Studies on Preparation of $^{99m}Tc$ Labelled 3-Iodo-2,4,6-trimethyl-iminodiacetic acid $(^{99m}Tc-IOTIDA)$ for Diagnosis of Hepatobiliary Disease (간.담도계 질환 진단용 $^{99m}Tc$ 표지 3-요오도-2,4,6-트리메틸 이미노 2초산$(^{99m}Tc-IOTIDA)$의 제조에 관한 연구)

  • For the development of $^{99m}Tc-labelled$ 3-iodo-2,4,6-trimethyl-iminodiacetic acid $(^{99m}Tc-IOTIDA)$, various experiments such as synthesis of IOTIDA, establishment of labelling conditions, determination of radiochemical purity, examination of stability, and organ distribution of rat were carried out. 1) IOTIDA was synthesized with a total yield of 42% from the starting material of 2,4-6-trimethylaniline via chloroacetylation, iodination, and condensation with iminodiacetic acid (IDA). 2) Freeze-dried instant labelling kits were prepared from aqueous solution $(pH\;5.8\sim6.0)$ so as to contain 40 mg IDA compound and 0.4 mg $SnCl_2$, per vial. Labelling of the contents of kit vials with $Na^{99m}TcO_4$, exhibited formation of two kinds of complex which was identified by ITLC-SA. After labelling, complex ( I ) was gradually converted to complex (II) with time. Labelling yield and radiochemical purity were above 99.5% based on the two complexes over-all. 3) $^{99m}Tc-IOTIDA$ maintained high radiochemical purity of above 99% until 6 hours after preparation at room temperature. Instant labelling kits stored at $4^{\circ}C$ for 6 month period also exhibited high labelling yield of above 99%. 4) Results obtained from animal experiments showed that most of the $^{99m}Tc-IOTIDA$ was rapidly excreted through hepatobiliary track into the intestines but with negligible renal excretion.

Calculation of Effective Half-life of Gamma Emission Radionuclide using Bio-kinetic Model (생체역동학 모델을 이용한 감마선 방출 핵종의 유효반감기 계산)

  • Patients administered radioisotope for medical purposes are regulated by each country to quarantine them until their body's radioactivity contents decrease below release criteria. To predict the quarantine period and provide it to medical staffs and patients, it is necessary to approach the assessment of the exposure dose of persons due to patients in a realistic manner. For this purpose, a whole-body effective half-life should be applied to the dose assessment equation instead of the physical half-life. In this study, we constructed a bio-kinetic model for each nuclear species based on the ICRP publication to obtain a whole-body effective half-life of 10 unsealed gamma-ray emitting nuclei from the notification of Nuclear Safety and Security Commission, and calculated the effective half-life mathematically by simulating the distribution of the radioisotope administered in the whole body as well as each organ scale. The whole-body effective half-life of $^{198}Au$, $^{67}Ga$, $^{123}I$, $^{111}In$, $^{186}Re$, $^{99m}Tc$, and $^{201}TI$ were 1,93, 2.57, 0.295, 2.805, 1.561, 0.245, and 2.397 days respectively. However, it was found to be undesirable to offer a single value of the effective half-life of $^{125}I$, $^{131}I$, and $^{169}Yb$ because the changes in the effective half-life show no linearity. A bio-kinetic model created for the internal exposure assessment has been shown to be possible to calculate the effective half-life of radioisotopes administered in the patient's body, but subsequent studies of radiolabeled compounds are required as well.

In silico evaluation of the acute occlusion effect of coronary artery on cardiac electrophysiology and the body surface potential map

  • Body surface potential map, an electric potential distribution on the body torso surface, enables us to infer the electrical activities of the heart. Therefore, observing electric potential projected to the torso surface can be highly useful for diagnosing heart diseases such as coronary occlusion. The BSPM for the heart of a patient show a higher level of sensitivity than 12-lead ECG. Relevant research has been mostly based on clinical statistics obtained from patients, and, therefore, a simulation for a variety of pathological phenomena of the heart is required. In this study, by using computer simulation, a body surface potential map was implemented according to various occlusion locations (distal, mid, proximal occlusion) in the left anterior descending coronary artery. Electrophysiological characteristics of the body surface during the ST segment period were observed and analyzed based on an ST isointegral map. We developed an integrated system that takes into account the cellular to organ levels, and performed simulation regarding the electrophysiological phenomena of the heart that occur during the first 5 minutes (stage 1) and 10 minutes (stage 2) after commencement of coronary occlusion. Subsequently, we calculated the bipolar angle and amplitude of the ST isointegral map, and observed the correlation between the relevant characteristics and the location of coronary occlusion. In the result, in the ventricle model during the stage 1, a wider area of ischemia led to counterclockwise rotation of the bipolar angle; and, during the stage 2, the amplitude increased when the ischemia area exceeded a certain size.

The Cellular Localization of GnRH and LHR in Aged Female Mice

  • Gonadotropin releasing hormone (GnRH) centrally plays a role in control of the hypothalamic-pituitary-gonadal axis-related hormone secretions in the reproductive neuroendocrine system. In addition, hormone receptors like luteinizing hormone receptor (LHR) are important element for hormones to take effect in target organ. However, ageing-dependent changes in terms of the distribution of GnRH neurons in the brain and LHR expression in the acyclic ovary have not been fully understood yet. Therefore, we comparatively investigated those ageing-dependent changes using young (1-5 months), middle (11-14 months) and old (21-27 months) aged female mice. Whereas a number of GnRH positive fibers and neurons with monopolar or bipolar morphology were abundantly observed in the brain of the young and middle aged mice, a few GnRH positive neurons with multiple dendrites were observed in the old aged mice. In addition, acyclic ovary without repeated development and degeneration of the follicles was shown in the old aged mice than others. LHR expression was localized in theca cells, granulosa cell, corpora lutea and atretic follicle in the ovaries from young and middle aged mice, in contrast, old aged mice had few positive LHR expression on the follicles due to acyclic ovary. However, the whole protein level of LHR was higher in the ovary of old aged mice than others. These results are expected to be used as an important basis on the relationship between GnRH and LHR in old aged animals as well as in further research for reproduction failure.

  • In the study, we endeavored to investigate the effect of phenylephrine, isoprenaline and prazosin on the tissue-specific vascular contractility and to determine the mechanism involved. There were few reports addressing the question whether thin or thick filament modulation is included in phenylephrine, isoprenaline and prazosin-induced regulation. We hypothesized that isoprenaline and prazosin play a role in tissue-dependent regulation of vascular contractility. Denuded arterial muscles of Sprague-Dawley male rats were suspended in organ baths and isometric tensions were transduced and recorded using isometric transducers and an automatic data acquisition system. Interestingly, sustained continuous contraction of thoracic and abdominal aorta. Furthermore, isoprenaline and prazosin together with phenylephrine inhibited transiently and persistently vasoconstriction of thoracic and abdominal aorta suggesting that additional mechanisms (e.g. decreased receptor density, chemical interaction, postreceptor signaling or distribution of agonists) might be included in the modulation of vascular contractility.

Differences in liver microRNA profiling in pigs with low and high feed efficiency

  • Feed cost is the main factor affecting the economic benefits of pig industry. Improving the feed efficiency (FE) can reduce the feed cost and improve the economic benefits of pig breeding enterprises. Liver is a complex metabolic organ which affects the distribution of nutrients and regulates the efficiency of energy conversion from nutrients to muscle or fat, thereby affecting feed efficiency. MicroRNAs (miRNAs) are small non-coding RNAs that can regulate feed efficiency through the modulation of gene expression at the post-transcriptional level. In this study, we analyzed miRNA profiling of liver tissues in High-FE and Low-FE pigs for the purpose of identifying key miRNAs related to feed efficiency. A total 212~221 annotated porcine miRNAs and 136~281 novel miRNAs were identified in the pig liver. Among them, 188 annotated miRNAs were co-expressed in High-FE and Low-FE pigs. The 14 miRNAs were significantly differentially expressed (DE) in the livers of high-FE pigs and low-FE pigs, of which 5 were downregulated and 9 were upregulated. Kyoto Encyclopedia of Genes and Genomes analysis of liver DE miRNAs in high-FE pigs and low-FE pigs indicated that the target genes of DE miRNAs were significantly enriched in insulin signaling pathway, Gonadotropin-releasing hormone signaling pathway, and mammalian target of rapamycin signaling pathway. To verify the reliability of sequencing results, 5 DE miRNAs were randomly selected for quantitative reverse transcription-polymerase chain reaction (qRT-PCR). The qRT-PCR results of miRNAs were confirmed to be consistent with sequencing data. DE miRNA data indicated that liver-specific miRNAs synergistically acted with mRNAs to improve feed efficiency. The liver miRNAs expression analysis revealed the metabolic pathways by which the liver miRNAs regulate pig feed efficiency.

Growth Response, Ecological Niche and Overlap between Quercus variabilis and Quercus dentata under Soil Moisture Gradient (토양수분구배에서 굴참나무와 떡갈나무의 생육반응, 생태 지위 및 중복역)

  • The Quercus variabilis and Quercus dentata, which are said to be relatively drought tolerant among the important genus Quercus that represent deciduous broad-leaved forests in Korea. These two species are widely distributed worldwide in Korea, Japan and China (northern, central, western and eastern subtropical regions). This study compared the ecological niche breadth and overlap according to growth response in 4 soil moisture gradients for the two species and tried to reveal degree of competition and ecological niche characteristics. The ecological niche breadth was 0.977±0.020 for Q. variabilis and 0.979±0.014 for Q. dentata, the latter being slightly wider. And they were similar in 5 traits (stem length, leaf lamina length, leaf width length, stem weight, leaf petiole weight), Q. variabilis was more dominant in 4 traits (leaves number, stem diameter, leaf area, leaf petiole length), and Q. dentata was more dominant in 7 traits (root length, shoot length, plant weight, root weight, shoot weight, leaf weight, leaf petiole weight). The ecological niche overlap for soil moisture between the two species overlapped most in plant structure-related traits and least in photosynthetic organ-related traits such as petiole length. As a result of principal component analysis, degree of competition between the two species for soil moisture was more severe when the soil moisture condition was low than high. Among the measured traits that affect the two-dimensional distribution, 8 traits (Leaves number, Shoot length, Stem length, Plant weight, Root weight, Shoot weight, Stem weight, Leaves weight) were correlated with the factor 1, and 2 traits (Leaf width length, Leaf petiole weight) were correlated with the factor 2 (r>0.5). These results show that the ecological response of the two species to soil moisture is not a few traits involved, but several traits are involved simultaneously.

Evaluation of Internal Dosimetry according to Various Radionuclides Conditions in Nuclear Medicine Myocardial Scan: Monte Carlo Simulation (심근 핵의학 검사에서 다양한 방사성핵종 조건에 따른 내부피폭선량 평가: 몬테카를로 시뮬레이션)

  • The myocardial nuclear medicine examination is widely performed to diagnose myocardium disease using various radionuclides. Although image quality according to radionuclides has improved, the radiation exposure for target organ as well as peripheral organs should be considered. Here, the aim of this study was to evaluate absorbed dose (Gy) for peripheral organs in myocardial nuclear medicine scan from myocardium according to various scan environments based on Monte Carlo simulation. The simulation environment was modeled 5 cases, which were considered by radionuclides, number of injections, and radiodosage. In addition, the each radionuclide simulation such as distribution fraction was considered by recommended standard protocol, and the mesh computational female phantom, which is provided by International Commission on Radiological Protection (ICRP) 145, was used using the particle and heavy ion transport code system (PHITS) version 3.33. Based on the results, the closer to the myocardium, the higher the absorbed dose values. In addition, application for dual injection for radionuclides leaded to high absorbed dose compared with single injection for radionuclide. Consequently, there is difference for absorbed dose according to radionuclides, number of injections, and radiodosage. To detect the accurate diseased area, acquisition for improved image quality is crucial process by injecting radionuclides, however, we need to consider absorbed dose both target and peripheral inner organs from radionuclides in terms radiation protection for patient.

Quality Assurance of Patients for Intensity Modulated Radiation Therapy (세기조절방사선치료(IMRT) 환자의 QA)

  • Purpose : To establish and verify the proper and the practical IMRT (Intensity--modulated radiation therapy) patient QA (Quality Assurance). Materials and Methods : An IMRT QA which consists of 3 steps and 16 items were designed and examined the validity of the program by applying to 9 patients, 12 IMRT cases of various sites. The three step OA program consists of RTP related QA, treatment information flow QA, and a treatment delivery QA procedure. The evaluation of organ constraints, the validity of the point dose, and the dose distribution are major issues in the RTP related QA procedure. The leaf sequence file generation, the evaluation of the MLC control file, the comparison of the dry run film, and the IMRT field simulate image were included in the treatment information flow procedure QA. The patient setup QA, the verification of the IMRT treatment fields to the patients, and the examination of the data in the Record & Verify system make up the treatment delivery QA procedure. Results : The point dose measurement results of 10 cases showed good agreement with the RTP calculation within $3\%$. One case showed more than a $3\%$ difference and the other case showed more than $5\%$, which was out side the tolerance level. We could not find any differences of more than 2 mm between the RTP leaf sequence and the dry run film. Film dosimetry and the dose distribution from the phantom plan showed the same tendency, but quantitative analysis was not possible because of the film dosimetry nature. No error had been found from the MLC control file and one mis-registration case was found before treatment. Conclusion : This study shows the usefulness and the necessity of the IMRT patient QA program. The whole procedure of this program should be peformed, especially by institutions that have just started to accumulate experience. But, the program is too complex and time consuming. Therefore, we propose practical and essential QA items for institutions in which the IMRT is performed as a routine procedure.

Study on dose comparison using X-Jaw split in VMAT treatment planning for left breast cancer including supraclavicular lymph nodes. (쇄골 상부 림프절을 포함하는 왼쪽 유방암의 VMAT 치료계획시 X-Jaw split을 이용한 선량비교에 관한 연구)

  • Purpose : The usability of X-Jaw split VMAT was evaluated by comparative analysis of the dose distribution between the treatment plan divided by X-Jaw and Full field VMAT treatment plan in left breast cancer treatment including supraclavicular lymph nodes. Materials and Methods : 10 patients with left breast cancer, including supraclavicular lymph nodes, were simulated using vacuum cushion, and 2 Full field Arc VMAT and 4 X-Jaw split Arc VMAT were planned The treatment plan was designed to include more than 95% of the Planning Target Volume (PTV) and to be minimally irradiated in the surrounding Organ at risk (OAR). Dose analysis of PTV and OAR was performed through dose volume histogram (DVH). Results : The Full field VMAT treatment plan and the X-Jaw split VMAT treatment plan of 10 patients were expressed as average values and compared. The difference between the two treatment plans was not large, with a Conformity index (CI) of 1.05±0.04, 1.04±0.03, and a Homogeneity index (HI) of 1.07±0.008, 1.07±0.009. For OAR, V5 in the left lung is 56.1±6.50%, 50.4±6.30%, and V20 is 20.0±4.15%, 13.52±3.61%. Compared to Full field VMAT, V5 decreased by 10.0% V20 by 32.6% in X-Jaw split VMAT. The V30 of the heart is 3.68±1.85%, 2.23±1.52%, and the Mean dose is 8.93±1.65 Gy, 7.67±1.52 Gy. In the X-Jaw split VMAT, V30 decreased by 39.3% and the Mean dose decreased by 14.1%. The left lung and heart, which are normal tissues, were found to have a statistical significance of that p-value is less than 0.05. Conclusion : In the case of left breast cancer treatment, which includes Supraclavicular lymph nodes with a large PTV volume and a length of X Jaw of 15 cm or more, the X-Jaw split VMAT shows improved dose distribution, which can reduce radiation dose of OAR such as lungs and heart, while maintaining similar PTV coverage with HI and CI equivalent to Full field VMAT. It is thought to be effective in reducing radiation complications.
