Factors that affect the muscular skeletal diseases of some industrial workers working in Changwon-si, Gyeongsangnam-do and the aspects of pain

  • In the present study, the general characteristics, job stress, working conditions, and aspects of pain of some industrial workers working in Changwon-si, Gyeongsangnam-do, were surveyed. In analyzing the relationship between job stress and the existence of pain, the variables "working speed" and "opportunities to develop abilities" were shown to have statistically significant relationships with the existence of pain ($p{\leq}.05$). Regarding the relationship between working conditions and the existence of pain, the variables" amount of work per hour," "amount of work per day," "number of parts handled during work," "work production per person," and" inconvenient postures or motions during work" were shown to have statistically significant relationships with the existence of pain($p{\leq}.05$). Regarding aspects of pain", within 1~3 years" was the most common answer to time of occurrence of symptoms, with a percentage of 27.6%; "appear almost always" was the most common answer to frequency of symptoms, with a percentage of 37.1%; "slight pain" was the most common answer to degree of pain, with a percentage of 50.5%; and "moderate" was the most common answer to encumbrance caused by pain to living and work, with a percentage of 41.2%. The aim of the present study was to determine the factors that affect pain due to musculoskeletal diseases in industrial workers and to define the aspects of pain in order to provide basic data for the preparation of measures to prevent musculoskeletal diseases. To control pain due to musculoskeletal diseases, factors that affect pain, as well as the aspects of pain, should be recognized early, and efforts should be made to supplement and improve systems for preventing recurrence.

무릎 전치환술 환자의 통증과 심리적 안녕에 관한 연구 (A Study of Pain and Psychological Well-Being in Total Knee Replacement Patients)

  • Purpose: The study investigated pain and falls as psychological factors in Total Knee Replacement patients and examined the relationship between these factors. Methods: Ninety-six total knee replacement patients aged 69.31±7.01 (male: 21, female: 75) participated in the study. Post-surgery pain was assessed on a visual analog scale, and fall risk scores were evaluated on fall fear and fall efficacy scales. Other psychological factors assessed included national anxiety and trail anxiety, fear prevention, Beck depression score, and psychological health measures. The data were analyzed using SPSS version 22.0. Results: A statistically significant correlation was found between pain before surgery and FES fall fear score before surgery (p < 0.05). A statistically significant correlation was found between pain after surgery and PWBS fall fear score after surgery (p < 0.05). A statistically significant correlation was found between state anxiety and PWBS, Trail anxiety (p < 0.05). A statistically significant correlation was also found between trail anxiety and BDI, PWBS (p < 0.05). State anxiety and fear avoidance before surgery were found to affect trail anxiety, and pain before surgery and fall fear score before surgery were found to affect FES. In addition, PWBS was found to affect BDI. Conclusion: The study confirms that increased pain in Total Knee Replacement patients can be a risk factor for falls. In addition, fall fears increase fear avoidance and cause psychological deterioration. This suggests that efforts should be made to reduce pain and to manage psychological factors.

Factors Influencing the Xerostomia Symptoms in the Patients with Temporomandibular Disorders

  • Purpose: This study investigated factors influencing the xerostomia symptoms in the patients with temporomandibular disorders (TMDs). Methods: Eighty-six participants over the age of twenty were randomly enrolled from patients with TMDs. The patients were diagnosed by Diagnostic Criteria-TMD (DC-TMD). Intensity of the pain and level of stress of the patients were recorded using TMD pain visual analogue scale (VAS) and stress VAS, respectively. The Symptom Checklist-90-Revised (SCL-90R), dry mouth symptom questionnaire, unstimulated salivary flow rate (USFR) and oral moisture were measured. Results: The patients who had above the mean of the TMD pain VAS had significantly higher scores on the stress VAS, subjective dry mouth symptoms and T-score of somatization. The patients who suffered from pain more than three months had significantly higher TMD pain VAS, subjective dry mouth symptoms and T-score of somatization. There were no significant differences in xerostomic symptoms according to the diagnosis of TMDs. Moreover, TMD pain VAS and the stress VAS did not correlated to USFR and oral moisture. Conclusions: The level of individuals' TMD pain and stress significantly affect their subjective dry mouth symptoms, however, it did not affect USFR and oral moisture.

선호 음악 감상의 음악치료가 말기암환자의 통증 정도에 따른 정서 및 스트레스에 미치는 효과 (The Effects of Music Therapy by Self-Selected Music Listening on Terminal Cancer Patients' Affect and Stress by Pain Level)

  • 목적: 이 연구는 선호 음악 감상이 말기암환자의 통증 정도에 따른 정서 및 스트레스에 미치는 효과를 파악하고자 함이다. 방법: 이 연구에 참여한 대상자는 경상북도 선린병원의 호스피스 완화의료 병동에 입원한 18세 이상의 말기암환자 20명이었으며, 대상자들에게 자신이 선호하는 음악을 선택하도록 하여 각 회기 당 30분씩 총 6회 제공하였으며, 실험 전후, 사전사후 조사를 하였다. 연구 도구로는 통증 자각 척도 중 숫자 통증 등급(Numerical rating scale, NRS), 정서상태 측정 설문지, 스트레스 설문지를 사용하였다. 통증인식 수준에 따른 집단 간 차이를 비교하기 위해 실험 전 자가 통증 척도에 따라 두 집단(통증 점수가 5점 이하인 집단과 통증 점수가 6점 이상인 집단)으로 나눈 후, 주 2회로 매 1회 당 30분씩 총 6회 실험하였다. 매 회기 사전사후 정서 변화를 측정하였고, 1회기 전과 6회 후 스트레스를 측정하였다. 정서 변화와 스트레스는 대응 2-표본 비모수 통계 처리하였고, 추가 분석으로 정서 변화 검사에서 1회기 전과 매회기 후를 반복 측정 분산 분석으로 처리하였다. 결과: 정서상태 변화를 위한 사전사후 검증에서 유의한 차이가 나타났고(P<0.001), 스트레스도 감소된 것으로 나타났다(P=0.001). 두 집단은 통증 인식 수준이 달랐지만 음악치료중재를 통해 모든 대상자들은 정서상태 변화와 스트레스 감소 효과를 나타냈다. 결론: 본 연구 결과들은 선호 음악 감상이 호스피스 완화의료 병동에 입원한 말기암환자들의 통증인식 정도 차이에도 불구하고 두 집단 모두에게 정서와 스트레스를 긍정적으로 전환하기 위한 효과적이고 적절한 중재임을 보여준다.

비 전기적 자극에 대한 자율신경계통과 골격근의 반응 (Autonomic and Skeletal Muscle Response to Non-electrical Cutaneous Stimulation)

  • Cutaneous stimulation has had a long history as a method of pain control. While there is general agreement that modern techniques such as electrical stimulation and massage often provide relief from acute pain and may in some cases significantly affect chronic pain, the mechanism by which these techniques affect pain remain unclear. Significant attention has been focused on the effects of stimulation on the autonomic nervous system(ANS) along with the increasing evidence of important ANS modulation of nociceptive activity throughout the pain pathway. However, inconsistent results on the presence and direction of ANS changes from cutaneous stimulation characterize the recent literature. The present study investigated a non-electrical cutaneous stimulation device, the Dermapoints massage roller, as well as an active placebo massage. The results indicate that the Dermapoints massage roller has both general effects associated with simple skin stimulation (such as increased skin temperature), as well as specific effects from increased stimulation by the tooth design of the roller. These specific effects include decreased muscle tension (at least for some muscle sites) and increased sympathetic activation. The results are consistent with a model of activation of Pacinian receptors as a possible mechanism for the antinociceptive properties of cutaneous stimulation.

The Effect of Spinal Decompression Therapy on the Pain and Posture in the Patients with Low back Pain

  • The purpose of this study identify that spinal decompression therapy effect on and pain, length Of leg distance(LLD), and muscle power and flexibility in patient with low back pain. The participants is 20 female and male with low back pain, and participant assign to decompression therapy group and control group at random. The decompression therapy apply to 20 minute 3 time for a week during 4 weeks. The Measurement items is pain, LLD, and muscle power, flexibility. The comparison between the before and after was Wilcoxon's U test, and 2 group after spinal decompression therapy application compared Mann-Whithney U test. Spinal decompression therapy reduced statistically significance the pain, LLD, and increased statistically significance the muscle power and flexibility increased the muscle power(p<.05). This study showed that spinal decompression therapy does affect pain, LLD, and muscle power and flexibility in patient with low back pain.

여성의 월경에 대한 태도에 영향을 미치는 요인에 관한 구조모형구축 (A Model of the Theoretical Structure of Factors Influencing College Womens도 Attitudes Toward Menstruation)

  • This study was designed to develop and test a model of the theoretical structure of factors influencing womens’ attitudes toward menstruation, specifically, premenstrual discomfort or pain, negative affect, disability and role acceptance. The conceptual framework was built through a review of the literature. For the purpose of testing the model, data were collected from April to June, 1992 with the use of Menstrual Distress Questionnaire, Menstrual Attitude Questionnaire, Sex Role Acceptance Scale. Data analysis was done with the SPSS PC/sup +/ for descriptive statistics and PC-LXSREL 7.13 for covariance structure. The study subjects consisted of 320 women college students in 2 three-year nursing junior colleges and 1 department of nursing of a college of medicine. The mean age of the subjects was 19, the mean age at monarch was 13.7, the mean menstrual cycle was 29.7 days and the mean menstrual duration was 5.3 days. The amount of menstrual discharge was moderate and the menstrual cycle variability was within 3 days. The score for pain, negative affect and disability were higher during the perimenstrual period than intermenstrual period. The fit of the hypothetical model with the six paths and the empirical data was high [X²(df=4)=. 57 (P=.966), GFI =.999, AGFI =.997, RMR=.008]. The hypotheses were supported on acceptable level of significance. 1) The higher the pain and negative affect, the stronger the disability. 2) The higher the pain, negative affect and disability, the more negative the attitudes toward menstruation. 3) The lower the women's role acceptance, the more negative the attitudes toward menstruation. The model was supported by the empirical data, and thus these results may help nurses and nursing scientists to understand and support these phenomena of womens’ health experience.

통증평가도구에 관한 고찰 (An Overview of Pain Measurements)

  • Objectives : The aim of this study is to introduce pain measurement tools that are considered suitable for clinical practice and research for Korean Medicine Doctors. Methods : We analysed some widely used and also useful pain measurement tools in terms of their methods and dimensions. Results : Diagrams, scales and questions are usually used to measure pain intensity, temporal pattern, treatment including exacerbating and/or relieving factors, pain location, pain interference, pain quality, pain affect, pain duration, pain beliefs and pain history. Specific pain measurements are also available for specific conditions such as Western Ontario and McMaster Universities Osteoarthritis Index, Oswestry Disability Index and Neck Disability Index. Conclusions : Faces Pain Rating Scale, numeric rating scale, visual analogue scale, McGill Pain Questionnaire and Brief Pain Inventory and commonly used pain measurements. Specific measurements should be considered depending on research topics.

암성 통증의 평가방법에 대한 고찰 (Investigation on Cancer Pain Assessment)

  • In general, cancer pain is neither fully recognized nor adequately treated. The inadequate pain control is due to failure of accurate assessment of cancer pain. This study was aimed to understand various characters of cancer pain and investigate available assessment scales which have been designed for, or frequently used with people with cancer pain. A total of 23 articles were selected from two different databases. The selected articles were analyzed according to three aspects of initial assessment, patient self-report and assessment of the outcomes of pain management. The characters of cancer pain is complex and includes physical, psychosocial, and spiritual dimension. Also, subjective pain can be classified into at least four specific factors, such as pain intensity, pain affect, pain relief, and pain quality. Based on various classification, the pain assessment scales can be divided into unidimensional or multidimensional. Among the more commonly used clinical tools are numeric rating scales, verbal rating scales, visual analog scales, and picture scales. Above all, in order to assess cancer pain objectively, the clinician must select appropriate assessment instruments which reflect pain definition and clinical purpose.

  • In chronic temporomandibular disorders (TMDs), constituent tissues such as muscles are sensitive to pain and psychological stress, which negatively affect the quality of life. In addition, since chronic TMDs is often accompanied by diseases such as psychological disorders and other chronic pain disorders, the diagnosis of those diseases and patient referrals are mandatory. The management of chronic pain in TMDs requires a multidisciplinary and holistic approach. Pharmacological therapy using cyclobenzaprine, serotonin and norepinephrine reuptake inhibitors, tricyclic antidepressants, progressive relaxation, and psychological approaches using cognitive behavioral therapy such as shifting negative thoughts about pain are all valid treatment options.
