灸((구)쑥뜸)시술행위의 개선을 위한 藥灸劑(약구제)의 활용 (Usage of Medicinal Moxibustion for the improvement of Moxibustion Treatment Procedure)

  • Objectives: To provide theoretical basis for the classification of medicinal moxibustion(藥灸劑) in Health Insurance Medical Benefit in Korea through investigating trend of judicial precedents on indirect moxibustion and usage of medicinal moxibustion in "Donguibogam". Methods: We analyzed statistical data of moxibustion from Health Insurance Review & Assessment Service and National Health Insurance Statistical Yearbook. We investigated major judicial precedents on indirect moxibustion to find out some trend and we searched the usages of medicinal moxibustion in "ZhenJiuDaCheng" and "Donguibogam". Results: According to recent judicial precedents, indirect moxibustion with equipment is no loner regarded as Korean Medical Procedure. In composition of 'Oriental Health Treatment', amount for acupuncture has gradually decreased instead, amount for moxibustion has increased steadily for 5 years. Medicinal moxibustion(藥灸劑) is often used as a form of indirect medicinal moxibustion with moxa in "Donguibogam". Argyi Folium, Moschus, Natrii Chloridium, Radix Preparata, Realgar and Olibanum are most frequently used for medicinal moxibustion in "Donguibogam". Medicinal moxibustions are composed of simple prescription or herb-pair or multiple prescription in "Donguibogam". Conclusions: In Health Insurance Medical Benefit in Korea, under the division of moxibustion, direct medicinal moxibustion and indirect medicinal moxibustion should be classified in addition.

한의사의 신의료기술 인식 실태 조사 (A Survey on the Actual State of Recognition of New Health Technology in Korean Medical Doctors)

  • Objectives : Nowadays, the assessment of new health technologies is gaining interest as an important issue for the safety of national health in the rapidly changing medical environment. The aim of this study is to understand how ignorant the korean medicine doctors are of new health technologies. Methods : The authors conducted a survey on the status of the ignorance of new health technologies in Korean medical doctors by e-mail. Results : Korean medical doctors' ignorance of new health technologies proved serious. The awareness of the law, however, was reached to some degree. The respondents answered that the present items of Korean Medicine listed in the medical care expenses by national health insurance system are too deficient to treat their patients effectively. Conclusions : It is strongly needed to try for more active registration of Korean medical new health technologies.

20년간(1987년~2007년) 한의원 경영수지의 경향 연구 (Trend analysis of financial balance of Korean medicine clinics during 20years(1987~2007))

  • Objectives : This study aimed to analyze the trend of financial balance of Korean medicine clinics during 20 years, and to provide basic information for adjusting the fee schedule of Korean medicine procedures in national health insurance(NHI). Methods : We collected 6 financial analysis reports for Korean medicine clinics from the Association of Korean Oriental Medicine(AKOM). The data on incomes, costs and EBIT(earning before interests and taxes) of subject clinics were abstracted, and their long-term trends were evaluated. Results : The proportion of insurance income in total income increased from 23% to 56% during 20 years. Among 5 treatment groups, 'non-insurance medication' took up 65~67% of total incomes in 1997, but its proportion decreased to 42.4% in 2007. 'medical procedure, physical therapy and others' increased from 12.4% in 1987 to 29.2% in 2007. The labor cost was major part of total cost and its proportion maintained from 52% to 54%. Cost of 'non-insurance medication' was on the decline from 41% in 1996 to 31.6% in 2007. EBIT were -17.4 million won in 1996, and 18.4 million won in 2007. Conclusions : Financial balance of Korean medicine clinics improved until early 2000, but it became worse in 2007. Though deficits from NHI procedures has been covered up by profit from 'non-insurance medication', health insurance became a major source of income during last two decades.

안면신경마비 환자의 최근 5년간 연도별 진료경향 분석 (Prevalence and Treatment Pattern of Korean Patients with Facial Palsy)

  • Objectives : While there are many studies about treatments of facial palsy, no study has been performed on general population of Korea, especially concerning about comparison between western medicine and oriental medicine. This study aimed to investigate magnitude of health visits and treatment patterns for Korean patients with facial palsy through the computerized database of Health Insurance Review and Assessment Service(HIRAS). Methods : According to the HIRAS database over 5 years' period from 2004 to 2008, the medical records of patients with facial palsy as a main diagnosis were extracted. Inclusion criteria of facial palsy are Bell's palsy(G510), Geniculate ganglionitis(G511), Melkersson's syndrome(G512), Other disorders of facial nerve(G518), Disorder of facial nerve, unspecified(G519) in western medicine. And Paralytic face(G016), Deviated eye and mouth(J01), The other facial palsy(J013) were included in oriental medicine. We compared the claim number of western medical care with that of oriental medicine treatment by year and month. Results : The total claim number of facial palsy was increasing on both western medicine and oriental medicine from 2004 to 2008. In western medicine, the claim number of Bell's palsy(G510) is the most. In oriental medicine the inpatients claim number of Deviated eye and mouth(J01) is the most, while outpatients claim number of the other facial palsy(J013) is the most. Conclusions : Medical database of HIRAS provided comprehensive and vast information on epidemiologic characteristics and treatment, which can be more reliable data to expect medical demand for facial palsy in condition that accurate diagnosis and standardized treatment is delivered in clinical settings.

만성피로의 체계화된 한의학적 치료법 연구의 필요성 (Need for Development of Oriental Medicine-derived Systemic Treatments against Chronic Fatigue-associated Symptoms)

  • Objectives: Fatigue is a prevalent symptom encompassing both acute and chronic manifestations. Most fatigue symptoms can be cured by taking a rest or removing underlying causes. However, chronic fatigue is frequently problematic due to its duration and effect on quality of life. There are no particularly effective therapies for chronic fatigue of unknown causes, and patients in Korea usually visit an Oriental clinic. This study aimed to analyze the current status of treatments and patients with chronic fatigue, and then map out of a strategy for development of generalized-treatments for chronic fatigue in Oriental Medicine. Methods: Clinical information related to chronic fatigue was selected from various different databases such as PubMed, KoreaMed, KStudy, DBPIA, OIM, and KOMS. Also, to understand current tendency of medical examination and treatment related with chronic fatigue, we requested Health Insurance Review & Assessment Service for clinical datum from 2003 to 2007. Results: The medical fees of National Health Insurance related with fatigue show an explosive year-on-year increase. On the other hand, it has been decreasing annually in the western medical fields. To take charge of clinical fields related with chronic fatigue by Oriental Medicine, we should make a unified diagnostic system. Then, we should also make standard evaluation tools and develop herbal drugs according to this unified diagnostic system. Conclusions: Fatigue-related symptoms will be a main target of Oriental medicine in the future. We expect that various studies related with chronic fatigue will be undertaken hereafter.

Traditional medicine in Taiwan: current status and future prospects

  • Oriental medicine is one of the main branches of traditional medicine left in the world. It includes acupuncture, acupressure, moxibustion, herbology, and qigong, all of which are still commonly used and officially recognized in some Asian countries, including Taiwan. In this article, the current educational, health insurance, and research and development status of traditional medicine in Taiwan will be introduced. The specific focus of the article is on efforts to integrate these ancient medical practices into modern medical science in our country.

의료기관 종류별 자동차보험 환자의 진료비 성향 분석 (Analysis of the propensity of medical expenses for auto insurance patients by type of medical institution)

  • 본 연구는 자동차보험 이용환자들의 효율적 관리방안을 모색하는데 필요한 기초정보를 제공하고자 보건의료빅데이터개방시스템에 등록된 2016년부터 2020년까지 5개년 자동차보험 진료비 심사자료를 대상으로 분석하였다. 분석결과, 자동차보험 입원진료비 내역 구성비율 1순위는 상급종합병원은 처치 및 수술료, 종합병원·병원·의원은 입원료, 한방의료기관(한방병원, 한의원)은 처치 및 수술료, 치과병원은 처치 및 수술료 이었다. 외래진료비 내역 구성비율 1순위는 양방의료기관(상급종합병원, 종합병원, 병원, 의원)은 진찰료, 한방의료기관(한방병원, 한의원)과 치과의료기관(치과병원, 치과의원)은 처치 및 수술료 이었다. 자동차보험 입원 건당 진료비에는 투약료, 마취료, 특수장비 비율이 영향요인이었으며, 외래 건당 진료비에는 이학요법료 비율이 영향요인으로 확인되었다.

  • After providing the purpose, scope and methods of present study in the first chapter. the second chapter discusses a theoretical overview on the social implication of medical expenditure and the medical insurance program for the aged population. In conclusion, to realize the reduction of aged population's medical expenditures, some possible plans are conceivable. Firstly, the payment level of medical insurance should be upgraded and the insurance coverage in oriental medicine treatment and the drug-store protection program need to be implemented. Secondly, the medical facilities and man-power have be expanded for the sake of reducing the social and geographical distances the aged population has to overcome to receive the medical benefits. Thirdly, the expansions of medical services for the home-stay aged are all the more required. Finally, a wide range of programs needs to be augmented to promote health for the aged population.

홍천군 데이터를 이용한 한의학 설문지의 문항간 신뢰성 분석에 관한 연구 (Reliability Study on YangSaeng Survey Using by Hong-Cheon Data)

  • Background : 'Yangsaeng' means health preservation concept in TKM(Traditional Korean Medicine). Health promotion using by traditional medicine was increased recently, but, has little measurement tools in evaluating traditional health care program. This study accomplished to analyze internal consistency reliability by Hong-Cheon survey data Method : Internal consistency reliability analysis within survey items Result : There was little reliability in mind and sexual life yangsaeng factor(Cronbach's alpha value 0.325, 0.273), higher reliability in lifestyle yangsaeng factor(0.718). And reliability was various within items according to age and health insurance type.

당뇨병성 말초 신경병증의 개괄과 한의학적 연구의 필요성 (Overview of Diabetic Peripheral Neuropathy and Need for Therapeutic Strategy using Traditional Korean Medicine)

  • Objective: This study aimed to analyze the current status of treatments and patients with diabetic peripheral neuropathy, and then map out of a strategy for development of generalized-treatments for diabetic peripheral neuropathy in Traditional Korean Medicine. Methods: We selected research materials from various databases such as PubMed, Google, KStudy, KoreanTK, OIM, KOMS and books. Also, to understand current tendencies of medical examination and treatment related with diabetic peripheral neuropathy, we requested Health Insurance Review and Assessment Service clinical data from 2003 to 2007. Results: It is reported that the incidence of diabetic peripheral neuropathy is increasing in an aging society. The medical fees of National Health Insurance related with diabetic peripheral neuropathy show a year-on-year increase. There are no particularly effective therapies for diabetic peripheral neuropathy in Western medicine, and in some papers, it was reported that treatment of diabetic peripheral neuropathy using Traditional Korean Medicine was effective. However, patients usually visit on Western medical center rather than seek Traditional Korean Medicine. To take charge of clinical fields related with diabetic peripheral neuropathy by Traditional Korean Medicine, we need more studies and experiments of diabetic peripheral neuropathy using Traditional Korean Medicine and should make a standardized protocol. Conclusion: Various studies related with diabetic peripheral neuropathy using Traditional Korean Medicine will have to be undertaken hereafter. We expect that Traditional Korean Medicine will play a vital role in treating of diabetic peripheral neuropathy.
