A Treatment Experience of Focal Hyperhidrosis in Sacrococcygeal Region -A report of two cases- (천미골 국소 다한증 환자의 치료 경험 -증례 보고-)

  • Essential hyperhidrosis is a socially disabling and emotionally embarrassing condition. Localized excessive sweating in the sacrococcygeal region is a rare form of focal hyperhidrosis. Although numerous treatment options exist, including botulinum toxin and sympathetic neurolysis, there has been no generally accepted form of treatment. The following cases describe the successful reduction of excessive sweating in the sacrococcygeal region, without side effects, after local applications of topical glycopyrrolate and the use of fast drying clothes.

  • Myofascial pain (MFP) is one of the most common causes of persistent orofacial pain. Patients with chronic myofascial orofacial pain may present with diffuse heterotopic pain, complicating the correct diagnosis. Treatment of chronic MFP should focus on the elimination of aetiologic factors. This article describes two cases of chronic MFP of the masticatory muscles, whose symptoms were exacerbated after masseteric nerve neurectomy. The patients had suffered from irrelevant treatment which did not resolve the symptom. Their symptom was managed by conventional treatment protocol. These cases emphasize the importance of correct diagnosis and evidence-based approach.

The Effect of Acupuncture with Magnetic Ring on Pain Threshold Using Algometer (자기장을 통한 침자극이 압통 역치에 미치는 영향)

  • Like acupuncture, magnetic therapy has been known to yield effectiveness when it is applied to relieve from fatigue, musculoskelectal diseases, sore sites, rheumatic arthritis and chronic pain syndromes. However, combined application of acupuncture and magnet has not yet been studied. This study is designed to investigate effectiveness of acupuncture therapy when in the magnetic field for the pain relief. Magnetic field was made by magnetic ring ($7{\psi}{\times}2.3{\psi}{\times}1.5mm$). Twenty-one male swimmers with latent muscular pain at the GB21 area in the university course of physical education in Daegu were chosen and divided into three groups; 1) acupuncture treatment group (n=7), 2) acupuncture treatment with iron ring group (n=7), 3) acupuncture treatment with magnetic ring group (n=7). Manual Acupuncture was given to the GB21 point for 20 minutes. The degree of pressure pain threshold (PPT, $kg/cm^2$) in GB21 was measured with algometer. Before acupuncture treatment, the PPT values were $6.08{\pm}1.69$, $6.39{\pm}1.72$ and $5.59{\pm}1.11$ in acupuncture treatment group, acupuncture treatment with iron ring group, acupuncture treatment with magnetic ring group, respectively. After acupuncture treatment, the PPT values were $6.48{\pm}2.33$, $6.31{\pm}1.31$ and $6.59{\pm}1.80$, respectively. Pressure threshold was significantly increased in the acupuncture treatment with magnetic ring group compared to the other groups. Based on these results, acupuncture treatment with magnetic ring produced better effects on pain threshold, and these effects can be considered to be associated with the currents or voltages induced by the acupuncture needle and magnetic ring at present.

The Influence of Contract-Relax Technique of PNF and Taping treatment on Pain and Range of Motion of osteoarthritis of Knee (PNF의 수축-이완 기법과 테이핑 치료가 퇴행성 슬관절염의 통증과 관절가동범위에 미치는 영향)

  • Purpose : The purpose of this study was to compare the influence of Contract-Relax Technique of PNF(Proprioceptive Neuromuscular Facilitation) and taping treatment on the ROM(Range of Motion), pain and gait speed of osteoarthrits of knee. Methods : In this study K hall for the aged were selected. Twenty-six subjects with osteoarthrits participated in the experiment. The subjects were partitioned into two groups are 13person PNF group and 13person taping group. All subjects randomly assigned to the PNF treatment group and taping treatment group. Both group received treatment for 3times a week during 4 weeks. (PNF group received treatment for twenty times at once). Algometer and VAS(Visual Analogue Scale) were used to measure pain, the ROM was meaured using goniometer and gait speed was measured using a stopwatch (30m-test). All measurements of each subject were measured at pre-treatment and post-treatment. The data were analyzed with SPSS WIN 11.5 using t-test. Results : The PNF group had a significantly increase of knee extension ROM and significantly decrease of pain after treatment 4 weeks(P<0.05). The taping group had a significantly increase of knee extension ROM and gait speed and significantly decrease of pain after treatment 4 weeks(P<0.05). Conclusion : These data suggests that PNF is beneficial to increase knee extension ROM and to decrease pain. Taping is beneficial to increase knee extension ROM and gait speed and to decrease of pain.

Comprasion of Effectiveness of CT vs C-arm Guided Percutaneous Radiofrequency Lumbar Facet Rhizotomy

  • Background: Facet joint have been implicated as a source of chronic low back pain. Radiofrequency denervation has demonstrated the most solid evidence. To increase safety and efficacy of treatment, computed tomography (CT) guidance injection has been used in several disease. The purpose of this study was to evaluate the efficacy of CT-guided radiofrequency rhizotomy in the treatment of facet joint pain. Methods: A total of 40 patients were randomized to undergo radiofrequency facet joint denervation under CT guidance or C-arm guidance. All patients were examined visual analogue scale (VAS) score before treatment, 1 month, and 3 months after treatment. Results: The VAS in both groups showed significant improvement over the 1-month interval. No significant difference in the VAS score among the group was observed. Conclusions: In this study there was no significant difference between CT guidance lumbar rhizotomy and C-arm guidance lumbar rhizotomy. Therefore CT-guided radiofrequency denervation of the lumbar facet joint was a minimally invasive technique that appears effective.

  • Hemifacial spasm is a distressing condition characterized by involuntary, intermittent, unilateral twitching of, all or parts, of the muscles innervated by facial nerve. This ailment is most common among middle-aged women. Neither causative agent nor reliable treatment has been established because etiology of idiopathic hemifacial spasm has remained undefined to date. This report describes two cases of hemifacial spasm treatment by injection of Botulinum toxin. An injection of Botulinum toxin provided relief of hemi facial spasm. Botulinum toxin therapy is an effective and convenient treatment of hemifacialspasm.

  • Objectives : This study is to report the effectiveness of Korean medical treatment in case of pubic fracture with pelvic pain. Methods : Pubic fracture patient with pelvic pain was treated by Gamigunggwui-tang and acupuncture. This study was evaluated by VAS (visual analogue scale) and radiography. Results : After treatment, VAS of pain decreased and radiography image showed that fracture is healing. Conclusions : We could prove Korean medical treatment is effective for recovery of pubic fracture and pelvic pain.

  • Objective : The purpose of this study is to find out the effects of Jungja and Jungseon-Hyeol on the patients of neck pain. Methods : A retrospective study has been carried out for 30 cases of neck pain patients who visited Dongseo Oriental Medical Hospital from August 1, 2001 to september 30, 2001. We have treated them with acupuncture treatment of Jungja and Jungseon-Hyeol. Results : We have found out the excellent effects of Jungja and Jungseon-Hyeol on the patients of neck pain. Conclusion : The acupuncture treatment of Jungja and Jungseon-Hyeol is useful on the patients of neck pain.

Does transcutaneous electrical nerve stimulation affect pain, neuropathic pain, and sympathetic skin responses in the treatment of chronic low back pain? A randomized, placebo-controlled study

  • Background: The purpose of this study was to assess the effectiveness of transcutaneous electrical nerve stimulation (TENS) in chronic low back pain and neuropathic pain. Methods: Seventy-four patients aged 18-65 with chronic low back pain were included in the study. Baseline measurements were performed, and patients were randomized into three groups. The first group received burst TENS (bTENS), the second group conventional TENS (cTENS), and the third group placebo TENS (pTENS), all over 15 sessions. Patients' visual analogue scale (VAS) scores were evaluated before treatment (preT), immediately after treatment (postT), and in the third month after treatment (postT3). Douleur Neuropathique 4 Questions (DN4), the Modified Oswestry Low Back Pain Disability Questionnaire (MOS), the Beck Depression Inventory (BDI), and sympathetic skin response (SSR) values were also evaluated preT and postT3. Results: A statistically significant improvement was observed in mean VAS scores postT compared to preT in all three groups. Intergroup comparison revealed a significant difference between preT and postT values, that difference being assessed in favor of bTENS at multiple comparison analysis. Although significant improvement was determined in neuropathic pain DN4 scores measured at postT3 compared to preT in all groups, there was no significant difference between the groups. No statistically significant difference was also observed between the groups in terms of MOS, BDI, or SSR values at postT3 (P > 0.05). Conclusions: bTENS therapy in patients with low back pain is an effective and safe method that can be employed in short-term pain control.

The Effect of Active Neck Muscle Training in the Treatment of Chronic Neck Pain (만성 경통 환자의 치료에 있어서 능동적 경부 근육 훈련의 효과)

  • The purposes of this paper were to investigate the effect of active treatment compared with a conservative treatment and to provide the information for physical therapy in patients suffering from chronic neck pain. Forty female subjects who were diagnosed with cervical radiculopathy participated in this study and were divided into the conservative and active treatment groups. The active treatment group consisted of 20 patients who were treated with therapeutic modalities and active neck exercise program during the admission (15 days) and one month after discharge. The conservative treatment group consisted of 20 patients who were not received with active neck exercise program. The assessment tools were made using visual analogue scale(VAS), neck disability index(NDI) and modified Zung depression scale(MZDS). All subjects were measured three times: before the admission, at discharge, and at one month after discharge. Data were compared by groups using independent t-test. VAS, NDI and MZDS scores measured at admission and discharge were not significantly different between the groups. On the assessment performed one month after discharge, VAS and NDI scores were significantly lower for the active treatment group compared with those of the conservative treatment group(p<0.05), but MZDS score was not significantly different between the groups. In the comparison of two treatment methods for chronic neck pain, active treatment was more effective than conservative treatment. The findings of the study indicate that active treatment in chronic neck pain has a positive effect in relieving pain and restoring neck function.
