역분화 방골성 골육종 - 증례 보고 1 례 - (Dedifferentiated Parosteal Osteosarcoma - A case report -)

  • Parosteal osteosarcoma is characterized as a densely ossifying lesion, usually occurring on the surface near the metaphyses of a long bone. The histological pattern is a well- differentiated mature bone trabeculae with a hypocellular spindle-cell stroma. The cytological details are those of a low-grade malignant lesion. The natural history of this lesion is indolent local growth, late invasion of the underlying bone, and infrequently, distant metastasis. However, there is a significant risk of eventual dedifferentiation into a high-grade lesion. We report here-a case of parosteal osteosarcoma dedifferentiated into a high-grade lesion, which occurred in the left distal femur of a 40-years-old woman, and discuss the experience in detail.

Osteosarcoma of the jaws in Koreans: analysis of 26 cases

  • Objectives: In order to assess clinical behavior, response to treatment, and factors influencing prognosis of Korean patients with osteosarcoma of the jaws (OSJ). Materials and Methods: A retrospective study of clinical, and pathological records of 26 patients with OSJ treated at the Department of Oral and Maxillofacial Surgery in Yonsei University Dental Hospital from 1990 to March 2017. Results: Of 26 patients, there were 9 men (34.6%) and 17 women (65.4%). Twenty-one of 26 patients had osteosarcoma of the mandible, and 5 of 26 patients had osteosarcoma of the maxilla. The histopathology of OSJ is highly variable, ranging from chondroblastic type (6 out of 26), osteoblastic type (10 out of 26), fibroblastic type (2 out of 26), to the rare variants like mixed type, small cell osteosarcoma types and more. All patients underwent gross total excision and only a few patients underwent neoadjuvant chemotherapy. Postoperative chemotherapy was given to most of the patients as adjuvant treatment or in combination with radiotherapy. The overall survival rate was 73.1% with an overall 2-year survival rate of 83.3%. The overall 5-,10-,15-year survival rates in this study were 73.5%, 73.5%, 49%, respectively. Using Kaplan-Meier analysis with log rank tests, the size of tumor (T-stage), and resection margins were found to affect the survival rate significantly. The chemotherapy was not significantly associated with improved survival rate. Conclusion: Surgical resection with a clear margin is the most important factor in disease survival. The role of chemotherapy and radiotherapy in OSJ remains controversial, and deserves further studies.

대퇴골에 발생한 역분화성 방골성 골육종 - 증례보고 - (Dedifferentiated Parosteal Osteosarcoma of the Femur - A Case Report -)

  • 57세 여자 환자의 대퇴골 원위부에 발생한 방골성 골육종이 이차적 역분화를 일으킨 예를 치험하여 이를 보고하고자 한다. 57세 여자 환자가 1987년 방사선 소견상 대퇴골 방골성 골육종으로 진단되었고, 1992년 절제 생검을 시행한 결과 통상적인 방골성 골육종이었고, 이배수성의 DNA를 나타내었다. 1998년 환자는 병적 골절이 동반된 재발성 종양으로 재입원 하였다. 절제된 종양은 통상적인 방골성 골육종의 부위뿐만 아니라, 고등급의 방추성 세포 육종의 특정을 보이는 역분화 부위가 흔재하여 있었다. 이러한 역분화 부위는 유세포 측정을 이용한 DNA 검사상 이수배수성을 나타내었다. 본 증례를 통하여 모든 방골성 골육종이 저등급 병변은 아니고, 일부의 저등급 병변이 불충분한 절제 후에 고등급의 종양으로 역분화할 수 있음을 알 수 있다.

소목 추출물의 구강암 및 골육종 세포주에 대한 항암작용에 관한 연구 (STUDIES ON ANTICANCER EFFECTS OF EXTRACTS CAESALPINIA SAPPAN ON ORAL CARCINOMA AND OSTEOSARCOMA CELLS)

  • Anticancer effect of methanol extract of Caesalpinia sappan L. on oral carcinoma (KB) and osteosarcoma (HOS) cells were investigated in this study. In order to elucidate the anticancer mechanism of Caesalpinia sappan L, we analyzed telomerase inhibitory effect of the methanol extract of Caesalpinia sappan L. In addition we prepared 5 fraction samples according to its polarity differences and analyzed anticancer effects on oral carcinoma and osteosarcoma cells. Following results are obtained in this study. 1. 50% cell proliferation inhibitory value ($IC_{50}$) of the methanol extract of Caesalpinia sappan L. against oral carcinoma (KB) cells and osteosarcoma (HOS) cells were $9.0{\mu}g/ml$ and $10.9{\mu}g/ml$, respectively. 2. The methanol extract of Caesalpinia sappan L. showed inhibitory effect of telomerase which is required for cancer cell immortality. Therefore, it seems that the anticancer effect of methanol extract of Caesalpinia sappan is at least partially due to telomerase inhibitory effect. 3. Five fraction samples were prepared according to its polarity and 88.7% of ingredient of total methanol extract was transferred to ethylacetate fraction. Thin layer chromatography analysis showed that dichloromethane fraction contained ingredient with relatively high polarity and ethylacetate fraction contained similar ingredient found in total methanol extract. 4. Anticancer effect was observed in n-hexane, dichloromethane, and ethylacetate fractions. The highest anticancer effect was found in dichloromethane fraction which had $IC_{50}$ value of 4.4 and $>4.0{\mu}g/ml$ against oral carcinoma (KB) cells and osteosarcoma (HOS) cells, respectively.

Predictive Potential of Glutathione S-Transferase Polymorphisms for Prognosis of Osteosarcoma Patients on Chemotherapy

  • Objective: To evaluate the predictive value of glutathione S-transferase (GST) gene polymorphisms for the prognosis of osteosarcoma patients receiving chemotherapy. Methods: A total of 159 patients were included in our study between January 2005 and December 2007., with follow-up until January 2012. Genotyping was based upon the duplex polymerase-chain-reaction with the PCR-CTPP method. Results: At the time of diagnosis, 15.4% of the patients presented with metastasis, while 22.3% developed metastasis during follow-up. At the time of final analysis on January 2012, the median follow-up was 45.5 months. Patients with null GSTM1 and GSTT1 had a higher event free survival rate than non-null genotype, but no significant association was found between the two genotypes and prognosis of osteosarcoma. Individuals with GSTP1 Val/Val genotype tended to live shorter than with the IIe/IIe genotype, and we found a significantly higher risk of death from osteosarcoma (adjusted HR=2.35, 95% CI=1.13-4.85). Conclusion: The GSTP1 gene polymorphism may have an important role in the prognosis of osteosarcoma patients with chemotherapy. Further analyses with larger samples and more genes encoding metabolizing and DNA repair enzymes are warranted.

Quercetin Enhances Cisplatin Sensitivity of Human Osteosarcoma Cells by Modulating microRNA-217-KRAS Axis

  • Quercetin can suppress osteosarcoma cell growth and metastasis. However, other effects of quercetin on osteosarcoma remain largely unknown. This research aims to evaluate the effects of quercetin in combination with cisplatin as treatment for osteosarcoma and investigate its regulatory mechanism. Cell viability and apoptosis in 143B cell line were determined after treatment with quercetin and/or cisplatin. RT-PCR and Western blot analysis were performed to determine the RNA or protein expression levels. Moreover, transwell assay was used to evaluate metastasis. Furthermore, rescue experiments were performed to investigate the potential regulatory mechanism of the treatment. Results showed that quercetin with concentration that was equal to or greater than $10{\mu}M$ inhibited 143B proliferation, while $5{\mu}M$ quercetin enhanced the cisplatin sensitivity of 143B cells. Expression of miR-217 was upregulated after quercetin and/or cisplatin treatment, while its target KRAS was downregulated both at mRNA and protein levels. MiR-217 knockdown led to the loss of enhanced cisplatin sensitivity while miR-217 overexpression showed the opposite effects, indicating that quercetin regulated cisplatin sensitivity by modulating the miR-217-KRAS axis. In conclusion, $5{\mu}M$ quercetin enhanced the cisplatin sensitivity by modulating the miR-217-KRAS axis. This finding suggests that quercetin may be administered with cisplatin to improve the treatment for osteosarcoma.

Clinicopathologic Features and Prognosis of Osteosarcoma in Turkish Adults

  • Background: Osteosarcomas are the most common solid malignancies of bone. In the last two decades there have been no concrete developments in their systemic treatment. In this trial we aimed to present our osteosarcoma patient clinical and demographic outcomes. Materials and Methods: Patients treated and followed up for osteosarcoma in Ankara Numune Education and Research Hospital from 2002 to 2012 were reviewed retrospectively. Results: A total of 21 patients (15 male, 6 female) were diagnosed with osteosarcoma. The disease was located at extremities in 76% and in 14% was metastatic at the time of diagnosis. Median disease free survival (DFS) was 36 months in non-metastatic patients and median progression free survival (PFS) was 2 months in metastatic patients (p<0.0001). Median overall survival (OS) was 80 months and 4 months, respectively (p=0.012). There were no survival differences in terms of presentation with pathological fracture, tumor size, tumor grade, alkaline phosphatase and lactate dehydrogenase level and type of chemotherapy regimen. Conclusions: Tumor site and stages are the most important prognostic factors for osteosarcoma. Extremity primary tumors have beter survival rates than non-extremity tumors. As a result of the use of effective chemotherapy the long term survival rates have improved from 10-20% to 60-70% in the last decades but we need more active agents, especially for metastatic cases.

골 연령이 미성숙한 골육종 환자에서 사지 보존술 후의 하지부동에 대한 고찰 (A Clinical Study of Leg Length Discrepancy after a Limb-Sparing Operation in a Skeletally-Immature Osteosarcoma Patient)

  • A limb-sparing operation has a definitive role in the treatment of osteosarcoma in the lower extremity of skeletally-immature patients. After a limb-sparing operation, leg length discrepancy remains as a major disability that should be corrected. This study was designed to suggest methods of tumor resection and proper timing of leg length equalization in skeletally immature osteosarcoma patients. From September 1990 to January 1998, we reviewed eight osteosarcoma patients in an immature skeletal age. There were 4 males and 4 females, and their mean duration of follow-up was 50.37 months (range : 25 to 88 months). Mean skeletal age was 8 years (range : 8 months to 11 years). The patients were classified according to the methods of tumor resection ; intercalary resection in 1 case, transepiphyseal resection in 1, intra-articular resection in 5, and extra-articular resection in 1. The results were as follows ; 1. The leg lengthening was begun when a patient's leg length discrepancy reached 4-5cm. 2. The age of final lengthening with permanent reconstruction was 14 years in males and 12 years in females (about 2 years before skeletal maturity). 3. When reconstruction was performed with a temporary spacer, the site of lengthening Was in the soft tissue, not in bone, and then a permanant reconstruction was done. 4. Reconstruction with a biologic spacer to preserve the joint function was a reasonable method for equalization of leg length. In conclusion, the appropriate choice of reconstructive method and the age at which to correct the leg length discrepancy in a skeletally-immature osteosarcoma patients are important factors for maintaining leg length at full maturity.

제 2족지 근위 지골에 발생한 저악성도 골육종 - 증례 보고 - (Low Grade Osteosarcoma in Proximal Phalanx of the Second Toe - A Case Report-)

  • 족부에 발생하는 골육종은 전체 골육종의 0.2~2%로 매우 드물며 장관골의 골간단부에 발생하는 저악성도 골육종은 전체 골육종의 약 1.9% 정도를 차지하는 드문 종양이다. 본 교실에서는 제 2족지 근위 지골의 종물과 심한 동통으로 내원한 38세 여자 환자에서 발생한 저악성도 골육종 1례를 경험하였다. 환자는 단순 방사선 검사상 좌측 제 2족지 근위 지골 전체를 침범한 골용해성 병변이 관찰되었으며 피질골은 얇고 팽대된 소견을 보였다. 골주사 검사상 단순 방사선 사진에서 보인 것과 동일한 위치에 흡수 증가의 소견을 보여 양성 골종양으로 판단하고 종양 절제 후 늑골을 이용한 관절 재건술을 시행하였다. 수술 후 조직학적 소견상 저악성도 골육종으로 판정되었고 이차적으로 좌측 제 2족지의 근치적 광범위 절제술을 시행하였다. 제 2족지 근위 지골에 발생한 저악성도의 골육종은 매우 드문 종양으로 광범위 절제술 후 좋은 결과를 얻었기에 문헌 고찰과 함께 보고하는 바이다.

  • 하악골에서 low grade 골육종은 매우 드물게 보고되고 있으며 조직검사상 conventional osteosarcoma와는 달리 pleomorphism을 보이지 않기 때문에 양성종양으로 underdiagnosis되기 쉽다. 그러나 정확한 환자의 진단은 적절한 치료와 환자의 예후에 영향을 큰 영향을 미친다. 그러므로 조기에 정확한 진단을 위하여는 의심되는 여러부위에서 조직검사를 시행하고 임상소견과 방사선소견 등을 병리의사와 공유하여야 한다. 이에 저자들은 초기에 진단이 어려웠던 하악골에 발생된 low grade 골육종을 경험하였기에 문헌고찰과 함께 보고하는 바이다.
