Breast implant-associated anaplastic large cell lymphoma (BIA-ALCL) is a recently described form of T-cell non-Hodgkin lymphoma now formally recognized by the World Health Organization classification of lymphoid neoplasms. The aim of this paper is to report the first case of BIA-ALCL diagnosed in a pregnant patient. It is well known that BIA-ALCL appears as an indolent lymphoma with a good prognosis when diag-nosed at early stages and clinical guidelines for its management have been clearly published. Nevertheless, they lack a standardized approach for BIA-ALCL during pregnancy. With limited experience in our case, treatment has been safely postponed after term without affecting patient's overall prognosis and without fetal complication. The fact that the disease was diagnosed at an early stage (stage I) undoubtedly influenced the course of treatment. A multidisciplinary approach weighing the risks and benefits of treatment is of paramount importance in order to ensure the best possible outcome for both the mother and her child and clinical update guidelines should be issued.
The characteristics of the records management systems of the Japanese colonial government can be summarized as follows. First, the Government General adopted a "decentralized retention" of public records. The Government General did not establish its own archives for central preservation of permanent public records. Colonial agencies established its own records office and the records office managed the records its agency created. Secret records and police records were exception. They were retained by the Secretary Office of the General Affairs Division and Police Division of the Chosun Government General respectively. Second, filing systems and retention periods of the public records followed the hierarchic structure of organization. In the headquarter of the government, records were filed by a "bureau-division-activity-file" classification system and a retention period of a file was given automatically by each unit the file belonged. A closed and cut-off file was retained and arranged according to its creating unit, creating year, and retention period. The filing system was easy to use once the filing system was established well, but to make it work effectively changes in activities and organizations should be on a reflected regular basis. It had an advantageous effect that permanent records could be preserved in a unified way throughout the organization. However, it is very critical to determine the permanent records in a professional way. Selection of the permanent records should be done professionally and in a historical perspective. Otherwise, the records retained as permanent records were not the records having an enduring value. And that was not done by the colonial government. Third, classification and scheduling of records were carried out by a creating division, rather than by the Records Office, mostly from the 1920s. Compilation of the records was also done by the creating agency. It implies that the records management lacked the professionalism. In conclusion, the records management system of the Chosun Government General wes nither modern nor user oriented. It managed the records for solely administrative purpose, i.e. effective colonial rule. The legacy of the colonial records system still exists in the public records system in Korean government. One should criticize the lack of will and efforts to modernize the public records system since the establishment of the Korean government while should reflect the historical origins of the records system in Korea.
From the 20th century, as international trade increases with the development of transportation and industries, a standard pictorial language to facilitate exchanges with others in different countries became necessary. However, it is not easy to have a full agreement on a standard pictorial language among different cultures. Therefore, the evaluation of standard pictograms has been the main discussion issue of the 150 since the organization's establishment in 1947. Although the 150 has a test method, 1509186, the participating countries do not follow the test because it does not reflect individual seeing mechanism for decoding pictogram which varies with his/her own experience. This study focuses on the theory of graphic symbol and seeing mechanism, and suggests what should be taken into account in the standardization process. In Chapter 2, the study investigates whether there be any cultural shock as we live in the net culture, and defines what reality is from a digital aesthetics view. What should be studied further is also presented, and a new method for revised international standard by means of analyzing seeing mechanism of decoding pictogram is suggested.
To show the analytic approach to website sue, this paper discusses style's function that is immediately related to users emotional and aesthetic satisfaction. And we make an analysis of several websites based on various visual factors. Style provides emotion, a shared experience, context that makes the message arts function immediately apparent, and interests. It is a principle design feature that parallels cognitive function. Visual factors of website interface could be extracted from various aspects which are organization level, medial level, and formative level. Based on the extracted visual factors, we analyze the style which convey emotional idea. We select several sites which are opposite side in the image map with son-hard and warm-cool axis, and then make an analysis of them in two points which are organization and formative factors and communication means for emotional effect. To understand the systematic approach to website style could be helpful for figuring out the design problems and bringing out many solutions. It could be base for concrete guideline for website page design.

This paper deals with the four aspects of the acquisition, construction and common use of modern and post-modern document DB in the NAK, NIKH, and AKS. First, The concept of Korean regional history includes provincial history, local history and regional history and as far as modern and post-modern history is concerned, the concept of regional history is on expansion. Second, National Archives of Korea has been systematically collecting and managing modern and post-modern regional history records in compliance to Public Institutes Records Management Law, enacted in 1999 and currently is in charge of handling public records of 373 central government administration offices, 514 regional government offices, Office of Education, universities and of other public agencies. National Institute of Korean History is working on a ten year project from 2004 to collect the scattered modern and post-modern regional history records and to classify them regionally and thematically. The Academy of Korean Studies has been collecting modern and post-modern regional history records and collection was initiated by Modern History Research. Those records that are collected from 1997 and 2005 are mainly from the liberation period. Third, characteristics of Central Archives Management System and Nara Records Portal System of NAK, Korean History Database System of NIKH and of The AKS' Korean Provincial Culture Electronic Encyclopedia are elaborated. Fourth, establishing 'Modern and Post-modern Regional History Records Council' as an affiliated organization of National Archives Management Committee is recommended, NAK leading the council and promoting further cooperation. In this section, an emphasis on allotted tasks of three institutes in order to achieve technology development for digitalized resource sharing, to improve on contents and to promote public and international use is placed as well.
Archival objects produced in relation to the activities of public institutions hold the inexpressible symbolism, historical value, and esthetical value as well in that the values of display and reuse of archives materials in the future are high as figures. The revised law of Archives Act in 2007 has provided a foundation for managing the archival objects in every public organization, by also including archival objects into the category of archives produced and registered in relation to their works. Although it could be seen as a reforming will to aim at the differential approach from the past to recognize the archival objects as records and make the managing will documented, it is still in an unprepared state to define the concepts on archival objects and to have methods to systematically manage and preserve them. Thus this paper has researched on the type classification of archival objects and their evaluation, as way of developing the methods for the management system for archival objects. On the premise that it should be preceded by clear definitions of concepts and distinctive understanding of their types first in order to build up the management system for archival objects, definition and type classification of the archival object have been attempted, based on the actual research materials and the current relevant laws. In addition, this study has analyzed the characteristics of selecting archival objects in order to search for which one they should obtain and manage among archival objects produced and reused in relation to the works of public institutions. On the basis of this, the plans for the national selection system and provision of selecting criteria for the subjects of permanent preservation have been suggested in this study.
This study is focused on clarifying the Joseon Dynasty Period's records management system mentioned on Kyeonggukdaejeon(經國大典). Through out investigations of the legislations of public records in 'creating stage', it was revealed. that is, records in creating stage had characters of original records and those was managed as the records themselves and circulated by sending and receiving, and finally those was kept and used in each agency. In this report, we present a case of successful treatment of a bowel fistula in the open abdomen by perforator flaps and an aponeurosis plug. A 70-year-old man underwent total gastrectomy and developed anastomotic leakage and dehiscence of the abdominal wound a week later. He was dependent upon extracorporeal membrane oxygenation, continuous hemodiafiltration, and a respirator. Bowel fluids contaminated the open abdomen. Two months after the gastric operation, a plastic surgery team, in consultation with general surgeons, performed perforator flaps on both sides and constructed, as it were, a bridge of skin sealing the orifice of the fistula. The aponeurosis of the external oblique muscle was elevated with the flap to be used as a plug. The perforators of the flaps were identified on preoperative and intraoperative ultrasonography. This modality allowed us to locate the perforators precisely and to evaluate the perforators by assessing their diameters and performing a waveform analysis. The contamination decreased dramatically afterwards. The bare areas were gradually covered by skin grafts. The fistula was closed completely 18 days after the perforator flap. An ultrasound-guided perforator flap with an aponeurosis plug can be an option for patients suffering from an open abdomen with a bowel fistula.
Clinical photography is an essential component of patient care in plastic surgery. The use of unsecured smartphone cameras, digital cameras, social media, instant messaging, and commercially available cloud-based storage devices threatens patients' data safety. This paper Identifies potential risks of clinical photography and heightens awareness of safe clinical photography. Specifically, we evaluated existing risk-mitigation strategies globally, comparing them to industry standards in similar settings, and formulated a framework for developing a risk-mitigation plan for avoiding data breaches by identifying the safest methods of picture taking, transfer to storage, retrieval, and use, both within and outside the organization. Since threats evolve constantly, the framework must evolve too. Based on a literature search of both PubMed and the web (via Google) with key phrases and child terms (for PubMed), the risks and consequences of data breaches in individual processes in clinical photography are identified. Current clinical-photography practices are described. Lastly, we evaluate current risk mitigation strategies for clinical photography by examining guidelines from professional organizations, governmental agencies, and non-healthcare industries. Combining lessons learned from the steps above into a comprehensive framework that could contribute to national/international guidelines on safe clinical photography, we provide recommendations for best practice guidelines. It is imperative that best practice guidelines for the simple, safe, and secure capture, transfer, storage, and retrieval of clinical photographs be co-developed through cooperative efforts between providers, hospital administrators, clinical informaticians, IT governance structures, and national professional organizations. This would significantly safeguard patient data security and provide the privacy that patients deserve and expect.
Myoepithelioma was recognized as a histological distinct entity by the World Health Organization (WHO) in 1991. Myoepithelial cells are believed to be of ectodermal origin. In salivary glands, the myoepithelial cells that surround the intercalated ducts are spindled, which is in contrast to the large stellate ones that envelop the acini. Myoepithelioma is a benign salivary gland tumor that consists entirely of myoepithelial cells. A 53-year-old man presented with a 1-year history of a painless mass originating from the right parotid gland. The mass grew rapidly reaching a size of approximately 6 cm. The patient had no facial paralysis. The authors performed right parotidectomy. Immunohistochemistry study of this tumor showed that it was positive for vimentin, positive for S-100, focally positive for pancytokeratin, and focally positive for p63 and that it had a Ki-67 labeling index (below 10%). Additionally, the tumor was negative for epithelial membrane antigen, negative for actin, negative for desmin, negative for CD34 and negative for anaplastic lymphoma kinase. The authors present a case of benign spindle cell myoepithelioma of the parotid gland in a 53-year-old man diagnosed after immunohistochemistry study, describing its importance, along with a brief review of the literature.
