MANAGEMENT OF FIBROUS HYPERPLASIA IN ORAL MUCOSA (구강점막에 발생한 섬유성 과증식의 처치)

  • There are a number of situations where the oral mucosa can be sucked or pressed to produce relatively banal but clinical distinctive changes. The labial and buccal mucosa and tongue may develop protuberances in areas where a tooth is missing or extra space is present. The mucosa is pressed and sucked into these spaces, thus leading to the development of a fibrous hyperplasia. This case report describes the management of fibrous hyperplasia in oral mucosa. Fibrous hyperplasia can be formed by habitual pressure or suction in oral mucosa. Treatment of fibrous hyperplasia consists of simple excision and, if feasible, elimination of the cause. And habit control is a important factor for preventing recurrence.

Effect on Wond Healing of Low Power Generating Laser Irradiation on Artificially Produced Wounds of Rabbits (저출력 레이저광선이 가토의 손상치유에 미치는 영향)

  • The author used rabbits in order to examine the effect of Ga-As low power generating semiconductor laser on artificially produced injuries of experimental animals. Artificially produced injuries include surgical wound of 3mm length, 2mm depth in size on ventral skin surface of rabbit and buccal mucosa, and electrical injury formed on opposite side of skin and buccal mucosa by electrical cauterization of same length and depth, and chemical injury formed by FC(Formocresol) solution applied on the anterior dorsal part of tongue. And then, on the experimental group, Ga-As laser was irradiated beginning on the day after the wound formation and continued to irradiate every each other day for five minutes. After1, 3, 6, 9, 13th day, certain number of animals of control and experimental group were sacrified, and wound site tissue was excised to make samples and was observed under light microscope. The following is the conclusions after comparing the healing procedure of experimental and control group. The following results were obtained : 1. Inflammation was decreased more rapidly in the experimental group than the control group. 2. In the surgical, the electrical and the chemical injuries in the oral mucosa, re-epithelialization was completed more rapidly in the experimental group than the control group. In the electrical injury on the skin, re-epithelialization was completed about 6 days after wound formation on both groups. 3. In the electrical and the surgical injuries on the oral mucosa, granulation tissue formation started at 3 days after injury on both groups, but in the chemical injury, it was completed about 3 days faster in the control group than the experimental group. In the surgical wound on the skin, it was completed about 9 days after injury, but faster in the experimental group. In the electrical injury on the skin, it was faster in the control group than the experimental group. 4. In the electrical and the surgical injuries on the oral mucosa, fibrosis was started at 6~9 days after injury on both groups, but regeneration of connective tissue in the experimental group was observed much more than the control group. 5. When comparing the effect of wound healing on skin and oral mucosa of control and experimental group, granulation tissue formation and re-epithelialization in the oral mucosa was more vigorous. In conclusion, the difference of timing and the sequence of wound healing process(inflammation, re-epithelialization, granulation tissue formation, fibrosis) following Laser irradiation between control and experimental group was not observed, but the healing tissue was observed much more in the Laser irradiated group.

A Case Report of the Allopuinol-Associated Angiokeratoma in the Oral Mucosa (Allopurinol과 연관된 구강내 혈관각화종(Angiokeratoma)의 증례보고)

  • Angiokeratoma is a cutaneous vascular disorder that occurs at any sites as trunk,extremities, fingers and toes etc. Although solitary or multiple cutaneous lesions have been reported, oral lesions have been very care. A 72-year-old man who had an exophtic, essile mass with dark red, black colored which located on buccal mucosa, was treated with excisional biopsy. He had no specific systemic history except for the medication of allopurinol, for treatment of gout since 10 years. Final diagnosis was determined as angiokeratoma by evaluation of clinical and histopathological finding, and the lesion has not been recurred for two months by decrease of allopurinol. It has been emphasized that the relationship between oral mucosal disease and the complication of allopurinol. Allopurinol is widely used for gout treatment, which we will report a case on the allopurinol-associated angiokeratoma in the oral mucosa.

Immunological Effect of the Cytochrome P450 to Alcohol and Stress in Guinea Pig (알콜과 스트레스가 Cytochrome P450 발현에 미치는 영향에 관한 면역학적 연구)

  • The purpose of this study was to investigate the effect of alcohol and stress on liver and buccal mucosa in guinea pig by immunological methods. Especially, Cytochrome P450 (CYP) which in oxidase during alcohol metabolism and bioactivator to carcinogen was used as an indicator in this study. 48 guinea pigs were used in this study. The experimental guinea pig were divided into three groups: The first was a group with giving alcohol-15%(v/v) ethyl alcohol, the second group was a with giving stress in the $0^{\circ}C$ water and the third was a control group. Every 4 guinea pigs of each group were sacrificed weekly-first, second, third, fourth week after experiment and extracted liver tissues and buccal mucosa. The liver tissues were observed by using immunoblotting technique (Western blot) and buccal mucosa were observed by immunofluorescence technique. The results were as follows: 1. By the alcohol and stress, Cytochrome P450 was amplified positive in the liver tissues at third week. 2. By the alcohol and stress, Cytochrome P450 was not detected in the buccal mucosa at any period.

Hodgkin Lymphoma of the Oral Mucosa : Case report and Literature review (구강점막에 발생한 호지킨림프종의 증례보고 및 문헌고찰)

  • Extranodal intraoral Hodgkin lymphoma is not common. We report the case of a 78-year-old male patient with ulcer of the mandibular oral mucosa that was not cured for about 3 weeks. In biopsy and histopathologic examination, it was found to be extranodal intraoral Hodgkin lymphoma. Early diagnosis of oral lesions led to early detection of lesions of the opposite neck lymph node in an additional PET-CT scan. We report this case and review relevant literature.

Personality Type Test(MBTI) of Korean College Students with Symptoms of Oral Mucosa Disease (구강점막질환증상자의 성격유형검사(MBTI))

  • The purpose of this study is to investigate the relationship between personality type and symptoms of oral mucosa disease. 393 college students completed the MBTI(Myers-Briggs Type Indicator) and a questionnaire and collected data were analyzed by SAS 9.2 program. The obtained results were as follows : 1. Dysgeusia(p<0.05) and xerostomia(p<0.01) occurred significantly more frequently in I type than E type. 2. Herpetic stomatitis, recurrent aphthous ulcer, glossitis, dysgeusia, burning mouth syndrome and xerostomia seemed to occur more frequently in S type than in N type. 3. Most symptoms of oral mucosa disease seemed to occur the most frequently in the type including NF among 16 types of personality of MBTI. 4. Most subjects had negative attitude in curing their symptoms of oral mucosa disease(p<.0001). Significantly more subjects with I type than E type had negative attitude in curing herpetic stomatitis(p<0.01) and dysgeusia(p<0.05). 5. Most symptoms of oral mucosa disease appeared to occur the most frequently in nervous or sensitive subjects. Therefore a guideline for the individual cure of oral mucosa diseases meeting personality type is necessary.

Effects of Isoproterenol on the Mast Cells in The Oral Mucosa of Albino Rats. (Isoproterenol이 백서구강점막의 비만세포에 미치는 영향)

  • The effects of isoproterenol, a sympathomimetic amine, on the mast cells in the oral mucosa were cytochemically studied in twenty four healthy male albino rats weighing approximately 100gm. seven groups of three each were injected intraperitoneally, as a single dose of 8mg/100gm. body wt., (dissolved in 0.9% saline 0.2㎖). Each groups were sacrificed following ether anesthesia at he time indicated, 1. 2. 3. 5. 7. 10. 14days after only a single injection. One normal group is not administered with any substances. The sections of oral mucosa were stained with 0.005% toluidine blue (Mowry's method) and alcian blue (pH 2.5). We found that isoproterenol made an initial decrease in the metachromasia and alcian blue affinity, and the number of mast cell, followed by an subsequent return to normal.

  • Pleomorphic adenoma is the most common of all salivary gland tumors, constituting over 50% of all cases of tumors of both major and minor salivary gland origin and approximately 90% of all benign salivary gland tumors. Of the major salivary glands, the parotid gland is the most common site of the pleomorphic adenoma. It may occur, however, in any of the major gland or in the widely distributed intraoral accessory salivary glands. The palatal glands are frequently the site of origin of tumors, and other parts of origins are as follows: upper and lower lip, buccal mucosa, tongue and occasionally other sites. The majority of the lesions are found in patients in the fourth to sixth decades, but they are also relatively common in young adults and have been known to occur in children. It is somewhat more frequent in women than men. The term "mixed tumor" has masquaeraded under a great variety of names throughout the years (e.g., enclavoma, branchioma, endothelioma, enchondroma), but the term "pleomorphic adenoma" suggested by Willis characterizes closely the unusual histologic pattern of the lesion. The accepted treatment for this tumor is surgical excision. The intraoral lesions can be treated somewhat more conservatively by extracapsular excision. In general, Lesions of the hard palate should be excised with the overlying mucosa, while those in lining mucosa, such as the lips, soft palate and buccal mucosa often can be treated successfully by enucleation or extracapsular excision. In our hospital, we experienced two patients who were identified pleomorphic adenoma which occurred at buccal mucosa, submandibular gland. The lesions were successfully treated by surgical excision.

Effects of Rotated Endotracheal Tube Fixation Method on Unplanned Extubation, Oral Mucosa and Facial Skin Integrity in ICU Patients (기관내관 순환고정방법이 중환자실 환자의 비계획적 발관 및 구강.안면 피부 통합성에 미치는 효과)

  • Purpose: The study was done to compare effects of two endotracheal tube (ET tube) fixation methods (rotated fixation versus conventional) on unplanned extubation and skin integrity for orally intubated patients in intensive care units. Methods: The research design was a non-equivalent control group with repeated measures design. Participants were 80 patients; 40 participants assigned to each group. ET tube for the experimental participants fixed with rotated method every morning. Unplanned extubation was assessed by bedside nurses using the unplanned extubation report form. Oral mucosa and facial skin integrity were assessed using oral assessment guide and facial skin integrity assessment guide at day 3, 7, 10 and 14. Results: There was no difference in the unplanned extubation rate between the two groups. Oral mucosa impairment scores for the rotated fixation method were significantly lower at day 7 (p=.044), 10 (p=.048) and day 14 (p=.037). Also facial skin integrity impairment scores for the same group were significantly lower at day 7 (p=.010), 10 (p=.003), and 14 (p=.002). Conclusion: Results of the study suggest that the rotated fixation method is effective for these patients, to prevent impairment of oral mucosa and facial skin integrity. Further research is needed to prevent unplanned extubation.

HISTOCHEMICAL STUDY ON THE ORAL MUCOSA OF FOLIC ACID-ADMINISTERED ALBINO RATS (엽산이 백서 구강점막에 미치는 영향에 관한 조직화학적 연구)

  • The effects of folic acid on the oral mucosa of albino rats were histochemically studied in the twenty four male rats, weighing abut 100gm. Seven groups of 3 each were injected with 0.2ml folic acid (folic acid 0.5mg. was dissolved in physiological saline 0.2ml) subcutaneously, during 1,2,3,5,7,10 and 14 days respectively. Oral mucosa of rats sere removed from upper molar region and fixed in 10% formalin, cold absolute alcohol, Carnoy's solution and acetone. The serial sections were histochemically stained by McManu's PAS reaction, Mowry's metachromasia, alloxan-Schiff reaction, and azo dye method for alkaline phosphatase. The comparative staining method was hematoxylin-eosin stain. The results were as follows: 1) Alkaline phosphatase reaction of stratum spinosum and stratum granulosum tended to increase after 7 and 10 days of folic acid administration. 2) PAS reactions of basement membrane and lamina propria increased after folic acid administration. 3) Metachromasia of stratum spinosum and stratum granulosum were slightly increased after 3,5 and 7days of folic acid administration and returned to the level of Control after 10 days. 4) In the oral mucosa, alloxan-Schiff reaction increased after 7 days of folic acid administration.
