  • Apoplexy is a disease of a morbid condition manifested as sudden syncope, unconciousness, distortion of face, hemiplegia and dysphasia, usually seen in the middle-aged. The symptoms and signs before sudden onset are headache, dizziness, numbness of extremities, palpitation, etc. This study was performed to investige causes of disease, therapies and prescriptions by insect medicine through the successive medical literatures, recent chinese medical literatures and chinese medical journals. The results are as follows; 1. The treatment of apoplexy is divided into stage of attack and sequela. In stage of attack, the principal therapies of apoplexy are calming the liver, suppressed Yang, dissipate phlegm and elimination weatness. In sequela, the principal therapies of apoplexy are invigorating Qi, promote blood circulation and dredge collateral. 2. Insects medicine have more strong effect than herbal medicine, because apoplexy is a kind of critically desease. 3. Insects medicine is effective in a convalscent stage and sequela of apoplexy. The proper dosage for stage of attack is a small dose of insects medicine(about 2-4g), increse gradually. In convalscent stage, about 4g, in sequela, patients need a large dose of insects medicine(about 8g). 4. Hirudo used to remove stagnated blood and to disperse swelling for the treatment of severe cases of blood stasis, such as cerebral infarction, sequela of cerebrovascular accident, contused wounds. Lumbricus used to for the treatment of convulsions due to high fever, and for hemiplegia and hypertension. Scolopendra used to subdue the endogenous wind for the treatment of various kinds of tics, convulsions and tetanus, and it's character is strong because it will be effective Sthenia-Syndrome of apoplexy. Scorpio used to subdue the endogenous wind for the treatment of various kind of tics, convulsions, tetanus and sequela of cerebrovascular accident.

  • Objective: Graves' disease, the most common cause of primary hyperthyroidism, is a thyroid specific autoimmune disorder. When resistance to medication is shown in spite of long term therapy with anti-thyroid drugs, radioactive iodine therapy would be chosen in Western medicine. However, this therapy has often been reported to cause patients have hypothyroidism, thus requiring them to take levothyroxine for the rest of their lives. In this study, we evaluate the clinical efficacy and safety of Ahnjeonbaekho-tang (AJBHT) on patients with Graves' disease. Methods: We prescribed AJBHT for 3 months to two groups: patients who had been taking antithyroid drugs were administered AJBHT after discontinuing the antithyroid drugs ($Com-Tx{\rightarrow}Single-Tx$), and patients who had not been taking antithyroid drugs were started with AJBHT (Single-Tx) immediately. We evaluated the thyroidal function test (TFT) and visual analogue scale (VAS) for clinical symptoms for 3 months. Results: Serum T3 and fT4 were significantly decreased in both groups and remission rate of thyroidal hormones were significantly improved in the Single-Tx group. The clinical symptoms of palpitation, fatigue, and heat intolerance were significantly improved in both groups. In the safety analysis, all patients were in normal range of liver, renal function blood test and common blood count. Conclusion: From these results, we suggested that AJBHT was effective on TFT and clinical symptoms of Graves' disease. The study supports that AJBHT may be a useful agent for patients with Graves' disease who are resistant to antithyroid medication or radioactive iodine therapy, and for patients at first diagnosis.

  • Background : Epidural morphine provides excellent postoperative analgesia but is often associated with side effects such as nausea, vomiting and pruritus. It has been reported that mixed agonist-antagonist, nalbuphine can reverse side effects of epidural morphine without compromising analgesia. This study was designed to compare the efficacy of each intravenous dose of nalbuphine for treatment of side effects following epidural morphine. Methods : All patients received continuous infusion(2 ml/hr) of epidural morphine-local anesthetics mixture(morphine 4 mg, 1% mepivacaine 50 ml and 0.25% bupivacaine 50 ml) following a loading dose (morphine 2 mg with 1% mepivacaine 7 ml). Patients requesting treatment for nausea, vomiting and pruritus randomly received intravenous nalbuphine 0.05 mg/kg(Group 1; n=20), 0.1 mg/kg(Group 2; n=20) or 0.15 mg/kg(Group 3; n=20). The severity of nausea, vomiting, pruritus, degree of pain, sedation and vital sign were assessed prior to and 30 min after each dose. Results : The severity of nausea, vomiting and pruritus decreased significantly in all groups(p<0.01). Pain and sedation scores were unchanged in all groups. One patient received nalbuphine 0.15 mg/kg, complained of dizziness, agitation and palpitation. His blood pressure who had increased to 170/100 after first dose. Conclusions : This study suggests that intravenous nalbuphine is good for treatment of side effects following epidural morphine, and the dose of Group 1, 0.05 mg/kg, may be recommended as an optimal dose.

  • We examined clinical and reseach data about abdominal palpation conducted in japanese oriental medicine from the early stages of its medical history to the latest research, so as to introduce it into korean oriental medicine. That is to say, we studied clinical significance and suitable oriental medicine prescription about following : sweat and moistness of abdominal skin ; temperature of abdominal skin ; palpation and visible peristaltic movement ; meteorism ; tonus of the whole rectus abdominis muscle ; local tonus of parts rectus abdominis muscle ; palpitation of abdominal aorta ; resistance tender on pressure in epigastric region, subcostal region, umbilical region, lower abdomen, para-umbilical region, cecal region, sigmoid region and inguinal region ; sound of fluctuating liquid in the epigastric region. The result was that abdominal palpation was an immediately applicable method of clinical diagnosis in oriental medicine, and is being proven by western medicine methodology today. Therefore we suggest that korean oriental medicine would advance greatly if abdominal palpation were applied.
