Surgical Treatment of the Atlantoaxial Osteoarthritis

  • Atlantoaxial facet joint osteoarthritis is rare, often undiagnosed because it may be misdiagnosed as occipital neuralgia, or degenerative cervical spondylosis. Unilateral occipitocervical pain aggravated by head rotation is a specific symptom. Conservative treatment is usually effective. But when the patient complains of intractable neck pain localized to occipitocervical junction and unresponsive to medical therapy, surgical treatment should be considered. Though a few reports of surgically treated atlantoaxial osteoarthritis has been published, surgical outcome is favorable. A case of a surgically treated atlantoaxial osteoarthritis is presented with a review of the literatures.

Effects of Aromatherapy Massage on Pain, Physical Function, Sleep Disturbance and Depression in Elderly Women with Osteoarthritis (향 요법 마사지가 골관절염 여성 노인의 통증, 신체기능, 수면장애 및 우울에 미치는 효과)

  • Purpose: This study was to investigate the effects of aromatherapy massage on pain, physical function, sleep disturbance and depression in elderly women with osteoarthritis. Method: The study design was a quasi-experiment design with a non-equivalent control group pre and post-test. The study pool included elderly female patients older than 65years old suffering from osteoarthritis. Results: Pain in the experimental group after the aromatherapy massage was significantly decreased than that in the control group. The physical disability in the experimental group after the aromatherapy massage was significantly decreased than that in the control group. The sleep disturbance in the experimental group after the aromatherapy massage was significantly decreased than that in the control group. The depression in the experimental group after the aromatherapy massage was significantly decreased than that in the control group. Conclusion: Aromatherapy may be adopted as an effective nursing intervention for osteoarthritis.

Bilateral Osteoarthritis of Coxofemoral Joint in a Thoroughbred Horse

  • A 3-year-old black Thoroughbred colt weighing 500 kg was admitted to the Equine Hospital of Korea Racing Authority (KRA) due to the chronic gait abnormality of the hindlimbs. The history revealed that the patient had run and fallen onto the fence of the farm about at the age of 1 year old and the lameness had been worsening insidiously since the accident. The degree of lameness was 2/5 in accordance with the AAEP lameness grading system in both hindlimbs at hand walking and trot. The lameness was exacerbated at lunging clockwise and counterclockwise on both hindlimbs. The definitive diagnosis of the bilateral osteoarthritis of coxofemoral joint was established using radiography under general inhalation anesthesia. The prognosis was considered to be poor with low expectancy for racing and the colt was expelled from the racecourse eventually. The clinicians are required to consider the possibility of osteoarthritis of coxofemoral joint presented with chronic hindlimb lameness although it is rare. This is the first report on the bilateral osteoarthritis coxofemoral joint in Republic of Korea.

  • Treatment of osteoarthritis of the ankle joint is similar to that of any other large joint and includes conservative and surgical treatments. Surgical option in severe osteoarthritis is joint fusion or joint replacement, whereas conservative treatment is limited and includes mainly ankle supports, physical therapy, and oral medication. Hyaluronic acid was discovered in 1934 and now has been widely used in the knee and shoulder joints. We reviewed the articles about an intra-articular hyaluronic acid injection in the treatment of osteoarthritis of the ankle joint.

Supramalleolar Osteotomy in Patients with Varus Ankle Osteoarthritis (내반 퇴행성 족관절염에 대한 과상부 절골술)

  • This article has reviewed the alignment of the ankle and hindfoot in varus ankle osteoarthritis. The indication and surgical technique of the supramalleolar osteotomy has been reviewed. Alignment of tibial plafond and hindfoot is variable in early stages of ankle osteoarthritis. Supramalleolar osteotomy is indicated for the treatment of ankle osteoarthritis in patients with minimal talar tilt and neutral or varus heel alignment.

  • This paper reviews the indications and surgical technique of supramalleolar osteotomy as one of the treatments for moderate degenerative ankle osteoarthritis. Although it is technically demanding and requires extensive preoperative planning, supramalleolar osteotomy will be a good treatment option for moderate degenerative ankle osteoarthritis. The osteotomy is designed to shift the weight bearing axis to the lateral side of the ankle joint and unload the medial side of the joint. In our experience, a supramalleolar osteotomy is effective to the treatment of moderate ankle osteoarthritis with a small amount of preoperative talar tilt and varus or normal heel alignment.

Effects of Heat Therapy Given at a Community Heath Clinic on Sleep and Pain in Rural Elders Who Have Osteoarthritis (온열요법이 퇴행성관절염 노인의 통증과 수면에 미치는 효과)

  • Purpose: The purpose of this study was to identify how heat therapy influences pain and sleep in rural elders who have osteoarthritis (OA). Method: A non-equivalent control group pretest-posttest design was used. Participants were 61 elderly farmers and they were assigned to an experimental group (31) or control group (30). The participants were living within the district boundaries of a public health clinic in North Choong Chung Province. The research was conducted from April 1, to 28, 2006. Results: The results of this study demonstrate that heat therapy is effective in reducing pain and promoting sleep in rural elders who have osteoarthritis. Conclusion: Heat therapy reduced pain, and improved sleep in rural elders, with osteoarthritis.

A Clinical Case Study of Osteoarthritis of Knee Joint treated with Traditional Korean Medical Therapy and Core Exercise (한방 치료와 코어 운동을 병행한 퇴행성 슬관절염 환자 1례에 대한 증례보고)

  • Objectives : The objective of this study is to report the effect of Traditional Korean medical therapy and core exercise on osteoarthritis of knee joint by observing a clinical case study. Methods : Patient with a diagnosis of osteoarthritis of knee joint, was treated by acupuncture, herb medicine, physical theraphy and core exercise. Results : After treatment, Visual Analogue Scale(VAS), Korean version of the Western Ontario and McMaster Universities(KWOMAC), Pain Rating Score(PRS) were reduced from 5 to 2, from 25 to 16, from 55 to 27, and CENTAUR test results were improved. Conclusions : This study showed that Traditional Korean medical therapy and core exercise has meaningful effect on osteoarthritis of knee joint, and more researches should be followed.

Effect of Miniscalpel-Acupuncture for Hand Osteoarthritis: Case Report (도침 치료를 통한 손가락 관절염 치료: 증례 보고)

  • This case report was conducted to introduce the use of miniscalpel-acupuncture for the treatment of hand osteoarthritis. A-76-year old female patient who had pain on both hands due to osteoarthritis was treated with miniscalpel-acupuncture for 7 weeks. The main outcome were the Numeral Rating Scale (NRS) and the grip strength by dynamometer. After miniscalpel-acupuncture treatment, NRS was decreased and the strength of grasp was increased. The result of this study suggests that the treatment of miniscalpel-acupuncture may be effectively used to decrease pain and increase grip strength of patients who suffer from hand osteoarthritis. Further research is necessary by the reason of the limited sample size of this study.

The Clinical Study on the Effect of Sulguanjul-bang No.1 on Osteoarthritis of Knee Joint (퇴행성 슬관절염 환자에서 슬관절방(膝關節方) 1호(號)의 유효성 연구)

  • Objective : This study is designed to find out the effects of Sulguanjul-Bang No.1 on osteoarthritis in knee joint. Methods : 45 patients with symptomatic osteoarthritis of the knee joint were enrolled in a short term 8-week open clinical trial. The 45 patients were taken Sulguanjul-bang No.1 two times a day before meals for 8 week. After 8 week treatment period, the following parameters were analyzed ; Visual analogue Scale(VAS) as first effective parameter, Lysholm index score, patient's global assessment, passive movement range of the knee joint. Results : Sulguanjul-bang No.1 treatment led to significant improvement in the pain and symptoms of osteoarthritis as determined by all efficacy measures. After 8 weeks of therapy, there was significant improvement in VAS and Lysholm index; squat, sit down and up, crepitaion, swelling. The range of motion was improved in 6 of 9 cases. The improvement in patient's global assessment was seen in 84%. Conclusion : Sulguanjul-bang No.1 was very beneficial effect in pain relief and improvement functions on osteoarthritis.
