Chronic Recurrent Multifocal Osteomyelitis of The Shoulder - A Case Report - (견관절에 발생한 만성 재발성 다발성 골수염 - 1예 보고 -)

  • Purpose: We report a case of chronic recurrent multifocal osteomyelitis of the shoulder. Materials and Methods: A 16 year-old male who had suffered from chronic recurrent multifocal osteomyelitis of the shoulder was diagnosed by clinical features and biopsy and was treated with arthroscopic debridement and Naproxen. Results: Symptoms was subsided without relapse during 16 months follow up. Conclusion: Chronic recurrent multifocal osteomyelitis is rare disease and it can be misdiagnosis because of its rarity and non-specific clinical presentation. This is a report of a case of chronic recurrent multifocal osteomyelitis of the shoulder in 16 years man.

Comparison of Treatment Methods in Completely Displaced Supracondylar Fractures of Humerus in Children - Open reduction and pinning versus Closed reduction and pinning- (완전 전위된 소아 상완골 과상부 골절의 치료 방법 비교 -관혈 정복 후 내고정술과 도수 정복 후 경피적 핀 고정술의 결과 비교-)

  • Purpose: To evaluate and compare the results of children with displaced supracondylar fractures of humerus treated with open reduction and pinning with closed reduction and pinning. Materials and Methods: From March 2002, we treated 17 patients with completely displaced supracondylar fractures under the age of 7 with a minimal follow up period of 6 months. 9 patients were treated with closed reduction and pinning and 8 patients were treated with open reduction and pinning. The clinical results were evaluated with Flynn's criteria. Time to bone union was also analyzed for these two set of patients. Results: The group treated with open reduction had 6 excellent and 2 good results for change in carrying angles, 5 excellent, 2 good and 1 fair results for motion loss evaluated by Flynn's criteria. The average time to bony union for this group was 5.8 weeks. The group treated with closed reduction had 6 excellent, 2 good, and 1 fair results for change in carrying angles and 7 excellent, 1 good and 1 fair results for motion loss. The average time to bony uinon for this group was 3.7 weeks. Conclusion: In treating completely displaced supracondylar fractures of humerus in children, the closed reduction method and open reduction method all showed good results. But the operation time, duration of hospital stay and time to union was somewhat shorter for the patients treated with closed redution and pinning.

  • Synovial hemangioma of the knee joint has been reported as a rare cause of pain, limitation of motion and hemarthrosis, often seen as an internal derangement of the knee. A 39-year-old woman presented with spontaneous hemarthrosis of the left knee joint and physical exam revealed a small painful mass adjacent to the medial side of the patella. Magnetic resonance imaging revealed an intra-articular tumor. Arthroscopic excision was performed and the histology was characteristic of hemangioma. Through this case, we discuss the diagnosis and treatment options for the synovial hemangioma of the knee.

  • This paper presents a case of an early malignant transformation of untreated ulcers in a patient with diabetes. This case shows that Marjolin's ulcer can occur not only after chronic injury, but can also develop in the early stages after the onset. Hence, an early biopsy for diabetic foot ulcers that fail to heal with acute treatment can enable an earlier diagnosis and treatment without amputation, resulting in a better quality of life for the patient.

  • Benign and malignant tumors are found in the foot, although the incidence is low. The most common bone tumor in the foot is osteochondroma, which is thought to develop in bones that form through the process of enchondral ossification. In particularly, osteochondromas in the foot mostly occur in metatarsal bones and phalanges. It is seldom found in talus. It is usually confused with osteophyte or enthesis. We report an osteochondroma case confirmed by roentgenographical and pathological investigation, after removing the masses form bilateral tali of a 19-year-old male patient presented with bilateral ankle joint pain for three years.

Surgical Treatment of Type II Talar Neck Fractures (제 2형 거골 경부 골절의 수술적 치료)

  • Purpose: To evaluate the clinical results and determine appropriate methods of surgical treatment about type II talar neck fracture. Materials and Methods: Among nineteen patients who received surgical treatment for type II talar neck fracture from May 2000 to May 2005. Fourteen patients with a follow-up period of more than 1 year were divided into two groups. Six patients reduced by closed reduction (Group A) with screw fixation and eight patients reduced by open reduction with screw fixation. We analyzed preoperative, postoperative and follow-up simple radiographs and reviewed patient hospital records retrospectively. Clinical results were evaluated by Hawkins scoring system. We analyzed pain, limp, range of motion of ankle and subtalar joint. Results: Five patients (83.3%) in group A and seven patients (87.7%) in group B had excellent and good clinical results. There were no complications including avascular necrosis, delayed union, nonunion. Conclusion: Closed reduction with screw fixation of talar neck fracture shows correct reduction and satisfactory results. But because of short term period of follow-up, we need long term results.

  • Nail bed malignancies are rare entities. Most nail bed malignancies are squamous cell carcinoma (SCC)s. Less than 10% of subungual SCCs occur in the foot. Fifty percent occurred on the hallux and approximately 25% on both the fourth and fifth digits. The correct diagnosis is often delayed because nail bed malignancies are frequently mistaken for benign or infectious processes. SCC on extremities is hard to distinguish from the benign lesion like chronic ulcer, fistula caused by chronic osteomyelitis, and abscess fistula. Attention should especially be paid in diagnosing the subungual lesion because paronychia is a common disease. SCC is the most common carcinoma second to malignant melanoma as a soft tissue malignant tumor in the West, and it involves mainly the head, neck and upper extremities but rarely involves lower extremities, particularly the toes. The authors emphasize the importance of a biopsy for chronic nonhealing lesions by presenting this case.

  • Background: The purpose of this study was to search the present state of physiotherapy and the development direction of a curriculum for physiotherapy educational program, especially in field of orthopedic manual therapy, in Swaziland. Methods: The developed curriculum is as follows: 1. Disease and Health State in Swaziland. 2. Medical Human Resources State of Swaziland. 3. Physiotherapy Services State in Swaziland. 4. Higher Education State in Swaziland. 5. Development Direction of a Curriculum for Physiotherapy Educational Program in Swaziland. Conclusions: There is no a physiotherapist in Swaziland and Swaziland government which has not a physiotherapy education programme in any university of whole country. So we need to the development direction of a curriculum for physiotherapy educational program for the future of Swaziland, especially in field of orthopedic manual therapy. The reason is firstly, most of people are obesity in this country, so many people complained a lot of joint and muscle pain. And secondly, also there are many musculoskeletal disease patients now a day in Swaziland because of HIV/AIDS. Lastly, Swaziland country is in a poor and developing state, so physiotherapist it is hard to make a purchase and setting physiotherapy modalities apparatus at Swaziland country.

  • The typical radiologic findings of a small radiolucent nidus with surrounding sclerosis and night pain responding to aspirin make the diagnosis of osteoid osteoma not too difficult. We report a case of multicentric osteoid osteoma with unusual clinical and imaging features which were strongly suggestive of subacute sclerosing osteomyelitis.

Discomfort on Wearing Thigh-Length Compression Stockings in Orthopedic Surgical Patients (정형외과 수술환자의 대퇴길이 압박스타킹 착용 불편감)

  • Purpose: This study aimed to investigate the degree of discomfort and to identify factors related with discomfort in patients wearing thigh-length compression stockings after orthopedic surgery. Methods: The participants were 105 patients wearing thigh-length compression stockings after orthopedic surgery at two hospitals in B city. Data were collected by the scale of discomfort in wearing compression stockings from July 5 to October 20, 2011. Results: Mean score of discomfort on wearing compression stockings was significantly higher at post-op. day 3 than day 1 (t=-2.30, p=.004). Discomfort scores for 'itching skin' and 'tightened leg' items were higher than others. There was significant difference in discomfort at post-op. day 1 according to surgical regions. The total score of discomfort at post-op. day 3 was positively related with the body mass index (r=.20, p=.041). Conclusion: Wearing thigh-length compression stockings to prevent deep vein thrombosis may cause physical and psychological discomfort for orthopedic surgical patients. To reduce discomfort in wearing compression stockings, nurses need to intervene discomfort and problems related to the stockings post-operatively.
