The Study on the annual average direct cost incidence per cancer patient (암환자 1인당 연 평균 직접비용 발생에 대한 연구)

  • Among the participants of the 2012 Korea Medical Panel survey, 308 people who have used emergency, hospitalization and outpatient services for cancer have been selected. The average annual direct cost per cancer patient was analyzed by adding up the patient's medical expenses, industrial copayments, and non-salary costs. The average annual direct direct cost of cancer spent by cancer patients is about 129,093,792 per male, 158,100,612 won for men and 110,482,075 for women.For those with health insurance, the total direct cost per person from cancer was 183,095,125 won and the beneficiaries were 46,241,705 won. By household income, the average annual direct direct costs per person were 112,459,971 won per patient in the household income quartile, 137,910,890 won for patients in the second quartile, 149,556,570 won in the third quartile and 112,730,461 won, quartile 5, respectively.Was 142,926,331 won.

Psychological effects and risk perception after genetic counseling

  • Purpose: Demand for genetic counseling on cancer predisposition syndrome is increasing. We evaluated the psychological effect on counselees after genetic counseling at a clinic in a single center. Materials and Methods: We surveyed a total of 72 enrolled participants who visited a genetic counseling clinic at the Samsung Medical Center (SMC). The initial survey was conducted before the first genetic counseling session, and the second survey was conducted after the second genetic counseling session. A total of 43 participants completed both the initial and second surveys. Results: The initial survey of 72 participants indicated higher feelings of guilt in the group with religion, higher depression and anxiety in the group with a diagnosis of self, and higher anxiety in the group on self-referral to the genetic counseling clinic. In the completed survey of 43 participants, overall decreased depression was observed after the second genetic counseling session (P=0.013). Risk perception and anxiety decreased in the group diagnosed with benign variant/variant of uncertain significance (BV/VUS, 25/3) and increased in the group diagnosed with pathogenic variant (PV, 15). Risk perception and anxiety differed between the BV/VUS and PV groups (P<0.001 and P=0.03, respectively). Conclusion: The genetic counseling clinic at the SMC was effective in ameliorating the depression score. Assessment of survey results revealed different depression scores, feelings of guilt and anxiety, and different effects of the genetic counseling clinic, depending on the subgroups. Understanding the needs and psychological characteristics of different groups is necessary for improving genetic counseling services.

An Observational Study in Manipur State, India on Preventive Behavior Influenced by Social Media During the COVID-19 Pandemic Mediated by Cyberchondria and Information Overload

  • Objectives: The coronavirus disease 2019 (COVID-19) pandemic is a public health emergency posing unprecedented challenges for health authorities. Social media may serve as an effective platform to disseminate health-related information. This study aimed to assess the extent of social media use, its impact on preventive behavior, and negative health effects such as cyberchondria and information overload. Methods: A cross-sectional observational study was conducted between June 10, 2020 and August 9, 2020 among people visiting the outpatient department of the authors' institution, and participants were also recruited during field visits for an awareness drive. Questions were developed on preventive behavior, and the Short Cyberchondria Scale and instruments dealing with information overload and perceived vulnerability were used. Results: The study recruited 767 participants with a mean age of about 45 years. Most of the participants (>90%) engaged in preventive behaviors, which were influenced by the extent of information received through social media platforms (β=3.297; p<0.001) and awareness of infection when a family member tested positive (β=29.082; p<0.001) or a neighbor tested positive (β=27.964; p<0.001). The majority (63.0%) of individuals often searched for COVID-19 related news on social media platforms. The mean±standard deviation scores for cyberchondria and information overload were 9.09±4.05 and 8.69±2.56, respectively. Significant and moderately strong correlations were found between cyberchondria, information overload, and perceived vulnerability to COVID-19. Conclusions: This study provides evidence that the use of social media as an information- seeking platform altered preventive behavior. However, excessive and misleading information resulted in cyberchondria and information overload.

A Study on the Types of Public Hospitals in the Region by Cluster Analysis (군집분석을 통한 지역거점공공병원의 유형화)

  • This study selected indicators that can represent the characteristics of general hospitals, including local medical centers and Red Cross hospitals, which are representative public health institutions, and analyzed clusters. And we present to benchmark in each cluster. According to the analysis, 276 general hospitals were classified into 13 clusters, and local medical centers and Red Cross hospitals were classified into clusters between 1 and 7 of the total 13 clusters because of their small size. Local medical centers and Red Cross hospitals, selected as excellent hospitals in each cluster, showed significant differences in management performance despite similar regional environment and medical performance, and among them, surgical consultation and internal medical care rates, inpatient and outpatient rates. In order for local medical centers and Red Cross hospitals to play their role as secondary acute hospitals in the region, inpatient care services and surgical functions must be activated.

A study on the space and requirements of evaluation and certification criteria for psychiatric institutions (정신의료기관 평가와 인증 기준의 공간 및 요구사항 연구)

  • Purpose: The study aimed to extract the space mentioned in the criteria for evaluation and certification of psychiatric institutions in which the most specific standards for medical services of psychiatric institutions are prepared, and to organize and analyze the requirements. Method: The implied space is derived by analyzing the evaluation and certification criteria. The derived space is re-classified by departments of the hospital, and the requirements are organized and analyzed. Results: First, a total of 41 spaces were derived. The derived spaces can be considered as spaces to be treated as important in psychiatric institutions. Second, as a result of reclassifying the derived space by department, 10 spaced for the hospital as a whole, 10 for wards, 12 for central/outpatient departments, 5 for pharmaceutical departments, 2 for catering facilities, and 2 for facilities. Third, if organized by psychiatric institution, there is a total of 40 spaces for psychiatric hospitals according to certification criteria, and according to evaluation criteria, there is 38 spaces for psychiatric hospitals and departments and 19 spaces for the clinics. The difference between the certification and evaluation criteria of psychiatric hospitals is insignificant. In order to promote the qualitative improvement of psychiatric institutions, it is necessary to gradually strengthen from the certification criteria. Fourth, the requirements reflecting the characteristics of a psychiatric institution were very limited to emergency exits and treatment rooms. It is necessary to prepare the space requirements for other rooms in consideration of the behavior of the mentally ill. Implications: The derived spaces and requirements can serve as practical reference materials for practitioners preparing to receive certification and evaluation of psychiatric institutions. In addition, it has meaning as a basic data for estimating the level of space and requirements of psychiatric institutions that are currently required in Korea.

Relationship between the Level of Local Extinction and Total Medical Service Uses (지역소멸수준과 지역의 총 의료이용 간의 관계)

  • Background: The purpose of this study was to explore the relationship between the local extinction index and total medical service utilization. Methods: A fixed effects model in panel analysis was performed for the 228 administrative districts in Korea. The statistical yearbook on the usage of medical services by region and Korean Statistical Information Service data were used from 2010 to 2019 for analysis. Medical service utilization was represented by the number of visits day, the number of inpatient days, and medical charges. Control variables were selected by using an Anderson model. The local extinction index was calculated using resident registration population data. Results: Descriptive statistics showed that the number of areas at risk of extinction increased from 61 to 95 for the study years. In addition, the number of visits, the number of inpatient days, and medical charges all increased during the study years. After controlling for variables affecting medical service utilization and doing a panel fixed effects model, the result suggested that a one-step increase in the local extinction index was significantly associated with a 12.29% decrease in medical charges of inpatients, a 7.33% decrease in medical charge of outpatient, a 5.21% decrease in the number of inpatient day, and a 5.54% decrease in the number of visits day. Conclusion: This study showed that the higher the region's extinction risks, the higher the region's total medical service utilization. The results of this study suggested that there was a disparity in medical service utilization between areas at risk of extinction and areas not at risk of extinction, so measures should be taken to address this disparity.

Differences in the Direct Medical Expenditures among Physical Activity Levels of the Community Residents (지역주민의 신체활동 수준에 따른 직접 의료비 지출의 차이)

  • The purpose of this study was to examine the differences in the direct medical expenditure of community residents according to their physical activity level. The quota sampling method was used to select samples from a youth community center, senior center, community cultural center, and community recreational sports center in G city in Gyeongbuk-do. Of the 773 questionnaires distributed, 716 were completed. The questionnaires consisted of the demographic characteristics, regular exercise and physical activity levels, health status and medical costs. While the beneficial effect of regular exercise and physical activity on dental costs was not (significant), its effect on the costs of visiting hospital services was (significant). The mean differences in the total monthly direct medical expenditures on outpatient services and medicine for those with very high, high, moderate, low, and very low levels of physical activity were 7,500, 26,299, 47,517, 9,314, and 9,9978 won, respectively. The result of this study supported the findings of previous studies that regular exercise and physical activity. are not associated with the reduction of medical expenditure.

The Need for Rehabilitation Day Care Program Service of Stroke Survivor's Family (재가 뇌졸중 환자 가족의 주간재활간호 서비스 요구와 관련요인)

  • This study was carried out to find out the basic data required to plan and develop Rehabilitation Day Care Program for the stroke survivor's family in Korea. The subjects comprised of 92 stroke survivor's family who discharged from 4 hospitals in Seoul during the past 2 years. The data were collected from August 3, 1998 to September 18, 1998, through interviews with questionnaires about general characteristics, activities of daily living, depression and service need of rehabilitation day care program at the outpatient clinics by trained nursing graduates. Data were analyzed with descriptive analysis, Pearson's correlation analysis, and Stepwise multiple linear regression analysis using SPSS/WIN 10.0 program. The results obtained are as follows; 1. The mean score of the general need of rehabilitation day care program of stroke survivor's family was 3.10(range 1-4). The highest need among the service categories of the rehabilitation day card program was self-care and restorative activities category(3.30), and health services referral category, recreation category, psychosocial activities category in order. The needs of each category are as follows. In the health services referral category, the need for dental examination and medical examination were highest, followed by the need for physical therapy and occupational therapy. In the psychosocial activities category, the need for family counselling was highest. In the self-care and restorative activities category, the need for ROM exercise training was highest, followed by bowel training, and ambulation training. 2. The need of family for rehabilitation day care program service displayed a correlation with the level of education, ADL, and the level of depression, and a reverse correlation with age, illness intrusiveness, depression, knowledge, subject and object burden and relationship with stroke survivors. 3. The stepwise multiple linear regression analysis revealed following results. For the need for rehabilitation day care program service, 22.6% of the variance was initially explained by level of family's knowledge about caring method for stroke survivors, 8.8% was the level of subjective burden and 5.4% was relationship with stroke survivors. In conclusion, above characteristics should be considered to develop stroke survivors' rehabilitation day care program.

Among the need for the effect related to the utilization of day care program, the need for survivors' physical and psychological well-being was highest and was followed by the need for caregiver's physical and psychological wellbeing. Pearson's correlation analysis revealed following results ; 1. The need for rehabilitation day care program service displayed a correlation with the level of education, ADL, and the level of depression, and a reverse correlation with age. 2. The need for the effect related to the utilization of rehabilitation day program displayed a correlation with the level of education, ADL, and the level of depression. The stepwise multiple linear regression analysis revealed following results : 1. For the need for rehabilitation day care program service, 28.4% of the variance was initially explained by one variable, level of depression. The level of depression plus two variables, survivors' age and ADL, explained 34.2% of the variance in the need for rehabilitation day care program service. 2. For the need for the effect related to the utilization of rehabilitation day care program, 12.4% of the variance was initially explained by one variable, level of depression. The level of depression plus one variable, level of education, explained 20.4% of the variance in the need for the effect related to the utilization of rehabilitation day care program. In conclusion, above characteristics should be considered when we are planning to develop stroke survivors' rehabilitation day care program.

Measuring Utility for Menopausal Symptoms Based on Time Trade-Off and Visual Analogue Scale Methods (시간교환법과 시각화척도 방법을 이용한 폐경 후 에스트로겐 결핍증에 대한 효용 측정)

  • The present study was conducted to measure health-related utility for post-menopausal symptoms among Korean woman. A survey questionnaire was developed to measure utility based on visual analogue scale(VAS) and time trade-off method (TTO). From January 29 to February 3, 2007, a face-to-face interview was conducted with 274 women aged 45 to 60 to ask about their subjective utility for hypothetical health status of mild or severe post-menopausal symptoms. Among the participants, 160 were recruited from outpatient departments of 3 hospitals and 114 from 2 sports centers located in Seoul. For mild symptom, the average utility score of the respondents was $0.63{\pm}0.14$ based on VAS and $0.61{\pm}0.26$ based on ITO. For severe symptoms, the average utility was $0.39{\pm}0.16$(VAS) and $0.44{\pm}0.27$ (TTO). For severe symptom, a higher utility score was observed for TTO than for VAS, which is consistent with earlier studies and thus supports the validity of the utility measurement in this study. Overall, the severity of post-menopausal symptoms of the respondents themselves was not significantly associated with the utility score that the respondents answered for hypothetical health status. Multiple regression analysis results showed that the utility score was significantly higher among respondents with older age, higher education, higher family income, and from hospitals. It is expected that the utility score measured in this study will enhance the understanding of the quality of life of women after menopause and will be used to conduct cost-utility analysis of health care interventions to treat post-menopausal symptoms.
