일부 암 종의 수술량과 병원 내 사망률의 관계에서 구조적 복잡성의 조절효과 (Moderating Effect of Structural Complexity on the Relationship between Surgery Volume and in Hospital Mortality of Cancer Patients)

  • Background: The volume of surgery has been examined as a major source of variation in outcome after surgery. This study investigated the direct effect of surgery volume to in hospitals mortality and the moderating effect of structural complexity-the level of diversity and sophistication of technology a hospital applied in patient care-to the volume outcome relationship. Methods: Discharge summary data of 11,827 cancer patients who underwent surgery and were discharged during a month period in 2010 and 2011 were analyzed. The analytic model included the independent variables such as surgery volume of a hospital, structural complexity measured by the number of diagnosis a hospital examined, and their interaction term. This study used a hierarchical logistic regression model to test for an association between hospital complexity and mortality rates and to test for the moderating effect in the volume outcome relationship. Results: As structural complexity increased the probability of in-hospital mortality after cancer surgery reduced. The interaction term between surgery volume and structural complexity was also statistically significant. The interaction effect was the strongest among the patients group who had surgery in low volume hospitals. Conclusion: The structural complexity and volume of surgery should be considered simultaneously in studying volume outcome relationship and in developing policies that aim to reduce mortality after cancer surgery.

미용서비스의 과정적, 결과적 품질과 소비자 만족에 관한 연구 (Service Quality and Consumer Satisfaction in Beauty Service ; Relating to the Outcome Quality and the Process Quality)

  • The purposes of this study were 1) to examine the conceptual structure of outcome quality and process quality of beauty service, 2) to investigate the relationships among beauty service quality, consumer satisfaction, word-of-mouth reputation and perceived risk, and 3) to find out the effects of related variables on consumer satisfaction. The questionnaire was developed based on pretest and previous studies, and completed by 435 female consumers that experienced beauty service during about one month in Busan. First, factor analysis showed that the process quality of beauty services consisted of four factors, such as Employee Service, Physical Service, Time-saving Service, and Economic Service. And the outcome service quality consisted of two factors, such as Socio-psychological service, Functional service. Second, the higher word-of-mouth reputation and the lower perceived risk were the higher perceived beauty service quality and the more satisfied with service. Third, the process quality of beauty service, word-of-mouth reputation, socio-psychological service quality effected on consumer satisfaction. Especially, the employee service was very important on consumer satisfaction.

  • 최근의 연구개발 사업은 산업체, 연구소, 학계 등이 함께 참여하는 공동협력 연구의 형태를 많이 띠고 있다. 공동연구개발은 연구개발에 따르는 비용과 위험을 분담하고 지식을 보완함으로써 연구개발에 있어서 규모의 경제성과 경제적 효율성이 달성될 수 있다. 그러나 연구개발 주체간의 힘의 불균형과 정보의 비대칭성, 신뢰의 상실, 이로 인한 기회주의적 행태 등으로 인해 그 성과가 미미하거나 실패로 끝나는 경우가 빈번하게 발생하기도 한다. 본 연구는 일련의 연구개발 과정을 지식과 정보라는 서비스 상품이 생산되고 소비되는 과정으로 인식하여 서비스 마케팅적 관점에서 접근하고자 한다. 또한 연구개발에 참여하는 여러 주체들 사이의 행동적 특성을 관계 마케팅이라는 시각에서 접근하고자 한다. 조직간 관계를 설명하는 변수들이 많이 있지만 본 연구에서는 공정성과 품질, 신뢰, 그리고 충성도 간의 관계를 규명하여 보고자 한다. 공동연구를 수행하는 과정에서 공정성과 품질이 신뢰 및 충성도에 어떠한 영향을 미치는지를 검증하는 것이 본 연구의 목적이다. 연구에 참여하는 기관의 종사자들에게 설문지를 돌려 자료를 수집하였으며, 다양한 통계분석을 거쳐 설정된 가설과 연구모형에 대한 검증을 하였다. 분석결과 절차적 공정성과 분배적 공정성은 조직 신뢰와 구성원 신뢰에 긍정적인 영향을 미쳤다. 과정 품질과 결과 품질도 대체적으로 조직신뢰와 구성원 신뢰에 긍정적인 영향을 미쳤으나 결과 품질은 구성원 신뢰에 유의적인 영향을 미치지 못했다. 그러나 절차적 공정성과 분배적 공정성은 모두 충성도에 긍정적인 영향을 미치지 못했다. 결과 품질은 충성도에 유의한 영향을 미쳤지만 과정 품질은 그렇지 못했다. 또한 조직 신뢰와 구성원 신뢰는 충성도에 모두 유의한 영향을 미쳤다.

성인여성학습자의 학습성과에 대한 구조분석 (A Structural Analysis of Learner on Adult Female Learners' Learning Outcome)

  • 본 연구는 평생학습에 참여하는 성인여성학습자의 학습성과에 영향을 미치는 요인들을 분석하고 관련 변인들이 학습성과와 어떠한 구조적인 관계를 형성하는지 고찰하는데 그 목적이 있다. 분석대상은 D지역 평생교육기관에서 평생학습에 참여하거나 평생학습경험이 있는 성인여성학습자 578명을 연구대상으로 하였다. 본 연구에서는 학습성과를 분석하기 위한 관련변인으로 학습자특성, 평생교육기관여건, 학습몰입, 학습만족도를 잠재변인으로 설정하여 14개의 관측변수를 두어 구조적 인과모형을 구성하였다. 연구결과, 모형의 적합도 지수의 값은 ${\chi}^2=224.267$ (df=69, p<.001), RMSEA=.062, TLI=.943, RFI=.920, CFI=.957, IFI=.957, NFI=.939로서, 모두 0.9이상으로 적합도 지수가 모두 만족되었다. 학습자특성의 학습성과에 대한 표준화계수는 .218이고 p<.001,평생교육기관여건의 학습성과에 대한 표준화계수는 .301, p<.001, 학습몰입이 학습성과에 대한 표준화계수는 -.149, p=.541, 학습만족도는 표준화계수 .405, p<.001로 학습몰입을 제외하고 모두 학습성과에 직접 영향을 주는 것으로 나타났다. 연구를 통해, 학습자특성과 평생교육기관, 학습만족도가 학습성과에 유의한 직접효과를 가지고 있음을 확인하였다. 학습몰입은 학습성과에 직접 효과를 주지 못하고, 학습만족도를 매개로 간접효과를 주는 것으로 나타났다. 본 연구는 평생학습성과를 설명할 수 있는 종합모델을 구축하고, 관련 변인들 간의 구조적 인과관계를 밝힘으로써, 성인여성학습자의 학습성과에 대한 전체적 측면에서의 구조를 설명할 수 있는 이론적 논의를 시도하였다는 점과 학습성과와 관련된 변인들간의 구조적 인과관계를 실증적으로 탐색했다는 점에서 의의가 있다.

  • Most of the theories of collective bargaining outcomes start with a set of economic variables. The economic constraints, pressures, and incentives influence the bargaining power relationship between labor union and employer. In this paper, the critical macro and micro economic variables that need to be considered in analyzing the economic context of collective bargaining power relationship is outlined. The focus is on the role that economic forces play in shaping the results of bargaining, that is the outcome of negotiations. In this study, the elasticity of the demand for labor is introduced as one of the most important economic aspects that influence bargaining power. Unions will be most successful in increasing wages when they enjoy an inelastic demand for labor. If the demand for labor is not naturally inelastic, some institutional arrangement for "taking wages out of competition" must be sought. Inflation, business cycle, and income policy are influential in shaping both parties' goals and expectations as well. In addition to the analysis of the economic variables, the nature of power is diagonized with some introductory notions about its care and feeding before proceeding to the details of the above issues.

Comparison of nutrient intake, life style variables, and pregnancy outcomes by the depression degree of pregnant women

  • The aim of this study was to analyse effects that the degree of depression have on the life style variables, nutrient intake, iron indices and pregnancy outcome. Subjects were 114 pregnant women who were receiving prenatal care at a hospital in Seoul. We collected data for general characteristics and lifestyle variables from general survey instrument and for depression score from the questionnaire on depression. Dietary intakes of subjects were estimated by 24 hour dietary recall method. Also we analysed iron indices and pregnancy outcomes. We classified subjects by 10 point, which was the average depression score, into two groups [Low depression score group (LS) : High depression score group (HS)]. As to the intakes of total calcium, plant-calcium, plant-iron, potassium, total folate and dietary folate, LS group was far higher than HS group (P < 0.05, P < 0.05, P < 0.01, P < 0.001, P < 0.05, and P < 0.01, respectively). As to pre-pregnancy alcohol drinking, LS group had 41.9% in non-drinker, which was far higher than 28% in HS group in non-drinker (P < 0.05). As for drinking coffee during pre-pregnancy, pregnant women who don’ drink coffee in LS group took 43.6%, which was higher than 38% in HS group (P < 0.01). Regarding delivery type, the cesarean section in LS group (18%) was significantly lower than that in HS group (45%) (P < 0.01). Bivariate analysis showed that birth weight was significantly associated with the gestational age (P < 0.01). The pregnant women with higher depression score tended to have undesirable life habit, which might affect negative pregnancy outcomes. A better understanding of how depression and intake of nutrients work together to modulate behavior will be benefit nutritional research.

인적자원관리시스템에 대한 공정성인식이 결과행위에 미치는 영향 (A Study on the Effects Comes up to Outcome Behavior of the Perceived Justice on Human Resources Management System)

  • This study aims at developing and empirically testing a comprehensive model of organizational justice derived from the prior empirical studies on organizational justice. The study seeks to understand the antecedents to and the consequences are addressed. The first question is what characteristics of the HRM systems determine the levels of perceived distributive and procedural justice. Hypotheses are derived from the two prominent theoretical models, i.e., the self-interests model (SIM) and the group value model (GVM). Those factors include neutrality, consistency, accuracy. ethicality. credibility. and reciprocity in the processes of making HRM decisions. All nine factors suggested by the two models aye adopted for this study as major determinants of both distributive and procedural justice. The second question of this study is related to the moderating effects of individual differences on the relationship between the SIM and GVM variables and perceived distributive and procedural justice in organizational contexts. The final research question deals with the consequences of perceived organizational justice. specifically job - related altitudes and organizational commitment. Job-related attitudes in this study refer to job satisfaction, job motivation, and job involvement, i.e., the willingness to make contributions. identification with organization, and intent to stay. In examining the consequences of perceived organizational justice, the study focuses on the interaction effects of procedural and distributive justice on the above nine outcome variables. The results show that both SIM and GVM variables significantly influence distributive and procedural justice. However, hierarchical regression analyses reveal that the GVM variables except more influence has been focused on the consequences of perceived distributive justice with regard to outcomes provided by the organization.

전측두엽 절제술시 해마체 절제 범위와 경련 예후 (The Seizure Outcome and Extent of Hippocampal Resection in Anterior Temporal Lobectomy)

  • Objective : Little consensus exists concerning which temporal lobe structures need to be resected or how much resection should be done during hippocampal resection. The purpose of this study is to identify whether the extent of hippocampal resection influences seizure after anterior temporal lobectomy. Materials and Methods : The extent of hippocampal resection was assessed in 96 patients who underwent temporal lobectomy for medically intractable complex partial seizures originating from a unilateral seizure focus in the anteromesial temporal lobe. Patients who had structural lesion were excluded from the study. Postoperative magnetic resonance imaging in the coronal and saggital planes were used to quantify the extent of the hippocampal and lateral cortical resection. The patients were divided into two groups. Patients who underwent hippocampal resection to the level of the cerebral peduncle were included in the partial resection group, and those who had resection to the level of the colliculus were assigned to total resection group. Seizure outcomes were defined according to the Engel classification and compared between the two groups. Neuropsychologic outcomes in the selected cases were reviewed. Results : The over-all seizure-free outcome(Engel classification 1) was accomplished in 75%(72/96) of the patients (mean duration of follow-up, 36.8 months). The total hippocampectomy group had a statistically superior seizure outcome than the partial hippocampectomy group(87.3% versus 58.5% seizure-free, p-value=0.001). Also, younger patients had a more favorable outcome. Other variables such as laterality, the extent of lateral cortical resection, age at onset and gender were not significant. The pre- and postoperative memory functions were evaluated in 24 patients. A worse postoperative memory outcome was associated with partial hippocampectomy. However this was not acceptable due to a former bias. Conclusion : The result of this study conforms that aggressive hippocampectomy resulted in a better seizure outcome.

Sports injuries: a 5-year review of admissions at a major trauma center in the United Kingdom

  • Purpose: Sports offer several health benefits but are not free of injury risk. Activity dynamics vary across sports, impacting the injury profile and thereby influencing healthcare resource utilization and health outcomes. The purpose of this study was to investigate sports-related major trauma cases and compare differences across sports and activity groups. Methods: A retrospective case notes review of sports-related major traumas over a 5-year period was conducted. Demographic, hospital episode-related, and health outcome-related data were analyzed, and differences were compared across sports and activity groups. The Glasgow Outcome Scale (GOS) at discharge was used as the primary outcome measure and the length of hospital stay as the secondary outcome measure. Results: In total, 76% of cases had good recovery at discharge (GOS, 5), 19% had moderate disability (GOS, 4), and 5% had severe disability (GOS, 3). The mean length of hospital stay was 11.2 days (range, 1-121 days). The most severely injured body region was the limbs (29.1%) and vertebral/spinal injuries were most common (33%) in terms of location. A significant difference (P<0.05) existed in GOS across sports groups, with motor sports having the lowest GOS. However, no significant differences (P>0.05) were found in other health-outcome variables or injury patterns across sports or activity groups, although more competitive sports cases (67%) required admission than recreational sports cases (33%). Conclusions: Spinal injuries are the most frequent sports injuries, bear the worst health outcomes, and warrant better preventive measures. Head injuries previously dominated the worst outcomes; this change is likely due to better preventive and management modalities. Competitive sports had a higher injury frequency than recreational sports, but no difference in health outcomes or injury patterns.

공과대학 학생들이 지각한 맥락적 지지와 진로미결정의 관계에서 대처효능감과 결과기대의 매개효과: 남녀 차이를 중심으로 (Coping Self-Efficacy and Outcome Expectancy as Mediators of the Relation Between Perceived Contextual Support and Career Indecision Among Engineering Students: Focusing on Gender Differences)

  • 본 연구에서는 선행연구와 사회인지진로이론을 바탕으로 지각된 맥락적지지가 대처효능감과 결과기대를 매개로 진로미결정에 영향을 미치는 인과적 구조모형을 설정하고, 그 적합도 및 개별 변수의 영향력을 성차를 중심으로 살펴보았다. 연구를 위해 서울과 경기도 소재 4년제 공과 대학생 672명을 대상으로 지각된 맥락적 지지, 대처효능감, 결과기대, 진로미결정으로 구성된 설문을 실시하였다. 구조방정식모형을 통한 자료분석 결과, 지각된 맥락적 지지가 진로미결정에 미치는 영향을 대처효능감과 결과기대가 완전매개하는 모형이 최적의 모형인 것으로 나타났다. 맥락적 지지는 대처효능감과 결과기대에 정적인 영향을 미쳤고, 대처효능감과 결과기대는 진로미결정에 부적인 영향을 미치는 것으로 나타났다. 한편, 성별에 따라 구조모형의 적합도를 검증한 결과 두 집단 모두에서 완전매개모형이 최적의 모형인 것으로 나타났으나, 개별변수의 효과는 남녀에 따라 다른 것으로 나타났다. 남자대학생들의 경우 대처효능감이 진로미결정에 가장 큰 영향을 미쳤으나, 여자대학생들의 경우 대처효능감은 진로미결정에 직접적인 효과가 없는 대신 결과기대가 가장 큰 영향을 미치는 것으로 나타났다. 이러한 연구 결과를 바탕으로 여학생들의 진로미결정을 개선하기 위한 방안과 후속연구들에 대한 제언을 논의하였다.
