Pemphigus vulgaris is a rin, chronic intraepidermal bullous disease with potentially fatal outcome. Oral lesions precede skin lesions in at least $70\%$ of cases, and in cutaneous disease, concomitant oral lesions are encountered in $90\%$ of patients. This disorder involve the skin and mucous membranes, especially the oral and pharyngeal mucosa, but may also involve the nasal, oropharyngeal, laryngeal and esophageal mucosa. Oral lesions are initially vesicobullous but rapidly rupture, leaving a painful erosion that shows little tendency to heal. Pemphigus vulgaris affecting the oral mucosa is still diagnosed only after considerable delay, because oral ulceration in common, and clinicians believed the lesions to be caused by more common conditions such as recurrent aphthous stomatitis rather than a rare disorder such as pemphigus vulgaris. The definitive diagnosis of pemphigus vulgaris should be undertaken as early as possible, so that treatment can be started at an earl·y stage. Because of the presence of nonspecific oral ulcer, high degree of suspicion is often required to ultimately make the diagnosis of pemphigus vulgaris and then we report a case of pemphigus vulgaris with a literature review.
구강 점막에 발생하는 동통성 궤양은 다양한 질환에 의해 발생할 수 있다. 구강내 병소가 발생한 후 피부병소가 발생하는 질환 또는 구강과 피부 동시에 병소가 발생하는 질환의 경우, 구강내만 국한하여 검사를 시행하고 피부 병소를 간과할 때는 진단이 어려울 수 있다. 본 증례에서는 구강 및 전신에 발생한 통증성, 미란성 병소가 나타나는 다형홍반 및 Stevens-Johnson syndrome(SJS) 환자를 경험하여 이를 보고하고 피부병소가 동반되는 구강점막 질환에 대해 고찰해 보고자 한다.

A 74-year-old female presented with a complaint of dry mouth, continuous spontaneous burning sensation in the tongue, and asymptomatic submucosal soft tissue mass on both sides of the lower labial mucosa. She refused to undergo total excision of the mass due to concern about the possibility of complications such as nerve damage because of the large size of the mass. As her clinical features and magnetic resonance imaging indicated the possibility of Sjögren's syndrome, a biopsy of the minor salivary gland of the right lower lip was performed. Consequently, she was diagnosed with mucosa-associated lymphoid tissue (MALT) lymphoma. Although the patient had typical signs and symptoms of Sjögren's syndrome, the histopathological result of MALT lymphoma made it impossible to determine whether the patient had a history of Sjögren's syndrome. For patients with risk factors for MALT lymphoma, such as Sjögren's syndrome, a biopsy of the labial minor salivary gland with immunohistochemical staining can be helpful in the diagnosis of not only Sjögren's syndrome but also MALT lymphoma.
Oral mucosal lesions with a papillary or verrucous surface are a heterogenous group of conditions with widely varying clinical behaviour. The etiology is variable and includes infective, traumatic or neoplastic processes. The majority of these lesions are precanceous lesion. Therefore, a biopsy is required to establish a diagnosis and the microscopic examination must be coordinated with the clinical findings. This paper reviews the clinical features and significance of papillary or verrucous lesions.
The authors have observed a case o hemangioendothelioma occurred on the left palate and buccal mucosa of a 40 year old woman. The results are as follows.
1) A blue-red round mass, 1.0cm in diameter, appeared on the palate and a blueviolet oval mass, 1.2cm in diamcter, on he buccal mucosa, and both of them showed a tendency to bleed after even slight trauma.
2) Microscopically the lesion was composed predominantly of masses of endothelial cells proliferating in and about the undifferentiated vascular channels.
3) well-vascularized tissue was made up of vascular spaces, showing infilitration of small round cells.
Oral submucous fibrosis (OSF) is a premalignant condition characterized by juxtaepithelial fibrosis of the oral cavity. The clinical manifestations include burning sensation to spicy food, blanched oral mucosa with palpable fibrous bands, trismus, depapillation of tongue, and dry mouth. As OSF is predominantly seen in people of the Indian subcontinent, cases of this disease have been rarely reported in South Korea. We present a case of OSF in a Sri Lankan man living in South Korea and emphasize the importance of enhancing knowledge of this disease while the number of immigrants is increasing.
To use cultured mucosa as a graft of full thickness, our laboratory has been involved in the development of techniques to grow epidermis together with connective tissue. Human oral mucosa was obtained at dental surgery. Under sterile conditions the tissues were cut into explants of 0.1 $cm^2$ which were placed in the center of 24 well tissue culture dishes and incubated in a growth medium. The growth medium used for epithelial was MEM(Minimum Essential Medium) supplemented with 10% fetal calf serum, 0.5% dimethyl sulfoxide, glutamine (0.292 g/l), epidermal growth factor (40 ug/ml), cholera toxin (30 ng/ml), hydrocortisone (2 ug/ml), insulin (40 ug/ml) and transferin (5 ug/ml). The medium for stratification of epithelial cells was MEM supplemented with 10% fetal calf serum, 0.5% dimethyl sulfoxide and glutamine (0.292 g/l). The medium used for fibroblasts was MEM supplemented with 10% fetal calf serum. With the three types of media used alternatively, a mucosa composed of epidermis and connective tissue was obtained after 3 weeks of culture.

Objectives: Surgical extraction of maxillary third molars is routine in departments devoted to oral and maxillofacial surgery. Because maxillary third molars are anatomically adjacent to the maxillary sinus, complications such as oroantral fistula and maxillary sinusitis can occur. Here we explore the factors that can cause radiographic postoperative swelling of the maxillary sinus mucosa after surgical extraction. Materials and Methods: This retrospective study reviewed the clinical records and radiographs of patients who underwent maxillary third-molar extraction. Preoperative panoramas, Waters views, and cone-beam computed tomography were performed for all patients. The patients were divided into two groups; those with and those without swelling of the sinus mucosa swelling or air-fluid level in a postoperative Waters view. We analyzed the age and sex of patients, vertical position, angulation, number of roots, and relation to the maxillary sinus between groups. Statistical analysis used logistic regression and P<0.05 was considered statistically significant. Results: A total of 91 patients with 153 maxillary third molars were enrolled in the study. Variables significantly related to swelling of the maxillary sinus mucosa after surgical extraction were the age and the distance between the palatal cementoenamel junction (CEJ) and the maxillary sinus floor (P<0.05). Results of the analysis show that the relationship between the CEJ and sinus floor was likely to affect postoperative swelling of the maxillary sinus mucosa. Conclusion: Maxillary third molars are anatomically adjacent to the maxillary sinus and require careful handling when the maxillary sinus is pneumatized to the CEJ of teeth.

A 60-year-old male complained of headache, nasal discharge, and diplopia for over one month with a history of left upper molar extraction, and he had recently experienced severe discharge of purulent exudate from his left antrum. Under the diagnosis of maxillary sinusitis, the Caldwell-Luc operation was performed, and several fragments of amorphous white mucoroid materials were removed. In the histological observation, sinus mucosa was relatively well preserved, but showed diffuse infiltration with eosinophilic polymorphonuclears. Huge molds of mucormycosis were associated with the surface of mucosa. He was treated with amphotericin-B deoxycholate, resulting in the uneventful healing of the antral lesion. The current case of antral mucormycosis was very rare but effectively treated by surgical removal of antral mucosa and the following antibiotic therapy for the strong inhabitants of fungal molds. We also presumed that the patient was superinfected with commensal fungus of mucormycosis during broad spectrum antibiotic therapy for the previous dental infection.
The age related changes in the life cycle of the progenitor cell population of murine oral epithelia was studied. Using radioautographic methods which have been adopted in previous cell cycle studies, the age-related changes of different phases in renewing cells of the palatal, buccal and lingual mucosae were determined. The results confirm published findings on cell cycle changes of epithelia with aging and illustrated further that mitotic phase which has hither to been considered stationary, also changes with aging. The major parts revealed by this study are as follows: 1) The basal progenitor cells in different regions of oral mucosa have different generation times. 2) The basal cell cycle time increases as a function of aging and the region most affected by aging appears to be the epithelium of the cheek. 3) The phases of the cell cycle affected by the process of aging are in increasing order of magnitude: M-, S- and G₁-phase. 4) The age elated change in the number of DNA synthesizing basal progenitor cells occurs at two age periods. Between 1 and 12 months of life it decreases, while from 12 to 20 months it increases.
