Simple Qualitative Sensory Assessment of Patients with Orofacial Sensory Dysfunction

  • Purpose: Oral and facial sensation is affected by various factors, including trauma and disease. This study assessed the clinical profile of patients diagnosed with sensory dysfunction and investigated their sensory perception using simple qualitative sensory tests. Methods: Based on a retrospective review of the medical records, we analyzed a total of 68 trigeminal nerve branches associated with sensory dysfunction in 52 subjects. We analyzed the frequency and etiology of sensory dysfunction, and the frequency of different types of sensory perception in response to qualitative sensory testing using tactile and pin-prick stimuli. Results: The inferior alveolar nerve branch was the most frequently involved in sensory dysfunction (88.5%). Third molar extraction (36.5%) and implant surgery (36.5%) were the most frequent etiological factors associated with sensory dysfunction. Hypoesthesia was the most frequent sensory response to tactile stimuli (60.3%). Pin-prick stimuli elicited hyperalgesia, hypoalgesia, and analgesia in 32.4%, 27.9%, and 36.8%, respectively. A significant association was found between the two kinds of stimuli (p=0.260). Conclusions: Sensory dysfunction frequently occurs in the branches of the trigeminal nerve, including the inferior alveolar nerve, mainly due to trauma associated with dental treatment. Simple qualitative sensory testing can be conveniently used to screen sensory dysfunction in patients with altered sensation involving oral and facial regions.

Survival Analysis of Oral Squamous Cell Carcinoma in a Subgroup of Young Patients

  • Oral squamous cell carcinoma (OSCC) is predominantly a disease of middle-aged men with long-term exposure to tobacco and alcohol. An increasing trend has been reported at a younger age worldwide. Clinical records of 100 patients under the age of 45 years treated specifically for oral cavity SCC in our hospital during a 10-year period were retrospectively analyzed to calculate the survival rates. An obvious male predominance coincided with smoking trend among Chinese young individuals and female patients were more likely to have no traditional risk factors such as smoking or drinking. The 5-year overall survival rate and disease-free survival rate were 61.0% and 75.5%, respectively, consistent with other published series over the decade showing a relatively better survival among the young. No significant differences clearly correlated with outcome when comparing non-smokers non-drinkers to ever-smokers and ever drinkers (P>0.05). Overall survival rate and disease free survival rate was found to be significantly higher in patients with early-stage disease than with advanced stage disease (P=0.001, P=0.009 respectively). The strong influence of clinical stage on prognosis emphasizes the importance of early diagnosis and treatment of oral malignancies for this unique clinical subgroup.

ESKAPE Pathogens in Oral and Maxillofacial Infections

  • Purpose: Most cases of oral and maxillofacial infections are usually easily treated by proper diagnosis, elimination of causative factors, and antibiotic therapy. However, the emergence and the increase of multidrug-resistant bacteria make treatment challenging. "ESKAPE" pathogens are the most common opportunistic organisms in nosocomial infections and have resistant to commonly used antibiotics. There are many medical reviews of ESKAPE pathogens, but few in dentistry. This study focuses on oral and maxillofacial infection especially with ESKAPE pathogens. The purpose of this study is to prepare feasible data about tracing and treatment of infection related to pathogens that may be beneficial to clinicians. Methods: A total of 154 patients with oral and maxillofacial infections were reviewed by analyzing retrospectively hospitalized data in the Department of Oral and Maxillofacial surgery, Chosun University Hospital, Korea, past 5 years from January 2014 to December 2018. Based on the medical records and microbiological tests, the results were divided into two groups: infections with ESKAPE pathogens and other bacteria. Results: A total of 22 species were isolated from 154 patients. The proportion of ESKAPE pathogens among all bacterial isolates collected from infected patients was 39.6%. Causative factors, especially in post-operative infection, showed a statistically significant correlation to ESKAPE infections (29 cases). And average of treatment period in ESKAPE group was longer than non-ESKAPE groups. Overall, Klebsiella pneumoniae (60.7%) was the most frequently isolated ESKAPE pathogen. And high antibiotic resistance rates had been detected in the ESKAPE during the five-year period. Conclusions: Infections with ESKAPE pathogens are now a problem that can no longer be overlooked in Dentistry. Based on results of this study, ESKAPE pathogens were highly associated with post-operative or opportunistic infections. Clinicians should be careful about these antibiotic resistant pathogens and use appropriate antibiotics to patients while having dental treatments.

  • The purpose of this study was to compare the soft tissue movements in facial profile predicted by a computer package with those that had actually accured following a sagittal split ramal osteotomy. The reliability of predicting the results of orthodontic surgical treatment was analysed. The study was based on the serial records of 30 consecutive patients who had been treated by means of a sagittal split ramal osteotomy. The serial lateral cephalometric radiographs used for the study were taken at the following stages: immediate preoperative : immediate postoperative : 6 months postoperative. A superimposition T1, T2-3 was generated to allow visual comparision. The results can be considered in relation to four important parts of the facial profile :the nose, upper lip, lower lip, and chin. The nose & Upper lip:The amount of movement of the upper lipwas not badly predicted for the average case. The lower lip: There was a significant trend over the whole sample for vertical positionof the lower lip to be less well predicted. The chin: The soft tissue movements of the chin were well predicted.

  • Purpose: The purpose of this retrospective report was to analyze long-term survival rate of sintered porous-surfaced dental implant ($Endopore^{(R)}$ Dental Implant system, Innova Corporation, Toronto, ON, Canada). Methods: 61 partially edentulous patients were received a total of 127 Endopore dental implants in the maxilla. Of the 127 implants, 24 implants were restored with individual (ie, non-splinted) crowns, while 103 implants were splinted to other implants. Medical records and radiographs were evaluated and analyzed by the cumulative survival rate, location of implants, implants length and diameter, crown/implant ratio and whether the implant was splinted. Chi squire test was used statistically. Result: Of the 127 implants, 8 implants (6.3%) were removed and and cumulative survival rate was 93.7%. Conclusion: Endopore implants showed satisfactory results after up to 8 years function periods in the edentulous posterior maxilla.

The effectiveness of elective neck dissection on early (stage I, II) squamous cell carcinoma of the oral tongue

  • Objectives: The purpose of this study was to evaluate and compare the treatment outcomes of partial glossectomy with or without elective neck dissection in patients with tongue squamous cell carcinoma (SCCa). Materials and Methods: A total of 98 patients who were diagnosed with tongue SCCa and underwent partial glossectomy between 2005 and 2014 were evaluated. Only 14 patients received elective neck dissection, and 84 patients received only partial glossectomy. Results: There were 56 men and 42 women with a mean age of 57 years and mean follow-up period of 33.7 months. There were 70 patients graded as T1 and 28 as T2. The total occult metastasis rate was 17.3%. The 5-year overall survival rate was 83.3% with elective neck dissection and 92.4% with observation. The 5-year disease-free survival rate was in 70.7% in the elective neck dissection group and 65.3% in the observation group. Conclusion: We retrospectively reviewed the records of 98 patients with tongue SCCa. These patients were divided into two groups, those who underwent elective neck dissection and those who did not. There was no statistically significant difference between the groups undergoing partial glossectomy with or without elective neck dissection.

사회적 기억과 구술 기록화 그리고 아키비스트 (A Study on Archiving of 'Social Memory' and Oral Record Focused on the Role of Archivist in the Stages of Oral Record Collecting and Planning)

  • 최근 기록학계의 화두는 '패러다임 변화'라 할 만 하다. 따라서 본 연구는 기록학 패러다임 변화와 관련하여 '사회적 기억' 기록화 개념을 통해 구술기록과 기록학의 적연성있는 관계설정을 이루고자 한다. 또한 이를 바탕으로 구술기록의 특성을 반영한 기록학적 구술기록 관리의 주체로서의 아키비스트의 적극적 역할론을 뒷받침할 것이다. 특히 기존 연구를 통해 이미 다뤄진 내용이라 할지라도 나름의 관점을 적용하여 새로운 의미를 도출하는데 초점을 맞출 것이다. 본 연구의 주요 내용은 다음과 같다. 첫째, '사회적 기억' 기록화 개념을 설정하여 논의하고자 한다. 이는 최근 기록학계의 '패러다임 변화'라는 화두와 관련 된다. 주로 국외 기록학자들 사이에서 활발히 이루어졌음에도 국내에서는 아직 본격적으로 다루어지지 않았기에 본 연구에서는 이 부분을 정리하는 것을 세부 목표로 할 것이다. 둘째, 기록관리의 제 단계 중 특히 수집 및 기획 단계에 초점을 맞추어 논지를 전개한다. 기록학적 관점은 생산 시점 이전 기록의 수집 및 기획 단계에서 시작하여 그 이후 단계를 통합적으로 인식하기 위하여 반드시 반영되어야 한다. 따라서 가장 중요한 것은 수집 및 기획단계이며, 이는 생산이 곧 수집을 의미하는 구술기록의 특성과도 밀접하게 연관된다. 셋째, 기록학적 관점에서 '구술기록'의 개념의 설정이다. 많은 구술사 연구자들에게 '구술자료'라고 통칭되는 구술면담을 통해 생산된 각종 자료들을 기록학적 관점에서 '구술기록'으로서 개념화한다. 넷째, 기록학에서 왜 구술사를 다루어야 하는지에 대한 의미 설정이다. 기록학에서 구술사를 다루는 목적과 그 당위성을 학문적으로 정당화시킬 필요가 있다. 왜 기록학에서 구술사가 중요한 것인지, 그것이 어떤 의미가 있는지 스스로 명확히 해야 할 것이다. 이를 통해 '구술기록'의 기록화는 어떤 의미를 지니는가에 대해 살펴볼 것이다. 기록화를 통하여 구술기록은 그 자체의 특성을 효과적으로 드러낼 수 있으며 궁극적으로 구술사와 구술기록의 가치를 재조명할 수 있음을 말하고자 하는 것이다. 마지막으로 구술사에서 아키비스트의 역할을 정립하는 것이다. 구술사에서 아키비스트는 단지 연구자들이 수집해온 구술기록을 정리하고 보관하는 보조자가 아니라는 점을 강조하고 설득할 것이다. 본 연구에서 다루는 위와 같은 이론적 논의를 통해 당위성을 얻은 기록학계에서의 구술사 연구는 단순히 기록학에서 구술사를 점유하겠다는 것이 아닌 다른 학문과의 연계 속에서 한국 구술사 연구의 도약이라는 방향에서 이루어져야 할 것이다. 이것이 가능하다면, 기록학계의 발전과 연구범위의 확장, 그리고 아키비스트의 역할 신장과 가능성 제고에 기여할 것이며 동시에 구술사 연구에 궁극적으로 긍정적 영향을 미칠 것이다.

보건의료 빅데이터를 이용한 유아기 우식증과 칸디다증의 연관성 분석 (Association between Candidiasis and Early Childhood Caries : Analysis Using Healthcare Big Data)

  • 이 연구의 목적은 칸디다증과 유아기 우식증의 연관성을 밝히고, 생후 1년 내의 칸디다증 또는 칸디다 구내염 경험이 유아기 우식증의 위험 인자로 고려될 수 있는지 알아보는 것이었다. 이를 위해 건강 보험 심사 평가원에서 2010년 1월부터 2012년 12월까지 태어난 어린이들의 의료 급여 이용 기록을 제공받았으며, 이 중 치과 진료 기록이 없는 경우는 배제하였다. 연구 대상을 칸디다증 경험 여부, 칸디다 구내염 경험 여부에 따라 나누어 치아 우식 유병률을 비교하였다. 또한 생후 1년 이내에 의료 기록을 갖는 어린이들을 만 1세 이전의 칸디다증 경험 여부, 칸디다 구내염 경험 여부에 따라 분류해 각 집단의 만 1세 이후 치아 우식 유병률을 비교하였다. 칸디다증 또는 칸디다 구내염을 진단받았던 어린이 집단의 치아 우식 유병률은 두 질환을 경험하지 않은 어린이 집단과 유의한 차이를 나타냈다. 만 1세 이전에 칸디다증 또는 칸디다 구내염을 진단받았던 어린이 집단에서 만 1세 이후부터 만 6세 이전까지 치아 우식에 이환된 어린이의 비율은 두 질환을 진단받지 않은 어린이 집단과 의미 있는 차이를 보였다.

국회기록정보 통합관리시스템 개발 방향에 관한 연구 (A Study on the Development of the National Assembly Archives and Records Integrated Management System)

  • 국회기록보존소는 국가기록원에서 개발한 표준기록관리시스템에 보존기록관리 기능을 일부 추가한 형태의 '국회기록관리시스템'을 개발하여 사용하고 있다. 그러나 시스템의 근간이 되는 표준기록관리시스템은 기록관 기능 수행을 위해 개발 보급되었기 때문에 영구기록물관리기관 기능을 동시에 수행하는 국회기록보존소의 업무 기능을 모두 반영하기에는 한계가 있다. 더구나 현행 국회기록관리시스템은 법규에 따라 이관받는 공문서 관리 중심이기 때문에 의정활동과 관련하여 수집한 국회의원 기록물, 역대 국회의장 등으로부터 채록한 구술기록, 영상회의록 등 다양한 유형의 기록물을 등록하여 관리하는 데에 어려움이 있다. 이에 본 연구에서는 현행 국회기록관리시스템의 문제점 분석과 국가기록원, 대통령기록관, 창원시, 문화재청에서 운용 중인 시스템 사례연구를 통해 향후 개발 예정인 국회기록정보 통합관리시스템의 기능 구성, 메타데이터, 목표시스템 등을 제안하였다.

Odontogenic infection involving the secondary fascial space in diabetic and non-diabetic patients: a clinical comparative study

  • Objectives: This retrospective study was performed to evaluate the clinical impact of diabetes mellitus on the prognosis in secondary space infection. Materials and Methods: Medical records, radiographic images, computed tomography, and microbial studies of 51 patients (25 diabetic patients and 26 non-diabetic patients) were reviewed. Patients were diagnosed as secondary fascial space infections with odontogenic origin and underwent treatment at Chonnam National University Hospital, in Department of Oral and Maxillofacial Surgery, from January 2007 to February 2009. Results: Compared to patients without diabetes, patients with diabetes were presented with the following characteristics: older age (diabetic patients: 62.9 years, non-diabetic patients, 47.8 years), more spaces involved (diabetic patients, 60%; non-diabetic patients, 27.3%), more intense treatment, longer hospitalization (diabetic patients, 28.9 days; non-diabetic patients, 15.4 days), higher white blood cell and C-reactive protein values, higher incidence of complication (diabetic patients, 40%; non-diabetic patients, 7.7%), and distinctive main causative microorganisms. Conclusion: These results suggest that the prognosis of diabetic patients is poorer than that of non-diabetic patients in secondary space infections since they had greater incidence rates of involved spaces, abnormal hematologic findings, more complications, and additional procedures, such as tracheostomy.
