock was established based on the cusp catastrophe theory. The results showed that the evolution equation of the local energy release energy of the surrounding rock was quartic function with one unknown and the release rate increased gradually during the mining. The calculation results showed that the gob was stable. The LERR per unit volume of the bottom structure was relatively smaller, which mean the stability was better. The LERR distribution showed that there was main energy release in the horizontal direction and energy concentration in the vertical direction which meet the characteristics of deep mining. In summary, this model could effectively calculate the stability of surrounding rock in the formation of gob. The LERR could reflect the dynamic process of energy release, transfer and dissipation which provided an important reference for the study of the stability of deep mined out area.

Economic Assesment for Track Ballast Structures based on Proper Maintenance Level (적정 유지관리수준에 기초한 궤도도상구조에 대한 경제성 검토)

  • In general, it is known that the concrete ballast track is possible to reduce a cost of maintenance activities cause of maintenance free by restrictive maintenance in application of track ballast structures. However, if deteriorations and fracture mechanism of a material are considered, proper maintenance activities and repairing are continually required. For the problems, we evaluated an economical efficiency of the gravel ballast and the concrete ballast on a proper level of maintenance activities. As the method based on history path is difficult to obtain sufficient reliabilities cause of an absence of real data for maintenance activities, we chose the delphi survey method by professional group, and we obtained the appropriate index and profiles for maintenance activities. As a result, we founded that effects for cost reduction can be expected by preventive maintenance management, and we proposed economical restrictions and life cycle cost for track ballast structures in a life cycle.

Ecological characteristics of areas naturally inhabited by Dolbae trees(Pyrus pyrifolia) on Mt. Kaya

  • The ecological characteristics of Dolbae trees, a valuable genetic resource for native pyrus fruit trees, were investigated at four naturally inhabited areas on Mount Kaya, Gyeongsangnam-Do. Most Dolbae trees were found on the southwest side of an old hiking path to a mountain valley at altitudes of 610∼670 m. The surrounding flora consisted of 46 families, 69 genera, 75 species, and 10 varieties. The natural habitat of the Dolbae trees was basically a deciduous broad-leaved forest with a predominance of Quercus serrata and ligneous plants(xylophytes) from the Fagaceae and Betulaceae families. The habitat was also found to be in a second transition resulting from forest disturbance and exhibited a degree of diversity of 2.901-5.065, based on Shannon-Weiner's index. Examination of the Dolbae trees found in Mount Kaya revealed diameter breast heights(DBHs) ranging from 10 to 60 cm, including six old and large Dolbae trees with a DBH of 50∼60 cm, which will be particularly valuable as a genetic resource. Accordingly, the current results suggest that the study areas are a secondary forest in a typical mid-temperate zone resulting from forest damage about 10 years earlier.

A Feasibility Study on the Damage Detection of Infinite Beams Using the Structural Intensity Measurement Technique (진동 인텐시티 계측 방법을 이용한 무한보의 손상감지에 관한 기초 연구)

  • The structural intensities have been applied to understand a source point and the path of vibrational energy flows in interested structures by many researchers. In this paper, a feasibility study was carried out to investigate the characteristics of a damaged beam with a inflicted open crack using the structural intensities. The damaged beam was taken as a continuous system with equivalent bending stiffness and the flexural vibrations were only considered in numerical simulation and experiments. A four(4)-transducer array was used to measure the flexural vibrations of the beam and the structural intensities were estimated by means of cross spectral density method. As a result, the magnitude changes of the structural intensities could be observed in the vicinity of the damage location and a damage index was newly proposed to identify the damage zone. It has been confirmed that the measurement of the structural intensities was simple and effective method to find out the damage zone.

A Study on the Sexual Values of Institutionalized Adolescents in Terms of Ego-Identity and Perceived Emotional Support from Care Teachers (육아시설 청소년의 자아정체감과 지각된 보육사의 정서적 지지에 의한 청소년의 성가치관)

  • The purpose of this study was to examine the effects of ego?identity and rare teacher's emotional support on the value of sexuality of institutionalized adolescents. The subjects were 102 middle school adolescents in Daegu and Cyeongsangbuk Province, Korea. Ego-identity of the institutional youth, perceived emotional support of care teacher and sexual values reported by the institutionalized adolescents. The data were analyzed with an AMOS 5.5 Program by comparative fit index and a SPSS Win program 12.0 using cronbach's $\alpha$, simple regression, hierarchical multiple regression and path analysis. The findings showed that both the degree of ego-identity and perceived emotional support of care teacher exerted a significant effect on the sexual values of institutional youth. Furthermore, it is important to note that the care teacher's emotional support influenced sexual values of the youth, and was mediated by the ego-identity of the institutional youth.

A Study on Improvement of Fire Service Deployment Standard in Korea (한국 소방력배치 기준의 개선에 관한 연구)

  • The purpose of this study is to offer the improvement for deployment of fire service force in Korea by settlement patterns on the basis of analysis for the present standard and deployment of fire service force. For the adequate deployment and operation of fire service force by settlement patterns, we carried out the analysis of the present standard calculated with allocation of the authorized strength. We also classified clusters for settlement pattern through the statistical methods. We proposed the standard for deployment of fire service force reflected with environmental and need factors through the introduction of standardized index.

Collision-Free Motion Planning of a Robot Using Free Arc concept (프리아크 개념을 이용한 로봇의 충돌회피 동작 계획)

  • This paper presents an effective approach to collision-free motion planning of a robot in the work-space including time-varying obstacles. The free arc is defined as a set composed of the configuration points of the robot satisfying collision-free motion constraint at each sampling time. We represent this free arc with respect to the new coordinate frame centered at the goal configuration and there for the collision-free path satisfying motion constraint is obtained by connecting the configuration points of the free arc at each sampling mined from the sequence of free arcs the optimality is determined by the performance index. Therefore the complicated collision-free motion planning problem of a robot is transformed to a simplified SUB_Optimal Collision Avoidance Problem(SOCAP). We analyze the completeness of the proposed approach and show that it is partly guaranteed using the backward motion. Computational complexity of our approach is analyzed theoretically and practical computation time is compared with that of the other method. Simulation results for two cally and practical computation time is compared with that of the other method. Simulation results for two SCARA robot manipulators are presented to verify the efficacy of the proposed method.
