A COMPARATIVE STUDY ON THE GENIOGLOSSUS MUSCLE AND ORBICULARIS ORIS MUSCLE ACTIVITY IN THE ANTERIOR OPEN BITE AND NORMAL OCCLUSION (전치부 개방교합자와 정상교합자의 이설근 및 구륜근 활성도에 관한 비교 연구)

  • This study was undertaken to investigate the activities of genioglossus and orbicularis oris muscle between normal occlusion and anterior open bite group. 39 subjects without the experience of orthodontic treatment and T.M.disorder were selected for this study. 20 subjects were normal occlusion. 19 subjects were anterior open bite. The twenty items were measured from the cephalometric headplates, and EMG recording of the genioglossus, orbicularis oris muscle were taken at rest position, water swallowing, jaw opening, isometric tongue protrusion, maximum tongue protrusion. All data were analyzed and processed with the computer statistical method. The following results were obtained: 1. Except at rest position. the muscle activities of genioglossus muscle in anterior open bite were higher than in normal occlusion with singificatn difference. 2. Except druing water swallowing, the muscle activities of orbicularis oris muscle in anterior open bite were higher than in normal occlusion with significant difference. 3. During maximum tongue protrusion, the geniolossus muscle of anterior open bite subjects showed the highest muscle activity. 4. Anterior open bite showed closer interrelationship between facial morphology ad the genioglossus, orbiculars oris muscle activities than that of nomral occlusion with significatn difference.

An EMG Study of the Feature 'Tensity'

  • Previous studies reveal that in English there is no EMG evidence fur the feature tense-lax distinction. The technique of electro-myography(EMG) was used to see if the existing claim holds true, particularly in unstressed syllable. It was found that in unstressed syllable, the peak EMG amplitude from the orbicularis oris superior muscle was significantly greater in /p/ than in /b/, while in stressed syllable this difference was negligible. It was hypothesized that in stressed syllable, /p/ and /b/ may be differentiated by the EMG activities from a muscle other than the orbicularis oris superior muscle, e.g. the respiratory muscles relating to 'aspiration' or depressor anguli oris muscle. In Korean, there was a clear labial gestures for the feature tense-lax distinction. The phoneme-sensitive manifestation of stress and some possible reasons for the inter-speaker variability in the data and the variability within a given speaker were discussed.

AN ELECTROMYOGRAPHIC STUDY OF THE MUSCLE ACTIVITY IN ANGLE'S CLASS II DIV. 1 MALOCCLUSION AND NORMAL OCCLUSION (근전도를 이용한 Angle씨 II급 1류 부정교합자와 정상교합자의 근육활성도에 관한 연구)

  • This study was performed to investigate the relationships between the EMG activity of the masticatory muscles in Angle's Class II div. 1 malocclusion and normal occlusion. This study was ranged from age 12 to 14 year-old for 30 male subjects: 15 subjects were Angle's Class II div. 1 malocclusion, and 15 subjects were normal occlusion with acceptable profile. Their cephalometric measurement were analyzed, and the EMG recordings from the anterior temporal, posterior temporal, masseter, and orbicularis oris muscles were analyzed during rest position, mastication of peanuts, and swallowing. All data was recorded and statistically processed with the VAX-11/780 computer system. The results were as follows: 1. The activity of muscles at rest was highest in the posterior temporal muscle with normal occlusion, as well as in those with Class II div. 1 malocclusion, and the posterior temporal muscle activity of Class II div. 1 malocclusion was higher than that of normal occlusion. 2. During mastication, all muscle activities of Class II div. 1 malocclusion were recorded lower than those of normal occlusion, and the activity of the anterior temporal muscle was higher than that of the posterior temporal muscle in both types of occlusion. 3. As for the activity in orbicularis oris muscle, it was greater in opening phase than in closing phase during chewing cycle with both types of occlusion. 4. During swallowing, the activities of the anterior temporal, masseter, and orbicularis oris muscles of Class II div. 1 malocclusion were recorded lower than those of normal occlusion.

Effects of Thread Embedding Therapy on Complete Facial Palsy

  • Objectives : The aim of this study was to observe the effect of Embedding therapy on complete facial palsy after primary treatment. Methods : 11 patients with complete facial palsy were treated with Embedding therapy. It was performed once a day, every two weeks. 15~20 Embedding threads were used in each Embedding therapy treatment. The total number of Embedding therapy treatments was 4 or 8. Frontalis muscles (including the Yangbaek ($GB_{14}$)), Orbicularis oculi muscle, Levator labii superioris muscle, Zygomatic major muscle, Zygomatic minor muscle (including the Georyo ($ST_3$) and Jichang ($ST_4$)), Masseter muscle, Buccinator muscle (including the Hyeopgeo ($ST_6$)) and Orbicularis Oris muscle were selected. Yanagihara's score and House-Brackmann scale were compared for before and after treatment to evaluate the effect of Embedding therapy. Results : Yanagihara's score increased significantly (p=0.003). House-Brackmann Scale decreased significantly (p=0.005). Three patients were extremely satisfied, six patients were satisfied, and two patients responded neutrally in regards to Embedding therapy. Conclusions : Embedding therapy can be effective in improving symptoms of complete facial palsy.

Electrophysiologic Pattern of Facial Muscles in Bell's Palsy (얼굴마비에서 얼굴근육의 전기생리학적 양상)

  • Backgrounds: Electrodiagnostic tests have been developed to estimate the degree of facial nerve injury during the acute phase. Side-to-side amplitude comparison with the affected side expressed as a percentage of the nonaffected side has been one of the most valuable electrophysiologic methods of assessing facial nerve functioning. This study was designed to know whether there is any difference in the side-to-side comparison of amplitudes and terminal latencies of the compound muscle action potentials (CMAP) of the facial muscles in the patients with Bell's palsy. Methods: Electroneurographic recordings with surface electrodes on the frontalis, orbicularis oculi, nasalis, and orbicularis oris muscles were made within 2 weeks post-onset (mean, day 7) in 39 patients. Results: Of the 39 Bell's palsy patients, 38 patients (97.4%) recovered satisfactorily within 6 months. The amplitude of CMAP in all patients was not reduced to 10% or less of that of the contralateral healthy muscle. The correlation of amplitude change between four facial muscles was relatively strong, but the correlation of latency change was weak. When the electroneurographic values were compared in the four muscle groups, the general linear models procedure did not show any significant difference for CMAP amplitude and latency changes (p=0.62-0.63). Conclusions: This study did not show any significant clinical advantage of electroneurographic recordings in more than one facial muscle at the early stage of Bell's palsy.

Direct Brow Lift Combined with Suspension of the Orbicularis Oculi Muscle

  • Background Although the conventional direct brow lift operation provides a simple means of managing lateral brow ptosis, the scars produced have been unacceptable. However, using the modifications proposed here, scarring showed remarkable improvement. This article reviews our experiences with the presented technique, mainly with respect to postoperative scarring. Methods Measured amounts of supra-eyebrow skin and subcutaneous fat were excised en bloc in the conventional manner under 'hyper-hydrated' local infiltration anesthesia. The lower flap and the edge of the upper flap were undermined above the muscular plane, and the orbicularis oculi muscle was directly suture-plicated and suspended upward to the distal frontalis muscle. Skin closure was performed in a basic plastic surgical manner. Results From April 2007 to April 2012, a consecutive series of 60 patients underwent surgery using the above method. The average width of the excised skin was 8 mm (range, 5-15 mm) at the apex of the eyebrow. Preoperative complaints were resolved without occurrence of significant complications. The surgical scars showed remarkable improvement and were negligible in the majority of the cases. Conclusions The direct brow lift operation combined with plication/suspension of the superior and lateral portion of the orbicularis oculi muscle provides a simple, safe, and predictable means of correcting lateral brow ptosis. The scars were acceptable to all of the patients. For proper management of the frontalis tone, upper blepharoplasty and/or repair of eyelid levator function must be considered in addition to brow lift procedures.

A Study of Surface Electromyography Measurement of Facial Muscles in Normal Person (정상인의 안면부 운동 시 표면근전도 측정 연구)

  • Background or Objectives : The purpose of this study is to measure surface Electromyography(sEMG) of facial muscles in normal person and to find method for standardizing of sEMG's value. Methods : We measured 3points on face, frontalis muscle($GB_{14}$), zygomaticus muscle($SI_{18}$), orbicularis oris muscle($LI_{19}$) of 40 normal person by sEMG. 40 normal person consist with two groups, each 20 male, 20 female. Average age of subject was $26.50{\pm}4.79$. SEMG instrument QEMG-4 XL was used. After training exercise of facial muscles, sEMG's root mean square value was measured once. Results : 1. In whole experimental group, frontalis muscle's both side average was $78.36{\pm}40.87$, zygomaticus muscle's both side average was $84.70{\pm}49.81$, orbicularis oris's both side average was $104.83{\pm}38.81$. 2. Left side of Frontalis muscle, both side of zygomaticus muscle are high marked in male than female in statistically. 3. In whole experimental group, average of ratio comparing smaller value with bigger value in difference between left side and right side was $19.60{\pm}12.88$ %. 4. Average of asymmetry index(AI) was $11.46{\pm}8.36$ %. orbicularis oris muscle's average of AI had least difference was $8.95{\pm}7.50$ %. zygomaticus muscle's average of AI had most difference was $13.95{\pm}8.90$ %. Conclusions : The result of this study could provide useful information of field of sEMG is used in oriental medicine treatment of facial muscles. To assess efficacy of treatment in facial muscles, we need to standardize facial muscle's sEMG values by using AI, ratio comparing values and etc.

A Study of Surface Electromyography Measurement of Orbicularis oris motion in Healthy People (정상인의 구륜근 운동 시 표면근전도(SEMG) 측정연구)

  • Objectives : To find an acupuncture point where more exact Surface Electromyography(SEMG) measurement can be drown, through the study of measurement of orbicularis oris. Methods : Of healthy people from 19 to 40 years of age, who did not fall under exclusion criteria (22 males and 22 females), were selected as subjects, after relaxation for 10 minutes, and they were told how to pronounce 'O' and 'U' with their lips puckered. The SEMG figures were measured with attaching disposable electrode on acupuncture point of right-and-left Hwaryo(LI19) and 1 cun away from Seungjang(CV24) on both sides when the subjects pronounced 'O' and 'U'. Results : The average value was highest on left 1 cun away from Seungjang(CV24) in pronouncing 'O' and 'U', and the average SEMG value was higher in the order of right 1 cun away from Seungjang(CV24), right Hwaryo(LI19), left Hwaryo(LI19). Average of the lower orbicularis oris is statistically higher than that of the upper orbicularis oris, which has significant meaning. However, there was no significant difference when compared by pronunciation. Average of percentage mark of differences of right-and-left measured value of each pronunciation and each acupuncture point : Pronounced 'O' and the upper part of orbicularis oris: $16.76{\pm}11.29%$, pronounced 'O' and the lower part of orbicularis oris: $22.41{\pm}12.92%$, pronounced 'U' and the upper part of orbicularis oris: $17.10{\pm}9.89%$, pronounced 'U' and the lower part of orbicularis oris : $19.20{\pm}10.82%$. Conclusion : The difference of pronunciation will not affect the results in SEMG measurement. In addition, the average of the lower orbicularis oris is statistically and significantly higher than that of the upper orbicularis oris.

Enzyme-Histochemical Study of Philtral Orbicularis Oris Muscle Fiber Types in Korean Male Cadaver (한국인 성인남성 사체에서 시행한 인중 구륜근 섬유들의 효소-조직화학적 분석)

  • The orbicularis oris muscle (OOM) is a very important muscle that originate from the second branchial arch and is innervated by the facial nerve. The aim of this study was to elucidate distribution types of two muscle fibers that composing OOM by using enzyme-histochemical examinations and tried to make a basis for a clinical application. The fresh frozen tissues from the superior and inferior portions of the OOM were taken from post mortem 65-year-old Korean male adult. Total five different sagittal sections were used on the midline of the philtrum, the middle portion of lower lip, the mouth corner, and each midlateral side of upper and lower mouth. We used enzyme-histochemical staining such as Periodic Acid-Schiff (PAS), Succinic Dehydrogenase (SDHase), reduced Nicotinamide Adenine Dinucleotide-Tetrazolium Reductase (NADH-TR), Adenosine Triphosphatase (ATPase) in pH 9.4, 4.6 and 4.3, and Modified Gomori Trichrome. There were about 30.24 % type 1 muscle fiber and 65.40 % type 2 muscle fiber in the midline of the philtrum (p < 0.05). Enzyme-histochemical staining is very useful and innovative method to elucidate characteristics of muscle fibers. We expect that chiloplasty and reconstruction of the lip portions for cleft lip patients, based on these results, are better to recovery function and aesthetic. However, we have some problems as an intramuscular variability and the inter-individual variation etc. Therefore we have to make progress these studies continuously to overcome these problems.

Correction of Lowerlid Entropion by Using Skin-tarsal Fixation Method (피부검판 고정술을 이용한 하안검 내반증의 교정)

  • For its lowerlid entropion correction, nonsurgical eyelid suture or surgical eyelid suture can be employed. Different types of surgical eyelid suture such as the Hotz method and the Callahan method, are generally performed, but were not sufficient for correcting of entropion. In our hospital we performed modified skin-tarsal fixation on 15 patients, 5 male and 10 female from January 20, 1999 to December 16, 2002. We corrected lower eyelid entropion with the excision of the skin and orbicularis oculi muscle and buried suture of tarsal plate and dermis which is a variant of the Callahan method. All the patients were satisfied with the operation results, and there was no case of recurrence of the lower eyelid pricking the cornea. During the 6 months period after the operation, the lower eyelids slightly turned inward but there was no further progression. The correction was performed excessively, therefore there was no case of recurrence after the operation and the results were satisfactory. There developed a dimple at the suture margin caused by skin contraction but it usually disappeared within 2 months after the operation. To avoid recurrence and to achieve satisfactory results, the orbicularis oculi muscle must be totally resected at the ciliary margin. We resected ciliary as well as pretarsal portion of orbicularis oculi muscle. Also, since most of the recurrences are originated from the medial side, the medial portion of skin flap must be tightly fixed to avoid recurrence and the upper part of the dermis should be fixed tightly to the lower border of the tarsal plate.
