A Clinical Study of Combined Eastern and Western Treatment of Acute Stage Stroke (급성기 뇌졸중 환자의 한양방 협진치료에 대한 임상적 고찰)

  • Object : This is a study of acute stage stroke patients designed for comparison of combined Eastern and Western medical treatments and Oriental treatments alone. Methods : 51 patients were diagnosed by Brain-CT and Brain-MRI scan as having suffered stroke. They had entered Dong-eui hospital within seven days of attack, and remained over seven days, all between November 2002 and August 2003. Patients were divided into two groups: a group treated with both Eastern and Western medical treatments, and a group treated only with Oriental medical treatments (but examinations were done and medications were given in hypertension, Diabetes Mellitus and so on). Results : The Eastern-Western medical treatment group showed significant changes in NIHSS. A rate of improvement figure of 24.94% was obtained for the Eastern-Western medical treatment group, and for the Oriental medical group a figure of 7.84% was obtained. Conclusion : The East-West medical treatment group had significant rate of development that measure for NIHSS by treated comparatively dependent patients. And oriental medical treatment group had significant rate of development that measure for NIHSS by treated comparatively independent patients. The NIHSS measure for the Eastern-Western medical treatment group shows a significant rate of improvement for relatively dependent patients, and the NIHSS measure for the Oriental medical treatment group shows a significant rate of development for relatively independent patients.

A Survey on Parent's Recognition and Utilization Patterns of Oriental Medical Care of Preschool Students in Seong-Dong district (성동구내 어린이집 원아 부모의 소아 한방치료에 대한 인식 및 이용에 관한 조사 연구)

  • Objectives: The purpose of this study is to examine the parental recognition and utilizing patterns of oriental medical care in children who live in Seong-Dong district. Methods 802 day-care center children's parents participated in the survey on recognition and utilizing patterns of oriental medical care, and 702 of the surveys were analyzed. Results: 1. 38.33% has received previous oriental medical treatments and most were received at the children's age under 12-24 months. Majority of the treatments took place in the clinical settings. By recommendation, some also received their treatment at the medical centers. One of the reasons why they receive oriental medical treatments was that they prefer to balance their treatment with the orental medical treatments along with the western's. 2. 53.85% of those who received oriental medical treatments were satisfied with their treatments. The effective treatment results were the major reason for the satisfaction. 3. 49.56% of those who had oriental medical treatments chose specific oriental medical facilities for the children's ailment. Most specific oriental medical facilities were local oriental medical clinics. Reputation fame were the main reason in choosing the clinics. 4. In 2009, 55.26% had received care in the specific oriental medical clinics for the diseases, such as respiratory symptoms, systemic symptoms, gastrointestinal symptoms, skin problems, neurological and psychiatric disease for infants, and respiratory symptoms, gastrointestinal symptoms, systemic symptoms, skin problems, neurological and psychiatric symptoms, urinary symptoms and musculoskeletal symptoms for toddlers. 5. The average duration of herbal treatment were 7-14 days, preferably 4-6 days, and preferred number of medical visits were once a week on Saturdays and preferred day for night cares were on Friday nights. Average treatment fee per treatment was less than 10,000 won which coincided with the preferred treatment fee per visit. 6. The very first medical institutions chosen by parents once their children are ill are the following; Western medical institutions, western medical clinics, pharmacies, oriental medical clinics, health centers, and lastly, oriental medical institutions. The major concerns in receiving oriental medical cares include pesticides in herbs and hazardous substances in heavy metals. Preferred form of oriental medicine was decoction, the negative feedback of oriental medicine was mostly due to the bitter taste of the oriental herbal decoctions. Preferred frequency of intake was twice a day with less than 10cc~20cc per single intake. Conclusions: Taken all together, we conclude that the growing recognition of Oriental Pediatric medicine is needed among parents.

Clinical Study of Oriental Medical Treatment on a Patient with Alcoholic Polyneuropathy (알코올성 다발성 신경병증의 한방치료 효과)

  • Alcoholic polyneuropathy is a disorder of the peripheral nervous system that interferes with sensory, motor, and autonomic nerve function. This study was to report the effect of Oriental medical treatment on a patient with alcoholic polyneuropathy. Conservative Oriental medical treatment was done to a man of 84 years who was diagnosed as a alcoholic polyneuropathy and suffered from both distal dominant leg weakness & symmetrical paresthesia and ataxic gait. The changes of symptoms, reflexes and sensory test were checked by Toronto Clinical Neuropathy Scoring System(TCNSS). Reflexes, motor and sensory impairments were improved after the Oriental medical treatment. I reported good effects of Oriental medical treatment on alcoholic polyneuropathy. Oriental medical treatment can be helpful to improve the symptoms of alcoholic polyneuropathy.

The Clinical Evaluation of East-West Medical Management for Central Poststroke Pain (뇌졸중후 중추성 통증에 대한 동서협진의 임상적 효율성 평가)

  • Background : Central poststroke pain(CPSP) can occur as a result of lesion or dysfunction of the brain from stroke and may cause many difficulty in the social activities and daily life. In this study, we evaluate the clinical effectiveness of east-west medical management for CPSP through VAS(visual analogue scale), infrared themography, MBI(Moderfied Barthel Index) and Rankin scale. Methods : We treated thirty patients with oriental medical treatment method and western & oriental medical treatment method. Each group has fifteen patients of the CPSP. We evaluated their pain(characterizes tingling and burning sensation, aching, hyperalgesia, and allodynia) through VAS(visual analog scale) pain score, the skin temperature of pain site by infrared thermography and assessed their mobility & rehabilitation ability through MBI(Moderfied Barthel Index), Rankin scale before and after pain treatment. Results : The skin temperature of pain site was lower than non-pain site. The difference of skin temperature improved from $0.65{\pm}0.45^{\circ}C$ to $0.39{\pm}0.25^{\circ}C$ after oriental medical treatment and $0.68{\pm}0.54^{\circ}C$ to $0.27{\pm}0.24^{\circ}C$ after western & oriental medical treatment VAS scores improved from $7.9{\pm}1.4$ to $4.7{\pm}1.6$ after oriental medical treatment and $8.1{\pm}1.3$ to $4.6{\pm}1.2$ after western & oriental medical treatment. MBI scores improved from $61.40{\pm}13.58$ to $85.00{\pm}13.85$ after oriental medical treatment and $52.26{\pm}13.52$ to $77.13{\pm}12.04$ after western & oriental medical treatment. And Rankin scale scores improved from $3.33{\pm}0.72$ to $2.46{\pm}0.74$ after oriental medical treatment and $3.60{\pm}0.82$ to $2.66{\pm}0.81$ after western & oriental medical treatment Conclusion : The difference of skin temperature and Rankin scale scores more significantly improved after western & oriental medical treatment than oriental medical treatment. According to the results, we thought east-west medical management is very useful treatment for CPSP and rehabilitation of the patients with stroke.

  • Objectives : The purpose of these 2 case studies are to report the effects of fatty liver children treated by oriental medical treatment. Methods : These patient was treated by herbal medicine, acupuncture, moxa, consultation. We observed the improvement of his disease by checking GOT, GPT, GGT, TG, and BMI. Results : By use of oriental medical treatment, we could make a considerable improvement of Child fatty liver. Conclusions These 2 case studies shows that oriental medical treatment can be an effective treatment for Child fatty liver. And the further study is needed with more cases and longer duration.

  • Objectives : The purpose of this study is to report a case of conversion disorder child treated by oriental medical treatment. Methods : We diagnosed the patient as Soeumin Woolgwang disease and treated him with herbal medicine, acupuncture, moxa, consultation. We observed the improvement of his disease by checking his pseudo-seizure, stool, headache and chest discomfort. Results : By use of oriental medical treatment, we could make a considerable improvement of pseudo-seizure and other symptoms(stool, headache and chest discomfort). Conclusions : This study shows that oriental medical treatment can be an effective treatment for conversion disorder. But the symtoms may recur, so we need continuous observation and management.

  • Objectives The purpose of the case study is to report the effects of stroke in childhood especially for Intracranial Hemorrhage sequela treated by oriental medical treatment. Methods The patient was treated by herbal medicine, acupuncture, moxa, and physical treatments. Results By using of oriental medical treatment, we could see significant improvement for Stroke symptoms. Conclusions This case study shows that oriental medical treatment can be an effective treatment for Stroke in Childhood. However, the further study with longer, and large clinical trials are needed for accuracy.

  • Objectives: The purpose of the case study is to report the effects of post traumatic stress disorder from sexual violence treated by an oriental medical treatment. Methods: The patient was treated by acupuncture at Baekhoi, Naegwan, Hapkok, Taeyang, Changmun and moxa at Baekhoi, Sin-Guel, Chungwan, Kwanwon. Results: The oriental medical treatment resulted considerable improvements for the patient. Conclusions: This study shows that the oriental medical treatment can be an effective treatment for sexual violence victims. However, the symptoms may recur, so we need to observe and manage the patients continuously.

  • The purpose of this study was to investigate the clinical application of relaxation training and oriental medical treatment to the SimDamHeoGup(心膽虛怯) type of ChilJungGuQiJung(七情九氣證) patient. In this case, a male patient, 31 years old, who lost his job two months ago, complained of the tension of body and anxiety. We treated the patient with relaxation training and oriental medical treatment such as herbal medicine, acupuncture treatment and aroma therapy. And we diagnosed his state SimDamHeoGup(心膽虛怯) type of ChilJungGuQiJung(七情九氣證), so we prescribed HaChuBoSimTang(夏朮補心場) for patient. In result, the patient’s tension of body and anxiety was improved, so he could recover his confidence slowly. Conclusionally, relaxation training and oriental medical treatment could be effective method to treat the patient of adjustment disorder with the tension of body and anxiety.

A Case Report of Oriental Medical Treatment for a Patient with Acute Paralytic Ileus (급성 마비성 장폐색 환자의 한방치험 증례보고)

  • Ileus is blockage of the intestine that prevents the contents of the intestine from passing to the lower bowel. In western medical treatment, surgery is mandatory for severe cases such as strangulation ileus. However more generally, conservative management is used for treatment. Examples of conservative management are decompression with tube, fluid treatment, enema and fasting cure. Until now, many oriental medical studies have been conducted on chronic ileus, but almost none on acute ileus. The results for the case in this report suggest that oriental medical treatment such as acupuncture, moxibustion, herbal medicine is effective in treating acute ileus. We think that further studies should be conducted on a combination therapy with other oriental medical treatment such as retention enema.
