Clinical Analysis of Mitral Valvular Disease (승모판막 질환의 임상적 고찰)

  • The clinical observations on the 63 cases of the mitral valvular heart disease treated in the department of Thoracic and Cardiovascular Surgery, Kosin medical college, during the period of 5 years from January, 1986 to December, 1989, are as follows. 1. Total 63 cases, 23 were male and 40 were female with sex ratio of 1: 1.7. 2. The age distribution was widely varied from 7 years of the youngest to 65 years oldest, average age was 35.3 year old. 3. The main clinical symptom and sign were D.O.E, general weakness, fatigability, palpitation, chest pain, hepatomegaly, pitting edema and both neck vein distension. 4. The preoperative N.Y.H.A functional classification were class II, III, IV in 28%, 54%, 16%. 5. The preoperative diagnosis were MS - 6.3%, MR - 53.9%, MSR - 36.5%, MVP - 3.1%. 6. All 63 patients were operated on the under direct vision using extracorporeal circulation, MVR in 18 cases, MVR and Tricuspid annuloplasty in 24 cases, DVR in 3 cases, DVR and Tricuspid annuloplasty in 5 cases, TVR in 1 case, Mitral annuloplasty in 4 cases, mitral valvuloplasty in 7 cases and mitral commissurotomy in 2 cases 7. The operative mortality was 7.9%, and 5 years actuarial survival rate was 89%.

Functional Intracardiac Paraganglioma

  • A 39-year-old man presented to the department of emergency medicine in Seoul National University Hospital complaining of chest pain, heart palpitation, and headache. Upon arrival, a computed tomography scan showed a 7.0 cm×6.2 cm lesion with the typical features of a paraganglioma. The patient was treated with an alpha-blocker and a beta-blocker prior to surgical intervention. We removed the tumor successfully, and histopathologic findings indicated that the tumor was indeed a paraganglioma. Since intracardiac paraganglioma is a rare disease, we present this case together with a literature review.

  • The patient in this case, 38 years-old female was adimitted for 8 days(2001. 7. 16 ${\sim}$ 2001. 7. 23). Her chief complain was lumbago, shoulder pain, headache, palpitation, general body weakness, insomnia, whole body heating and dyspnea. we diagnosed this patient as hyperthyroidism with originated from yinhuhwawang(陰虛火旺) and bigihu(脾氣虛). we treated this hyperthroidism patient with herbal medicine(Chonglijagamtang-gamibang(淸離滋坎湯加味方), bojungikgitang-gamibang(補中益氣湯加味方)), Acupunture, Moxa, etc. after oriental treatment during 8 days, pain(shoulder pain, headache, lumbago, whole body pain) was decreased and the symptoms were all diminished. in the days to come, we need to study further study about the superiority of Oriental Medicine healing a hyperthyroidism patient.

In Dong-Eui-Bo-Gam, Ondamtng & its Gagambang are applied for the sudden palpitation, epilepsy, the insomnia of exhaustion and an acute convulsion which are caused by the asthenia of heart and gall bladder(心膽虛怯). 2. In Dong-Eui-Bo-Gam, Guibitang & its Gagambang are applied for amnesia, amenorrhea, upper abdominal pain, the itching sign the pubic region, the hernia of the vulva which are caused by the asthenia of heart and spleen(心脾兩虛). 3. In Dong-Eui-Bo-Gam, Soyosan & its Gagambang are applied for epilepsy, the hemorrhage in time of coughing, the intermittent fever, metrorrhagia, various woman's diseases which are caused by the depression of vital energy of liver(肝氣鬱結).

  • Hyun-chim Therapy was made for help the people who didn't learn breath training. The doctor acupuncture and treat 'Hyun-chi-sul' and it is a means by which patient can use 'universal Qi'. Ul-zeung comes from obstruction of qi by stress. The mind affect the body. The patient has depressed mood, irritable sign, chest discomfort, flank pain, angry stats and some strange feeling on the throat. In the case, two female patients, who complained of depression, insomnia, anxiety, palpitation, low energy, etc. We treated the patients with Hyun-chim Therapy. In result, the symptoms which they complained were improved.

  • The purpose of this study is to report the effects of oriental demonstration treatment for Panic Disorder. Panic Disorder is an anxiety disorder, characterized by panic attacks which are recurring episodes of fear accompanied by somatic symptoms such as shortness of breath, palpitations, chest pain, choking, dizziness, trembling and/or faintness. Objectives: We experienced one case of Panic Disorder treated with Asian medical treatment, acupuncture and herbal medication. 20-year-old female patient suffered from Panic Disorder with nausea and insomnia. Methods: Shihosogantang-gami had been applied to the patient. Results: After treatment her Panic Disorder was improved. Conclusions: Shihosogantang -gami of Asian medical treatment could be safe and effective.

  • From August 1990 to October 1991, we performed operation on 5 patients with cardiac myxoma, which was located in the left atrium, There was 1 male and 4 female patients ranging in age 45 years to 61 years. Chief complaints included exertional dyspnea, palpitation, cough, orthopnea, general weakness. M-mode and two dimensional echocardiography provided an accurate diagnosis in all cases which was utilized as safe, reliable and noninvasive imaging modalities. In all cases, myxoma was excised successfully. Follow up is current. No recurrent myxoma has been identified clinically or by echocardiography. In this series, excellent results were obtained by simple excision of the tumor, with or without a margin of normal atrial septum.

An Unusual Biatrial Cardiac Myxoma in a Young Patient

  • This is a report of a biatrial cardiac myxoma in a young man with a 10-month history of exertional dyspnea and palpitation. The echocardiogram revealed biatrial myxoma prolapsing through the mitral and tricuspid valves during diastole. All cardiac chambers were enlarged and dysfunctional. The electrocardiogram revealed a rapid ventricular response with atrial flutter rhythm. The masses were resected and diagnosed as myxoma by a histological examination. The follow-up echocardiogram revealed significant improvement in ventricular function and reduction in the cardiac chambers' volume. There was no evidence of myxoma recurrence. The most probable cause of the patient's heart failure was considered to be tachycardia-induced cardiomyopathy.

  • Panic disorder is one of common disease clinically, and it is included in the anxiety disorders. This case is about a patient who is 43 years old lady, suffered by a sense of unease, irritation, palpitation, chest discomfort, heating sense of the upper part. etc. This patient was treated by Bosimgunbitang(Buxin Jianpi Tang) in early stage, and Palmulgunzatang(Bawu Junzi Tang) in chronic stage. This paper describe the process and contents about the way the patient was treated.

An Extra-adrenal Pheochromocytoma Presenting with Spontaneous Intracerebral Hematoma

  • We report a 18-year-old man, who has been taking antihypertensive medication for 1month in a local clinic, presented with a sudden onset headache followed by left blindness. He experienced palpitation and chest discomfort during physical exertion since 2years before admission, but unfortunately has been ignored. Brain CT showed intracerebral hemorrhage in the left temporoparietal area, but cerebral angiogram and magnetic resonance image revealed no vascular anomaly. He was managed conservatively, and headache and visual loss were improved over time. Subsequently, on the evaluation of hypertension, he was diagnosed as having extra-adrenal pheochromocytoma on left paraaortic area from the results of endocrinological evaluations, abdominal CT scan, and $^{131}I$-MIBG scintigraphy.
