Strategies to Improve Use of Medicines (의약품의 적정 사용을 위한 전략)

  • This study reviewed published studies on interventions used by hospitals, health insurance programs, or governments to improve use of medicines in foreign countries. Interventions to improve use of medicines are classified into two categories: 1) information strategies-dissemination of educational materials, group education, one-to-one educational outreach, drug utilization review, and feedback; 2) managerial strategies- formularies, prior authorization, and financial incentives. Dissemination of educational materials, which is a common intervention, was unsuccessful in changing physicians' prescribing behaviors. Problem-based small group education was more likely to change behaviors than didactic large group education. One-to-one educational outreach(academic detailing) was among the most effective strategies used to change prescribing behaviors. Prospective drug utilization review (DUR) program was more successful in improving use of medicines than retrospective DUR program. Feedback intervention has been reported to be ineffective to change behaviors. Formularies are frequently used to control medication use by most health insurance programs. Financial incentives provide physicians economic incentives according to appropriateness of prescribing. However, few published studies have assessed the efficacy of formularies or financial incentives. Prior authorization requires physicians to get authorization from health insurers before prescribing a certain group of drugs which is usually of high costs or risk. There is no magic bullet for quality use of medicines. Multifaceted interventions that help to predispose, enable, and reinforce desired behaviors are more likely to be successful.

An Analysis of University-based Continuing Care Retirement Communities' Services and Programs Management - With a Focus on Three Cases in the United States - (대학연계형 연속보호체계형 노인주거환경의 서비스 및 프로그램 운영 분석 연구 - 미국의 사례를 중심으로 -)

  • This study aimed to identify services and social and leasure programs that will be needed for helping senior residents to be aging healthfully and comfortably in the university-based continuing care retirement community (UBRC). This study conducted case study targeting three representative university-based CCRCs in the east area of United States such as Oak Hammock (OH) at Florida state, Kendal at Lexington (KL) and The Collonades (C) at Virginia state. There were three types of services in the UBRC, which were meal services, healthcare services, and personal services. Those services were different from the level of care such as independent living, assisted living, nursing home and memory care. Meal services and personal services were very similar among three UBRC, whereas healthcare services of OH were higher level of quality and diversity than the others. The social and leisure programs were categorized in-house programs, university outreach program and intergenerational programs between residents and students attending to related university. In-house programs and university outreach program were almost similar among three UBRC, whereas intergenerational programs of OH were well organized and actively operated than the others. This study results are expected to provides essential information for the development of Korean style services and programs of university-based CCRCs in the future.

International Health Project for Improving the Level of Mother and Child Health in Developing Countries: Focusing on KOICA CTS Cases in Vietnam (개발도상국 모자보건 수준 향상을 위한 국제보건사업: 베트남 KOICA CTS 사례를 중심으로)

  • We developed the world's first wireless ultrasound diagnostic device. For the smooth delivery of pregnant women, the WHO requires four prior examinations, and the use of ultrasound diagnostic devices is essential for this purpose. However, income levels and medical facilities in developing countries are falling short. We conducted KOICA's CTS program in Vietnam using a self-developed radio ultrasound diagnostic device. We supplied Sonon300C, a portable wireless ultrasound diagnostic device, to health centers and hospitals in Huong-Hoa district, Vietnam, and conducted an outreach program in an effort to further enhance business performance. As a result, the rate of ultrasound diagnostics in the region reached 100% and the percentage of trained graduates increased to 59%.

  • In this paper, the strategic plan for Honam Sea Grant College Program will be discussed. The main activity area of Hanam SGCP is research and development, education and public information, outreach. Honam SG theme areas and priorities of its activity area will be determined to enhance the sustainable use and conservation of marine and fishery resources for Honam region. In the education and public information, marine education program executed by MOMAF and relative institutions is introduced and suitable program direction will be suggested. The network composition between HSGCP and other institution in domestic or abroad is also tackled. An interchange of beneficial information is accomplished through the effective network with other organizations. Finally the result of survey to develop the pending problems of Honam area will be discussed.

Findings of Core Elements for Library Outreach Model of Social Workers in the Library (사회복지 전문가 참여형 도서관 아웃리치 모델 개발을 위한 핵심 요소 도출)

  • The libraries and social welfare centers carry out various programs for the information poor. The libraries have focused on accessing and utilizing information, while social welfare centers have centered on improving the quality of life. This study have investigated as follows. Firstly, this study examined the cooperation model between libraries and social welfare centers or librarians and social workers in public libraries in Korea and California, USA, and analyzed the international library policies and guidelines in literature review. Through the definition of terms and related concepts, we confirmed the range of the information poor in this study. Secondly, this study executed to interview librarians and social workers in Korea and USA about the way of cooperation for the information poor. These interviews have been understood the public library outreach programs in California and the program of the National Library for Children and Young Adults' Out of School Youth Reading Program (2018). Finally, this study suggested the core elements to be considered in developing a library outreach model for the information poor based on the implications derived from literature review and interviews.

  • Both domestic and overseas archives provide various forms of outreach services including exhibitions, educations, and publications for extended users. In addition, the publication program is portable and has excellent effects within its scope. Providing various and useful information about archives is significant because they can be important resources for historical research and provide good guidelines for archival users. This study investigated the current status of the publication programs, types of publications, the publishing organizations, and publication process managed by both the National Archives and Records Administration (NARA) and the National Archives of Korea (NAK). Based on the comparison of the publishing programs of the two archives, the study analyzed the limitations of the publication program of NAK and gave suggestions for its improvement.

  • This study is a part of three year project of “A Study on the Development, Evaluation & Outreach of Marriage Preparation Program”. The purpose of this study is to analyse the unmarried and married couples' attitude about marriage and family, marriage preparation status, their needs of marriage preparation program and ingredients of marital happiness to developing base line data of the marriage preparation program. The result showed that communication & conflict resolution, financial management, relationship characteristics are the major contents area to be considered to develop marriage preparation program. Critics and recommendation for need assessment and educational methods were added.

Nurses' Perception of Home Visit Nursing Care Services at the Outreach Community Center (찾아가는 동 주민센터 방문건강관리사업에 대한 방문간호사의 인식)

  • The purpose of this study is to promote the efficient operation of the Home visit nursing care services by identifying the perception and experience of visiting nurses at the community center. To this end, a methodological triangle study method was applied to visiting nurses. As a result of analysis the delivery system of the home visit nursing care services, the perception and participation experience of nurses, it was found that it is necessary to supplement the program standards such as the number of visiting target, health assesment, and information system for the efficiency of the visit. It is suggested that policy alternatives should be developed by sharing the major issues raised in this study.

Risk Perceptions of Noncommunicable Diseases among Cambodian Adults: A Cross-Sectional Study

  • Purpose: In Cambodia, noncommunicable diseases (NCDs) account for 64% of all deaths. A lack of risk perception of NCDs leads to poor measures of their prevention and management. This study aimed to investigate Cambodians' risk perceptions of NCDs based on the health belief model. Methods: A cross-sectional design was used, and using convenience sampling, participants included 200 Cambodians aged 40 years or older. A face-to-face administered structured questionnaire was used to assess demographic characteristics, health behaviors, and risk perceptions of NCDs. Results: Of the constructs of NCD risk perception, perceived severity (88.2%) and benefits (86.3%) were high, but relative to these, perceived cues to action (64.1%), barriers (63.5%), and self-efficacy (58.1%) were low. Conclusion: It is important to improve perceived self-efficacy in government health promotion, outreach, and improvement programs and to reduce perceived barriers through medical tests either by facility-based delivery or via outreach health services in Cambodia.
